On several occasions it sounds as though something 'metal' is sliding around inside the panel of the front passenger door.i have taken it to the dealer 3 or 4 times and they cannot verify it.when i went out with a tech, once, although it had made the sound as i turned out of my driveway, it wouldn't duplicate with the technician.they wouldn't take off the panel to investigate.given the problem with airbags this makes me very nervous and i hope that i am never in a crash.
Driver in a 2005 ford focus 4 door turned left in front of me. My prius v hit the ford focus on the front passenger side corner. My prius was damaged in the center of the front. The speed of my vehicle is less that 40 mph, the speed limit. I believe i was traveling about 30 mph before i hit the brakes had about 25 feet to stop. When we hit the ford it was like hitting a brick wall, my prius poppedup off the ground about a foot. Neither of the front air bags deployed. My wife, the passenger in my prius fractured her sternum due to the pressure of the seat belt. Updated 06/06/2014
My front and driver's side air bags didn't open when my car was hit onthat side by another car driven by a lady who ran astop sign; and struck my moving car; totaling my car. I suffered sertous chest, neck and back sprains that still l bother me. I was struckpretty hard, i suffered a terrible jolt to my chest area when my car came to a hard stop, and i still hit the steering wheel. This accident costs me my job and i am no longer able tostand or walk for long periods of time.
My horn no longer works. I took it into the dealer and it was diagnosed as having a faultyspiral cable. According to information on the internet, the spiral cable also plays a role in the deployment of the driver airbag. This part was under a recall for vehicles a couple of years prior. I feel that if this is a known issue and an extreme safety issue then it should also be fixed under recall. The dealer price to fix this is over $800. I have opened a case with toyota #210-128-2281, but they don't see it as a safety concern and will not offer assistance to remedy the faulty cable.
The contact owned a 2012 toyota prius v. While driving 5 mph and making a left turn, a vehicle crashed into the front passenger side of the contact's vehicle. As a result, the air bags deployed, but the driver side air bags failed to fully inflate. The contact sustained injuries to the chest that required medical attention. A police report was filed and the vehicle was towed to an independent mechanic, who deemed the vehicle destroyed. The manufacturer was not made aware of the failure. The failure mileage was 23,000.
All the brake lights popped on and check engine light . There was a radal in the engine i got the check engine light check it was multiple mis fire so i changed spark plugs and coil packs .yesterday the vehicle stalled while moving and wouldn't go no where else . There is a recall on the car that my fiance just got in the mail and toyota in goldsboro nc doesn't want to do anything about it .
On 09/19/2019 key west toyota completed the tires change of my vehicle as i requested. In addition, key west toyota performed "107649 j0v recall j0v01b 0.70 update to the power management ecu with new software" as showing on the invoice. The following day, on friday 09/20/2019, while i was driving the vehicle lost power and stalled in the middle of the traffic, putting my life and safety at risk and my heath and general well-being. I sent back the vehicle to key west toyota, and since then the issue is unresolved, and i am not even able to reach anyone on the phone or through the email i sent from key west toyota website "contact us".
Headlights burn out in less than 6 months due to degradation of the wiring harness. As the vehicle ages, headlight life decreases dramatically. This is a safety hazard. In the most recent bulb failure, both bulbs failed on the same day - and only became apparent that night..
The contact owns a 2012 toyota prius v. The contact stated that while driving 25 mph, smoke emitted from the dashboard and the vehicle's mileage fluctuated. The contact also stated that her insulin pump malfunctioned while in the vehicle. The vehicle was taken to the dealer where the smoking failure was not diagnosed but the contact was informed that the smart key's frequency interfered with the insulin pump. The manufacturer was contacted about the failure. The vehicle was not repaired. The failure mileage was 156.
The navigation, bluetooth & radio system was turned off on it own and became corrupted when the car dealership tried to update the systems software. The hardware was then rendered inoperable and the systems computer was then replaced, the warranty did not cover the defects in the system. The estimated repair exceeded $1000us.due to the dealerships actions in trying to run a software update, the repair costs were paid by the dealership. The failure of the computer system seemed to be known by the car dealer. Toyota was never directly contacted by car owner as only the car dealer handled the repair issues.
My 2008 toyota prius's dashboard indicators (odometer, warning signs and everything) showed symptoms of disappearing momentarily during second half of the year 2016.upon restarting, it used to come back. However, on february 18, 2017, the dashboard light won't come back even after multiple time restarting.drove to work in that condition hoping the dashboard indicators will come back but they didn't. Made an appointment with dealership to check it out on monday. Before the appointment, the indicators came back so i cancelled the appointments.when discussing the problem with a friend, i was told that toyota is aware of this problem and have extended the warranty for such repairs.upon hearing this, i called toyota customer service to inquire and was told that the extended warranty is expired and can't fix the problem under warranty.driving without the dashboard lights is a safety issue that toyota shall address for all affected vehicles.
Toyota prius v 2012 lost power as i was driving out of the parking lot. I had to make emergency stop on the side of the road because inverter/electric motor failed. Mechanic ran computer test and found that problem codes indeed pointed to failure of inverter.i called local dealer mccord toyota and was told my car does not fall to extended warranty of inverter and did not offer any help. Same day i called toyota customer care at 1-800-331-4331 and they told me that my car does not fall under inverter warranty based on my vin number, and gave me case number 2005072419. I was able to call my insurance company to arrange towing from my location to repair shop. Repair shop completed work and vehicle is now in operable condition. I feel lucky that i was not on freeway and did not have my children with me when it happened.
Vehicle was serviced for safety recall j0v.after service, in car electronics systems did not work properly: navigation, bluetooth/phone integration, audio system.dealer refused to acknowledge that recall service was responsible.
Driving in the evening, using headlights.turn vehicle off and exit.within an hour, get back into vehicle, start, turn on headlights and they (low beams) fail to illuminate.high beams work, as do parking lights, but the low beams stopped working, and they still fail to work.driving without headlights or perpetual use of high beams is a safety hazard and distraction for other motorists causing an unsafe condition.have made an appointment with local toyota dealership for maintenance, in early feb. 2014.the vehicle is still under factory warranty, less than 36 months old with fewer than 36,000 miles.
The contact owns a 2012 toyota prius v. While driving approximately 10 mph, the vehicle stalled and failed to restart. The vehicle was towed to a dealer where it was diagnosed that the intelligent power module needed to be replaced or the software needed to be updated. The vehicle was not repaired. The manufacturer was notified of the failure. The vin was not included in nhtsa campaign number: 14v053000 (electrical system, hybrid propulsion system). The approximate failure mileage was 67,000. ...updated 07/30/15updated9/23/15
On june 5th, 2019, i took my car in to round rock toyota and they performed the 2018 toyota prius v recall fix. Since then, my mpg has gone from an average of 41 miles per gallon, which has stayed consistent since i purchased the car brand new in june 2012, down to 32 mpg. I took the car back to round rock toyota for a diagnosis on sept 30, 2019 and they said that they cannot find anything wrong, there are no error codes, and that my now greatly reduced 32 mpg fell within toyota standards. I am writing to file a complaint that their 2018 recall for the toyota prius v has a problem and needs to be revisited or the vehicles should be taken off the road. I am co-filing a complaint with the epa as well.
The contact owns a 2012 toyota prius v. The vehicle was repaired under nhtsa campaign number: 15v449000 (hybrid propulsion system). While driving approximately 60 mph, the vehicle stalled after the traction control warning indicator illuminated. The vehicle restarted and was able to be driven in the limp-home mode and failed to accelerate past 40 mph. The vehicle was driven to the dealer where it was diagnosed that the inverter failed and needed to be replaced. The vehicle was not repaired. The manufacturer was not made aware of the failure. The approximate failure mileage was 100,475.

