At approximately 14:35 on saturday may 24 2014, the car behind me was hit by an suv that didn't brake when he came to a totally stopped long line of traffic on hwy 101 sb.i estimate that the suv was traveling at a speed of at least 55 mph when it struck the car behind me. The car behind me then struck the back of my 2012 prius plug-in vehicle pushing the front of my car into another suv in front of me.4 vehicles were involved, others took heavy damage. None of my airbags deployed, and i began feeling lower left side neck pains the day after the accident.
On september 8, 2012 my wife was on her way to our rental and was hit as she readied to turn to the house. Our 2012 toyota prius 3 was thrown into the air and bounced back to a diagonal position across the road. Upon impact, the steering wheel air bag did not deploy until after impact--her seatbelt did not restrain her from hitting her chest on the steering wheel nor did the knee air bag deploy until after she hit her knees on the lower dash. Fortunately for her and me, she suffered deep bruising but no broken bones etc. As of today, 9/26/2012, her chest still hurts but the pain is diminishing. I contacted toyota and filed a complaint on 9/8/2012 and was told they would investigate--case #[xxx], my concern since we have heard nothing is that you too will hear nothing and if this issue exists in other prius cars on the road, someone will suffer more than my wife. If this is an isolated case--good for toyota but how does this help us? i called toyota today for an update but have not yet heard back. If more information is needed plz contact me. I was driving on the highway 59 feeder road southwards towards university blvd in the left most lane. A car came and hit the passenger side of my car. Due to the impact my car rolled over onto the curb, hit the grass and flipped back into the lane again. I was in complete and utter shock and trauma and soon enough i noticed the blood on me. My left side (head, shoulder and elbow) was paining badly. My seat belt was still on, my leg was on the brake and windows shattered and glass was all over the place, air bags were not deployed. I turned off the engine, moved shift rod into park position and applied the hand brake.a man and his son (i believe) witnessed the accident and came to me within a few minutes. They called ems. While waiting for emergency to arrive, i got my cell phone from my backpack, which was on the passenger seat, and called my father.a few minutes later, another person, came up to me and said he witnessed the accident as well and stayed near my car. I did not move from the drivers seat or open the door because i was too afraid to open the door...few minutes later, the emergency personnel came. They opened the car door and cleared a few things and put me on a stretcher. They put a neck brace and restrained my head from moving and strapped me onto the stretcher. They picked up my backpack as well.my dad came as they were putting me on the stretcher. I was immediately rushed to the nearest er. ..i have concern about the airbag. I just want to know where to go to get checked for recalls i received
2012 toyota prius.consumer writes in regards to malfunction of vehicle seat belt and airbags.the consumer's wife was involved in an accident on september 8, 2012. The vehicle received severe front-end damage to the extent of almost $12,000. When the vehicle was stuck by an on-coming vehicle, her seat belt did not prevent her from hitting her chest on the steering wheel nor did the air bag deploy in time.she received severe bruising, severe pain and discomfort, and stress. Also, the lower dash knee air bag failed to deploy in time to prevent severe bruising to his wife's knees. Toyota sent the consumer a letter stating there was no evidence of any manufacturing defect with the air bags or seat belt.
I was involved in a front end collision with a bobcat front end loader. I struck the implement broadside going 35 mph.there was no damage to the implement. My car was totaled and the airbag deployed. After going to the hospital and a chest x-ray, ultrasound, and ct scan, it was diagnosed i had fractured my sternum. I am six feet tall and had my seat all the way back. I am questioning whether there have been any other people sustaining a fractured sternum due to an air bag deployment at 35 mph?
Takata recall. Nhsta shows recall but also shows that toyota doesn't have a remedy. Why not?!?nothing has happened regarding the defect but i'm concerned it may. Bloomberg says takata uses ammonium nitrate (fertilizer) as a propellant which is apparently unstable.
While traveling in a rain storm at 17mph, i tried to engage the brakes to prevent from crashing into an 18 wheeler and the brakes did not work. I repeatedly tried to stop the car and the car was accelerating even thought i was applying the brakes not the gas.the car would not shut down, go into park or neutral. I did crash into the 18 wheeler because the engine was accelerating and i wanted to stop.when i crashed the airbags did not deploy.even after impact the car was still accelerating like it was trying to push the 18 wheeler. Eventually the car powered down after repeatedly pushing every button possible. I had the car towed to a toyota dealership. I phoned your hotline but the vin number i gave had a number that was wrong my prior confirmation number is 10542285. Toyota is aware and i am waiting for them to call me back.

I was rear ended while stopped, by a vehicle reported to be doing 40 mph. My vehicle sustained rear bumper damage from the car hitting mine and front bumper / engine compartment damage from the car i subsequently hit. Total damage in excess of $11,000.the air bag did not deploy, but i didn't know if the impact was severe enough to activate it. However, when reviewing the repair invoice, i noticed the air bag sensor was replaced. Apparently it should have activated the system but didn't.

My 2012 prius was hit in back on passenger side at approximately 80-85 miles/hour while my speed was 62 m/h. With impact from hit in the back, my car went flying head on into the median wall (those temporary cement walls used during reconstruction). Two things happened were:1. After the first impact my prius had total power loss, not even emergency flasher light would turn on;2. I experienced a front end collision in my new 2012 prius with 2,500 mileage.the collision vehicle speed was nearly 30 mph.the seat belt did not restrain me and the airbag did not deploy.i was taken to the hospital and sustained a fractured sternum due to hitting the steering wheel and whiplash.there was significant damage to the front end of the vehicle. Toyota's response after their own inspection was "our inspection found no evidence of a manufacturing or design defect with the airbag or seat belts."

Wife in accident with side impact and front impact approximately 35-40 mph ran off of a road with no shoulder side impact with oak tree then front impact with another oak tree. No airbags deployed. The vehicle was involved in a multi vehicle accident, where the car was hit twice on the drivers side, the 2nd impact hitting the drivers door causing damage to the door and fender along side of the engine compartment.the air bags illuminated due to the crash, but there was no deployment of the airbags.all party's were concerned why the malfunction light came on during the crash, and should the bags have deployed.the body shop said they think it was cause the door was misaligned after the impact.concerned that the system may deactivate itself during a crash and not deploy the airbags.

My car will turn on, but it will not shift into drive or reverse until numerous attempts.i usually have to turn it on and off for about 10-15 times before it will allow my to shift into gear.the key fob will lock and unlock the car.the car will still turn on, ac will still come on, radio still works.the only thing that doesn't work on the first 10-15 tries is the sifting into gear.priuschat forum talk about this issue a lot.they all have similar stories with the same details as mine.it takes me about 5-7 min to get my car started.once i finally get the car in gear, no issues pop up for the rest of the drive. All of this started about within the last month for me.
I have never received a recall mail regarding the car. I used an insurance tracking plugin, and all the problems started.pcs error message on my dashboard, due to a leak in brake booster they said. My dealership will not tell me it's a recall, made it seem like they helped me instead of charging as high as $4000. Over the last nine months i have had to replace the bulbs in my 2012 toyota prius 6 times. When i brought it to the dealer they stated that it was because i was not using toyota certified bulb and i paid over $150 to have them inspect the headlights and replace the bulbs.after i had the certified bulbs installed, a bulb went out again. When i returned to the dealer, i was told that i needed new wiring for a cost in excess of $250.00. The technician stated that tsb-0204-12 was issued for for 2010-11 prius for this problem and that the 2012 prius have the exact same wiring harness, but that toyota was not going to fix that problem for teh 2012 models and that i would have to absorb the cost myself.

My prius was just fine then one day it stalled while i was driving in the middle of the free way.i took my vehicle in to elmore toyota in westminster, ca, they found it had a failed inverter and it was still covered under warranty.it took 2 weeks for them to fix it, but when they returned my car, the slip indicator was now on and would not turn off.they claim it was like that prior to the failed inverter, this was false, my car was running perfect.they then wanted to charge me nearly $1k to fix a problem i did not have prior to failed inverter.i feel the situation was connected but they refuse to cover it.
There has been a safety recall on my vehicle for the years 2004-2009 models of the toyota prius, but my model year is 2012 and i have the exact same issue.1) my check engine light comes on and off and it appears to be the hybrid electric water pump issue related to the coolant.the cap pops off and spills out.2) when the light comes on, i am assuming the car is starting to overheat and is putting myself and my 3 year old daughter at risk as this is the primary car we use to transport her.we have just stop taking her out at all due to this issue.when the car overheats, i pull over and it can be risky as sometimes, i am on the highway or local streets.i do not want this issue to cause problems for other other drivers as well while i am trying to pull over where ever i am.3)the problem as i stated above has been acknowledge by you and toyota but just for 2004-2009 models.4) the problem has not been inspected by toyota, the police, or third party.i have been the only person checking the car and i am using a diagnostic check engine light tool to read the code and looking under the hood to find the coolant pump leaking and popped out just as indicated in the recall for other models.5) yes, the check engine light and high temperature light goes on and off.the issue first appeared around november of 2020.
When our neighbor with a 2013 prius key fab enters our 2011 prius it disables the ignition. It will sometimes allow the car to start by touching our key fab to the ignition button however if engine stops, like at a stop light, it will disable the ignition again not allowing you to start the car unless you put your key fab up to the start button again. The dealer knew about the situation and agreed it is a safety hazard but said toyota will not fix the problem.
Car becomes immobile - will not shift into reverse or drive.computer keeps sending the message to put it in park and depress brake, but once that is done, any attempt to shift into reverse or drive triggers the message, endlessly.the problem is apparently intermittent.occurred for two hours yesterday, after occurring to a lesser extent in preceding days (in the days preceding the total breakdown, i was able to shift after a slight delay and a few attempts).the dealership to which i had it towed said it was fine today.everything checks out. Cigarette/power outlet fuse always blows very frequently - even with a gps use of cellphone charger only. Have been to toyota dealership and have not addressed the issue after several complaints. Replacing fuses does not address the issue. This is a safety issue for people dependent on gps and communication specially for long travels.## vin failed ##
Driving 70 mph car began to go into high rpm, then accelerated to 74 from 70 mph. I tried to decrease speed then vehicle shut off got a long beep and message on display "check hybrid system, pull over immediately" all lights on dash were indicating failures. Coasted to safety with 5 passengers in vehicle across traffic at 9:30 pm to shoulder. Tried to restart several times then vehicle went into high rpm then engine knocked loudly and was completely disabled. Lost brakes, pedal was hard, lost power steering, difficult to maneuver. I still had 2 bars indicating plenty of fuel in car. Prior fillup was 7.6 gallons at pump, usually get over 500 miles per tank, trip meter only showed under 400 miles since fill up so had plenty of fuel and there was no low fuel indicator. Reported all problems to dealer. They only noted the car shut down but told me they confirmed everything i said and that toyota engineers are trying to resolve this since they are aware of this type of problem. So far dealer and toyota have not addressed the high rpm, the acceleration, loss of electronics and hybrid system failure and told me they havenever heard of any toyota ever having a report of a high rpm or acceleration without intent to accelerate form the driver. Vehicle is unsafe, has 4944 miles and this problem occurred about a month after purchase. Toyota still does not know what the problems are and in possession of the vehicle.
Door locking mechanism, security issue, vehicle will not lock due to faulty lock actuator--- may effect other actuators. Vehicle stationary and on start up for motion.

When the headlights are on, the speedometer does not display when the dashboard lights are dimmed.all other displays dim,but the speed of the car is no longer displayed.

New prius does not work bluetooth wise with existing phone. Prius i just traded in did. No resolution from dealer (toyota world o newark delaware) and no resolution from toyota corporate. I was told they could do nothing and that i would have to purchase new phones. We are driving vehicle without bluetooth without cell phones in our ears thereby creating safety hazard.
While driving at night, all lights, especially red in color, which are viewed from front windshield, appear as three distinct images.the source light, then a distinct shadowed light above and below the source.this safety issue is extremely compounded when driving at night in high traffic situations where there are hundrends of tailights in front of me.all those lights are essentially trippled causing confusion and distraction.toyota refuses to aknowlege the issue.through research, i have found that this is a common issue with many frustrated owners just living with it because toyota wont fix it.it is a huge safety issue.please help make toyota rectify this huge safety issue before it causes preventable accidents!!

