Cpsc:when we opened the car to go to the store we noticed a smell of something burning.then saw smoke coming up from the seat at the point where the car seat mirror was focusing the sun's reflection. I immediately removed the mirror, took a picture of the hole that was burned, and poured bakingsoda on the smoldering hole to stop further damage. Upon inspection of the mirror, i discovered that it had a slightly concave shape which allowed it to focus the light of its full surface on a small area. Updated 07/24/2017

Two of these reversible, 5 point harness safety seats have broken off the reclining base that isdesigned to stay attached to the main body of the seat. The contact owns a 2008 bms pathfinder 200 cc scooter. The contact was driving approximately 40 mph, and noticed that the speedometer displayed an inaccurate reading. The failure recurred whenever the vehicle was in motion. The vehicle was not diagnosed or repaired. The manufacturer was made aware of the problem.the approximate failure mileage was 50. The vin was unavailable.

Cpsc: "vinsic usb type c car charger it was charging my nexus 5x with a usb c-c cable, and it was charging my girlfriend's samsung galaxy s6 edge+ with a micro usb cable. At some point inoticed that my phone was no longer charging so i stopped using the car charger. I took the unit apart and noticed that the spring on the center terminal had overheated tothe point that a few coils of the spring were welded to each other and the spring had actually melted and fallen apart.melted and fallen apart. Yesterday 10/18/05 prtc omniride bus 147 picked up at west falls church metro in northern virginia at 5:30.the heat was on.the driver said he was trying to get the air conditioner to run and it would not put out any cool air.he said he reported to his supervisor who told him to drive the bus.you can not be held accountable for his lack of critical thinking skills (leaving the heat running trying to get it to blow out cool air), but you are accountable for putting a bus on the road that is unsafe.once we got on 66 westbound we were okay until we got outside the beltway where traffic started backing up.when the traffic started backing up and the bus had to creep along, the engine would die.this happened three times between the dunn loring and vienna metro stations.it happened approximately 4 more times before we could connect with the linton hall bus that was then able to take us into manassas.the people on the bus say that they have complained about this bus before.this bus is unsafe and should not be on the road.i am planning to report this incident to dot, also, for an investigation to see why the repair process is not sufficient enough to fix the problem and continue putting a problematic bus on the road.i instructed the driver (george) that he should refuse to take an unsafe bus out, that as long as he was willing to drive it, you would continue to give it to him.prtc could be held legally liable for liabilities incurred because of any incidents related to a faulty bus.

The dealers advised the contact that battery needed to be replaced. The contact also had to replace the battery cables prior to the failure. The current and failure mileages were 956.

He also stated that manufacturer was no longer in business and there was a new manufacturer. He contacted the manufacturer and they advised him to contact the new owners. He called the new owners and he was advised that to get the vehicle serviced he would have to travel over 300 miles. The vehicle was not repaired. The failure and current mileage was 24.

This vehicle manufacturer is not listed in your system.this is a street legal 200cc motorcycle purchased from sunl motor sports.2005 sunl model lb200-2 motorcycle purchased new with less than 6 miles on motorcycle at time of purchase.please see www.sunl.com for more information on sunl.i was advised that this was a reliable source of transportation that i could use to travel from home to work and back.problems with motorcycle began by 2nd day of ownership.after purchase on 4/19/06 i discovered that:1.the factory installed mirrors are not adjustable to view traffic.this resulted in me not being able to have any vision to either side or behind without physically turning my head and almost caused me to be an in accident.after the manf stated no other mirrors were available i had to purchase mirrors intended for another type of bike and attach to mine.2.motorcycle engine is not capable of functioning under normal use on a daily basis as a means of transportation.the engine on my motorcycle went out within the first 25 days of ownership and after the repaired it the engine problem re-occurred within 2 days.this failure has caused me to have to leave the roadway very quickly due to the loss of power and almost caused me to collide with traffic.to date i have owned the vehicle 37 days and it has been in for repairs for approximately 18 of those days and is still inoperable as of today.the engine has been replaced and it only lasted 2 days even with me not exceeding 50 mph on the vehicle.this has caused me to experience a considerable loss of time and money after purchasing a brand new motorcycle.

An authorized dealer was not located in the area; therefore, the manufacturer agreed to ship the repair part to the owner. The contact had concerns of the safety risk involved. Cpsc the consumer was driving his truck. When he went to pass a vehicle, the gas pedal got stuck on the mat. There are horizontal grooves on the mat that cause the pedal to get stuck.the consumer had to shift into neutral to get the truck to slow down. He was able to shut the engine off.the consumer stated the vehicle reached 96 mph before he was able to regain control of the truck.