My 2012 prius v started showing the warning lights for brakes, abs, anti-skid and engine lights. This happened while driving on a regular road. Brakes still work but feel spongy. Scanned for codes and it says c1391 which requires replacing all the main brake components. The cost is over $3000 because so many parts involved. There's seems to have been a recall for other models of prius but the 'v' not included. Many v owners have experienced this problem. All the parts shown in the attached picture need to be replace cause toyota doesn't know or can't figure out where the problem is. Car only has 68,000 miles on it
Was driving down the street in my prius when all of a sudden the car went totally dead.it did not go into a limp-home mode.i was dead in the water in the middle of a street all of a sudden.we had to get out and push it out of the street. Mechanic says its a known issue with prius and inverter. That was pretty dangerous and i was glad we werent on the freeway.we were unaware of any recall.the car went into "disco mode" - every warning light it had was on and blinking but the car was completely dead.
After repair of stalling engine condition: vsv valve replaced, a software update subsequently was performed on my car (jov recall). Ever sincethat software update it hasn't been safe to drive my car : unsafe breaking, really hard breaking, especially on highways and untimely dashboard lighting related to trac/vsc, check engine and hybrid warning light
After the latest hybrid software upgrade on my car we noticed that the gas mileage dropped by 10%. Then as we were driving in heavy traffic on a two lanesfour different warning lights came on two which were for the braking system and two others for the hybrid system. Took the car to the toyota shop and they found the hybrid batters needed to be replaced at 63,000 miles.

My 2012 prius v had about 3000 miles on the speedometer when i began to notice that from a start, when i would put the lever into drive it would go into reverse.it is very intermittent.i cannot make it deliberately happen.i have taken the car to the dealership over this issue twice.they could not get the car to make the shifting error.because they cannot replicate the error, they will not give credibility to the problem. This problem has occurred probably 20 times since it began happening.i am now keeping a log on each occurrence. It has been insinuated that i do not know how to shift this vehicle.after watching the how to shift video, i don't think the dealership understands the shifting mechanism.however, this did make me more conscious of my actions during shifting.i actually did catch the shifting problem as it occurred.my hand was on the lever in the drive position with the hand pressure being down.the car was in reverse with the beeping and camera on with the lever in full drive position.i am very concerned that the car will go into reverse and i would not be aware of it because i had put it into drive.this is an accident waiting to happen.
The contact owns a 2012 toyota prius v. The contact stated that after coming to a complete stop, the contact turned left with the accelerator pedal depressed however, the vehicle failed to exceed 9 mph. The contact drove the vehicle home and had vehicle towed to liberty toyota (4397 route 130 south burlington, new jersey 08016, (609)-386-6300) to be diagnosed. The mechanic located the failure at the inverter. The vehicle was repaired. The manufacturer was not contacted. The failure mileage was 95,000.

My 2012 prius v is only 90k miles. Starting from about 2 weeks ago, the warning lights including brake system warning light, abs warning light and slip indicator light were suddenly lit on when i was driving, then i found the regenerate brake become very hard and difficult to stop which was malfunctioning. These warning lights were kept on all the time when i drive the car. The car threw out code c1391 when given a diagnosis. After some online work, i found this is a very common problem for prius v. I went to the dealership however the dealership states that my car, based on its vin, is not in one of the certain cars covered by zjb phase 2 which covers brake booster and brake booster pump assemblies for certain 2011-2015 model year prius vehicles.the failure of the brake could be a deadly deficiency to all the people in the car when driving. Now i have to think to stop using this car because of the severe security risk. Toyota should take action to solve all this problem for all involved cars, not certain cars. My car is made in 2012, the diagnosis code c1391 given by my car are falling into one of the four codes associated with the brake problem covered by zjb phase 2. It is ridiculous that my car was not issued this extended warranty.
On jan. 3, 2018 while moving at a minimum speed (less than 15 mph) i applied the brakes and several warning lights illuminated (brakes, abs, traction control) along with a warning sound. This was on a city street, driving in a straight line. I was able to brake to a stop. I then attempted to continue to drive at a very low speed but soon lost all ability to brake the vehicle and i coasted to a stop. I had the vehicle towed to a local toyota dealer. I was told the brake actuator had failed and was charged $3288 plus tax for the repair. I received the car on jan. 9, 2018 from the dealer. On jan. 12, 2018 the car failed once again in the exact same manner. The dealer kept the car until feb. 1, 2018 at which time they returned the car to me at no charge, stating they had changed the same exact part as before. The car has 84,000 miles and i purchased it new. I understand toyota has issued recalls for this issue on other models but not on my vehicle. I have retained my repair invoices but am unable to upload them at this time.

From a standstill, under light winter conditions, there have many times when the vehicle will not/can not move.when the accelerator is depressed power is sent for a fraction second (very quick pulse) and then all power is cut off and nothing further happens until you lift off accelerator and try again.it does the same thing in reverse.the only way out of this cycle is to physically push the car to get it moving(slow walking pace) and then the traction control behaves very much like our 2012 camry se v6.a couple times we had our camry right next to the prius(side by side) and the camry was able to get itself moving(with tc on) and prius would not budge.both have the factory tires, the prius tires are in better shape than the camry.i know the two vehicles are vastly different and that this comparison may be of little help; i simply want to express that the conditions the prius will not function under should not be thought of as extreme, but rather ordinary winter conditions.it seems that the prius traction control parameters do not even give the car the chance to get moving before shutting the system down.

This complaint is for a 2012 prius v that i've notified my local dealer and toyota about a possible safety issue relating to the brake booster. The prius v is currently not included in the zjb customer support program and i believe it should be.brake pedal feels very firm and inconsistent and much harder to activate the brakes. There is constant (every few seconds) brake booster actuation/whirring consistent with symptoms of a leaking/defective brake booster per my local dealer. Warning lights for brake, abs, stability control, and tire pressure are on.
At least 2 or 3 times a week, when shifting the car into drive when the car is stopped, the car goes into reverse instead.the first few times i thought i had made a rushed error, so i started watching quite carefully.you can even be backing up and then brake and shift the car specifically into drive and it stays in reverse.it might take 3 or 4 attempts to get it into drive.i have realized this must be why they installed the beeping in the system to alert you that the car is in reverse, it must be a more pervasive problem than simply in my car.i always visually confirm my motions now, and 2 or 3 times a week i will enter the car and put it in drive and it goes into reverse instead.this is an accident waiting to happen.
My toyota prius v only has 121,153 miles on it and during a short drive to work where we never reached speeds over 50mphs our prius v overheated causing a blown head gasket and all sorts of combustion issues. The repairs were $3000. This is a 2nd family car which has always been maintained by expert, license auto mechanics. This should never happen on a car with such low milage! i have learned that this is a common issues with this toyota prius model and toyota should file a recall and fix the overheating issues.
Engine-engine have broken head gasket. Coolant reservoir started to drain. Coolant slowly leaks in internal combustion engine due to broken head gasket. Causing shaking engine because coolant cannot combust inside the engine.-also egr cooler was fully clogged by carbon that caused failure of carbon air circulation.brakeslowly failing brake actuator in the process. When pressing brakes, whining sound is present and brakes slowly fails to hold pressure in the system when brakes are applied.-brake system was fully inspected and no leaks were found and brake fluid holds in full level.
My 2012 prius v has several unresolved problems.on those occasions when the gas engine cranks the car, there is a loud engine rattler.the engine sounds as if there isn't any oil.turning off the engine and re-crank does not always fix the problem.the front passenger seat sometimes rattle.two different toyota service departments are unable to determine and fix the problem.one of the shops replaced one of fiftybutton like leather seat fasteners.this seemed to resolve the problem.two weeks later the problem started again.now, i suspect the front passenger doorspeaker is the problem. Occasionally, the rear driver window squeaks.these problems have been reported to service shops but they want to keep the car over night.i did not buy a new car for over night sleepovers at the dealer.finally, two weeks ago a rock hit my windshield.five days later the windshield crack grew from a tiny ding to 15 inches.i have owned thirteen new cars in this life time.i have suffered windshield cracks but none of them suggested that the glass was too thin. This is my first toyota.
