The front end of the vehicle was severely damaged, and the air bags did not deploy. The driver sustained neck, shoulder, and chest injuries that required medical attention. A police report was filed. The vehicle was towed to a tow yard. On several occasions, prior to the failure, an erroneous warning message displayed while driving, indicating that the vehicle was driving too closely to the preceding vehicle, even though another vehicle was nowhere in the vicinity. The contact indicated that the failure occurred on multiple occasions, including shortly before the crash. The manufacturer was notified of the failures. The local dealer was not contacted for diagnostic testing or repairs. The failure mileage was 1,500.
I am filing a complaint against tesla regarding the mcu infotainment on the model x. My vehicle is 2 years old and began to suffer mcu failure after upgrading to tesla's hw3. The mcu failures has resulted in severe lag, low vehicle startup. Vehicle start up has taken anywhere from 10 minutes to 20. Recently, a family member was required to be driven to the hospital. Due to the mcu causing my behave slowly, 911 was required to be called for an ambulance. The mcu failure is severely effecting medical emergencies. Tesla should upgrade all mcu 1 vehicles to the mcu 2 for free. .

The contact is original owner of a 2018 tesla model x. The contact has experienced multiple instrument cluster (ic) failures while driving where the ic goes blank. Contact states that when the ic goes blank the speedometer and turn signals are not present, and it is not possible to assess the speed of the vehicle or signal lane changes. Contact provided the following incident dates: 29-jun-2018: instrument cluster (ic) goes blank for 20 minutes on hwy. Contact stopped to charge vehicle and ic came back on. Contact called tesla service (52 derwood cir, rockville, md 20850, 301-738-6523) and was told to reset vehicle. 28-sep-2018: ic goes blank for 20 minutes while driving on hwy. Contact stopped to charge vehicle and ic came back on. 05-oct-2018: ic goes blank for 15 minutes on while driving local road. Contact called tesla roadside service (1-877 798 3752) and was told to reset the vehicle with foot on brake. Ic came back on after reset. 07-nov-2018: ic goes blank for 5 minutes on while driving local road. Ic came back on after contact performed reset 12-nov-2018: vehicle serviced at tesla service (52 derwood cir, rockville, md 20850, 301-738-6523). Ic was not repaired nor replaced. Contact was told to reset the vehicle to correct problem. Invoice [xxx] attached. 12-nov-2018 (continued): contact submitted a photograph of blank instrument cluster (ic) for tesla serivce review. On the tesla service invoice, the contacts concern is mischaraterized as ā€instrument cluster rebootsā€¯, when the contacts actual concern was that the ic goes blank while driving. The contacts experience is that the ic does not automatically come back on (reboot) and has been repeatily told by telsa to reset the vehicle to get the ic back on. The contact questioned tesla service employee adam higbee about why the ic was not replaced. Adam told contact that the ic goes blank because the media control unit (mcu) is unable to communicate with the mcu. He said that they updated the firmware, and a vehicle reset can fix the blank ic if it happ
The central display unit/media unit (mcu) had more and more frequent reboots, freezes, blank screens.when it occurs, my autopilot will stop function, traffic information will not work, radio/speaker will be silent, no turn light signal clicking is audible.the manufacturer refused to fix the problem, giving all types of excuses.
While driving on interstate highway at 75 mph, the instrument display and muc display both go blank. The cruise control did not disengage. No display of vehicle speed, rear camera or other parameters. The display did not return after approximately 2 minutes. The driver then instituted a reboot while still driving. After another 1 to 2 minutes, the instrument panel began to redisplay information. The muc subsequently displayed information. This is a first time event for this vehicle.
All screens in the car shut down while driving and there is no access to your speed, blinkers, control of lights, control of car safety features, wipers, etc.tesla was notified multiple times and each time they tried to correct the issue, it was not resolved.it has been a year since the issue started and has happened over a dozen times, resulting in unsafe driving conditions.tesla keeps stating they "remotely diagnose" the problem and told me to keep an eye on it and see if it happens again, although they told me less than 2 weeks prior to call immediately if it happened again.they have refused to fix the problem and it has become a safety issue with driving the vehicle.