Cpsc:came out to my car after work. Upon entering it smelled like an electrolytic capacitor blew.could not locate were it was coming from. Until i reached for the car charger for my cell and it was stuck melted to the seat.the consumer stated the charger was plugged into the 12v outlet and still worked. Updated 05/09/2017

I would like to complain about the hid headlights.they are simply too bright.when a vehicle approaches me at night (hid) i have trouble seeing what is directly in front of me.can we please have headlight standards that address this issue?

I cannot narrow this down to a specific vehicle, because it is so widespread, but over the weekend, i barely escaped what would have been a head-on collision with a vehicle on a 2-lane road that had crossed over the center lane unbeknownst to me.i was not able to see this vehicle due to severely degraded plastic in the headlight lens, which i have seen on more vehicles than this.i am concerned that carmakers are cutting costs and sacrificing safety by making such a crucial part of the safety equipment made out of plastic.please consider requiring the use of glass for headlight lenses in the usa.

This not about any special make of car. It is a complaint on the new projection headlights. These have a great tendency to blind oncoming drivers as the car bounces on uneven road way (which is 90% of al roads.) they need to be made illegal on all vechicles

The contact owns a 2010 piaggio gts300 scooter. While driving approximately 20 mph, the vehicle suddenly stalled. The vehicle was towed to the contacts home. The failure recurred three different times. The contact stated the vehicle would not start for at least two hours after stalling. The vehicle was not taken to an independent mechanic or dealer for diagnostics. The mechanic stated the failure may have occurred due to a defective fuel pump. The vehicle was not repaired. The approximate failure mileage was 400.

So, i wanted to buy a small, efficient 4x4; something that would do ok in the snow, but wouldn't use too much gas, and i wanted one for less than $15k new. So, after coming back for a trip to italy, i discovered that if i wanted to import a fiat panda (which best fit my needs), i'd have to find a registered importer. Or become an importer myself. And pay a bond equal to 1.5 times the price of the vehicle.this is clearly suboptimal. The eurozone and japan both have first-rate safety and environmental regimes; why must a personal import of foreign cars begin with the assumption that whatever safety or emissions regulations the vehicle was originally designed to meet were inadequate, especially in light of the huge numbers of older and/or poorly maintained vehicles still on the road, but exempt from scrutiny because they were *sold here originally*.this seems to exist only to frustrate the informed consumer. I hope my government can do better.

Cspc: manchester rv propane tank s/n 1119471, cat #6879, has a broken weld on bracket from thepropane tank, causing the tank to drop onto the spare tire and create a potential danger to therv. *dt

My shifter broke in my lotus elise.i was shifting while driving and it just snapped off.this has happened to many of us now, and the part should be recalled and replaced.it's just a matter of time before someone gets injured or killed.

In re:the wisconsin department of corrections transportation van, which has a metal box with a metal seat in which the prisoners are placed.the consumer feels that there is no need for the box and seat because the prisoners are kept in full restraint while in transit.ultimately, if the van were to crash, the prisoner(s) would be killed, or seriously injured.scc

No warning lights illuminated to indicate there was a problem.the brake locking up caused a hole to burn through the tires.this has happened twice, and the tire has been replaced on the trailer twice.the service dealerrepaired the vehicle;consumer didnot know what was done to the vehicle.after driving approximately 200 yards the brakes locked up again.the manufacturer was working on giving a trade for the trailer. Running of red lights, speeding. I was almost hit by a truck from basic energy service dot number 496976 or 496970, tx dot number 27167, tx lic number 2dg824on 4/28 this truck (with load) ran a red light with his horn blaring. I was able to swerve out of his way before he rammed me from behind. His speed was to great to stop when he had 200+ feet when the light changed to red not yellow he sped upand just laid on his horn.

The contact owns a 2005 bad boys electrical boogie model b258 (na).the rear driver's side axle has failed eight times.the axle metal is not strong enough to sustain the boogie weight and causes the axle and bolts to fail.the manufacturer has sent new parts; however, the rear driver's side axle continues to fail.the mileages were not applicable.

Dt: the contact stated while driving the left rear wheel came off the utility trailer without warning.he managed to pull over.upon inspection he discovered the lugs were loose.he tightened the lugs,and took the trailer to a repair shop.the mechanic replaced six wheel studs, two rims,and two tires.these repairs were covered under warranty.after the repairs were donethe studs came loose again.he took the trailer back to the repair shop for further repairs which have not been completed at this time.

Cpsc: i had a problem with a tire on my truck so i had to go todiscount tire for a new tire and on the request for my new tire (dynapro atm 275/65/20) made by hankook. I already have 3 others on my truck. I was informed that discount tire can't sell methe tire due to the fact "they are not sealing properly to stock truck rims".