Bought car on 1/23/13 and drove home. Next day got a call they forgot to give us access key so drove 27 miles to dealership and smoke on dashboard. Pulled into service and sales manager got in car drove around and said all is fine. Came home an odometer showed 564 miles after i drove 27 there and 27 back. The car had 156 night we bought it the day before. We are old and disabled and do not drive often. Odometer now showing almost 900 miles in less then the 3 weeks we have owned it. Reported the smoke on dashboard to toyota corp office and told them how hot the dashboard got and odometer jumping. The man thought it was funny and said he hoped i did not burn my hand. Now nav system not working and radio sounds tinny. Called lakeland toyota and talked to owner [xxx] who says he is an accountant and knows nothing about cars. Took car to another dealership and was told nav system software 2.1 should be a 2.2 and claimed to have it updated. The car then added another 100 miles again. We need help to make sure car is safe to drive and need miles to stop adding up even when we are not driving.also the history on mpgone time showed 99.9 and then 26 and then 44.7 and then 49.we had to put gas in it three times in it in less then 3 weeksbecause gage on dashboard keeps showing getting low but then it only takes 6 gallons. I put no for fire but i did see smoke on the dash above the computer and it was very very hot. So not sure if there was an electrical fire or not or just a surge or short. After toyota power management software recall installed "power management upgrade" software in my car i immediately experienced serious problems with my ignition /start of hybrid/gasoline engine. After pressing ignition, i had to wait 15 to 20 minutes and press start button on/off repeatedly until the gasoline engine ignite but hybrid failed to start.it is not normal because a prius hybrid engine starts firstnot the gasoline engine. Toyota has disabled some important functions in its power managementrecall software update that affects the hybrid engine performance and failure to start. I drive my prius 3-4 times a month and i did not have any problem before the recall software upgrade because prius owners manual clearly says that the battery with be disconnected by battery saving function when your car is parked for long time, several days, weeks. In fact, when your car is parked for just days, toyota power management software (recall) is draining the car battery and reconfiguring the relays to power up the gasoline engine.toyota owners manual clearly states that when you areparked, "power management software disconnects the auxiliary battery from the hybrid system."however, after the recall, not only auxiliary battery is not disconnected but there are lots of "activity" in hybrid engine that drains the batteries. I called the toyota service and they now say you have to keep the car on a charger all the time when parked for several days. Of course , it is a huge safety problem to keep the car on a charger all the time whenparked for days because the battery is housed in thecabin and the ventilation does not exist unless you park your car outdoors. At the same time toyota recommends keeping the car on a charger , it also warns of explosion caused by charging the batteries in a garage.
I went to start my prius v.after it was started,a number of the warning lights came on including the abs and stability control.i was stunned since i had my 30000 mile check up in may with no problems.when i took it to the dealer for repair, the dealer determined that rodents had eaten through a number of the electrical systems because of the soy basedwiring.(i understand now that it's "biogradable soy based wiring insulation.)i generally park my car at only two locations - my home and my work.i had never seen any rodents of any type!i can't imagine what would have happened if one of the wires to the engine, or airbags, or any system for that matter separated while i was driving.apparently, toyota is aware of this problem but is trying to downplay it!i was told it's not a warranty item.my car requires two new wiring harnesses, the alarm wiring, and the airbag harness plus other repairs.
Horn stopped working in november 2018. Dealer tells me that there is an opening in the spiral cable which is causing horn not to work. Entire spiral cable needs to be replaced. This vehicle has had driver's airbag recalled once, maybe twice. Did spiral cable get damaged in one of these recalls? i know that there was a recall in earlier toyotas with same part, but not this year and model, wondering if there should be.

The contact stated that the cabin air filter did not have a mesh grill which was allowing the rodents to enter through a hole in the cabin air filter then into the glove compartment console and into the interior of the vehicle. The dealer was contacted and informed the contact that the wires were soy based causing the attraction of rodents to the hood of the vehicle however the vehicle was not taken for an inspection. An independent mechanic was also contacted who confirmed that the cabin air filter did not have a mesh grill which resulted in the rodents entering through a hole into the cabin air filter then entering into the glove compartment console accessing the interior of the vehicle. The vehicle was then taken to a mechanic where a hardware cloth grill was installed on the cabin air filter. The vehicle was repaired and rodents were no longer entering the vehicle. In addition, the contact was required to clean the feces remaining in the interior of the vehicle utilizing a mask, gloves, and disinfectant and in order to avoid contact with a virus from the feces, the contact was advised not to vacuum let the virus become airborne. The contact was concerned that the rat feces in the vehicle could result in a fatal disease in the event a child would ride in the vehicle and accidentally eat the feces. The contact was concerned that if rodents entered though the air conditioning unit and defecated inside, activating the air condition unit if there was feces, a virus could become airborne. The manufacturer was notified of the failure then informed the contact that the failure was not covered under warranty and that the failure was an outside influence which was not their responsibility. The approximate failure mileage was 40,000. The vin was not available.
On my way home i suddenly saw a sign on the screen "check the hybrid system. Stop in a safe place". I pulled over, smelled smoke and saw smoke in the rear. I opened the trunk ans frightened by flame. I pulled things out and covered the flame with two big towels and succeeded in putting it out, although i got burned on my fingers. I called geico. They sent a towing car and it took my car to my apartment. When geico took my car to a collision center, they said it is total loss after inspecting it. I claimed it to toyota. They inspected it, but said that there was no evidence that the car had electrical or mechanical defect. I did nothing but driving the car, but it caught fire. Whose responsibility is that?i believe the producer should take the responsibility but they do not. Where can i be compensated for the loss?
I have never received a recall mail regarding the car. I used an insurance tracking plugin, and all the problems started.pcs error message on my dashboard, due to a leak in brake booster they said. My dealership will not tell me it's a recall, made it seem like they helped me instead of charging as high as $4000. Vehicle stability control, anti-lock brake, and traction control lights intermittently turn off.a few times it shut down the hybrid system and tell me to stop .now all those lights are all on and i am driving without vss, abs and traction control.braking is getting harder.i took it to the dealer and they told me the cost is $3500.i did some research and found out that this is a known prius problem problem
I went to start my prius v.after it was started,a number of the warning lights came on including the abs and stability control.i was stunned since i had my 30000 mile check up in may with no problems.when i took it to the dealer for repair, the dealer determined that rodents had eaten through a number of the electrical systems because of the soy basedwiring.(i understand now that it's "biogradable soy based wiring insulation.)i generally park my car at only two locations - my home and my work.i had never seen any rodents of any type!i can't imagine what would have happened if one of the wires to the engine, or airbags, or any system for that matter separated while i was driving.apparently, toyota is aware of this problem but is trying to downplay it!i was told it's not a warranty item.my car requires two new wiring harnesses, the alarm wiring, and the airbag harness plus other repairs.
On july 28, 2017, i was driving along with my three children.i pulled into a driveway to park my vehicle behind a parked vehicle.there was approximately a 2 foot span between the back of the vehicle and the front of my 2012 toyota prius.to the right of the parked vehicle was a parked truck.being that i was pulling into a space to park my vehicle, i was traveling extremely slowly.i began to press on my brake to park the vehicle and instantly knew there was a problem.upon depressing the brake, several lights began to flash within the dash to include but not limited to the abs, warning light, and the light with vehicle swerve lines.other lights were also flashing, but i do not recall each one.the brake pad instantly went straight to the floor with no pressure.the vehicle would not stop and continued to travel forward with acceleration.to avoid hitting the vehicle in front of me i began to turn my wheel to the right.the vehicle continued to accelerate and came into contact with the truck that was to my front-right.my prius failed to slow or stop.it continued to accelerate and began pushing the truck up the driveway and into the metal garage door with the brake pedal being depressed the entire time.eventually, the car came to a stop and the lights continued to flash prior, during, and after the incident.after the car was towed tread wheel marks were found on the driveway where the wheels spun and left an imprint in the asphalt.i and my children were uninjured however, the incident was incredibly traumatic.the vehicle was deemed a total loss by my insurance company and as of yet to be processed for diagnostic testing by toyota.the toyota care service was informed of the accident.toyota believes it to be associated with the intelligence component of the vehicle, however determination is currently unknown until diagnostic is performed.
Problem with braking system involving, the other day the car was on and suddenly three (3) warning lights came on: brake light, abs light and traction control light.there is also now a pumping sound coming from under the interior of the dash (on it at least sounds that is where it's coming from.the car has 151,428 miles on it.i've included pictures of the current eror code, mileage and dash lights that are on.

My prius was just fine then one day it stalled while i was driving in the middle of the free way.i took my vehicle in to elmore toyota in westminster, ca, they found it had a failed inverter and it was still covered under warranty.it took 2 weeks for them to fix it, but when they returned my car, the slip indicator was now on and would not turn off.they claim it was like that prior to the failed inverter, this was false, my car was running perfect.they then wanted to charge me nearly $1k to fix a problem i did not have prior to failed inverter.i feel the situation was connected but they refuse to cover it.

There has been a safety recall on my vehicle for the years 2004-2009 models of the toyota prius, but my model year is 2012 and i have the exact same issue.1) my check engine light comes on and off and it appears to be the hybrid electric water pump issue related to the coolant.the cap pops off and spills out.2) when the light comes on, i am assuming the car is starting to overheat and is putting myself and my 3 year old daughter at risk as this is the primary car we use to transport her.we have just stop taking her out at all due to this issue.when the car overheats, i pull over and it can be risky as sometimes, i am on the highway or local streets.i do not want this issue to cause problems for other other drivers as well while i am trying to pull over where ever i am.3)the problem as i stated above has been acknowledge by you and toyota but just for 2004-2009 models.4) the problem has not been inspected by toyota, the police, or third party.i have been the only person checking the car and i am using a diagnostic check engine light tool to read the code and looking under the hood to find the coolant pump leaking and popped out just as indicated in the recall for other models.5) yes, the check engine light and high temperature light goes on and off.the issue first appeared around november of 2020.
2012 toyota prius ii, dealer maintained, now needs new engine due to blown head gasket.mileage 151000.the car has run perfectly until a recent engine light came on.when it was brought into the dealership, the diagnosis was misfiring of 2 cylinders and coolant leaking into cylinders 3 and 4.there was no previous engine problems.an official complaint was filed with toyota customer service.they responded that there was no fault on their end and i should have the engine replaced at an authorized toyota dealer.a search for this trouble resulted in several other complaints of the failure of head gaskets in this year and model prius.
Problem with head gasket, low miles, cold engine starting, stationer, and some times in motion
When idling there is a loud rattle and multiple random misfires in the engine. Appears to be a head gasket leak problem from reading forums. I have been to multiple repair shops they cannot determine the problem.
Driving 70 mph car began to go into high rpm, then accelerated to 74 from 70 mph. I tried to decrease speed then vehicle shut off got a long beep and message on display "check hybrid system, pull over immediately" all lights on dash were indicating failures. Coasted to safety with 5 passengers in vehicle across traffic at 9:30 pm to shoulder. Tried to restart several times then vehicle went into high rpm then engine knocked loudly and was completely disabled. Lost brakes, pedal was hard, lost power steering, difficult to maneuver. I still had 2 bars indicating plenty of fuel in car. Prior fillup was 7.6 gallons at pump, usually get over 500 miles per tank, trip meter only showed under 400 miles since fill up so had plenty of fuel and there was no low fuel indicator. Reported all problems to dealer. They only noted the car shut down but told me they confirmed everything i said and that toyota engineers are trying to resolve this since they are aware of this type of problem. So far dealer and toyota have not addressed the high rpm, the acceleration, loss of electronics and hybrid system failure and told me they havenever heard of any toyota ever having a report of a high rpm or acceleration without intent to accelerate form the driver. Vehicle is unsafe, has 4944 miles and this problem occurred about a month after purchase. Toyota still does not know what the problems are and in possession of the vehicle.

Starting in spring 2013 and continuing during the rest of the spring, summer and early fall in the hottest months, after parking my prius c outside, it always had issues starting, which continues to this day. I will put the key in the ignition, start the car, all the dash lights come on and the car will usually not allow me to put it into reverse or drive. I have to take the key out of the ignition and start it again at least twice before it starts normally as it does when the weather is cooler and/or when the car is parked in my garage. A couple of times, i managed to get the car to go into reverse, but it was almost impossible to steer as if the parking brake was on. Then the vehicle stopped, leaving me in the path of oncoming traffic. This has been happening since i bought the car and the dealership says they can't fix it unless they can recreate the problem. I have taken pictures and viseo, but that makes no difference to them. This is a dangerous problem and needs to be fixed. This is the second complaint i have filed re: this matter as my first does not seem to appear here.
I went to the charles maund toyota dealer at austin, tx for a recall. Nhtsa recall id 18v684. According to the official recall remedy from nhtsa, "if the vehicle has experienced an inverter failure with certain hybrid system faults related to this condition, the inverter assembly will be repaired or replaced free of charge."my car lost power on the highways several times recently. The error code is p0a08, which is related to inverter failure with certain hybrid system faults, so the inverter assembly should be repaired or replaced free of charge. But the dealer didn't want to replace or repair the inverter for my car. There is still a potential threat on my car to lose power on highways.
After toyota power management software recall installed "power management upgrade" software in my car i immediately experienced serious problems with my ignition /start of hybrid/gasoline engine. After pressing ignition, i had to wait 15 to 20 minutes and press start button on/off repeatedly until the gasoline engine ignite but hybrid failed to start.it is not normal because a prius hybrid engine starts firstnot the gasoline engine. Toyota has disabled some important functions in its power managementrecall software update that affects the hybrid engine performance and failure to start. I drive my prius 3-4 times a month and i did not have any problem before the recall software upgrade because prius owners manual clearly says that the battery with be disconnected by battery saving function when your car is parked for long time, several days, weeks. In fact, when your car is parked for just days, toyota power management software (recall) is draining the car battery and reconfiguring the relays to power up the gasoline engine.toyota owners manual clearly states that when you areparked, "power management software disconnects the auxiliary battery from the hybrid system."however, after the recall, not only auxiliary battery is not disconnected but there are lots of "activity" in hybrid engine that drains the batteries. I called the toyota service and they now say you have to keep the car on a charger all the time when parked for several days. Of course , it is a huge safety problem to keep the car on a charger all the time whenparked for days because the battery is housed in thecabin and the ventilation does not exist unless you park your car outdoors. At the same time toyota recommends keeping the car on a charger , it also warns of explosion caused by charging the batteries in a garage.
Moldy odor emits from vents.had round rock toyota dealership attempt to correct the issue four times and also had called up toyota corp but issue was never resolved.on the fourth attempt at the dealership, toyota had authorized a carbon-type cabin filter that was suppose to be able to remove the odor but it did not help.did not fix or replace the root cause of the issue.food was never allowed in this vehicle; air vent was selector for fresh (not recirculate).this is my third but last toyota.it breaks my heart to have purchased a brand new prius and by 3000 miles, strong moldy odor exists.dealer had sprayed lysol, provided ac refresh kit but evaporator or ventilation system is faulty.toyota bulletin # t-sv-0142-13 (sept 12).
Driving the interstate at 65 mph the engine stopped- with no warning of any kind,the air, dash lights, radio, etc all continued to work.we coasted to the side of the road & restarted the engine.we purchasednew & had all the required maintenance done at the toyota dealership.we learned of a recall on the news in february & took the car in for the repair.(a software problem related to the car engine stopping while driving)when we learned of the recall we rememberedthis had happened to us about a month earlier, again, onthe interstate at 65 mph & thought it was just a fluke at the time.we were very upset when this latest incident happened after having the recall repair done on the car & having driven a few thousand miles between incidents.we drove the car directly to our dealer's service dept and reported these facts to the service manager.they immediately took it in and tried to detect any evidence of the incident.they told us that they also contacted toyota corporate technical services for assistance.they were not able to find any evidence of the incident.as we are now afraid to drive the car, they gave us a car to drive until the problem could be resolved. We don't want to see this car sold to an unsuspecting driver.we feel this is a life threatening defect andtoyota should repurchase the car from us at a fair value so that we could replace it with another car.we contacted the toyota customer experience center yesterday by phone and filed a formal complaint, they opened a file and gave us a case number.today we were contacted by phone by a toyota regional analyst and were told that since they cannot find any evidence of the incident by using their computer system we would need to file a lemon law complaint.we told him that we expect toyota to resolve our issue - we should not have to prove the defect.help.
This vehicle has been burning oil for the past 10,000 miles. Vehicle has had all services and synthetic oil changes done from day 1 at the dealership. Oil changes every 5,000 miles since day 1. At 90,000 miles the car started burning oil excessively. Complaints and diagnostics were attempted at toyota franchise dealerships but they failed to find the issue. Finally at 100,000 miles the engine failed due to head gasket failure. This vehicle is classified as a partial zero emission vehicle and it is registered in a state that follows california air resource board regulations and is being serviced in a state that follows as well. Under the pzev regulations, toyota warranties emission related components for 15 years or 150,000 miles. Under toyotas warranty guide it states seals and gaskets are covered. The manual does not clarify any further leaving the coverage level vague and questionable. Upon trying to get toyota to replace the engine, they gave us a week of no responses and finally declining the claim even when the manual states it should be covered. The oil burning issue with this motor is a big problem that toyota has been trying to cover up.toyota is quoting $13,000 for engine replacement.they have a service bulleting regarding the oil burning because it's a known issue, but they only honor claims under the 5 year 60,000 miles power train warranty.