Main touchscreen failed while reversing the car leading to loss of rear view camera. Car was being back out from a crowded parking area after an event risking the health and safety of pedestrians and other vehicles. Failure was sudden and complete without any warning signals. Failure has happened at least twice in the past 2 months, and 2-3 times before that since 2018. It seems to be getting more frequent. Dealer claims this a "software issue" that engineers are working on
Driver distraction issuewhen i bought the tesla model x, basic voice commands were available -- e.g., to call using a smartphone, linked to the car via bluetooth. The operation, as new, was straightforward, and matched the then-current manual, available in 2018.as is known, tesla updates its car software, &, in fact, makes it mandatory that the car's sw be updated, or risk voiding the warranty.normally, the new sw updates improve the operation of the car. However, starting in december 2019, the sw updates, & each tesla 'owners manual' that corresponds, fell out of sync. The manual would describe functionality, and the actual software, simply did not have the functionality. I've read each of the updated "model x owners manual north america",(january 2020 & october 2020), downloaded from the tesla website. In each case, the car fails to conform to the operational details explained at, e.g., the october 2020 (2020.44) manual, page 53. On that page, tesla explains, twice, that a way to activate a voice command, is to 1. Press the voice button;2. State the voice command, e.g., "call john doe"3. Press the voice command.twice, on the same page 53, tesla wrongly instructs the driver to 'tap the voice button again' or 'press again'.in hundreds attempts to make the model x to respond to 'bookend' presses of the voice button, it has failed to operate in the manner described in the manual, since december 2019.however, the car does respond to the following sequence of inputs, which the manual also describes:1. Instrument cluster in the car froze for over ten minutes along with all audio in the car.displayed speed as 67 miles per hour for the entire time despite accelerating and decelerating. Instrument cluster including speedometer did not work until reset in the car.

Tesla model x dies two times.1) at apx 1k miles (within 3 months of new purchase of new vehicle, 1st owner), it died on freeway with no power and stopped. Due to spacing from the rear card, no accident occurred and hence no fatality. Tesla service center found nothing with this issue. No resolution on diagnosis and or did not care safety of the driver and passengers.2) at 9k miles identical issue but with an additional issue of model x going in forward directions when it showed revers on the instrument panel. Everything in car was frozen only option was forward direction at low speed but with reverse mode sign!! i had not other control over the tesla model x. I have taken video at this time for an evidence.tesla states that they did not find any issue and down loaded display driver again!! i find it an excuse and tesla does not understand seriousness in safety issue. Me and family is afraid to drive this car after two incidents.
I heard that nthsa is launching a investigation into investigating tesla for a update that the company released to reduce the range of the vehicles to address a battery issue. I want to let you know that i have experienced the issue on 2018 tesla model x. I used to charge the vehicle at 90% for my daily commute and it used to show me 208 miles as the range. But ever since the update its showing me 196 as the range at the same percentage of charging (90%)
Central console screen stopped working for a few minutes. During this time frame, there is no way to open the rear doors of the vehicle. The only recommended option from the manufacturer is to remove the speakers on the rear door panel and then pull on emergency release. There is no way a passenger can remove the speakers without tools. This can lead to a safety issue, were the rear passengers have no mechanical way to open the door to escape the vehicle. Similarly when the screen is not responsive, there is no way to activate the rear defogger or defog the front windscreen. This situation drastically reduces visibility and can lead to an unsafe condition on the road. When the front display is blank; the driver gets no positive feedback whether a turn signal has been engaged - no visual indication or audible clicking noise. When this happens the driver is unable to change lanes safely; not knowing whether blinkers are on or off. When these conditions happen vehicle was in motion in both freeway driving conditions.
Essentially new car purchased 3 months ago with less than 5000 miles on it. Car failed while driving at high speed on highway and could not put in park, car was struck in drive mode. Tons of errors and safety issues. Parking brake failed. Airbag and abs systems failed. This car failed one month prior also with safety issues at high speed and tesla said they fixed it but clearly not.
Hello, i thought itwould be best to send my latest service note about my car.tesla continues to avoid fixing or addressing the problem.we are not sure where else to turn and see so many other complaints about this online.i finally decided it was time to reach out to you. Thank you in advance for any assistance or guidance you can provide.' hi there are no time stamps, as it has for the past 6 months, the screen is perpetually slow. I assume it is in the records, we have reached out to service many times. We've had the screen replaced, we've cleared out the memory, gone through reboots and we still have issues. We can rarely get to our reverse camera in a timely manner, and all features accessible via the touchscreen are unusable. Our vehicle is no longer safe to drive. We've recently gone online and recognize that we are having the exact same issue as other vehicles. We've gone from the biggest tesla fan and turned out 50+ people to the vehicle, to have lost faith in the brand. Especially with the safety issues this is causing us. We have gone through months or troubleshooting and repairs and it is still not resolved. What do you suggest for our path forward? how do we get this in front of tesla's leadership team?'lee schor
Audio warning glitch in tesla model x.scenario is that the car is in park with the passenger's door and hatchback open.car is placed in to reverse gear and the audio tone sounds warning of open door and open hatchback.passenger's door is closed by occupant and audio signal stops, although rear hatchback is still open.audio tone is still silent if the vehicle moves in reverse gear although the hatchback is still open.risk of potential and actual damage to vehicle, building structures, and people nearby due to audio tone silencing prematurely. Apparently there are no safety-interlocks preventing starting of the engine, such as transmission in 'p' or 'n'. Further, there is a wand shifter, but gear selection is completely under software control. There is no interlock such as with a traditional prndl selector. This is an inherently unsafe design.civil docket describing the accident and tesla's response are attached.note in tesla's response: 1) they do not state how many minutes elapsed between driver exiting the vehicle and toddler entering it. They use the vague term "moments." it could well have been 10 or 15 minutes. 2) they claim it is not reasonably foreseeable that a child would manage to shift a vehicle into gear, which is patently false. Safe vehicle design absolutely includes consideration of unsupervised children playing at the controls.