Our 2012 prius v has continuously been burning oil at an alarming rate. It was put through an oil consumption test at the dealership and they claimed not to know what the problem was (just that the car was getting older and we should probably buy a new one), even though they have issued a tsb to dealerships about this problem.at this point, we are barely able to drive the car any distance without having to put more oil in. We have always kept up on the maintenance for the vehicle, we recently replaced the head gasket and the coolant in the car, but now the car just runs hot with the fan running all the time and the oil is consistently burning.we've never had to put oil into the car ourselves between maintenance sessions and now they have us coming in to change oil every 5k miles instead of every 10k miles as before.even then, we are still having to add oil in between visits. I bought the car as a commuter vehicle, it has about 200,000 miles on it, driving it 5 days per week, about 100 miles per day. Depending on the quote to fix it (from a non dealer because they won't acknowledge there's a problem), i will probably have to trade the car in at a minimal value in order to purchase a new car, which i really cannot afford to do right now.this problem is only occurring when the car is moving. There is no leak, as it would be evident on my driveway.i mostly drive the car on the highway.
It is having heavy miss fire which make the enging shaking too much many times
Engine misfiring at start. While at intersection in city and turning left, accelerating and triggering the combustion engine, misfiring causing jerking and lack of acceleration almost causing a stalled car in oncoming traffic. Check engine light on. Service says misfiring caused by faulty head gasket. Appears to be symptomatic of gen 3 prius.
Approximately 2 months ago i started having problems when starting the car in the mornings or after the vehicle sat for a while. When starting the vehicle it would shake and run rough but would run good after it warmed up. I took it to my local toyota dealer and a manifold and a sensor was ordered by the dealer. The next day i went back to the dealer to have the parts installed. I sat in the waiting area for over 2 hours only to be told it was not the manifold and sensor. After a scope was used to diagnose the problem. This time i was told i either had a leaking gasket or a crackedengine head. This prius v only has 94245 miles on the engine. It has always had regular maintenance at the toyota dealer. The dealership quoted me to replace theengine at $6000.00. I have never had a vehicle with less than 100,000 miles usage toneed a new rebuilt engine especially a toyota product. I am very disappointed in toyota. This was a known concern on toyota's part and it was not properly addressed in a recall, now i am left holding a very expensive bag. How many other unsuspecting, customers will you lose besides me
The contact owns a 2012 toyota prius v. While driving various speeds, the air conditioner was activated and there was a strong odor in the vehicle. The vehicle was taken to the dealer (toyota of renton, located at 150 sw 7th st, renton, wa 98057; (425) 228-4700) where it was diagnosed that the odor was due to mold. The dealer cleaned the vents and the vehicle was repaired, but the failure recurred several other times. The vehicle was taken to the same dealer, but they used the solution to clean the vent the second time. The vehicle was repaired, but the failure still occurred. The manufacturer was not contacted. The approximate failure mileage was 50,000.
Took in car for oil change /tire rotation , a software update was performed without my authorization from toyota on to my prius v , car went from getting 40 mpg highway to 32-33 mpg highway . City mpg dropped from 41 mpg to 36-37mpg . Dealer claims car has wrong tires which i agree, except those tires were the same tires on the car before and after the software update. Calculated the gas mileage myself before the recall and was pretty close to what the car's computer said ti was , calculated the gas mileage after the software update was done and it came very close to what the computer said.
Engine shake on start up when vehicle is not in motion , seems to be an issue with power.
My car is burning excessive oil
Excessive oil consumption started 70,000 miles. Burn oil 1qt every 1000 miles getting worse
1)intermittent regen braking failure causing brakes to become more difficult to press thus harder to stop the car in a timely manner. Usually when there is stop and go driving will the regenerative braking fail. 2) clogged egr valve causing a loud engine knock resulting in a blown head gasket.
When i brake the car shakes a noticeable type of shake and starts to make a weird stalling noise.
My pruis is burning oil at a faster rate than it should be.
It started with shaking on startup, idle and low speeds. Check engine light came on. Diagnosed with multiple cylinder misfire. Ignition coils replaced, but did not solve the problem. Subsequent analysis indicated coolant in the combustion chamber due to a failed head gasket.
Vehicle has been consuming oil to the point where the vehicle needs monthly oil changes. Was told many toyota prius models have had an oil consumption issue and that i should file a complaint.
The abs brake pumpexperienced an "internal failure" at mileage of 113k. There is a customer service notice that covers 2012 toyota prius and prius plug in for the same issue regardless of mileage until 2019, and then up to 150k miles after that. However, my 2012 prius v was not covered. This abs brake pump just suddenly failed, there weren't any leading indicators. Toyota knows it supplied defective parts for the abs brake pump. There is already a lawsuit of other 2012 prius v owners complaining of the same.however, toyota has not corrected their service notice. They won't release the details of how they can justify coverage to all other 2012 prius, and prius plug-in, but not prius v. The repair is significant ~ $3,500.also, last year my engine block was cracked and needed to be replaced. Thankfully, this happened ~ 80k miles, so it was still under extended warranty. But this failure was surprising for a toyota. I had wondered why i had to change the oil more quickly than the recommended 10k, i would change it every 5-7k, or at least have it checked. It seems they have the same issues with their 2020 rav 4s. I'm surprised they haven't fixed an issue from a 2012 vehicle yet by 2020.
The engine went stalled in the middle of the highway. Lucky enough, i was able to stop the car safely without any collusion. Brought the vehicle to a mechanic and diagnosed with excessive consumption of engine oil. The engine had catastrophic damage and needed a replacement. I spoke to piercey toyota service director ken johnson and he stated there is no active recall for the vehicle and do not believe it is the vehicle issue. I do not believe that is the case, further investigation is needed to prevent other same model drivers from a deadly engine stalling crash.
My car has an issue i first noticed soon after i bought it, and that now happens at least twice a week. The date i've entered is the approximate first date i noticed the problem occur. I describe below a more recent instance of the same behavior, the time when it felt the least safe. /// the recurring behavior is that while decelerating with my foot on the brake pedal, if the car goes over an obstruction like a rock, a rough patch of road, or a pothole, it sometimes lurches forward before resuming deceleration. The sensation is of accelerating, but even if it's only that the car momentarily stops decelerating it's disconcerting and in a few instances has gotten me close to an accident. /// the time it came closest to causing an accident was on 3/21/13, when i feared the car was going to roll into a crosswalk as people crossed. With my foot on the brake, coming to a stop at a stop sign, we went over uneven pavement and the car lurched forward while only a few feet from the crosswalk. Several people waited close to the edge of the sidewalk at a bus stop. One of them seemed ready to step outto cross the street, and i braced for the worst. /// ultimately i stopped a few feet before the crosswalk, but the situation felt completely unsafe since it involved losing stopping power while coming to a stop sign with pedestrians present.
Faulty head gasket, engine shaking on startup, coolant draining prematurely, white exhaust, low compression and power.
Car stalled on highway. Was just merging onto highway, the car stalled and died. Was able to very, very slowly pull off to the side of the road . This was a few months after the software update for the inverter. Could have been a catastrophic accident!!! got towed to a toyota dealer. They replaced the inverter and parts with an updated one and software too. There was no charge tome. However, i believe the 2012 inverter was inferior and all these models should be updated with new inverters. This is an accident waiting to happen!
My car has started to burn oil at an alarming rate: 2 quarts between oil changes at 5,000 miles.this does not seem normal.there does not appear to be a leak.
At about 100,000 miles the engine began using (burning) oil at the rate of about 1 quart every 1000 miles.now at 150,000 miles the oil usage is 1 quart per 500 miles. The oil has been changed using the recommended lubricant more often than required since the car was new. Toyota is aware of the problem and has redesigned the pistons and rings but but offers no financial help for cars off warranty. The dealer estimated repair cost is over $5000.