My prius is using 1 quart of oil per 900 miles, the toyota dealership did a oil study and verified that indeed the car is using 1 quart per 900 miles, but will not do anything about it and stated my car is no longer under warranty and could be rebuilt for 9000 dollars-this is a commuter car only and has only been used for open road driving.

I own a 2012 prius with the technology package, including radar adaptive cruise control. I believe these same radar sensors are also used to detect imminent collisions in front of the vehicle, even when cruise control mode is not engaged. About a month ago, while driving north on interstate 5 in downtown seattle at approximately 60 miles per hour, and without cruise control engaged, my prius went into "imminent collision mode" despite the fact that there was nothing in front of me -- no vehicles, no debris, no obstructions. Before slamming on the brakes, my seat belt was "tightened" and the brakes were automatically applied at full force, even without my foot touching the brake. This was incredibly jarring and disturbing, and could have been a serious accident if there was a vehicle behind me. Fortunately, there was no vehicle behind me at this time. After about two seconds of full braking, the vehicle "disengaged" collision mode and allowed me proceed and return to speed. I called the toyota of seattle dealership and spoke to their service department. They were unaware of this issue, and suggested that i bring the vehicle in for servicing. They confirmed there were no outstanding recalls on the vehicle. They told me it would cost at least $100 for a basic diagnostic, and if sensors had to be replaced, it could cost hundreds or even thousands of dollars. Because i am currently unemployed and looking for work, i did not bring my vehicle into the service center. I feel this is toyota's obligation to fix, and shouldn't require me spending money out of pocket. I would like some kind of response from toyota on this issue. I have avoided using the radar cruise control since this moment, but the fact that it occurred while not in this mode makes me feel that the car could "take control" at any moment and cause an accident. My headlights have burned out so many times i've lost count. Spent close to $600.00 on bulbs. Why is toyota not being held responsible for this issue? this has happened while just starting the car from parked position to driving down the highway.
The low beams have been intermittent and then go out all together. This has happened twice in the past four months each time bulbs last about two months before they blow. It has happened while driving as well as after going somewhere and then getting back in car and trying to return home no lights.
I have had to replace headlight bulbs multiple times in the past two years. Reading reports, it is the exact same issue as the 2010 and 2011 prius owners had. I am now having to have my headlight covers resurfaced because my passenger side headlight is not at all visible at night. The dealership is recommending replacing the compartment where the bulb goes.
Passenger side low beam headlight will not work. I replaced the bulb, and checked the fuses, and it still will not light. This has been an ongoing problem with this car.
While driving at night, all lights, especially red in color, which are viewed from front windshield, appear as three distinct images.the source light, then a distinct shadowed light above and below the source.this safety issue is extremely compounded when driving at night in high traffic situations where there are hundrends of tailights in front of me.all those lights are essentially trippled causing confusion and distraction.toyota refuses to aknowlege the issue.through research, i have found that this is a common issue with many frustrated owners just living with it because toyota wont fix it.it is a huge safety issue.please help make toyota rectify this huge safety issue before it causes preventable accidents!!
Low beam headlights keep going out. Passenger side has been replaced many times. Moisture is visible in light assembly.
I have to replace both low beam head lamps approximately every three weeks. I have bought and replaced 5 sets since owning the car in june 2015. I currently have 40,000 miles on the car.

There is a tripling of light sources through the front windshield from things such as oncoming vehicles headlights, vehicle taillights, emergency vehicle lights, street lights, intersection stop/yield lights and any other strong light source.the issue is apparent at all times however the issue is significantly acute in the evening hours as there are more light sources during the dark.
When driving at night i have an issue where the light source from headlights have a rainbow affect. Also tail lights are seen three times for every one light. Ie. Center brake light is illuminated on vehicle in front of me, there is a replicated light above and below the actual light. This is also an issue with traffic lights as there are 3 distinct lights for every one light. This may also be referred to ghosting.
Headlights keep blowing out. Since purchasing the car new, have had to replace headlights five time. One headlight is out as we speak.
My 2012 prius was hit in back on passenger side at approximately 80-85 miles/hour while my speed was 62 m/h. With impact from hit in the back, my car went flying head on into the median wall (those temporary cement walls used during reconstruction). Two things happened were:1. After the first impact my prius had total power loss, not even emergency flasher light would turn on;2. In the past year we have replaced multiple low beam halogen headlights.one week ago, both burned out causing a nighttime safety issue.i researched the lighting issue and learned there is tsb t-sb-0204-12 for headlamp durability enhancement and a customer support program "zhb" for this identical problem.in the zhb program, toyota will make the necessary repairs free for only 2010-2011 prius'. My 2012 prius had both headlights go out while driving on an interstate. Luckily no damage was done. My dealer ran my vin to see if toyota had a warranty extension as it is a well documented voltage problem for the car. I was told that the 2010 and 2011 prius do have such coverage but the 2012 does not. Seems very shortsighted on toyota's part. It clearly foreseeable that someone could be harmed or killed and that this issue should be addressed.

Both driver and passenger side low beam headlights fail frequently (every 3-6 months) making driving in the early mornings and nighttime very dangerous, as they fail unexpectedly. The most dangerous situation for us with the headlight failure is when we were driving at night in the mountains and both low beams failed at the same time. Thankfully the high beams were functioning. The fact that there is a recall for 2010 and 2011 models but the 2012 model prius is excluded us very concerning considering the issue is the same across years.

I am now changing the bulbs every couple of months.toyota officially states in their'customer support program zhb certain 2010- 2012 model year prius coverage for halogen low beam headlamp bulbs' that this problem is covered under their warranty: if the owner experiences the condition described above, he/she should contact a local authorized toyota dealership for diagnosis. If the condition is verified, the dealer will install voltage adjustment wire harnesses and replace the halogen low beam headlamp bulbs with new ones at no charge to the customer.' when i took my car into a toyota auto nation in hayward for service on saturday 3/9/19.i was told this warranty expired on february 2019, and if i wanted to fix this problem it would cost between $500- $600. I explained that i had a document from toyota headquarters that has a different date of october 31, 2019. The service person said, he would check the date and email me back. He emailed me back on with this reply: 'unfortunately as per toyota your vin doesn't not support that tsb and because your vin does not apply to the tsbourparts departmentcannot order parts to support the remedy for your vin as they are unavailable.'i feel that toyota does not want to repair this known safety hazard, unless i pay out of pocket for the parts and labor. I have noticed ghosting of images most obvious while night driving. Particularly noticeable in led lights. It looks like a duplicate image of the lights created. It can be very glaring and hard to judge distance of other vehicles.

Low beam headlamps continually fail, and are very dim even after installing new bulbs. I've had to replace bulbs no less that 4 times since 2012! not only is this an expense, butmore importantly, poses a significant hazard. There is no warning or specific mileage. I've never had to replace headlight bulbs in any other car i've owned. Something must be wrong with theelectrical system for this to continually be happening.

I suspect that some lithium cells have been damaged after the incident.i also suspect that there may be other issues as well such as something wrong with the inverter module related to a recall on other models of prius during my search for an answer.i need assurance from someone that this vehicle is now safe to drive.besides the case number from my complaint to toyota usa,aaa has record of my recent towings on 04/09/16 and on 04/12/16.

My husband was driving the car preparing to turn to enter the freeway when the prius lost all driving power spontaneously at the same time prompted with a warning on the screen 'check hybrid system and park in a safe place'.the car lost all driving power including power steering but my husband was able to glide the vehicle across several lanes to a parked position before the freeway entrance. The car never resumed its ability to drive despite turning it off and an attempt to restart. Vehicle was in motion traveling about 50 mph when an effort was made to pass another vehicle on a 2 lane road, the vehicle lost power while we were passing, we barely got back into our lane and just barely avoided a head on collision. Then the vehicle slowed to about 15 mph and limped to a stop a mile later. We could have died.
I pulled up to a car wash and once i was on the track i tried to put the vehicle into neutral and it would not work. The automatic car wash track started to try to pull the vehicle forward. As im frantically trying to make it go into neutral, the vehicle started to hop off the track and was getting violently thrashed around as it was getting pulled forward causing the person running the wash to have to manually hit a panic button to stop it. Fortunately there was no damage to either the vehicle or the car wash but it is very disturbing that it would not work. It took several employees to be able to figure out how it worked. After starting the car, it would not go into gear.the dashboard was lit but i could not get the car moving. After fiddling around for sometime, it finally clicked and i was able to go into reverse. The car functioned fine until later that day i made a drop-off stop and the same thing happened. The dash board was lit but i could not get the car moving.i was stalled almost in the middle of the road. The next day i took it to the local dealership and they did the most extensive computer generated diagnosis on the car but could find no indicators of the problem. I reported this to toyota headquarters who said they'd investigate but there is no internal system for reporting back to me on their findings. I did research on the consumer reports website and saw others have reported similar problems. My fear is getting 'stuck' in unwanted times and undesired locations. The fact that the company doesn't recognize the problem is troubling. I plan to get a new car rather than worry about this one.

I was getting ready to pull out of a parking space in a parking lot. The car was in park, so i pressed on the brake, started the car and put it in reverse. All of a sudden there was a loud roar and the car shot full speed in reverse, completely out of control. It clipped the bumper of a parked car and some shopping carts, then backed into a wall which stopped the car. Miraculously no one was injured. The car has been totaled.

After toyota power management software recall installed "power management upgrade" software in my car i immediately experienced serious problems with my ignition /start of hybrid/gasoline engine. After pressing ignition, i had to wait 15 to 20 minutes and press start button on/off repeatedly until the gasoline engine ignite but hybrid failed to start.it is not normal because a prius hybrid engine starts firstnot the gasoline engine. Toyota has disabled some important functions in its power managementrecall software update that affects the hybrid engine performance and failure to start. I drive my prius 3-4 times a month and i did not have any problem before the recall software upgrade because prius owners manual clearly says that the battery with be disconnected by battery saving function when your car is parked for long time, several days, weeks. In fact, when your car is parked for just days, toyota power management software (recall) is draining the car battery and reconfiguring the relays to power up the gasoline engine.toyota owners manual clearly states that when you areparked, "power management software disconnects the auxiliary battery from the hybrid system."however, after the recall, not only auxiliary battery is not disconnected but there are lots of "activity" in hybrid engine that drains the batteries. I called the toyota service and they now say you have to keep the car on a charger all the time when parked for several days. Of course , it is a huge safety problem to keep the car on a charger all the time whenparked for days because the battery is housed in thecabin and the ventilation does not exist unless you park your car outdoors. At the same time toyota recommends keeping the car on a charger , it also warns of explosion caused by charging the batteries in a garage.
I went into toyota of fort walton beach, fl to remedy a recall (nhtsa recall number 18v684). She said that this recall is not on their list and thus would do nothing. I want this recall repaired as presently there is a safety risk. There are phrases in this recall of "1 unrepaired recall" and "recall incomplete".