Blf note: asking tom to look over for review, 8-8-2017cpsc: bought 2 brand new tires for my car, and one of them blew out and completely shredded one hour and 38 miles away from goodyear service center. Very scary as i was traveling on theinterstate in bad conditions next to a truck. The consumer stated the tire was returned and another tire was provided. Cpsc: purchased a hawk 250 from tx powersports. I have driven it 31 miles. Still under the 90 day warranty, the reartire locked up and destroyed the wheel and rear swing arm. Tx powersports refused to honor their 90 day warranty. Tx powersports said that rickey power sports, theimporter/distributor/manufacturer of the hawk 250, said that the parts are not covered . Letter from jerry shipman (ok) regarding nhtsa's early warning reporting (ewr) regulations. Mr shipman opposed the issuance of quarterly early warningreport regulations because it would put manufacturer's of small-to-medium trailers out of business, s10-0303(brebber ssgn).

Fax letter from sean johnson manager calico trailer manufacturing inc. (ak) opposing regulations to submit quarterly informational reports, early warning reports (ewr) dot 2003-4246. Mr johnson claims that the new regulations ar unfair to small-to-medium size trailer companies, under 26000 lbs, and should be rescinded or radically changed, otherwise it could put him and many of his colleagues out of business. Nhtsa's new rules require all large manufacturers of motor vehicles (a tractorwas included in large motor vehicles), starting april 1, 2003,to compile quarterly informational reports called early warning reports(ewr) containing complex dataabout vehicle's accidents, safety related defects, and customer' service complaints. Because johnson's company produces more than 500 trailers annually it was lumped together with ford, gmc and freuhauf bound by tread act. Also, according to nhtsa's own estimates, johnson will be forced to spent $225000within the next few months to meet nhtsa's requirements. Afterwards, $145000annually to comply. Also, those trailer companies under 26000 lbs are rarely involved in serious accidents or have serious safety related defects. *nlm

Mr priesgen opposesfederal government's irrational intrusion into the day to day intrusion into the daily operations of trailer industry. Mr priesgan's company manufacturs small-to-medium size trailers, less than 26000 lbs gross vehicle weight.nhtsa'sewr regulationsif not rescinded or substantially modified would put priesgen and many of his colleaguesout of business. Nhtsa's new rule requires all large manufacturersof motor vehicles (a trailer is included in the definitionof a motor vehicle), starting april 1, 2003, to compile and submitinformational reports entitled early warning reports or ewrs containing complex detailed data about vehicle accidents, warranty claims,safety related issues , and customers requirements. Because mr. Priesgan company produces over 500 trailers he is lumped with giants such as ford, gmc and freudhheun who are bound under tread act.according to nhtsa's own estimates, mr priesgenwill be forced to spent $225000 within nextseveral months to comply with the requirements.then, another $14100this yearand thereafter to comply with ewr requirements. Mr priesgen insists that neither he or his colleaguescould meet those fiscal requirements. Furthermore, trailer industry is very rarely involved in major accidents or defect issues as specified by the ewr requirements. *nlm

The cab was blue in color and pulling a empty car carrier trailer. The license plate number on the trailer was [xxx]. The trailer number was [xxx]. The cab number was [xxx]. The name of the company was jack cooper transport..the usdot number on the door of the cab was [xxx].we had been in the left speed lane, trying to keep a safe distance from the vehicle in front of us, doing about 8 miles over the limit, and the driver had been tail gating us. My wife was getting very upset and gesturing him to back off and he would only get closer. When he finally passed us on the right, there was not enough room for him to cut back over into our lane so he just came over anyway before his trailer was clear of our front end. If i hadn't slammed on my brakes and swerved onto the shoulder, he would have collided with us and probably taken my wife's side of the car off. The truck turned left onto rt. 309 at 2:39 p.m.i reported this to the maryland state police at the time and provided them this information.i also reported this to the company and my contact was[xxx]jack cooper transport company, inc.|[xxx]11071 guilford rd. Cpsc: purchased a hawk 250 from tx powersports. I have driven it 31 miles. Still under the 90 day warranty, the reartire locked up and destroyed the wheel and rear swing arm. Tx powersports refused to honor their 90 day warranty. Tx powersports said that rickey power sports, theimporter/distributor/manufacturer of the hawk 250, said that the parts are not covered . Cpsc:came out to my car after work. Upon entering it smelled like an electrolytic capacitor blew.could not locate were it was coming from. Until i reached for the car charger for my cell and it was stuck melted to the seat.the consumer stated the charger was plugged into the 12v outlet and still worked. Updated 05/09/2017

Cpsc: i had a problem with a tire on my truck so i had to go todiscount tire for a new tire and on the request for my new tire (dynapro atm 275/65/20) made by hankook. Consumer writes in regards to receiving aftermarket parts on vehicle after auto accident. The consumer requested information concerning the safety and validity of aftermarket parts being provided by the insurance company.