The contact owns a 2012 toyota prius. The contact stated that the vehicle failed to start. The contact mention the vehicle had to be given a jump start in order to start. The vehicle was taken to a dealer where the failure was unable to be duplicated. The manufacturer was made aware of the failure. The vehicle was not repaired. The approximate failure and current mileage was 17,500. The vin was not available.
A groaning noise at all temperatures so far.it begins and then goes away after i drive for a block.it is very loud and it can be heard by others around me.it is embarrassing and doesn't sound like it is healthy for the longevity of the vehicle.
Front head lights went out within a few weeks of each other.replaced both bulbs.then they went out again with no warning in about a month.had to use brights to drive home in the dark.dangerous!replaced them again and now one of the head lights is out again after only 2 weeks even though it is a different brand.headlights have gone out while driving and when it is parked.
Headlights low beam fail every month or soand i have replaced 16 bulbs in last year.this creates a dangerous situation for us.also the cost is great.i have never had to replace headlights before in other vehicles. When is a recall going to be demanded ?when somebody is killed because of this

It is having heavy miss fire which make the enging shaking too much many times
While driving in interstate traffic the car lost power and would only go 11mph.when i got to a safe place to pull over the hybrid system showed failure and several other warning lights.the car had a previous recall for power management completed in 4/2019.this is an invertor failure
The contact owns a 2012 toyota prius v. The contact stated that the hybrid warning indicator illuminated and the vehicle shook, vibrated, and stalled. The failure code was cleared by autonation toyota south austin (4800 s ih 35 frontage rd, austin, tx 78745, (512) 518-6900) before the contact received notification of nhtsa campaign number: 18v684000 (hybrid propulsion system). The dealer repaired the vehicle. Shortly after, the contact was driving and the hybrid warning indicator illuminated and the vehicle stalled. The same dealer diagnosed that the head gasket may need to be replaced. A second dealer, charles maund toyota (8400 research blvd, austin, tx 78758, (512) 270-5055), diagnosed that the inverter needed to be repaired. The manufacturer was not contacted. The approximate failure mileage was 200,000.
The contact owns a 2012 toyota prius v. While driving at a slow rate of speed, the vehicle shifted into neutral and came to a slow stop. The contact was unable to get the vehicle out of neutral after the failure. The vehicle was towed to the dealer and it was determined that the inverter failed on the vehicle. The dealer stated this was a known issue with toyota's hybrid vehicles, but could only replace it with the same model inverter that would fail again. The manufacturer would not confirm what the dealer stated about this being a known issue. The vehicle was repaired. The failure mileage was 33,634. The vin was not available. The car was parked on a steep incline while i closed a gate behind me to a gated driveway.when i got back in the car and put the car in drive, i took my foot off of the brake and before i could get it to the gas pedal the car immediately rolled back and into the gate.the car released as if it was a standard shift car that needed a clutch and slammed into the gate.i have taken the car to other safe spots that are on hills and found that this car can not hold a stopped position on an incline.
When i brake the car shakes a noticeable type of shake and starts to make a weird stalling noise.
My car is burning excessive oil

I owned this vehicle since brand new. It had happened more than a dozen times.when going over bumps or uneven surfaces, with the same pressure applied to the break, the 2012 privus failed to stop and lurched several feet before stopping.we had an incident that we almost ran into pedestrian yesterday when this issue occurs.this is very concerned to my family.we bought this vehical back to the dealer ship where we purchsed it.(expressway dorchester toyota in masschusettes)they said they could not fix it because they could not reproduced at the dealship.the service of manager claimed this is working as designed from the (tracking control).i feel this is a false statement, this does not happened to my other cars that has traction control.toyota should really starting fixing it before someone gets killed!

The contact owns a 2012 toyota prius v. While driving at an unknown speed, the position of the driver side head rest failed to provide adequate support. As a result, the contact sustained pain to the head, neck, lower back, and left leg that did not require medical attention. The vehicle was not diagnosed nor repaired. The manufacturer was not notified of the failure. The failure mileage was 31,000.
Car experienced greatly diminished braking power on interstate 75 prior to approaching heavy traffic backed up to exit.many warning lights illuminated on the dash: brake, abs, etc.toyota has already issued a warranty extension for this issue for the standard model prius, and i believe this is a safety hazard on the prius v wagon as well.
2012 prius v 68,000 miles on vehicle brake issues from the time car was purchased new from a toyota dealer.history... When braking and going over rough surface or bump, the brakes would seem to momentarily disengage and then reengage after the surface smoothed.the car seemed to lunge or skip forward and take longer to stop.alerted the dealership, they checked and said everything was fine.this has continued throughout my ownership of this vehicle. Recently during an emergency stopping action on a freeway, my prius was the 3rd car in a 3 car accident.the 1st car for some reason, slowed drastically, and was rear ended by the car in front of me.i felt i was at a good distance behind the car in front of me and i reacted and fully depressed the brakes and felt this car continue to roll and contact the car in front.the brakes did finally take hold, but i felt a hesitation which lengthened the stopping distance.i have researched and found there are dozens of reported issues with the hybrid's brake systems in both prius models as well as some reported issues with lexus hybrids.i wonder how many other incidents have occurred with the millions of prius vehicles that have not been reported.there seems to be issues with the computerized/mechanical braking integration and possible issues with the actuator.
Car only has 68,000 miles on it
I was in stop-and-go traffic. I had accelerated to about 25 mph when i needed to slow down, i stepped on the brake and at about the same time hit a bump in the road. The car not only was not braking (i was stepping on the brake at the time) but indeed seemed to accelerate briefly before the brakes re-engaged (i had my foot on the brake--depressing it--the entire time). This has happened from time to time with my vehicle--it has happened when backing out of my garage, and also when breaking while driving over road bumps or potholes. This is a safety hazard that needs to be addressed.
Slowed for red light that was visible for a great distance so coasting and applied brakes and they weren't there....i hit the car in front of me, he hit the car in front of him....mostly dings in fenders....my hood was up on one side, and front panel moved....nobody was hurtpolice came and sat in the car and applied brake and it went to the floor.he said "you have no brakes; do not drive the car, have it towed".
I began experiencing what i described to the dealer as a "hiccup" in my brakes. I complained of this problem the two times i took the car in for routine maintenance (first and next six months), and each time they found "no problem" with the brakes. It was not until this snowy rough winter with all the attendant pot holes in the street that i correlated this braking problem with uneven pavement. I know this happens when my right front tire hits a pot hole, and it may happen when any tire does, but if i'm apply the brakes - especially hitting the brakes at the moment one tire dips into the hole -they do not engage and in fact there's a slight lurch forward, almost an acceleration. The car is going to the service department tomorrow with the hopes that the problem can be located. This truly feels dangerous to drive.

Loud scraping noise coming from rear of car---had the car in to toyota 5 times. The noise continues to get louder as time passes.-- its now loud enough to drown out the radio.--toyota service says it is just rust on the rotors.--this is the 4th new toyota i have owned,-- but this the only one i have ha d this problem with.-- which leads me to believe that there is a fix for this problem.--ii was told by the service manager that toyota knows of this problem and are working a recall to fix it.---so where is the recall??????
Brake noise when pulling forward in am . Dealer said fix is coming for months, service bulletin never came out. Worried that may lead to brake failure.
The brakes booster and brake booster pump assembly is defective and needs to be recalled since it's a manufacturer issue.
Nhtsa recall number 15v449manufacturer recall: for / hybrid inverter/power module (ipm) crash involving prius v (owned by another person - my mother was hit and killed by prius v) occurred on sr 105.prius v traveling n on outside lane crossed inside lane and 11' grass median bordered by concrete into s bound inside lane and collided my mother driving oncoming vehicle on inside s bound lane. Woman driving prius v was killed on impact, and my mother died later that day at uf hospital jacksonville. Two witnesses driving behind my mother saw crash and said the prius v appeared to be accelerating across median. Crash occurred approx. 2 months before toyota's recall on july 15, 2015 of 109,000 prius v hybrids. The prius v that killed my mother was part of this recall. In feb 2014, there was a recall on the prius ecu.the pruis v had the same ecu, but toyota chose not to recall the prius v until almost a year and a half later, 2 and a half months after my mother's fatal crash. If they had recalled the prius v in 2014, would my mother still be alive?