Touch screen and main display unit and controller problems. It goes black very often, it becomes slow many times, restart takes a long time, sometimes bluetooth stop working for all devices, no sounds chimes, no audio sometimes, the blank screen stays for long time sometimes during the drive. This is big safety concern. I went to service few times and most recently today and tesla service denied the service or fix siting everything works. They asked me to restart the screen. They told if screen goes blank weekly it's acceptable.
On march 1, 2021, wife and i was driving east bound on route 80 highway in ohio.twice the vehicle braked while driving in cruise control.it was a clear day.there were no cars around.no bridges or obstacles.and the road lines where clear.the first braking occurred while we were on right lane on a two-lane highway.the second braking occurred (around 5:15pm) while we were in the middle lane of 3-lane highway.we were driving approximate 65mph.at both braking, my wife was driving.when i happen, we both confirmed there was no cars or obstacles around us.we contacted tesla, who after reviewing their logs says it was working in "normal".that the vehicle saw something and braked.even though both myself and wife can confirm there was nothing around us, particularly since covid has significantly reduced any traffic.there has been a history of issues.on other occasion, one of my side doors indicated there was something there, even though i was the only vehicle in completely empty parking lot.only after many complaints an attempts did they finally replace the sensor.it seems that they are repeating their denial of claims by two eye-witnesses of the issues.
Most tesla model x owners are complaining of an acceleration shudder that happens due to a halfshaft alignment design flaw widely acknowledged by all tesla service centers.when it gets worse due to inaction / lack of recall by tesla, serious catastrophic failure can occur while car is in motion.wide spread problem.my issue was first noticed with less than 1,500 miles.service centers seem to have stopped replacing front end half shafts until redesign is done.but no one knows if or when that might happen.tesla needs oversight in this area.google "tesla acceleration shudder model x"and you can read about it from hundreds of owners.