I received the warning,please check hybrid system. After checking the dtc the following code appeared on the computer: p0a08:dc/dc converter circuit status which indicates an inverterproblem. Looking at the recall website using vin number, showed no recall concerning this. The problem happened many times while driving on a city or a highway
When i drive over any type of bump, (pothole, speed bump, etc) and my foot is on the brake....the car will suddenly speed-up instead of slow down. Very dangerous situation and it happens every time.

I own a 2012 prius with the technology package, including radar adaptive cruise control. I believe these same radar sensors are also used to detect imminent collisions in front of the vehicle, even when cruise control mode is not engaged. About a month ago, while driving north on interstate 5 in downtown seattle at approximately 60 miles per hour, and without cruise control engaged, my prius went into "imminent collision mode" despite the fact that there was nothing in front of me -- no vehicles, no debris, no obstructions. Before slamming on the brakes, my seat belt was "tightened" and the brakes were automatically applied at full force, even without my foot touching the brake. This was incredibly jarring and disturbing, and could have been a serious accident if there was a vehicle behind me. Fortunately, there was no vehicle behind me at this time. After about two seconds of full braking, the vehicle "disengaged" collision mode and allowed me proceed and return to speed. I called the toyota of seattle dealership and spoke to their service department. They were unaware of this issue, and suggested that i bring the vehicle in for servicing. They confirmed there were no outstanding recalls on the vehicle. They told me it would cost at least $100 for a basic diagnostic, and if sensors had to be replaced, it could cost hundreds or even thousands of dollars. Because i am currently unemployed and looking for work, i did not bring my vehicle into the service center. I feel this is toyota's obligation to fix, and shouldn't require me spending money out of pocket. I would like some kind of response from toyota on this issue. I have avoided using the radar cruise control since this moment, but the fact that it occurred while not in this mode makes me feel that the car could "take control" at any moment and cause an accident. Vehicle is manufactured without any regard for preventing mice from entering the passenger interior--- air intake vent under windshield and 2 rear decompression vents located in the rear cargo compartment above the wheel wells are open to the exterior allowing unimpeded access to the interior of the car. Had mice in this car 2 weeks after purchase in 2012 and have been dealing with this problem for 7 years. Just recently, had toyota install 1/4" hardware cloth over these 3 areas and this should help significantly to prevent entry. All preventative measures to prevent mice entry have been diligently taken by me, without success. Mice can make a person very sick. Their excrement carries hantavirus..... And the mice carry ticks, which can lead to lyme disease. Simply manufacturing the car with these vents properly screened would greatly reduce the chance of occupants contracting a serious disease.the mice also do a lot of damage to the car itself, in my car the driver seat belt was chewed, and i have had to replace the cabin air filter several times because of nesting in this compartment(after having been unknowingly breathing the mice excrement as the air passes through this filter on its' way to the interior. Toyota needs to be pressured by your agency to correct this very serious health hazard that their car is presenting. Their is no legitimate reason for this situation to exist. The cost to toyota would be negligible.the benefit to all prius owners would be very significant, likely preventing many cases of disease. Please take this situation very seriously, as is the case. I was involved in a front end collision with a bobcat front end loader. I struck the implement broadside going 35 mph.there was no damage to the implement. My car was totaled and the airbag deployed. After going to the hospital and a chest x-ray, ultrasound, and ct scan, it was diagnosed i had fractured my sternum. I am six feet tall and had my seat all the way back. I am questioning whether there have been any other people sustaining a fractured sternum due to an air bag deployment at 35 mph?
While driving to a get together the back seat middle passenger restraint became separated from the locking clip that holds seat belt over passengers lap. Making it dangerous to drive with my childrenin the back seat. It will not clip together anymore. I just bought this vehicle 3 months ago and it had no problems at that time.
The driver seatbelt buckle will not unbuckle
My wife was involved in a two car crash when a car coming in the opposite direction made an illegal left turn directly in front of her. It happened so quick that she never had time to hit her brakes. She was traveling between 30 and 35 mph in a 35 mph zone on a two lane road. I would think that at that speed if all her safety equipment worked she would not suffer any injuries. Her seat belt failed causing the seat latch to fail and she ended up jammed into the bottom and under side of the dashboard. She suffered a blow to her chest and injuries to her knees, lower legs and foot. She had surgery on her right knee so we could get her on her feet again and is awaiting surgery on her right shoulder. She had a pre existing heart condition that was raised to emergency status after the blow to her chest and she had surgery for that and they found a short circuit in the middle of her heart and zapped it. There is only a 2% chance it will ever return. I have pictures of the failed seat belt and seat. The seat belt is in the fully extended position and locked and can not be released even by yanking on it. I am writing this report so this situation can be checked and if found faulty can be corrected before any one else goes through what my wife is going through.
The latch system design on the seats on the new prius v is flawed. Instead of having four latch points, one set of two for the left seat and one set of two for the right seat, they have only three latch points, one in the middle of the left seat, one in the middle of the right seat and one in the middle of car. Our child safety seat has three buckles designed to work with the latch system, one on the left, one on the right and a top tether. The left and right buckles cannot attach to the same latch point because then the child safety seat will pivot. This means one must install the child safety seat at an offset to the car's seat, using the middle and right latch points for a child safety seat installed on the right side. But then the top tether, which must go over the top of the seat down to a latch point, is no longer as secure as it should be, since the latch point is then off-center with regard to the top tether. If the child is old enough that they need to use the car's seat belts with the child safety seat (e.g., a booster seat) instead of the restraints built-into the child safety seat, one can't buckle in the seat belt because the buckle intended for the right side of the car is buried underneath the offset child safety seat. The middle buckle is designed to not work with the side seat belts. If one uses the left side buckle with the right side seat belt, it's not as secure, and then the left side passenger (or a second child safety seat) has no seat belt.so a proper use of the seat belts requires the child safety seat to be centered on the seat, but then either the latches are connected at an offset or they are connected to the same latch and pivot. Either way the child safety seat is not really secure.
I experienced a front end collision in my new 2012 prius with 2,500 mileage.the collision vehicle speed was nearly 30 mph.the seat belt did not restrain me and the airbag did not deploy.i was taken to the hospital and sustained a fractured sternum due to hitting the steering wheel and whiplash.there was significant damage to the front end of the vehicle. Toyota's response after their own inspection was "our inspection found no evidence of a manufacturing or design defect with the airbag or seat belts."

The vehicle received severe front-end damage to the extent of almost $12,000. When the vehicle was stuck by an on-coming vehicle, her seat belt did not prevent her from hitting her chest on the steering wheel nor did the air bag deploy in time.she received severe bruising, severe pain and discomfort, and stress. Also, the lower dash knee air bag failed to deploy in time to prevent severe bruising to his wife's knees. Toyota sent the consumer a letter stating there was no evidence of any manufacturing defect with the air bags or seat belt.
I was driving down the interstate with my factory installed seat warmer on the driver's seat on. I got out at a gas station to go to the bathroom.when i sat down on the toilet i felt an excruciating pain on my left upper back side of the thigh.the car seat gave me second degree burns on the back of my upper left thigh. I went to the er the next morning when more blisters appeared so that the open and closed blisters could be cleaned and dressed.i was given a tetanus shot as well.i contacted toyota the day after the incident after being seen by the er. I spoke with a claims adviser on dec. 1, 2014 who asked me to email pictures of the driver's seat and pictures of my burns.i was then contacted dec. 9, 2014 by toyota to be told a toyota inspector would inspect the vehicle on dec. 18, 2014. The vehicle was inspected and i have not been given any information to date (12/31/14) nor has my vehicle been fixed. I am awaiting toyota to contact me on the outcome of the inspection. The dealership told me i cold not have the seat warmer fixed until toyota went through their process of documenting the incident due to me being injured.

The drivers seat does not raise far enough to allow shorter drivers to see over the dash board , base of windshield and rear of hood. This prevents shorter drivers from being able to see the the ground anywhere near the car.giving the drivers a 2 dimensional view of the traffic and objects between 3 and 5 feet high. Anything below 3 foot tall is not visible for 30 to 50 feet in front of the car.the raised belt line is also causing this problem of 2 dimensional views verses 3 dimensional views.this is a serious safety issue.. It could be fixed with increased travel height for the drivers seat.as i have had to offer several friend chair cushions to prop them up so they could see..when i looked in the car. I don't think do to the windshield angle.. That i being almost 6 foot tall could see the ground anywhere near the front of the car either.i don't fit in these cars.

I am 5'3".the headrest of my car is designed to angle forward at the top.this pushed my head down at a 15* angle.it is painful to drive.i will have toremove the headrest completely, losing any protection in any accident and losing the support it shoud give my head now.i understand this design is mandated by your agency and that all people in the 5'0" to 5'4" height range experience this problem.since the average height for american women is 5'5" your regulation discomforts and/or endangers half of the women in the country.change the regulation.allow, or require, headrests that can swivel to change the angleof alignment.
Head restraint (head rest) on driver seat does not stay in place.after i adjust it, it rises very slowly.if i do not continually readjust it (3 or 4 times a day), it rises above my head.i consider this a safety concern.this has been happening since i bought the car (new) and i have asked toyota several times to repair this defect, and they say that it is 'normal'.i have owned many cars during my lifetime, and this has never happened before in any of them.it does not happen to the passenger head restraint.i do not think it is 'normal', and if it is 'normal' for toyota, then all toyota cars have a safety problem.i have contacted toyota and they just refer the problem back to my local dealer, who continues to give me the same answer.
I went to start my prius v.after it was started,a number of the warning lights came on including the abs and stability control.i was stunned since i had my 30000 mile check up in may with no problems.when i took it to the dealer for repair, the dealer determined that rodents had eaten through a number of the electrical systems because of the soy basedwiring.(i understand now that it's "biogradable soy based wiring insulation.)i generally park my car at only two locations - my home and my work.i had never seen any rodents of any type!i can't imagine what would have happened if one of the wires to the engine, or airbags, or any system for that matter separated while i was driving.apparently, toyota is aware of this problem but is trying to downplay it!i was told it's not a warranty item.my car requires two new wiring harnesses, the alarm wiring, and the airbag harness plus other repairs.
I was driving on i-45 at 55 or 60 mph and the brake pedal went to the floor. It is still that way, brake fluid level is ok. Toyotacare plan is expired. Is there a recall for these vehicles? thanks
When braking during normal driving conditions, a mixture of city and highway, the car makes a sound like the abs is kicking in. When coming to a stop, the noise persists until releasing the brake pedal. Shortly after this started, the abs, traction control, and brake warning lights came on. I took the vehicle to my local toyota dealership where i was told it was the brake actuator needed replace at the cost of $3600. My 2012 prius only has 76k miles on it.

My toyota prius 2012 with 95k mil.abs, traction and brake lights came on while driving on highway, the breaking wasn't like used to be, it was mushy after the warning lights, took it to toyota dealership they estimated $3500 for the brake replacement the fault codes c1241, c1247, c1391 found break booster and pump assy, needed. This should be recalled by toyota, its safety issue, i have owned the car since it had 41k miles and regularly maintained the car throughout my ownership period.
I received the warning,please check hybrid system. After checking the dtc the following code appeared on the computer: p0a08:dc/dc converter circuit status which indicates an inverterproblem. Looking at the recall website using vin number, showed no recall concerning this. The problem happened many times while driving on a city or a highway
I started my car and my dash lights for brakes, anti-lock, anti-skid came on and brakes became spongy. This happens intermittently. I parked the car and later checked the lights and the car was operating normal. I drove the car around the neighborhood to try to duplicate the issue. Could not duplicate. The next day i started the car the lights came on again and brakes were not operating normally. Spongy and no anti-lock. I checked recalls and found the issue was recalled in 2010. I also found numerous complaints on prius forums for this issue covering 2012 prius. I think this is a safety issue toyota should recall for 2012 as well.

Vehicle is parked outside, and during morning cold start, the vehicle produces a moaning/wooing sound for approx. 50-75 yards with vehicle moving under 25 mph.the sound is being generated from the front wheel wells.sound generated is related to vehicle speed.the faster the vehicle moves, the more frequent the sound being produced during the first 50-75 yards. Strange sound is gone after that, and does not return until the next cool/cold start.owner took the vehicle back to dealership for inspection, and dealer took off the front wheel to inspect brakes, wheels, and tires, and was not able to locate or reproduce the problem.abnormal sound is only produced during morning cold starts and when vehicle is left to cool(i.e. Parking lot at work).