On sunday december 17, 2017 at about 3:00 pm i was driving with my family (wife and two kids) in south florida on us highway 1. The weather was clear and i was traveling about 55 mph. Then without any warning a number of trouble indicator light illuminated on the dash. When the lights came on i was driving in a straight line with just enough gas pedal input to maintain my 55 mph speed. Startled by the lights i lifted my foot off the throttle and attempted to apply the brakes. When i attempted to apply the brake pedal i had little to no brakes and i had to 'stand on the brake pedal' (press it as hard as i could) to get the car to slow and eventually stop while pulling onto the shoulder of the road. I was scared and shocked at the same time. I turned the car off and back on thinking it might fix the problem. It did not. I was able to get the car home and the very next morning took it to south dade toyota. I was able to get it home and to the dealer by driving very slow keeping a very large space cushion and standing on the brake pedal when i needed to stop. While i did not crash understandably i was scared because you should not just lose brakes out of the blue. At the dealer they informed me that the brake actuator failed and needed replacement costing $5734.94. I did research and found that toyota issued a warranty extension zg1 for this exact problem but was told by the dealer that my vin was not part of the program even though my vehicle experienced the exact problem.i reached out to toyota cooperate and while they agreed that yes i had the same problem my vin was not part of the program (they gave me a case # 1712180647). However they did offer $2207.57 as a good will gesture towards the repair. I paid $3527.37. Toyota knows of a life safety problem and has chosen to limit the program. My mileage was 106,451 and under the program would have been covered.
Purchased prius v in march 2012, june 2012 took vehicle in for service and to have dealer look into noise coming from rear brakes. Dealer stated they could not hear the noise and saw nothing upon inspection.noise became more frequent and louder, took vehicle back to dealer and they could hear the noise.upon inspecting the rear brakes they found rear rotors had abnormal markings in them from improper machining.brake pad friction material glazed.rotors and pads replaced.after a week the noise returned.took vehicle back to dealer and was told this was a problem which toyota has been experiencing for months but has found no solution too. Was told i would be informed when a solution was found and i could bring the vehicle back then.i feel this is a safety concern which needs to be looked into.
Brake is having groaning/ grinding/owl hooting sounds.the grinding sound appears when you applied the brake, and when the car almost at complete stop, is the worst.groaning and owl hooting sound are always there, specially in the morning.in the afternoon is not as loud as in the morning, but still noticeable.been to toyota dealer and they inspected the brake and said there is nothing wrong. Also saying that the sound is normal for a regenerate brake system.but i don't agreed to it.toyota need to find a fix for it, and we don't know is the brake really safe!
I stated to the dealership on june24,2013 that the car feels like it runaway when i brake and hit a pothole in the road.they road tested my car and couldn't duplicate any abnormal issues with the braking. They visually inspected the brakes and they found nothing.there have been incidents where i brake and hit a pothole and jut forward either almost passing a red light or almost hitting a car in front of me.it is a very scary situation.i would have done this complaint sooner, but i was not aware of this website. I just happen to search to see if anyone else has the same issue with their 2012 prius v.this scary dangerous situation still continues till this day.
Brake light/tsc light on dash was on. There appeared to be no difference in drive-ability, but there was a humming noise present at idle and 20-30 mph. Took car to dealer 4/30/2019 for check. Coggins toyota in jacksonville, fl determined the cause was a, abnormal leak in the accumulator. They replaced the brake booster/master cylinder assembly. After the new equipment was installed, the technician noticed the new pump assembly had excessive noise and vibration internally so another replacement part was ordered. New pump assembly brake booster was installed and car was put back in service 5/19/2019. Total cost of this service was $2,745.11. We later learned that a customer support program csp zjb was issued by toyota for 'certain" 2010 phd prius, "certain" 2011-2015 prius, and "certain" 2012-2015 phv prius. As per the csp zjb mailing to our relative who also owns a prius, the problem was "with the brake booster and brake booster pump assemblies". According to dianne, customer service rep with toyota, these vehicles included in the csp zjb were produced between october 2010 to november 2015 and included some 946,100 vehicles. My vehicle was not included in this program as the vin number did not match the affected vehicles. However, we had the same problem with our vehicle that the program was initiated to include. Upon submitting a request for reimbursement from toyota for the cost associated with repairs, it was rejected. We believe the problem for which the service program was initiated is the same problem we experienced and our vehicle vin should be added by toyota to the list of affected vehicles so that it becomes eligible for expense reimbursement.
The contact owns a 2012 toyota prius v. While driving at any speed, the brakes made noise and the abs failed. The vehicle was taken to venice toyota (941-451-5150, located at 900 u.s. 41 bypass, venice, fl 34285) where it was determined that there was no recall. The manufacturer was not made aware of the failure. The vehicle was not diagnosed or repaired. The failure mileage was approximately 110,000.
The master brake cylinder broke down on my car.the warning lights finally came on however the issue was noticeable before that as the car braking at low speeds created slight stuttering stops when coming to the end of a complete stop plus more importantly, we noticed braking was getting more difficult at certain times. This issue is creating a cost of over $3000 to repair.numerous others experienced this same issue with the previous prius model (gen 3) and toyota acknowledged it and extended the warranty to those models but nothing seemed to been have corrected with the prius v.
Brakes release and car accelerates when braking over potholes or uneven pavement at low speeds.
My 2012 prius v has been making a groining, howling noise since i purchased it. Noise comes from front and rear brakes. Has taken the car for repairs but they have not been able to fix it. It is very annoying and can be very loud. People inside and outside the car can hear it, so its quite embarrasing. Noise comes and goes its not constant. Toyota says they are aware of the problem but have no repairs at this time. I drive my family in this car and its scary to know there is an abnormality with the brakes. Beware the dealer will tell you to follow through with a third party arbitration service. I did and my case was denied.
On multiple occasions when braking if i hit a bump in the road or pothole the cars speeds forward for a few feet. It feels like the brakes release causing the car to stop slowing down. This is very discomforting. So far i have not been in any accident though i fear that this could contribute to an accident if i had to stop suddenly.
When you start to drive it makes a loud howling sound, it is getting louder as time goes on.people look at us like we are crazy for driving with bad brakes.we have had it in to the dealer many times to just be told it is the way it is.no repair is necessary.it is getting more and more annoying to listen to.it lasts longer as time goes on.come on toyota lets get this taken care of before there is a fatal collision due to this!it is getting scary to drive and feel safe.
Takata recall. Right after recall no 18v-684 update system on my prius at local toyota dealer my car brakes pedal become veird . When i just start on the engine thn start moving and stoping, the brake pedal is really up and not moving down and brakes car really quick. At the same time when i start driving i hear that rear writing brakes for about 1-2 miles. Then it come off and brake pedal goes down little bit to normal position and works great. This is happen every time when shot down engine and start again to go somewhere. Its really dangerous bacous the position of brake pedal sometime up sometimes down, can not predict how soon you can stop when its needed. It always happen now when i drive in my city roads. I came back to dealer that did update system on my car, they said that i need to pay to find out what is going on with my car, they will charge me even if this happen after theire recall update system on my car. This is horrable.
There are two things that occur with my brakes. One is a consistent and very loud groan with braking after any length of time sitting idle. The other is when the brakes are applied and i may be over a bump in the road or a pot hole or rocks the car lurches forward quickly as if i have lost control of breaking. Almost like hydroplaning without water present. I have taking the car to the deal consistently since the noise began and yet to have a fix for it. They say there is nothing wrong with the brakes. However i specifically had someone sit in my car to hear the groan because they said they were unable to duplicate it. They heard the noise and again could do nothing to make it stop.