My 4 year old daughter climbed into the 3rd row of our tesla model x while it was parked. She wanted to sit in the back seat and use a "big girl" seat belt, instead of her car seat. She attempted to put the seat belt on and even clicked it into place with the strap over her lap. However, she somehow got the chest/shoulder strap wrapped around her neck. My daughter & wife thought that letting the belt retract a little would allow the remove the tension and allow the belt to be fully extended. That didn't happen. Luckily, one of my child's arms was in a raised position and prevented the seat belt (now in a loop around her neck) from literally strangling her. My wife called me in a panic and hurried home. My daughter sat in the back seat crying and in a panic. I met them in the street when she arrived. The car was still on when i jumped in the back seat. I initially thought i could let the seat belt retract a bit and then the tension would release so that i could remove her. The problem is the belt never allowed me to extend it. Instead, the seat belt continued to remain tight and engaged even as it was retracted. In the end, i had to hold the seat belt in place so it wouldn't get any tighter and my wife ran into the house and grabbed scissors. Once i had secured the seat belt and had scissors in hand, i asked my wife to take a cell phone video of it. I can share that video with you if you like. Ultimately, we had to cut the seat belt. We have not alerted tesla of this issue and we have not yet had the car repaired. I'm not familiar with seat belt safety requirements, and i realize my daughter is very young and having the seat belt wrap around her neck is likely an uncommon situation, but it seemed odd to me that the belt performed in this manner. I'm alerting the nhtsa in case this is a product safety issue that warrants action.
The contact owns a 2018 tesla model x. The contact stated that the front driver's side seat back was fractured. The contact stated that the seat back failed to remain in an upright position. The vehicle was taken to tesla service center located at 5600 kearny mesa rd, san diego, ca 92111, (858) 285-3905, where it was diagnosed with needing the front driver's side seat to be replaced. The vehicle was not repaired. The manufacturer was notified by email about of failure. The failure mileage was approximately 56,000.*dt
I brought my car to brooklyn tesla service because my driver seat started to rocking (its moving like it got loose) after technician duplicated the issue, he said it's a "normal wear and tear" and that tesla seat can start moving after a certain time if the driver is heavy. They refused to do anythingi am concerned about safety and i'm sure that loose driver seat is dangerous.in addion the seat has metal bar on buttom cusion that is paintfull for "heavy" drivers, also a very big safety concern because you can't really drive in pain and this as per tesla the seat is designed this way.
I've been informed by the vehicle manufacturer, that the tesla model x 7 seat vehicle must make a loud 15 second long alert when shifting into drive, when in the event that the vehicles fold flat seats are folded flat. This same alert is not present on tesla model x 5 seat vehicle models, and i would like to inform nhtsa that tesla maybe violating regulations in their produced 5 seat models, or making up regulations to ignore software bugs that hinder the operation of their vehicles and distracting the driver for 15 seconds when using the vehicle daily.

When slowly pulling into a parking spot, the car accelerated on it's own jumping the curb and slammed into a chain link fence. I pressed the brake pedal but it didn't respond. My ten year old child was in the back seat and thankfully we are both unharmed. This is the second incident of the car accelerating on it's own. The first time this occurred was during my first week with the car and when i was slowly driving around a traffic circle in a residential area and the car took off on it's own. No damage other than misalignment that time but i was very scared and called tesla service. It took several weeks for them to respond with the data report from the car and said that the vehicle increased speed due to the driver pressing the accelerator.i accepted their explanation and assumed that i was indeed at fault even though i was positive that i didn't press the accelerator. I'm incredibly saddened by this situation as i truly loved this car but i'm too frightened to drive it again. It's currently at a body shop.

While attempting to drive, the vehicle suddenly would not move. Upon inspection, the contact discovered that the front passenger wheel had separated from the vehicle and made contact with the wheel well. The vehicle was towed to an independent mechanic and repaired. The cause of the failure was not determined. The manufacturer was notified of the failure. The dealer was not contacted. My tesla model x falcon wings do not open at times. They sense an obstruction when there is none. My kids have been stuck in the car because the doors do not open. I had to crawl through the trunk to get my kids out of their car seats. This occurs when the car is park/not in motion.