The brakes felt very spongy and the car was having a difficult time coming to a complete stop. The abs, brake, and stability control lights turned on at the same time. Also, a whirring noise close to the passenger side started happening every 2-3 seconds. After some research i later learned that the noise is most probably coming from a brake pump assembly that has failed and is not able to keep pressure. The car is currently parked since i donā€™t feel safe to drive it. I contacted a local dealer to see if this part can be changed for free since this is not normal wear and is unsafe. While driving the vehicle on a dry surface street, the "check hybrid system" light illuminated.the brake pedal felt like i had to apply more pressure to stop and it seemed to travel further toward the floorboard than normal.
As i was approaching a turn i began to brake and the brakes failed. I tried pressing the brake pedal harder and making sure that i had proper foot placement and it felt like the car accelerated at that point. Since the car was not slowing down i was unable to make the turn and ended up crossing the road diagonally and going up onto the curb. Repeating several-second groaning or grinding noise heard from rear wheel area upon startup motion. The noise is severe and sounds like low-pitch metal grinding. It's potentially brake related but could be suspension bushings or other components rubbing against one another. It does not sound like typical brake glazing. The noise occurs in all weather conditions and typically after vehicle has been sitting for several hours or overnight. Noise continues for several minutes at stop and start. Noise is loud enough to startle an experienced driver. The absolute severity and indication of the sound is enough to question safe ongoing functionality of braking and/or suspension systems. The dealer state the abs brake system need to be replaced at a cost of $4,504.63 plus taxes. I explained to the dealer the car is making a sound like the abs is kicking in when it come to a stop. I red toyota extended the warranty for this issue due the brake actuator failure due manufactory issues. The dealer state my vin is not there. I have owned a toyota prius 2012 since february 2012 and have had multiple issues with the braking system. I have noticed that if driving over adepression in the road (e.g. Pot hole or man hole cover) while depressing the brake the vehicle accelerates. It seems that the vehicles brake system releases and you have to reapply the brakes. I have had the vehicle inspected by the dealer 2 times and they have been unable to identify the problem. I have notice various other complains on this website and wanted to add my experience. Hopefully this will eventually lead to the government forcing toyota to recall 2012 prius models before someone is seriously hurt.
I was exiting the park and my toyota prius' brakes failed, careening me onto a major thoroughfare (south orange avenue) on a hill, while my car began accelerating. I hit the park button and my car slowed until i was able to get control again and turn up a hill to a stop. When i turned my car off and on again, the brakes were functioning normally.
Car was in motion and brake pedal went half way to floor and abs, brake, traction control and tire sensor lights came on. Master cylinder or brake booster seem to have failed. Dealer didn't recognize codes and could only recommend changing spark plugs when i just had them replace 6-8 months prior.explained that did think that had anything to do with brakes giving out.they reset codes and sure enough happened again within the next 30 days.toyota admitted to since receiving many reports of certain internal malfunctions of brake booster assemblies.4 trips later to dealer the response is the codes are not coming up (dtcs) c1391, c1252,c1256 orc1253 in order to be covered it's voluntary customer support program which has brake booster and brake booster pump coverage.very frustrating!
I was traveling north on the highway. I was going between 50 and 55 mph when the traffic light turned to yellow and then to red. When i applied the breaks nothing happened. My vehicle actually appeared to speed up. I released the brake then pumped the break twice then again applied the break, this time stomping harder and with both feet. My vehicle did not stop or even slow. I went through the light, resulting in a car accident. I t boned another car.

More than once i was driving about 40/45mph, once on a curve, anyhow as i was going through the curve it felt as though the brakes engaged, without me touching them. I understand the safety thing, however at both instances it was not necessary. It caused the car to pull to the left. Almost causing me to loose control.
Abs warning light indicating trouble with the braking system, i took the car to an independent auto repair store and the mechanic checked the codes on his computer and received 2 codesthat indicating that the abs pump needs to be replaced. Lost front brakes completely driving down road. I was able to slow car with manual brake on shifter in conjunction with rear brakes. A serious issue.
On july 28, 2017, i was driving along with my three children.i pulled into a driveway to park my vehicle behind a parked vehicle.there was approximately a 2 foot span between the back of the vehicle and the front of my 2012 toyota prius.to the right of the parked vehicle was a parked truck.being that i was pulling into a space to park my vehicle, i was traveling extremely slowly.i began to press on my brake to park the vehicle and instantly knew there was a problem.upon depressing the brake, several lights began to flash within the dash to include but not limited to the abs, warning light, and the light with vehicle swerve lines.other lights were also flashing, but i do not recall each one.the brake pad instantly went straight to the floor with no pressure.the vehicle would not stop and continued to travel forward with acceleration.to avoid hitting the vehicle in front of me i began to turn my wheel to the right.the vehicle continued to accelerate and came into contact with the truck that was to my front-right.my prius failed to slow or stop.it continued to accelerate and began pushing the truck up the driveway and into the metal garage door with the brake pedal being depressed the entire time.eventually, the car came to a stop and the lights continued to flash prior, during, and after the incident.after the car was towed tread wheel marks were found on the driveway where the wheels spun and left an imprint in the asphalt.i and my children were uninjured however, the incident was incredibly traumatic.the vehicle was deemed a total loss by my insurance company and as of yet to be processed for diagnostic testing by toyota.the toyota care service was informed of the accident.toyota believes it to be associated with the intelligence component of the vehicle, however determination is currently unknown until diagnostic is performed.
Pivot bolt inside steering wheel came loose,was warned it had dropped to where it could interfere with brake linkages. Filed complaint with toyota and never heard back so filing this complaint.(1)a large bolt inside a sealed area should not be able to come loose if installed correctly (manufacturing error - single instance or systematic?) and (2) a loose bolt should not be a risk for a possible catastrophic outcome - loss of brakes (design flaw,linkages should not be susceptible to being jammed - that's what they warned me about when theyasked for the $330 to fix it).this was discovered when i took the vehicle in for regular servicing,they told me the steering felt funny.

I having a problem with the brake booster on my prius and the toyota dealership i went to said i need to replace both the master cylinder and booster pump which would be 3000 dollars worth of parts and labor. After research this issue i have found serval people have the same issue. Also the parts are on back order, showing you how big of issue this is.the breaking system feltspongy, mytraction control braking system and abs lights came on. Thankfully i had just a couple of feet to my driveway.
I was traveling about 20 miles an hour during traffic hour when i saw the car stopped in front of me.i had at least 4 car distance when i applied brakes.normally, i can feel the seat belt tighten, cars jerking or shake due to anti-lock brake when you step on the brake hard.but this time,i did not feel the car slow down when i stepped on brake all the way down, it continue to cruise a little before it slow down, but by then, i have already hit the car in front of me.this car was serviced regularly, during my last visit 3 weeks ago, i was notified that my car is on the recall list for software update.
I have never received a recall mail regarding the car. I used an insurance tracking plugin, and all the problems started.pcs error message on my dashboard, due to a leak in brake booster they said. My dealership will not tell me it's a recall, made it seem like they helped me instead of charging as high as $4000. A number of lights came on...abs, anti skid, system failure, etc. The brakes started to feel very funny and started to make a strange noise. Took it to toyota and they said it was not safe to drive because of the brakes. Needed the brake system (actuator is the name i believe).
The brake light, abs light, traction control light all came on at once while driving normally.it did not affect the drivability of the vehicle.i took it to the local toyota dealership to diagnose the problem.the service specialist said that this is a known problem with these cars and quoted a $2500 service price to replace it.if it is this known and this expensive to replace then it should be included in a recall.that would make it a design flaw by the manufacturer.
My car has the same brake booster/master cylinder defect that the 2010/11 model prius were recalled for. Brake booster or brake pressure accumulator failure on 2012 models causes loss of braking effectiveness. Serious safety issue that can cause death. Toyota is well aware of this. This happens in motion at highway speeds.
The car has been suffering all of the brake symptoms for the recall of the 2010 prius brake booster system. I've repeatedly complained about losing braking power when the abs light comes on when i go over uneven surfaces such as railroad tracks or a pothole. The brakes have benn farting from day one while sitting at a light or parked if the brakes are depressed and then slightly released. And now today the abs brakes are on with the battery light and there is limited braking power. I have to push almost all the way down before the brake engages. Toyota tells me i need to replace the brake booster system for $3500 and there are only 70,000 miles on my 2012 car. I feel my brakes have the same issues as the 2010 recall and more years including mine are affected and should be recalled.
When the brakes are applied and going over a rough road surface, or pothole, the brake warning light appears, and the brakes suddenly stop functioning until back on a smooth road surface.this happens all of the time under these conditions.it is really scary when the vehicle suddenly loses all braking while trying to stop.i have a 2012 toyota prius and a 2010 toyota prius.they both have the same problem with the abs braking system.cannot understand why this is not a full recall on this potentially dangerous condition.
The vehicle emits a repetitive 'woo......woo.....' that varies with speed/wheel revolution (something rubbing during each revolution). To the best of my ability to tell, it sounds like the groan is coming from the rear-passenger side.the sound is usually present after the car has sat overnight or after prolonged parking and last for 100-300 feet of driving. I've even heard it after four hours of being parked at work. It will typically make this sound for a few days after a rain event. The sound has occurred in both forward and reverse and with the emergency brake on or off prior to departure. I purchased the vehicle in april and have heard it all the way to the present.during a tire replacement i made note of the issue to the dealer, who attributed it to rotor oxidation. I responded that i've heard the grind/groan of rust being worn off the rotor when first apply the brakes, and this is not that sound. I went in with a recording on my phone and the technician agreed it wasn't rust. They repaired the tire and tried to reproduce the condition without success.they did tear down the brake and found nothing out of the ordinary. The fact that this issue persists on multiple vehicles of this model is indicative of a manufacturer's defect that should be addressed.

The vehicle was traveling over 60 mph when the abs, brake and traction lights came on and the brake became immediately spongy and i had to depressed even harder (past the foaminess) to reduce speed. A motor could be heard trying to turn over on the brake system. The car speed was reduced to 25-30 mph on the highway but the spongy brakes continued. Within 5 min i pulled off the highway and into a parking lot. Parked the car, turned it off and on to see if that would remedy the situation and nothing changed. I then called the toyota dealership's service department to book an immediate appointment and arrived at the dealership approximately 40 minutes later. The car was looked at and i was told that the brake actuator was the issue and that they would like to contact toyota to see if this was in warranty or part of the recall. I was told the next day that the cost of installation and parts would be at least $4,000 and that i would need to pay the full amount as toyota was taking no responsibility. I called the toyota customer service line, explained my situation and was told the same thing as i was told at the dealership. I feel that this brake actuator (its parts, its composition and/or its installation) is a systemic issue with toyota products and a known issue with toyota prius' in 2009 and 2010. Tangential to this issue, my father owns a toyota car as well and has also had to have his actuator replaced this year. Toyota should be held responsible for all faulty parts and installation and purchase of new parts. My maintenance record is exemplary and my driving record is as well so this is not regular wear and tear in my opinion.
While traveling in a rain storm at 17mph, i tried to engage the brakes to prevent from crashing into an 18 wheeler and the brakes did not work. I repeatedly tried to stop the car and the car was accelerating even thought i was applying the brakes not the gas.the car would not shut down, go into park or neutral. I did crash into the 18 wheeler because the engine was accelerating and i wanted to stop.when i crashed the airbags did not deploy.even after impact the car was still accelerating like it was trying to push the 18 wheeler. Eventually the car powered down after repeatedly pushing every button possible. I had the car towed to a toyota dealership. I phoned your hotline but the vin number i gave had a number that was wrong my prior confirmation number is 10542285. Toyota is aware and i am waiting for them to call me back.
My prius v five was purchased in august 2012. Location of the car is nw of wa state where temperatures are currently in the mid 50s (and below) with high level of humidity. Since the beginning of october 2012, i'm experiencing a noise that would best be described as an "owl hooting." my problem is especially noticeable after the car has parked for an extended amount of time (eg. In the garage overnight). Sometimes, merely parking for 10 minutes on the school's parking lot is enough to make the noise appear again. The "hooting" definitely "oscillates" with the rotation of the tire and emanates from my rear right side, but is especially loud when making left turns. The noise disappears after driving a few hundred yards, but is loud enough to turn heads.i'm able to reproduce this problem every single morning since october 2012.the service techs were able to confirm the noise, but couldn't repair the defect. Instead, an open case/ticket was submitted to toyota's headquarter.

Brake booster assembly has failed on my car preventing the regenerative braking to function normally and it could potentially lead to the brakes failing all together. Upon further research it seems to be a common issues with these vehicles and toyota has a "customer support program" under the code "zjb" that will replace the faulty brake booster assembly. My car is still technically covered under the service bulletin that toyota sent out but when i called several dealers and toyota corporate, they notified me that my car was ineligible due to mileage.i was quoted over $3,000 at various dealers to replace the part to what amounts to a basically very expensive brake job. I am concerned for my safety in terms of the brakes failing while driving. This is my only vehicle and i have started to resort to driving on the city streets as opposed to the freeway.
For some time i have noticed that when braking, if my car hit a bump, the brakes would disengage for a second or two even if the brake pedal was fully pressed, and the car would lurch forward until the brakes reengaged. It was a similar sensation to hydroplaning, even though it would occur on a dry road. The antilock braking system light would be on while the brakes were out, and i am told that the brake lights on the back of my car also were on, but no brake function for a second or two. A few days ago i was braking at a stoplight in normal weather conditions and the brakes did not work at all. I had to stop the car using the emergency brake. The mechanic has been unable to find anything wrong with the car. My car only has 30k miles on it and has been well-maintained.

The car failed to slow down or stop, and instead went over the curb, veering to the right and side swiped the left side of my neighbor's car, narrowly missing his garage.the car then side swiped a tree on my left (on property between my and neighbor's property.) the front end of the car crashed into a tree, saving me from rolling about 900 feet to the street below. The entire time of this incident the brakes never engaged (pedal was down to floor) and i frantically tried to steer to avoid hitting anything. A witness, standing on the street, confirmed my brake lights were on. Car was serviced by toyota (world class inspection) 15 days before the incident. Inspection report indicated brake fluid level and brake lining were "checked and ok." after the incident the car was towed to a repair site where the operator demonstrated to me that the brakes were nonexistent and the brake fluid level was still appropriate. Car was deemed totaled.
The brake, abs, and stability control light come on and the brakes require more pressure to function. The problem is intermittent, but has been happening more often and for longer. The rotors, pads, and brake fluid were looked at within the last month and have no issues. I am getting a code c1391 abnormal leak in the accumulator. I own a 2012 prius with the technology package, including radar adaptive cruise control. I believe these same radar sensors are also used to detect imminent collisions in front of the vehicle, even when cruise control mode is not engaged. About a month ago, while driving north on interstate 5 in downtown seattle at approximately 60 miles per hour, and without cruise control engaged, my prius went into "imminent collision mode" despite the fact that there was nothing in front of me -- no vehicles, no debris, no obstructions. Before slamming on the brakes, my seat belt was "tightened" and the brakes were automatically applied at full force, even without my foot touching the brake. This was incredibly jarring and disturbing, and could have been a serious accident if there was a vehicle behind me. Fortunately, there was no vehicle behind me at this time. After about two seconds of full braking, the vehicle "disengaged" collision mode and allowed me proceed and return to speed. I called the toyota of seattle dealership and spoke to their service department. They were unaware of this issue, and suggested that i bring the vehicle in for servicing. They confirmed there were no outstanding recalls on the vehicle. They told me it would cost at least $100 for a basic diagnostic, and if sensors had to be replaced, it could cost hundreds or even thousands of dollars. Because i am currently unemployed and looking for work, i did not bring my vehicle into the service center. I feel this is toyota's obligation to fix, and shouldn't require me spending money out of pocket. I would like some kind of response from toyota on this issue. I have avoided using the radar cruise control since this moment, but the fact that it occurred while not in this mode makes me feel that the car could "take control" at any moment and cause an accident. The brake doen't work when i was driving out of the highway. I pressed hard of the brake and cannot stop the car. And i crushed into a big truck. The car crushed once with the truck and the momentum made the car to hit the traffic light pole again. Terrible.