My abs actuator has completely failed. I was getting code c1391 when i first had the problem diagnosed and recently it finished failing completely. I was on the highway when it did and i severely lost brake pressure and almost got in a car accident because of it. This issue was recalled on regular prius hatchbacks but because the prius v's abs pump system is slightly different to compensate for the different weight of the prius v, the prius v doest fall under the extended warranty to have the issue fixed even though it is the exact same issue that the regular prius' have been having which they extended the warranty on. Im extremely frustrated because i have called my dealership as well as the toyota helpline to report this and they say that theres nothing they can do for me and that my car is just not covered under the extended warranty because the warranty didnt apply to prius v's even though its the same part having the same issue. I've been quoted $4,000 to have this fixed and i can't afford that right now.
The brakes failed while driving. All the abs, brake lights and a continuous beep sound turned on. I almost had a crash. It was the brake actuator pump and i read about multiple owners had complains with it.
My 2012 prius v with only 55,000 miles. Abs lights came on and buzzing sound every 3 seconds from just in front of steering column. Toyota dealer charged $270 for computer diagnostic then gave me a parts and labor estimate for replacing brake booster and master cylinder of $4400!!! independent mechanic can order parts from another toyota dealer in mainland us for $1400 less than the $3200 that dealer in hawaii quoted and labor for much less. But still will be costing me about $2500.searched recall sites and regular 2012 prius models had recalls for exactly the same issue but no recall on 2012 prius v brakes?
The contact owns a 2012 toyota prius v. The contact stated that while driving at various speeds, the abs warning light illuminated and the brake pedal failed to operate correctly. The contact coasted the vehicle to the shoulder of the road, and depressed the brake pedal extremely hard for the vehicle to stop. The failure occurred intermittently. The vehicle was taken to bryan college station toyota (728 n. Earl rudder fwy, bryan, tx 77802, (979)776-0404) where it was diagnosed however, the mechanic was unable to duplicate the issue. The technician changed the oil and one week later the failure recurred. The vehicle was taken back to the dealer where it was diagnosed a second time and it was determined that the brake booster was faulty. The vehicle was not repaired. The manufacturer had not been notified of the failure. The failure mileage was approximately 220,000. The vin was not avaiilable. ## vin failed ## 2012 toyota prius ##
Brake booster slowly failing....when pressing brakes it doesn't smoothly hold pressure
November 4, 2017- car was parked in our driveway. Put the car in reverse to back out of our driveway. The brakes failed. No matter how many times the brake pedal was repeatedly pushed down, the car just would not stop. It actually felt like the car was accelerating!the car ran over our mailbox at the end of our driveway. The mailbox got dragged under the car and the car rolled out onto the street. Finally, we pushed in the power button and that was how we were able to get the car to stop. Our mailbox got totaled and damage was done to the undercarriage of our car, as there is twisted metal hanging down scraping the ground. The brakes started to work again a few minutes later, as i was able to pull it back into the driveway off of the street.on september 19, 2017 ' car was parked in a parking lot. When reversing out of the parking space, the brakes did not work. After repeatedly pumping the brakes, they finally engaged just before it hit a car parked behind us. We thought this was a freak one-type event, so we didn't report it.
Unlike other 2011-2015 toyota prius vehicles the prius v is not covered under toyota's csp-zjb customer support program. My 2012 prius v has all the indications of this brake booster and pump problem... Multiple lights on the dash (brake, abs, pcs ..... On; codes c1252 & c1256; and brake boost pump running every 6-8 seconds plus a brake pedal that you can push to the floor in 4-5 seconds. In hard braking i've had to pump the brake pedal to get stopped. It's my wife's car but she will not drive it anymore. Took the car to a toyota dealer who was aware of this problem and started to have it fixed at their expense until he put the vin # in his computer. His estimate was $2500+ to have the problem fixed after i paid to have the problem diagnosed even though he knew what the problem was. I declined and called toyota customer service which was a waste of time...sorry, your car is out of warranty and your car does not have a problem we will cover. On the net, there are multiple prius v owners with the same problem that started in 2008. Until someone with a bigger stick (nhtsa), toyota is saying there isnota problem with the prius v brakes.
Brakes sometimes grab very sharply when touched while in motion, causing forward momentum of body to slam on brakes. Almost caused accident when i touched brakes with someone tailgating too closely.also traction is interrupted when hitting the metal abutment to bridges.
Absbrake is not working. While i was driving on a city traffic and come to a complete stop at the traffic light, the brake pedal went deep when i step on it and it took me more time to stop than i expected it. I went and got it red the code c1391 abnormal leak in accumulator.
11/15/19 originally the car showed brake, abs, antiskid and engine idiot lights went to the dealer with a code c1391. They said i need to replace the master cylinder and the power booster for $2600. Then they said it was policy to also replace the brake booster pump assembly for an additional $2,000 or a total of $4600. After i complained that they had recalled 972,000 other prius but didn't include the prius v they said i only needed to replace the master cylinder and power booster and said they would do it for $1,516.89. So then i go to take the car to the service department today 12/4/19 and while sitting there the idiot lights all turn off and the brake start to function normally own its own... I ask the service representative what his thoughts were and he said i should still make the repairs, but he couldn't say what they would do it came back. So i'm going to drive the car for a week to see. While i'm waiting can you please do a recall on the brake pump assemebly since these folks dont even know what the heck is going on with this car. It appears its not either device and its a computer glitch that they haven't resolved...help us prius owners please!!!
1)intermittent regen braking failure causing brakes to become more difficult to press thus harder to stop the car in a timely manner. Usually when there is stop and go driving will the regenerative braking fail. 2) clogged egr valve causing a loud engine knock resulting in a blown head gasket.
Driver was making a sharp left turn into a parking space at low speed and attempted to brake to a stop. The vehicle did not slow down at all despite hard pressure on brake pedal but instead accelerated, jumped a curb in front of the vehicle and hit a wall which stopped the car. No one was injured but front bumper badly gouged and front door slightly dented. Driver was able to put the car in reverse and get off the sidewalk. Vehicle is now at toyota dealer for inspection.upon reflection 2 other incidents occurred where what felt like a momentary speedup occurred again at low speed while turning and braking. But these 2 were while moving forward and not attempting to bring the vehicle to a complete stop.
I bought prius v in february 2012.i've noticed few times issues with brakes.while decelerating with my foot on the brake pedal to slow the car to a stop and if the car goes over a pothole or slight bump on the road then car accelerates forward before coming to a stop.this is safety hazard since it almost led to hitting the other car's rear bumper.also, i've gotten into an accident in december 2013 where there was another brake issue.i stopped my car in the incline road by putting my foot on the brake pedal.then, i changed the gear shift to rear and slowly took my foot off the brake pedal so that i can back into the street parking which was only about 4 to 5 feet away.suddenly, car sped down the road going rear direction to the directly opposite street and hit two parked cars.i had put the foot down on the brake pedal when i noticed the car going rear fast but it would not stop the car from going down the road.i had no control over the car or the speed.this is major safety hazard since someone could have been seriously injured with losing control of the brake and acceleration.i've owned other toyota car (camry) before but never had this issue until this prius v.my car has been in body shop for over a month with repairs.i plan to take the car to the toyota dealer to check out this brake issue.
Brought car in for recall j0v00d and immediately afterwards the abs,traction-control, brake light came on. The lights went on and off and then stayed on. Went back to dealer and was told it needed a $3800.00 repair.was told that certain vin numbers were affected, but not my vin#. Not sure how this is possible since i am reading all over the internet that this is a problem on this year car also? i was driving on highway us 19 in clearwater, when this all happened.
Complete brake failure on the freeway!!! abs, brake and traction control lights lit up. Almost crashed! code c1391-abnormal leak in brake actuator pump.how long will it take this agency to make toyota fix this?? its a safety issue!!! stop picking your asses and do something about it!!!!