When the front display is blank; the driver gets no positive feedback whether a turn signal has been engaged - no visual indication or audible clicking noise. When this happens the driver is unable to change lanes safely; not knowing whether blinkers are on or off. When these conditions happen vehicle was in motion in both freeway driving conditions.
Trunk flew open while driving after screws attaching latch to hood stripped or loosened to point of detaching.
Using the self-park system, from a stationary position and pressing the touch screen prompts to use the self-park, and having hands and feet away from the gas, brake and steering wheel, and parking between two cars, the car accelerated backward on its own and i had to slam my foot on the brake to stop it but it still hit a pole going in reverse and damaged the body of the car. Had i not hit the brake my son and i would have been injured. Tesla has denied responsibility.
When slowly pulling into a parking spot, the car accelerated on it's own jumping the curb and slammed into a chain link fence. I pressed the brake pedal but it didn't respond. My ten year old child was in the back seat and thankfully we are both unharmed. This is the second incident of the car accelerating on it's own. The first time this occurred was during my first week with the car and when i was slowly driving around a traffic circle in a residential area and the car took off on it's own. No damage other than misalignment that time but i was very scared and called tesla service. It took several weeks for them to respond with the data report from the car and said that the vehicle increased speed due to the driver pressing the accelerator.i accepted their explanation and assumed that i was indeed at fault even though i was positive that i didn't press the accelerator. I'm incredibly saddened by this situation as i truly loved this car but i'm too frightened to drive it again. It's currently at a body shop.

Came home and noticed did the car head problem with the suspension. I am filing a complaint against tesla regarding the mcu infotainment on the model x. My vehicle is 2 years old and began to suffer mcu failure after upgrading to tesla's hw3. The mcu failures has resulted in severe lag, low vehicle startup. Vehicle start up has taken anywhere from 10 minutes to 20. Recently, a family member was required to be driven to the hospital. Due to the mcu causing my behave slowly, 911 was required to be called for an ambulance. The mcu failure is severely effecting medical emergencies. Tesla should upgrade all mcu 1 vehicles to the mcu 2 for free. .

Driver distraction issuewhen i bought the tesla model x, basic voice commands were available -- e.g., to call using a smartphone, linked to the car via bluetooth. The operation, as new, was straightforward, and matched the then-current manual, available in 2018.as is known, tesla updates its car software, &, in fact, makes it mandatory that the car's sw be updated, or risk voiding the warranty.normally, the new sw updates improve the operation of the car. However, starting in december 2019, the sw updates, & each tesla 'owners manual' that corresponds, fell out of sync. The manual would describe functionality, and the actual software, simply did not have the functionality. I've read each of the updated "model x owners manual north america",(january 2020 & october 2020), downloaded from the tesla website. In each case, the car fails to conform to the operational details explained at, e.g., the october 2020 (2020.44) manual, page 53. On that page, tesla explains, twice, that a way to activate a voice command, is to 1. Press the voice button;2. State the voice command, e.g., "call john doe"3. Press the voice command.twice, on the same page 53, tesla wrongly instructs the driver to 'tap the voice button again' or 'press again'.in hundreds attempts to make the model x to respond to 'bookend' presses of the voice button, it has failed to operate in the manner described in the manual, since december 2019.however, the car does respond to the following sequence of inputs, which the manual also describes:1. All screens in the car shut down while driving and there is no access to your speed, blinkers, control of lights, control of car safety features, wipers, etc.tesla was notified multiple times and each time they tried to correct the issue, it was not resolved.it has been a year since the issue started and has happened over a dozen times, resulting in unsafe driving conditions.tesla keeps stating they "remotely diagnose" the problem and told me to keep an eye on it and see if it happens again, although they told me less than 2 weeks prior to call immediately if it happened again.they have refused to fix the problem and it has become a safety issue with driving the vehicle.