Sunday dec. 11th: pulling out of chicago garage, car makes odd rotational noise. The sound seems to go away after about a block or so of driving. The dealership is closed and i am booked up with clients, so i continue to test and see if temperature or environment effect the noise.the noise appears to only happen on a 'cold start' but all other factors do not seem to show an impressive change.dec. 15th: i call toyota corporate and file a complaint, then per their direction, call the dealer and make an appointment to have the noise problem fixed. Jan. 13th: after several calls to corporate, and having taken the car in three times to the dealership (midtown toyota- chicago), i have been informed by one of the service employees that toyota is having a problem with many of these models.they think it is a back brake issue, perhaps defective part(s). There was a rattling or knocking noise coming from under the dash area of the passenger side whenever driving over bumps at slower speeds on city streets, often felt tramsmitted to the steering wheel.upon taking it to the toyota dealer, they found that there was free play in the intermediate steering shaft, which they said, could eventually fail.it had to be replaced.
Pivot bolt inside steering wheel came loose,was warned it had dropped to where it could interfere with brake linkages. Filed complaint with toyota and never heard back so filing this complaint.(1)a large bolt inside a sealed area should not be able to come loose if installed correctly (manufacturing error - single instance or systematic?) and (2) a loose bolt should not be a risk for a possible catastrophic outcome - loss of brakes (design flaw,linkages should not be susceptible to being jammed - that's what they warned me about when theyasked for the $330 to fix it).this was discovered when i took the vehicle in for regular servicing,they told me the steering felt funny.
I had to return a 2012 prius i rented from avis corp. The carbon fiber steering wheels on the 2012 prius vehicles are decomposing in the florida heat and sun. The steering wheel is putting carbon (glass) fibers into people's hands. They make our hands red and itch from the carbon glass fibers poking our hands. You can see the fibers shining in the sunlight at the right angle. You guys need to look into this. Probably need to cover wheels with leather or something.also, it appears toyota does not answer their support phones and fixed their support (complaint) web site not to work. Vehicle is parked outside, and during morning cold start, the vehicle produces a moaning/wooing sound for approx. 50-75 yards with vehicle moving under 25 mph.the sound is being generated from the front wheel wells.sound generated is related to vehicle speed.the faster the vehicle moves, the more frequent the sound being produced during the first 50-75 yards. Strange sound is gone after that, and does not return until the next cool/cold start.owner took the vehicle back to dealership for inspection, and dealer took off the front wheel to inspect brakes, wheels, and tires, and was not able to locate or reproduce the problem.abnormal sound is only produced during morning cold starts and when vehicle is left to cool(i.e. Parking lot at work).
More than once i was driving about 40/45mph, once on a curve, anyhow as i was going through the curve it felt as though the brakes engaged, without me touching them. A dead mouse was found in the cabin air filter.the dealer charged me to change the filter and clean out the air system.i thought it was a fluke.i had several unexplained messes in the car (blamed my children) then found a live mouse in the glove compartment.opened up the glove box all the way and exposed what was behind it and found the mice had shredded up the insulation around the wiring system.we removed a gallon sized baggie of insulation that made a very cozy and large mouse nest.now the dealer wants to charge meagain for a mouse issue.where will this end?in reading on the internet, it appears toyota prius has a big problem with not sealing up the car for vermin.hanta virus is a very big concern for me, as is the thought of a mouse scurrying near or over me while i am driving.
My 2012 prius v has a serious reflection in the point of veiw of the side view mirrors, when the front windows are rolled up, and the sun is high noon(+-5 hours) position. This is caused by the white dash trim that the sun hits.## vin passed ## toyota prius v 2012 ##.
Interior water leakage that drips down from the center console down to the driver and passenger floors. First occurrence occurred around 14k miles. It is possible that it was leaking before but i did not notice it because i have weather proofing floor mats, the wetness occurs under the mat. The dealer says clogged ac tubing by debris. They cleaned it out but charge me $100. Second occurrence in dec 6 2013 at 16k miles. It rain the night before. I suspect it is a design flow if there are trend for similar complaints or my car has a defective part(s). The consistent dampness under the floor mats could lead to long term issue with the car.
There is a noticeable halo effect in the windshield when viewing oncoming lights.it is bothersome and irritating on the eyes especially when driving for long periods of time.i am writing toyota directly to learn when a different windshield can be provided.

On september 8, 2012 my wife was on her way to our rental and was hit as she readied to turn to the house. Our 2012 toyota prius 3 was thrown into the air and bounced back to a diagonal position across the road. Upon impact, the steering wheel air bag did not deploy until after impact--her seatbelt did not restrain her from hitting her chest on the steering wheel nor did the knee air bag deploy until after she hit her knees on the lower dash. Fortunately for her and me, she suffered deep bruising but no broken bones etc. As of today, 9/26/2012, her chest still hurts but the pain is diminishing. I contacted toyota and filed a complaint on 9/8/2012 and was told they would investigate--case #[xxx], my concern since we have heard nothing is that you too will hear nothing and if this issue exists in other prius cars on the road, someone will suffer more than my wife. While driving, a loud popping noise (cracking) was heard with no observed damage, and later that day the windshield grew a long crack in windshield while the car was parked.an edge crack formed on the windshield extreme outer edge in the chamfer about 2/3 of the way up on the driver?s side a-pillar.the windshield by it?s design appears to be rather thin, and the windshield edge is exposed, not covered or embedded in plastic or rubber or trim material.the windshield edge as observed has a very small chamfer and does not have a generous bevel (round) that is highly polished such as found on the passenger window edges or older windshields i am familiar with. I took the car to toyota of bowie and was told by the service manager, he believes there was an impact to the windshield at the very extreme edge of the windshield where the crack root is on the chamfer and their dealership would not replace it under any warranty.the service manger did not look at the crack with any tools, for example, a magnifying class or the use of a ball point pen to determine any impact location or if it is in fact a stress crack that has formed.the service manager did not offer to arrange to have the car repaired and told me to go to a glass company. When the service manager was asked about any problems or concerns with toyota windshields or exposed glass edge windshields with a small chamfer, he said there were no problems or recalls.i have only owned the car eight months and plan to write toyota to learn when a more robust crack resistant windshield can be made available.note:pictures can be made available.
I was involved in a front end collision with a bobcat front end loader. I struck the implement broadside going 35 mph.there was no damage to the implement. My car was totaled and the airbag deployed. After going to the hospital and a chest x-ray, ultrasound, and ct scan, it was diagnosed i had fractured my sternum. I am six feet tall and had my seat all the way back. I am questioning whether there have been any other people sustaining a fractured sternum due to an air bag deployment at 35 mph?
On july 28, 2017, i was driving along with my three children.i pulled into a driveway to park my vehicle behind a parked vehicle.there was approximately a 2 foot span between the back of the vehicle and the front of my 2012 toyota prius.to the right of the parked vehicle was a parked truck.being that i was pulling into a space to park my vehicle, i was traveling extremely slowly.i began to press on my brake to park the vehicle and instantly knew there was a problem.upon depressing the brake, several lights began to flash within the dash to include but not limited to the abs, warning light, and the light with vehicle swerve lines.other lights were also flashing, but i do not recall each one.the brake pad instantly went straight to the floor with no pressure.the vehicle would not stop and continued to travel forward with acceleration.to avoid hitting the vehicle in front of me i began to turn my wheel to the right.the vehicle continued to accelerate and came into contact with the truck that was to my front-right.my prius failed to slow or stop.it continued to accelerate and began pushing the truck up the driveway and into the metal garage door with the brake pedal being depressed the entire time.eventually, the car came to a stop and the lights continued to flash prior, during, and after the incident.after the car was towed tread wheel marks were found on the driveway where the wheels spun and left an imprint in the asphalt.i and my children were uninjured however, the incident was incredibly traumatic.the vehicle was deemed a total loss by my insurance company and as of yet to be processed for diagnostic testing by toyota.the toyota care service was informed of the accident.toyota believes it to be associated with the intelligence component of the vehicle, however determination is currently unknown until diagnostic is performed.
Turning right from a stop sign stop, the car took off in an unintended acceleration and could not be stopped until it hit a curb. The force broke the ball joint of the driver's side wheel causing the wheel to turn approx 45 degrees. The car then skidded about 20 yards and came to a stop with the motor racing. It was a single car crash with no bodily injury.

The contact had previously experienced the same defect with the front passenger's side splash protector. The contact indicated that the defect was due to the vehicle being too low to the ground and when parking the vehicle, the splash protectors would make contact with the curb causing them to become loose and eventually detach from the vehicle. The contact also stated that the protectors would also make contact with the tires once they became loose. The vehicle was diagnosed and repaired at the dealer. The manufacturer was notified and no solution was offered. The vin was not available. The failure mileage was 5,000.
The windshield causes most light sources, particularly led lights, street lights, and oncoming headlights (basically any bright light source), to appear with a "ghosting" image. The "ghosting" appears in the windshield vertically below the light source as one, two, or even three images. The dealer has replaced the windshield twice already, but there has been no improvement. In fact, the first and second replacements are worse than the original. I also own a 2008 toyota sienna, and i drive various chevy equinoxes and ford escapes where i work - i do not experience the "ghosting" effect in those vehicles.personnel from the dealer's service department as well as representatives from toyota have also observed this phenomena with the windshields in my prius plug-in. I was told by a representative in the service department after the last replacement that even the toyota engineers weren't happy with the replacement. It has now been two weeks since the last windshield was installed. I notified the service department (during my drive home that same day) at the dealership that the windshield was unsatisfactory, and i was told that they would notify me when toyota decided what to do about it next. To date not only have i heard nothing, but the last three phone calls to the service adviser have gone to voice mail, and i received no reply.
Mice in cabin.since i've owned this car (bought new) i've had several occasions where mice get inside the passenger compartment.i've looked on-line and have seen numerous complaints and suggested solutions.most have said their dealers deny any design problems and will not fix the problem.frustrating!
Vehicle is manufactured without any regard for preventing mice from entering the passenger interior--- air intake vent under windshield and 2 rear decompression vents located in the rear cargo compartment above the wheel wells are open to the exterior allowing unimpeded access to the interior of the car. Had mice in this car 2 weeks after purchase in 2012 and have been dealing with this problem for 7 years. Just recently, had toyota install 1/4" hardware cloth over these 3 areas and this should help significantly to prevent entry. All preventative measures to prevent mice entry have been diligently taken by me, without success. Mice can make a person very sick. Their excrement carries hantavirus..... And the mice carry ticks, which can lead to lyme disease. Simply manufacturing the car with these vents properly screened would greatly reduce the chance of occupants contracting a serious disease.the mice also do a lot of damage to the car itself, in my car the driver seat belt was chewed, and i have had to replace the cabin air filter several times because of nesting in this compartment(after having been unknowingly breathing the mice excrement as the air passes through this filter on its' way to the interior. Toyota needs to be pressured by your agency to correct this very serious health hazard that their car is presenting. Their is no legitimate reason for this situation to exist. The cost to toyota would be negligible.the benefit to all prius owners would be very significant, likely preventing many cases of disease. Please take this situation very seriously, as is the case. I also will be calling you to discuss this over the phone. Thank you for your help to correct this problem!

When starting to drive the car after is has been sitting for about 3 hours and especially first thing in the morning, the vehicle makes a groaning noise which seems to be coming from the left rear tire area.it lasts for about the first 50 feet and then it stops making the noise.this has happened with temperatures varying between 40 to 75 degrees.when it is really hot in the morning, the noise is not present.the dealer has looked at the vehicle several times and they say that this is a national problem with no solution.i have owned this car for 4 days short of a year with 12,250 miles on it and it does it all the time. This is very frustrating because the car is fine otherwise.