Brake activator unit failed after 4 months of reporting the issue to toyota. It follows the conditions indicated on toyota wen zg1. Vehicle was in motion at highway speed when braking malfunction occurred. Vehicle was controllable and allow for secure stop. Brake response was sluggish and hard on the foot.
The actuator has been making noise for the past year . I took it to the dealer about feb 2019 and since my warranty had just expired the dealer would not fix. And today it's been worse . Every time i am at a stop it makes tons of noise.
The brake loses its grip over a rough road bump while pulling in for a stop. Very very not safe, i almost rear ended a car in front of me a couple of times. Creates extremely unsafe feeling. Brake loss happens all the time over bumpy road only.
My entire breaking system went out while driving to work on a busy road, where i had to pull over to the side to see what was wrong, as i had never seen those five lights light up ever, as this is not a typical issue that would ever break without a manufacturing issue. I was never notified of a specific issue with certain 2011-2015 prius vehicles that are involved in a customer support program zjb. I have now paid $100 to toyota to diagnose this problem, which was not necessary, as it is obviously the exact issue (5 lights luminated) from the customer support program, and they have told me that i was not notified of the issue because my vin is not included in the program. It has been diagnosed with the exact break booster issue listed in the letter sent to other vin numbers. I drive children around in this car, and this issue is severe and should clearly be for a larger number of cars than they are admitting. I have now been told that they willnot pay for the $3,000 in repairs because it is not included. This is unacceptable. This is a safety hazard on the road and unbelievable that i would pay extra money for a toyota because it is supposed to be a safe and reliable car, only to find out they will not admit when they have a widespread break manufacturing issue with their vehicles. My only recourse though toyota i am told is to make a case number and if my vin happens to be added to the list, they may reimburse me. This is unacceptable. Toyota needs to recall all vins for this issue, so consumers are aware that their breaks may fail at any moment.
My 2012 toyota prius v (mileage:55155) has had a failure of the rear brake pads and rotors due to excessive rusting.please note the front brakes are in great shape and show very little wear.the service manager was quick to point out that he was only a franchise store and was not responsible for the design of the rear brakes. He stated that this happens occasionally on the hybrid cars they service and if the manufacturer had coated the brake (i believe he was referring to the slide area of the caliper where the brake pads rides) that the brake pad would not have hung up and worn out along with destroying the rotors.i am a former licensed aircraft mechanic and a licensed engineer with the state of illinois.clearly there is no reason for the rear brakes to fail before the front brakes and certainly not the rotors.this car has been serviced by the dealer every 5000 miles, which demonstrates the failure was not due to neglect.this failure mechanism needs to be addressed before it causes an accident.
While pulling into a parking space (speed less than 5 mph) and applying the brakes the pedal went to the floor and the car seemed to speed up. The car jumped the curb and struck a tree.a good samaritan assistedand actually got in the car and found the brake pedal went to the floor. Shortly after the accident the brakes returned to normal operation. This is the second time that the car has experienced similar problems. In 2014 while pulling into the driveway the brake pedal went to the floor and struck the house. Shortly after the brakes returned to normal operations. Toyota was notified and inspected the car's computer and braking system per case number 1410012557. After the inspection toyota said that there were no brake issues found with the car and stated that it was safe to drive. Obviously there is an issue with the braking system in these cars.
A recently purchased used toyota prius v which was owned by a dealer a matter of 2 months ago was sold to me and the abs/brake actuator failure began. The vehicle is outside of the 150k/10yr warranty they want to cover, however is extremely dangerous and has already nearly caused me to be in an accident as the brake power massively fluctuates.i was driving on the highway at the 75mph from phoenix to tucson approx 120 miles. The light came on part way through and suddenly the brakes went from being sensitive and responsive to needing a lot of pressure applied to properly slow. When a diesel big rig swerved in front of me to avoid an obstacle that i would easily have been able to brake from typically, i was nearly slammed into the back end of him with very little power.the light flipped back off about time i made it to tucson, then when coming to a stop at a dangerous intersection on my way through city streets home crossing kolb and valencia (a site of many an accident), i nearly went into the intersection when the brake system suddenly cut out again.when i spoke to the local dealership this morning they told me this was a known problem but since it was out of warranty i'd have to cover 2500 to replace what is a known defect with this vehicle. That's [xxx] and dangerous.redacted to protect personally identifiable information pursuant to the freedom of information act (foia), 5 u.s.c. 552(b)(6). *cc
On 11/3/2020, several warning lights appeared on the dash.after researching the combination of lights and sounds from the brake pedal i determined that the brake actuator is failing.per my research, continuing to operate the vehicle can result in a loss of braking assistance.other toyota prius models are covered under extended warranties (reference zjb), but the prius v models, which use the same actuator, are not.an accident/incident has not yet occured, but this seems like a huge case of negligence on toyota.
When driving on uneven ground, like pot holes, the brakes fail. Literally if you have your foot on brake when driving over a pot hole, you can feel the brakes release and the car won't stop. I've had similar issues on bumpy ground. The car has always had the problem since i purchased it, probably on a weekly basis, and i've wondered about the safety and whether it's just related to abs. I've also inquired at the dealership. Luckily, i've never been in an accident because of the problem.but, this weekend my friend with a prius had an accident because of the problem, so i decided to report my complaint.
Since i first bought the car in 2012 i noticed that every now and then when i pushed down the brake pedal to slow down for a stop, if the car hit a slight bump in the road, the car would momentarily accelerate instead of braking. It felt like i was losing control of the car. (note: since i did not record the dates, please note that i have chosen august 1, 2017 as a random date just so i could continue on to the next step but this has happened from time to time since 2012.)
While driving the brakes went out. I brought the car to toyota and they said the brake booster is defective and needs to be replaced the price to do so is about $3,000 they also said they don't know why this happened as in some cases you never have to replace a brake booster nor master cylinder
Took car in for service at 40,251. Was told brake pads are at 1mm for both rear brake pads, and that rotors were rusted. Told for safety it should be replaced at this time. There had been no indication that brakes were faulty. No warning lights, no noise, no change in braking sensation. Prior service at 35,397 indicates brakes in good condition with both rear pads at 7mm. Also, no mention of rotor rust. Service done at 30,109 also indicates brakes in good condition with rear pads at 7mm.multipoint inspection paper for 40k servicewas not attached to bill. Requested inspection paper for 40,251 service, and all other inspection papers. Car was bought at warren ohio toyota, and was always taken there for service. Was told no inspection papers are kept longer than 6 months, and they could not provide anyinformation about this carsprevious inspections results. The most recent inspection paper was mailed to me. It indicates the rear brake pads were at 0mm and 1mm. A 0mm pad. There was nothing that occurredduring drivingto indicate the pad was down to metal.i have contactedthe warren ohio toyota general manager and service manager with my concerns over this rapid decline in the braking system with no warning in less than 5,000 miles of driving. Most of the drivingi do weekly is highway miles. I have not received any response from the general manager orservice mangerdespite repeated calls and emails. This rapid decline in my braking system with no warning is a serious safety concern. Either my2012 prius v has a faulty braking system, or the brakes were fine andtoyota of warren ohiolied about the condition of the brakes.

Steering column broke. Driving out of a parking lot. Heard and felt a loud clunk. Thumping feeling when turning steering wheel. Was told by dealer that the steering column failed. Only 130,000 miles. Should never happen on any car.
The steering column was a bit loose when i bought the car used from a rental office at i 95 toyotabrunswick georgiait has gotten worse at 130000miles about 90% highway they tell me it will cost $1100 dollars to replace the whole column.
The contact owns a 2012 toyota prius v. The contact stated that the power steering was inoperable. While driving, the steering wheel became very loose and the vehicle went in any direction when the steering wheel was turned. Also, the vehicle shook when turning. There were no warning indicators illuminated. The dealer and manufacturer were not contacted. The vehicle was not diagnosed or repaired. The manufacturer was not made aware of the failure. The failure mileage was 178,000.