I was traveling on highway 99 in ca on 08-23-05.a truck drew up on the back end of my personal vehicle, which was traveling about 70 mph at the time.he was so close that i could not see the lower portion of the grill in my rear view mirror.i tapped my brakes to try & get him to back off but to no response.i pulled off to the side so that he could fly past.as he passed i caught the name of his trucking company "ren marc".i sped up to get the company's number so i could call & complain.i was doing about 80 mph to keep up with him to get the info i needed.i called the company & no one spoke english on the phone.i then decided to call you to report this danger.one # listed on the truck was #ca 167444 but the dot # listed was 1310844.i don't no if this is the right place to formmy complaint but no one at the company seems to understand english (literally) so i know they will not do anything to push safer driving.i hope you can help.

Very scary as i was traveling on theinterstate in bad conditions next to a truck. The consumer stated the tire was returned and another tire was provided. Mr lisko thinks that such reports would put him and his colleagues out of business.

Letter from peter hokanson opposing those provisions of the early warning reporting (ewr)regulation that requires manufacturers that produce over 500 small-to-medium trailers (those with a gross weightrating, gvwr, of under 26000 pounds) to issue a comprehensive early warning report to nhtsa. Such reporting would put mr hopkanson and some of his other collegues in trailer industry out of business.

Wortington welding , inc requested for a manufacturer's identification and password to be used to digitally sendtread information to nhtsa secured website. Also, enclosed the names and requested information for the primary and secondary contacts to be used in cas eof problems or questions once the company's data has been submitted.

Re: additional information. The consumer's vehicle was repossessed four days after the payment due date. She complained of a defect with the horn and refused to sign a new contract.*ph*tc

Blf note: sent consumer an email requesting missing child safety seat information on 8-8-2017cpsc:several child safety seats were purchase by the state of nm child protective services. Two of these reversible, 5 point harness safety seats have broken off the reclining base that isdesigned to stay attached to the main body of the seat. This presents a huge safety issue if the seat is in use and the car is involved in a crash of any magnitude.updated 07/19/2017

Request for information regarding the requirements needed for an auto manufacturer to perform a recall.*mr

The contact owns a 2004 rolling r golf cart (na). The contactstated while traveling at approximate speeds of 18-20 mph, the vehicle would not accelerate and after several attempts, the vehicle stalled. The dealer advised he was aware of the failure in his type of vehicle and advised if he were to repair the motor, the vehicle would catch on fire. The contact believed the defect to be an extreme safety issue. The vehicle was not repaired. The current and failure mileages were not available.

Cpsc the consumer was driving his truck. When he went to pass a vehicle, the gas pedal got stuck on the mat. There are horizontal grooves on the mat that cause the pedal to get stuck.the consumer had to shift into neutral to get the truck to slow down. He was able to shut the engine off.the consumer stated the vehicle reached 96 mph before he was able to regain control of the truck.

This vehicle manufacturer is not listed in your system.this is a street legal 200cc motorcycle purchased from sunl motor sports.2005 sunl model lb200-2 motorcycle purchased new with less than 6 miles on motorcycle at time of purchase.please see www.sunl.com for more information on sunl.i was advised that this was a reliable source of transportation that i could use to travel from home to work and back.problems with motorcycle began by 2nd day of ownership.after purchase on 4/19/06 i discovered that:1.the factory installed mirrors are not adjustable to view traffic.this resulted in me not being able to have any vision to either side or behind without physically turning my head and almost caused me to be an in accident.after the manf stated no other mirrors were available i had to purchase mirrors intended for another type of bike and attach to mine.2.motorcycle engine is not capable of functioning under normal use on a daily basis as a means of transportation.the engine on my motorcycle went out within the first 25 days of ownership and after the repaired it the engine problem re-occurred within 2 days.this failure has caused me to have to leave the roadway very quickly due to the loss of power and almost caused me to collide with traffic.to date i have owned the vehicle 37 days and it has been in for repairs for approximately 18 of those days and is still inoperable as of today.the engine has been replaced and it only lasted 2 days even with me not exceeding 50 mph on the vehicle.this has caused me to experience a considerable loss of time and money after purchasing a brand new motorcycle.

Cpsc:she indicated that the right rear wheel wiggles while it is being used and the issue began shortly after purchase.she also indicated that a screw from the car seat lock bar fell off about a month ago. She was able to identifywhere the screw came from and installed it back. *dt

Dt: the contact stated while driving the left rear wheel came off the utility trailer without warning.he managed to pull over.upon inspection he discovered the lugs were loose.he tightened the lugs,and took the trailer to a repair shop.the mechanic replaced six wheel studs, two rims,and two tires.these repairs were covered under warranty.after the repairs were donethe studs came loose again.he took the trailer back to the repair shop for further repairs which have not been completed at this time.