The entire time of this incident the brakes never engaged (pedal was down to floor) and i frantically tried to steer to avoid hitting anything. A witness, standing on the street, confirmed my brake lights were on. Car was serviced by toyota (world class inspection) 15 days before the incident. Inspection report indicated brake fluid level and brake lining were "checked and ok." after the incident the car was towed to a repair site where the operator demonstrated to me that the brakes were nonexistent and the brake fluid level was still appropriate. Car was deemed totaled.
Three different door lock actuators (front left, front right, rear left) have failed and been replaced.symptom is that door does not lock or unlock from key fob or lock control on driver's armrest, also fails to lock automatically when shifting into gear if that option is set.door must be locked and unlocked manually using the rocker switch in the inside door handle.clearly there was a batch of defective parts used during manufacturing.
The contact owns a 2012 toyota prius.the contact stated that while parked, there were rodents entering the interior of the vehicle. The contact stated that the cabin air filter did not have a mesh grill which was allowing the rodents to enter through a hole in the cabin air filter then into the glove compartment console and into the interior of the vehicle. The dealer was contacted and informed the contact that the wires were soy based causing the attraction of rodents to the hood of the vehicle however the vehicle was not taken for an inspection. An independent mechanic was also contacted who confirmed that the cabin air filter did not have a mesh grill which resulted in the rodents entering through a hole into the cabin air filter then entering into the glove compartment console accessing the interior of the vehicle. The vehicle was then taken to a mechanic where a hardware cloth grill was installed on the cabin air filter. The vehicle was repaired and rodents were no longer entering the vehicle. In addition, the contact was required to clean the feces remaining in the interior of the vehicle utilizing a mask, gloves, and disinfectant and in order to avoid contact with a virus from the feces, the contact was advised not to vacuum let the virus become airborne. The contact was concerned that the rat feces in the vehicle could result in a fatal disease in the event a child would ride in the vehicle and accidentally eat the feces. The contact was concerned that if rodents entered though the air conditioning unit and defecated inside, activating the air condition unit if there was feces, a virus could become airborne. The manufacturer was notified of the failure then informed the contact that the failure was not covered under warranty and that the failure was an outside influence which was not their responsibility. The approximate failure mileage was 40,000. The vin was not available.
Design of car includes a flaw causing a huge vertical blindspot on both the right and left side of the car. Because there is a bar that runs vertical on both sides at the beginning of the driver and front passengers' windows, this, along with the pillar coming down the windshield contribute to an extremely large spot where objects in road cannot be seen. This is especially hazardous for pedestrian foot-traffic crossing perpendicular to the car. Peds in crosswalks or in parking lots are at times, completely masked by this blind spot by the combination of the front windshield pillars and front window sub-pillars. Other cars (such as honda insight) with a dramatically angled windshield avoid this by using a smaller sub-pillar on the front window, allowing for things to be seen far more easily. This issue should be addressed before it causes a pedestrian injury or death. This is not something that can be repaired.
I was getting ready to pull out of a parking space in a parking lot. The car was in park, so i pressed on the brake, started the car and put it in reverse. All of a sudden there was a loud roar and the car shot full speed in reverse, completely out of control. It clipped the bumper of a parked car and some shopping carts, then backed into a wall which stopped the car. Miraculously no one was injured. The car has been totaled.
Turning right from a stop sign stop, the car took off in an unintended acceleration and could not be stopped until it hit a curb. The force broke the ball joint of the driver's side wheel causing the wheel to turn approx 45 degrees. The car then skidded about 20 yards and came to a stop with the motor racing. It was a single car crash with no bodily injury.
On cold mornings the car makes a groaning noise from the rear for aprox a 1/2 block. My dealer was able to verify the problem when i left the car overnight parked outside. They said they are waiting for toyota to come up with a fix. The car now has just over 10,000 miles but it has been going on for most of the time we have owned it.
Repeating several-second groaning or grinding noise heard from rear wheel area upon startup motion. The noise is severe and sounds like low-pitch metal grinding. It's potentially brake related but could be suspension bushings or other components rubbing against one another. It does not sound like typical brake glazing. The noise occurs in all weather conditions and typically after vehicle has been sitting for several hours or overnight. Noise continues for several minutes at stop and start. Noise is loud enough to startle an experienced driver. The absolute severity and indication of the sound is enough to question safe ongoing functionality of braking and/or suspension systems. When starting to drive the car after is has been sitting for about 3 hours and especially first thing in the morning, the vehicle makes a groaning noise which seems to be coming from the left rear tire area.it lasts for about the first 50 feet and then it stops making the noise.this has happened with temperatures varying between 40 to 75 degrees.when it is really hot in the morning, the noise is not present.the dealer has looked at the vehicle several times and they say that this is a national problem with no solution.i have owned this car for 4 days short of a year with 12,250 miles on it and it does it all the time. The lining on the deflector rubber gasket/seal of the sunroof has been deteriorating. I have heard from many other prius iii owners that this is a common defect for the 2010-2013 year models. I believe it if is this common, toyota should be responsible for replacing it.
My prius was just fine then one day it stalled while i was driving in the middle of the free way.i took my vehicle in to elmore toyota in westminster, ca, they found it had a failed inverter and it was still covered under warranty.it took 2 weeks for them to fix it, but when they returned my car, the slip indicator was now on and would not turn off.they claim it was like that prior to the failed inverter, this was false, my car was running perfect.they then wanted to charge me nearly $1k to fix a problem i did not have prior to failed inverter.i feel the situation was connected but they refuse to cover it.
Nhtsa recall #18v-684i was driving on the street when cheq hibrid sistem sign came on, after i got this masage, i did drove for a while til stop on a safe place, when turning it back on with the break pedal pushed, the sistem did not get a ready light, which means able to move, so i called aaa (a towing service).we went to a west covina toyota dealer. The service man manny told me it maybe is about the recall on this vehicle.when vehicle was ready manny told me on the phone that it was a loose wire and when i got at the dealer he show me a switch (not a wire) that it is imposible to unlock it by it self, it has to be move by a finger in order to unlock it or lock it back on, so my point is that i feel that they are telling me that just to charge me and i believe that they are making this up. I was driving on the freeway, about 65 mph, going to work on 06/12/18 at approximately 9:45 am arizona time.all of a sudden i heard a loud bang (explosion) and looked around my car.i noticed in my rear view mirror black glass flying off the roof of my car.there were other cars around me but no trucks or semi's.i cautiously changed lanes until i could pull off the freeway.when i got out of my car i looked at the roof and found that the sunroof of my car was shattered and had dropped into the area below the roof.i could still hear glass cracking and see it falling into the well under the roof area.i had had the inside shield closed so there was no glass on me.i slowly drove to my local toyota dealership to ask for help.they said that they had never seen this before and that i probably had something hit my roof.i told them that i didn't see anything flying at me and that there were no trucks/semi's around me.then they told me they couldn't do anything but would help me call my insurance company to file a claim.i did (it took state farm over and hour to figure out where to connect me for a glass claim).i had to take it to a body shop and get an estimate, then get it approved, and get a rental car.even at the body shop i could still hear the glass cracking.everyone kept telling me that something must have me but i didn't feel anything when it happened, just the loud bang like an explosion.
The design of both honda accord and toyato prius, and likely othernew modelcars, makes it very difficult to see when backing up. The back up camera is placed such that it is easily covered with road debri (dirt and snow) so it often useless. The deisgn of windows, seat and camera do not sufficiently allow one to see over ones left shoulder. Perhaps this is more because i am barely 5'1:" and the car's safety forbids using a seat cushion to raise me higher.backing up is frightening but many car lots require you to pull in front forward so pulling out when pedestrians might be behind or other cars backing up is a serious problem.i did not have this problem when i drove a 2007 ford ranger or a 1995 le baron convertible.so far i have managed to not have an accident but think cars' designs do not consider this situation sufficiently for safe driving..
I switched into neutral and pushed the park button to park. I leaned forward to grab my wallet and felt the car accelerating over the parking curb. I immediately slammed the brakes and the car accelerated into a building. I am fortunate to be alive because if the airbag would have activated, i would have broke my neck.
Excessive oil consumption. When i checked the oil level before the next scheduled oil change, i found no oil on the dipstick. On my last oil change, i was not told i had a problem with my oil consumption. Had to buy oil in a hurry as i did not want to drive the car without oil.
Took my car into dealership because of a noise it would make when i started to brake and when i was stopped at a light.the car would also vibrate and shake. The dealership told me the abs actuator needed to be replaced which would cost over 4 grand! i couldn't believe it. I'm scared to drive the car now.
I was in freeway going home last night after visiting my mom by point loma area san diego. I was on 8fwy going east, then i merge to 805fwy south which i always and usually take same route. As i went on ramp of 805 south going on a steep high altitude, my 2012 priusv stall and i can't accelerate when i was stepping into my gas pedal which i have already pushed it all the way in. So i quickly put emergency lights on and slowly guided my car to the right shoulder. I called aaa which i've been a member for so many years. And believe me it is really a life saver. This safety could have been worst from my experience of been driving for more than 30 years. I got it towed all the way home which was about 7 miles. The traffic was heavy on a saturday night and i waited for almost 2 hours for my towing. When i got up this late sunday almost noontime, i went out to check on my car and it is still saying "check hybrid system" with all the lights on my module was on. I started it and noticed there was an acceleration so i drove it back to my driveway since the tow truck just drop it off on the curveside close by my residence. It drove ok, and then i started my research online and i recently bought a code reader but not sure if it will work on my hybrid 2012 prius v. Nothing came out on the reader. I called my buddy which was a certified ase mechanic but has less experience on hybrid. But he has a more expensive code reader and analyser equipment and tools. I also check the toyota headquarters and you website about my car on safety recalls. I just had taken this vehicle to the dealer for some sort of worldwide safety recalls on all this typical model. I've also read an additional recall and they just have launched it 01/2019. I've plug in my vin# on all of this sites for a check but nothing is coming out. But their complaint is similar to what's going on to my car. It's "check hybrid system" and i believe it's the inverter issues. Please update me and also the toyota hq.
Vehicle is manufactured without any regard for preventing mice from entering the passenger interior--- air intake vent under windshield and 2 rear decompression vents located in the rear cargo compartment above the wheel wells are open to the exterior allowing unimpeded access to the interior of the car. Had mice in this car 2 weeks after purchase in 2012 and have been dealing with this problem for 7 years. Just recently, had toyota install 1/4" hardware cloth over these 3 areas and this should help significantly to prevent entry. All preventative measures to prevent mice entry have been diligently taken by me, without success. Mice can make a person very sick. Their excrement carries hantavirus..... And the mice carry ticks, which can lead to lyme disease. Simply manufacturing the car with these vents properly screened would greatly reduce the chance of occupants contracting a serious disease.the mice also do a lot of damage to the car itself, in my car the driver seat belt was chewed, and i have had to replace the cabin air filter several times because of nesting in this compartment(after having been unknowingly breathing the mice excrement as the air passes through this filter on its' way to the interior. Toyota needs to be pressured by your agency to correct this very serious health hazard that their car is presenting. Their is no legitimate reason for this situation to exist. The cost to toyota would be negligible.the benefit to all prius owners would be very significant, likely preventing many cases of disease. Please take this situation very seriously, as is the case. I have a recall on my prius for software and it says on your site that if i have a problem with my invertor they should fix it or repair it for freemy car had this problem 1 week ago and my invertor failed and i went to toyota today they gave me an appointment today and they said they contacted toyota japan and the recall got rejected for the invertor part they said i should fix my invertor first then they will do the recall of the software! and it costs 3000 jds here in jordan its too expensivewhat should i do?
I own a 2012 toyota prius. On the dashboard the brake light, the abs light, and the traction control light are all on. In addition to the warning lights the brake accumulator pump is constantly buzzing without ever stopping. I took the car to our local dealer and i was told that it is a leaky accumulator pump defect. I was also told that this is not a ware and tearitem and that it is in fact a faulty item from the manufacture. I called toyota corporate for help but i was told that the car is out of warranty and that there is nothing they can help me with. Problem with braking system involving, the other day the car was on and suddenly three (3) warning lights came on: brake light, abs light and traction control light.there is also now a pumping sound coming from under the interior of the dash (on it at least sounds that is where it's coming from.the car has 151,428 miles on it.i've included pictures of the current eror code, mileage and dash lights that are on.
My passenger side sun visor just completely fell out while driving down the road. It has rarely been used (my car is a 2012 with high miles and most of the time i travel alone). This could have easily caused a distraction or crash. I noticed that there are several complaints about other toyota models / years.
Received a warning check hybrid system - it was code poa93. Ended getting the inverter pump and radiator replaced do to this code. Appears to be a known issue with toyota with there prius's. The car died rolling into a toyota dealer.
2012 toyota prius has been consuming large amounts of oil since 89000miles.dealer completed an oil consumption test, of course if passed.dealer stated rave4's and camry's have had same issue and they issue recalls.toyota has yet to accept responsibility and replace the pistions to correct the oil consumption.
Three doors stopped opening and closing with the remote key and inside lock/unlock buttons in a span of three weeks. Not able to open and close the door with the remote key or lock/unlock button is a safety risk.toyota confirmed that all three door lock actuators failed. Each one costs $800 to replace with a total cost of $2400.it is inspected by toyota dealership, toyota 101, 386 convention way, redwood city, 94063. The car had only 36,448 miles. The problem appeared all of a sudden. First one lock failed. Soon all three locks failed.
In early october i received a recall notice from toyota for a "failsafe driving mode" hybrid system fault that could cause the vehicle to "stall" increasing the possibility of vehicle crash during critical driving conditions. It stated i would be notified when the fix was available. 2 1/2 months later, i've yet to hear from toyota and my dealership states they have not received remedy parts or procedures from toyota. All they could do was give me a phone number at toyota corporate. I called the number and the person had no further information on when a fix for the problem would be available. She stated the original notice stated a time frame of 2-3 months; in fact, it does not contain any time period indication. It is inappropriate for fix for something of this potential magnitude to take so long. I'm concerned that toyota, as they have done with other recall issues, is merely delaying to avoid costs of implementing the fix and endangering all owners further than they already have. The lady stated i could contact the dealership about providing a rental vehicle during the period, something the dealership has denied they will do. I bought my vehicle trusting toyota would stand behind the vehicle and they are failing to do so.