Vehicle in motion when steering column adjustment released and would not relock causing the steering column to move forward and back, up and down. The car was difficult to drive because the steering column would not stay fixed in one position. Took the car to the dealer where they removed the steering column and reinstalled a bolt that had come out of the steering column and reinstalled the bolt with thread lock. Cost of repair was $532. I believe this is a safety issue and other models should be checked for the loose bolt.
I felt a loose noise behind the wheel of the car.took the car to service where i had bought it and they certified that the steering shaft needs a replacement @ 47,000 miles.prius models from previous years have a recall for this issue.the issue is not solved, yet they continue to have the defect and no other recall has been issued for the same problem.the repair estimate is around 500.00i have had about ten cars in my life and in none have i ever had the worry or not even the sound of a loose steering shaft.we are talking about the steering wheel...and i have a family of 5.
Noise ("clunking") when turning left & right as if there is "play" or something is loose somewhere in the steering system. Worried about safety and understand there were previous recalls of the shaft on previous year models. Do not want to replace the entire steering shaft as dealer suggests if not necessary.
I was driving, trying to park my car, when the steering (power failed) it took all might strengthen to turn the wheel. The car was serviced by toyota regularly - there were no previous signs or indicators that any problem existed. Steering system failed; had the car towed to a toyota dealership. At 50,000 the toyota dealership said the steering column failed, not getting any electricity to the steering column. Needed a new steering column.
I am filing a claim to notify other consumers about an ongoing issue i've experienced with my 2012 toyota prius v. We purchased the car in january 2012 and since then we've had 3 incidents (that we know of) in which rodents have infested our vehicle..the first incident occurred (august 2012), we brought it to the dealers attention and were told there was nothing they could do but we could contact toyota headquarters. The dealer determined that the rodent had climbed in to the rear of the vehicle through an air vent, and chewed the wires of the rear harness system (the wiring system that affects the hybrid technology). The estimated repair was $2 - 3k.after contacting toyota headquarters, i was told toyota does not consider this to be a defect in the design of the car or a safety issue. I was told it was an environmental issue and i needed to do something to correct my surroundings. They advised that i open a claim with my insurance provider to cover the costs for the rear harness wiring damage.after researching this, it appears there are 6 openings/gaps within the vehicle that rats can get into to access the interior cabin. Neither toyota or the dealership offered to block/cover these vents to prevent any future attempts by a rodent getting in. They didn't offer any suggestions or advice as to what i should do to prevent this. This morning (april 27, 2013) we woke up and realized the rodents had entered the vehicle again, this time chewing up my child's car seat and the leather of the rear seats. There are numerous people that have experienced this issue dating back to 2007 and yet toyota has done nothing to repair this design/safety flaw. Following is a link to a prius forum where prius owners complain of this issue:http://priuschat.com/threads/toyota-claims-not-aware-of-mouse-problem.113690/ .
I was completing parking the car in a head-in parking space in a parking lot.with my foot on the brake, the car experienced unintended sudden acceleration.the car jumped a concrete sidewalk, crossed the 6-8 foot wide sidewalk, and crashed into a metal security fence.
The rear hatchback latch is weak and the latch assembly needs to be replaced. This prevents the rear hatchback door from being opened or making it difficult to open. It will cost just shy of $700 to fix.of the four priuses my family has owned (prius 2005 and 2 prius 2007 and prius v 2012) have had issues with the rear hatchback latch.toyota please fix this problem.
A groaning noise at all temperatures so far.it begins and then goes away after i drive for a block.it is very loud and it can be heard by others around me.it is embarrassing and doesn't sound like it is healthy for the longevity of the vehicle.
Car only has 68,000 miles on it
On june 5th, 2019, i took my car in to round rock toyota and they performed the 2018 toyota prius v recall fix.
Car suddenly had no pick up. There seems to be problems with the engine accelerating/switching gears. This was extremely dangerous while i was driving. Also if just had the car inspected a few months prior.

I live in an apartment complex in illinois, and i do not have a garage.i think that some, if not all, of my tires were stolen overnight while i was sleeping.i do not have a car alarm.i bought 4 new and very inexpensive tires today.the tire theft could have happened in my parking lot, or at an auto-body shop like ntb while i was getting an oil change.i think it was the ntb in dekalb, illinois.i am not 100% sure if this is where it happened.i am not sure when it happened.my tires were so bald, that i almost landed in the ditch.can you please start a dot tire identification number registration form online via your website?it would be easier to track tires bought by honest and hard-working middle-class americans.also, are all 4 dot id numbers supposed to be the same if you buy 4 new tires all at once?the dot id number is located on every tire approved by the nhtsa.i would upload pictures of all 4 of my new tires if that was the option.please help honest people save money and time!mc from illinois.

The brake booster/acctuator has failed on my prius at about 76000 miles and i bought it used with 35000 miles. I have found out that this has been an issue with prius vehicles since 2006 and enhanced warranties have been issued on models from 2006 as well as 2010-2015. I have tried to call toyota to have them fix the issue but they say that my vehicle isnt covered under the warranty for some reason. What they said was that my parts maker wasnt covered under the warranty but if its a problem that has been prevelant in other prius vehicles, i dont understand why this wouldnt be covered regardless of who made the part. It has affected my braking randomly and has almost caused me to have an accident a couple times already. When diagnosed i got error code c1391 which was covered under the warranty enhancement letter i found yet toyota will not fix my vehicle.
Safety risk that vehicle could lose power and stall. Manufacturer recall number j1v. Nhtsa recall number 18v684 as of october 4, 1918.it is now november 1, 2018.called the dealership this week and the report was that the manufacturer has not fix to date.question:what relief is available until fix discovered?free use of car provided by dealership?
When the vehicle hits a bump, rock or uneven road while decelerating, the vehicle suddenly accelerates. It feels like when you skip a step. I have had the vehicle since december 2012 and this has happened enough times that i do not think its fortuitous. The sudden acceleration makes the car travel a couple of feet. Updated 12/5/2014
The contact owns a 2012 toyota prius v. The contact stated that while driving approximately 10 mph and decelerating, the vehicle erroneously accelerated after hitting a bump in the road. The contact crashed into the preceding vehicle. There were no injuries associated with the crash. The police were not contacted. The vehicle was later taken to the dealer who was unable to diagnose the failure. The manufacturer was not contacted. The failure and current mileages were approximately 9,700. The vin was unavailable.
Engine shake on start up when vehicle is not in motion , seems to be an issue with power.
When i brake the car shakes a noticeable type of shake and starts to make a weird stalling noise.
I am beginning to notice more and more tiny specks evenly distributed on the windshield that appear to be in the material in the middle of the windshield because no amount of window cleaning can make them go away. The specks are very distracting when traveling into the sun and i believe are a safety hazard.
2012 prius has serious defect in the windshield lamination. Bright light sources are tripled at night.a ghost image appears above and below the original light source.light sources may be, headlights, taillights, signal lights, and even reflected lights off certain road signs.as you approach the light source, the two ghost images will merge into the primary light source.the safety hazard is extreme eye fatigue at night, and severe visual distortion that is directly related to the brightness of the light source.
When operating the prius v on a sunny day, the reflection of the side dash panel blocks the view of the side rear view mirror.the reflection occurs in both the right and left side glass.this is a safety issue and has been reported to the toyota dealer.their response was they were aware of the problem and it was normal!this safety issue has also been reported to toyota.they assigned a case number and responded the problem was known and their was no known repair.they also stated the issue has been classified as a design issue. This is my first, and last, toyota. This issue could be normal for a toyota, but it is unacceptable for me as it is a safety issue and must be fixed.i inquired if i could purchase black panels to minimize the reflection and was told the panels were only available in light grey and light bisque (both very reflective). Updated 8/12/13
I was completing parking the car in a head-in parking space in a parking lot.with my foot on the brake, the car experienced unintended sudden acceleration.the car jumped a concrete sidewalk, crossed the 6-8 foot wide sidewalk, and crashed into a metal security fence.