Parking car in handicapped spot - speed approximately 2 mph - gas engine cut in and the car accelerated at a very high speed.it went over space stop (concrete) - crossed the sidewalk onto grass and i had both feet on the brake and stopped the car approx. 3 feet from a wall of a building and shut off the engine.the handicapped sign scraped the whole right side of my car and there are several dents.the sign was fine.
The brake, abs, and stability control light come on and the brakes require more pressure to work . Getting a code c1391 abnormal leak in the accumulator.but i was never informed of this recall and there are no open recalls associated with my car
While traveling in a rain storm at 17mph, i tried to engage the brakes to prevent from crashing into an 18 wheeler and the brakes did not work. I repeatedly tried to stop the car and the car was accelerating even thought i was applying the brakes not the gas.the car would not shut down, go into park or neutral. I did crash into the 18 wheeler because the engine was accelerating and i wanted to stop.when i crashed the airbags did not deploy.even after impact the car was still accelerating like it was trying to push the 18 wheeler. Eventually the car powered down after repeatedly pushing every button possible. I had the car towed to a toyota dealership. I phoned your hotline but the vin number i gave had a number that was wrong my prior confirmation number is 10542285. Toyota is aware and i am waiting for them to call me back.
2012 toyota prius.consumer writes in regards to vehicle sudden acceleration. The consumer stated as he entered a parking lot and turned to go down an aisle, he hit the curb too closely and the vehicle accelerated at an incredible speed. He put his foot on the brake and it went to the floor; he tried to turn the power off, but the vehicle did not stop. At the rate the vehicle accelerated, he passed by 2 parking aisles and was stopped by hitting a parked vehicle, which the force of his vehicle was pushed under a fence and onto a railroad track. The vehicle was badly damaged because the emergency brake was on and the impactdestroyed the transmission. The dealership reviewed the computer data and concluded the vehicle did not accelerate on its own. They implied his foot was on the accelerator and not the brake.
Driving 70 mph car began to go into high rpm, then accelerated to 74 from 70 mph. I tried to decrease speed then vehicle shut off got a long beep and message on display "check hybrid system, pull over immediately" all lights on dash were indicating failures. Coasted to safety with 5 passengers in vehicle across traffic at 9:30 pm to shoulder. Tried to restart several times then vehicle went into high rpm then engine knocked loudly and was completely disabled. Lost brakes, pedal was hard, lost power steering, difficult to maneuver. I still had 2 bars indicating plenty of fuel in car. Prior fillup was 7.6 gallons at pump, usually get over 500 miles per tank, trip meter only showed under 400 miles since fill up so had plenty of fuel and there was no low fuel indicator. Reported all problems to dealer. They only noted the car shut down but told me they confirmed everything i said and that toyota engineers are trying to resolve this since they are aware of this type of problem. So far dealer and toyota have not addressed the high rpm, the acceleration, loss of electronics and hybrid system failure and told me they havenever heard of any toyota ever having a report of a high rpm or acceleration without intent to accelerate form the driver. Vehicle is unsafe, has 4944 miles and this problem occurred about a month after purchase. Toyota still does not know what the problems are and in possession of the vehicle.

There is a triple effect in visibility when a light source (eg oncoming traffic headlights) fall on the windshield. This issue is evident from all angles and both front seats. Has a distracting and a straining effect on the eyes. Feels like wearing bi-focal glasses.

While driving, a loud popping noise (cracking) was heard with no observed damage, and later that day the windshield grew a long crack in windshield while the car was parked.an edge crack formed on the windshield extreme outer edge in the chamfer about 2/3 of the way up on the driver?s side a-pillar.the windshield by it?s design appears to be rather thin, and the windshield edge is exposed, not covered or embedded in plastic or rubber or trim material.the windshield edge as observed has a very small chamfer and does not have a generous bevel (round) that is highly polished such as found on the passenger window edges or older windshields i am familiar with. I took the car to toyota of bowie and was told by the service manager, he believes there was an impact to the windshield at the very extreme edge of the windshield where the crack root is on the chamfer and their dealership would not replace it under any warranty.the service manger did not look at the crack with any tools, for example, a magnifying class or the use of a ball point pen to determine any impact location or if it is in fact a stress crack that has formed.the service manager did not offer to arrange to have the car repaired and told me to go to a glass company. When the service manager was asked about any problems or concerns with toyota windshields or exposed glass edge windshields with a small chamfer, he said there were no problems or recalls.i have only owned the car eight months and plan to write toyota to learn when a more robust crack resistant windshield can be made available.note:pictures can be made available.
The windshield has a vertical "prism" effect with any light source coming through it.
Vehicle windshield has an apparent critical defect in production. Light passing through the windshield is replicated above and below the source of light. As light source becomes more distant, light is replicated 2, 3, or 4 times more than its source (as distance increases). This creates a major hazard during night driving and driving when the sun is low enough in the sky where it can be seen through the windshield. In some cases, the effect is blinding. I have found this to be a major safety hazard and one that could result in injury or fatality. More recently, after the time change, i have been leaving for work when the sun is lower in the sky and this creates a blinding effect. I have had two near misses in the past two days because of this. I am also having to drive home in the dark which is dangerous as oncoming headlights are amplified in intensity and can appear to be as if the oncoming lights are high-beams. This issue has existed since i took delivery of the new car and when talking to the selling dealer, i am continually brushed off and have yet to have it fixed or even looked at. Dealership personnel admit they have heard of the issue, but cannot provide information as to the fix or how toyota is dealing with these complaints. At this point, with two near misses recently, i see the car as being unsafe for driving. I will be contacting toyota to file a complaint regarding this issue. I will also be contacting the dealership management to arrange drop off of the car until it can be made safe to drive.in my own research, i have found numerous complaints detailing the same issues online and a couple videos showing the exact same issues. Head lights from oncoming traffic at nighttime have a "halo" and "prism" appearance. Additionally, reflective road signs are also very distorted, seeing what appears to be multiple signs. Since that time eyes have been tested with no change from last exam and new glasses are now being worn. There is no difference. My wife states she sees the same distortion when driving after dark. This has been a continual problem since we purchased the vehicle. In addition, the windshield was replaced in august, 2012, as a result of hail damage. It was replaced with genuine toyota parts. There is no difference.

There is a ghosting image seen through the front windshield on my new 2012 toyota prius ii that is affecting night time driving and the visibility of lights, especially red lights. When you are operating the vehicle at night and you approach a light there is a ghost image of the light above and below the actual light source until you get within a few feet of the light source then it disappears. This is becoming a safety issue for myself as well as many other 2012 prius owners that i have talked to about this. I have 20/20 vision with no need for glasses and this is beginning to cause a strain on my vision, as i imagine for others an even greater strain. The windshield has a serious optical illusion during night time driving.the windshield creates a ghost image of the original light source - the ghost image appears above and below the original light source.for example, at night the red traffic light will have a ghost image of the red light above and below it, ultimately showing three red lights (ghost image(1), original light(2), and another ghost image (3)).this can also be seen with the yellow and green traffic lights, oncoming car headlights, brake lights, street lights, and pedestrian walk signs.
On oct. 23, 2013 at 6am in the morning in about 45 degrees fahrenheit, my toyota prius 2012 rear window emitted a loud punching sound and then shattered when i turned on the rear wiper.i googled the incident and we're not the only ones who experience this.there have been similar incidents on other prius in other years as well.thus, we are reporting it and hope this will be looked into.our car is barely a year old and under warranty but our local toyota dealer said warranty will not cover this.thank goodness our auto policy will and we don't have to pay a deductible.

I have noticed ghosting of images most obvious while night driving. Particularly noticeable in led lights. It looks like a duplicate image of the lights created. It can be very glaring and hard to judge distance of other vehicles.

New 2012 toyota prius plug-in. Visibility through front windshield is blurry - particularly noticeable at night when light sources are viewed through the windshield. Light sources produce multiple images (up to two additional) below the original light source. Problem is also noticeable during daylight hours, particularly during cloudy conditions. Ghosting effect worse at a distance, does not seem to occur when light source is very close to vehicle (for example within 10 feet). Ghosting effect is quite noticeable at night. I am worried that the problem may cause headaches, visual fatigue, and accidents.

The effect is so pronounced that it is impossible to read lit highway safety signs at night because the ghosting obscures the letters completely. The lights look fine through the side glass, so it is clearly an issue just with the windshield. I contacted the dealer and they dismissed my complaint. However i see that many people have mentioned the exact same thing online and our local news channel in the san francisco area kabc channel 7 has begin a "7 on your side" investigation. Please urge toyota to recall these dangerous windshields.
Brand new prius 2012while driving at night,lights, especially red in color, when viewed from front windshield, appear as three distinct images. One dark image along with two faint image one above and one below the dark one.i got the car in nov end, reported the issue in dec 17 to toyota, then went to dealer before christmas. Technician there acknowledged the issue and ordered a new glass. Windshield was replaced on 11 jan but saw the same issue, technician test drove the car and acknowledged the issue is still there.toyota rep will come and look into the issue. Design of car includes a flaw causing a huge vertical blindspot on both the right and left side of the car. Because there is a bar that runs vertical on both sides at the beginning of the driver and front passengers' windows, this, along with the pillar coming down the windshield contribute to an extremely large spot where objects in road cannot be seen. This is especially hazardous for pedestrian foot-traffic crossing perpendicular to the car. Peds in crosswalks or in parking lots are at times, completely masked by this blind spot by the combination of the front windshield pillars and front window sub-pillars. Other cars (such as honda insight) with a dramatically angled windshield avoid this by using a smaller sub-pillar on the front window, allowing for things to be seen far more easily. This issue should be addressed before it causes a pedestrian injury or death. This is not something that can be repaired.
Prius is not safe to drive at night. The windshield distorts light sources and causes a ghosting/multiplying effect. Traffic signals images are doubled, tripled or more - not safe as it is distracting and confusing. This issue is very prominent at night. It is not just traffic signals but other light sources as well. In december 2012, i contacted toyota and was referred to dealer. Service manager confirmed issue and said that he is waiting on toyota on how to fix - no resolution after one year of waiting.
There are very dangerous blind spots on both rear driver and passenger sides of the toyota prius v . Vehicles located near the mid to rear of the car can not be seen in mirrors or by glancing over the shoulder when changing lanes. I have had numerous near collisions due to this design defect. I have brought this to the attention of the toyota corporation and the local dealership and have had no co-operation in dealing with this safety hazard. Brand new 2012 prius driving out of the dealership at night, noticed a rather bothersome trait: lights viewed through the windshield have a prism effect where there is a "ghost" image located above and below the actual light source. I wanted to know if this was defective windshield, or just how the prius is due to the very low angle of the windshield. It is definitely a prism type effect as it differs by color, and as you approach the light source, the ghost images merge into the single image.this happens with all light sources, the sun (worst of all), headlights, tail lights,street lights, traffic lights, etc.when driving at night this ghosting of the light sources is really distracting and makes it look like you are looking at a blurred image. Windshield optics cause light sources to triple with ghosting above and below the source of light.this is causing difficulty in judging distance, distraction and eye strain while driving.
The windshield causes most light sources, particularly led lights, street lights, and oncoming headlights (basically any bright light source), to appear with a "ghosting" image. The "ghosting" appears in the windshield vertically below the light source as one, two, or even three images. The dealer has replaced the windshield twice already, but there has been no improvement. In fact, the first and second replacements are worse than the original. I also own a 2008 toyota sienna, and i drive various chevy equinoxes and ford escapes where i work - i do not experience the "ghosting" effect in those vehicles.personnel from the dealer's service department as well as representatives from toyota have also observed this phenomena with the windshields in my prius plug-in. I was told by a representative in the service department after the last replacement that even the toyota engineers weren't happy with the replacement. It has now been two weeks since the last windshield was installed. I notified the service department (during my drive home that same day) at the dealership that the windshield was unsatisfactory, and i was told that they would notify me when toyota decided what to do about it next. To date not only have i heard nothing, but the last three phone calls to the service adviser have gone to voice mail, and i received no reply.
Wind shield wiper switch quit working at 62,500 mile.the wind shield wiper motor is now making grinding noises and needs to be replaced(74,000 miles). Both parts broke when we were traveling out of state,during rain storms.

The replacement windshield had the same defect the case was taken to arbitration when toyota refused to find a remedy for the safety defect. Arbitration was lost due to "lack of evidence". More than once i was driving about 40/45mph, once on a curve, anyhow as i was going through the curve it felt as though the brakes engaged, without me touching them. I understand the safety thing, however at both instances it was not necessary. It caused the car to pull to the left. Almost causing me to loose control.
Vehicle is parked outside, and during morning cold start, the vehicle produces a moaning/wooing sound for approx. 50-75 yards with vehicle moving under 25 mph.the sound is being generated from the front wheel wells.sound generated is related to vehicle speed.the faster the vehicle moves, the more frequent the sound being produced during the first 50-75 yards. Strange sound is gone after that, and does not return until the next cool/cold start.owner took the vehicle back to dealership for inspection, and dealer took off the front wheel to inspect brakes, wheels, and tires, and was not able to locate or reproduce the problem.abnormal sound is only produced during morning cold starts and when vehicle is left to cool(i.e. Parking lot at work).
Turning right from a stop sign stop, the car took off in an unintended acceleration and could not be stopped until it hit a curb. The force broke the ball joint of the driver's side wheel causing the wheel to turn approx 45 degrees. The car then skidded about 20 yards and came to a stop with the motor racing. It was a single car crash with no bodily injury.