I verified that there was no traffic, that there was an area where cars were not parked on the left side of the street and could use a parking driveway on the right that will allow more space to perform the u turn. I gently pressed the break to further slowdown and then veered a little to the right into the driveway at less than 5 mph. I started to turn to the left while still gently pressing the break. As soon as the front right wheel left the driveway and before i had completed the u turn, the tesla all of a sudden, on its own spontaneously accelerated to full power (a tesla x can go from 0-60 miles in approx. 4 seconds). My foot was still on the break, i did not move it and therefore i did not press the accelerator. There were no objects obstructing the pedals. In a small fraction of a second, the tesla traveled at full speed towards the tacoma car parked ahead on the left (less than 20 feet). Something in the tesla system triggered the sudden spontaneous full acceleration resulting in this collision.the tesla had major damage in the front, the air bags were deployed. I had a major bruise on my abdomen and several small bruises on my chest. The tacoma suffered damage in the front, moved backwards and hit the car parked behind it. There were no passengers in the other vehicles. It is my firm belief that this accident occurred due not to a driver's error but to a technological error within the tesla vehicle. I found more than 60 complaints for spontaneous sudden acceleration for tesla vehicles that have been filed to nhtsa. What these complaints have in common are that vehicles are traveling slowly trying to park or make turns at low speed when sudden spontaneous full acceleration was triggered.
As the vehicle self driving system was active it came to a full stop in a red light. The vehicle stopped behind an 18 wheeler. As i was in a full stop, i stopped looking to the front of the car and looked for a document and checked messages. Suddenly the vehicle accelerated fully, impacting the 18 wheeler in front of me. My reaction was not fast enough as i was very confused, eventually i hit the breaks and activated abs system but not with enough time to stop before impact. This has been confirmed by the tesla report that the vehicle acceleration occurred with out drive input. They seem to ignore the fact that i was at a red light and state it is operator error not to be attentive, but regardless, accelerating at full speed with no input towards another vehicle is a hazard to the population. They have not shared the report from engineering, only read the statement over the phone once and have not been able to ask for the written report. Significant avoidance in occurring from the company.
On march 18 had to make a panic emergency stop to avoid a collision.i heard a popping noise and immediately had grinding and scraping from the left front wheel.sound worse with right turns, less with left turns.scraping continued with driving and car was taken to tesla service dublin as a safety issue, not safe to drive.tesla confirmed that two bolts holding the left front brake caliper broken and had to replace them.tesla took photos, but i did not receive them.wheel noise resolved.
Autopilot system would not work from the moment it left the lot brand new. The warnings on the forward cluster display lower center started out displaying "surround vehicle display limited" then changed to displaying "autopilot features limited". Never had the calibration in progress displayed while driving off the lot. It was taken to the tesla service center in vienna, va to address due bill items. After 2 weeks at the tesla service center it was determined that a wire in the harness leading to the right b pillar was damaged. A wire was added outside of the harness to repair it. Request to have the harness replaced were denied and no assurances in writing from tesla that the rest of the harness was free of any damage. I never had a "calibration in progress" returning the vehicle home. Is the autopilot function calibrated? we have the full self driving package. Inspection of the work accomplished by tesla i discovered the anchor bolt, anchor bolt cap missing and a couple of clips broken on the trim panels.

Note: i am not a tesla short seller, i am real owner, driver of tesla for the last 19 + months.1.sudden acceleration happened many times during autopilot, vehicle doesn't decelerate even when another vehicle is in front, i had to take control from autropilot with this sudden acceleration and i apply brake. This has been many many times at different locations (freeways ~65mph, expressways ~45/50mph speeds). I did not apply any gas pedal/other techniques to increase the speed, but the vehicle suddenly without any intervention speeds up, i was terrified with this and took control from autopilot. Contacted tesla earlier regarding this, but their poor customer service, service center did not help much.2.automatic emergency braking...which is supposed to stop if any objects infront, does not stop. Its just the visible clear objects, vehcile does not stop even at lower, higher speeds.i request nhtsa to kindly take note of these issues with teslas and appreciate your action in this regard.
The central display unit/media unit (mcu) had more and more frequent reboots, freezes, blank screens.when it occurs, my autopilot will stop function, traffic information will not work, radio/speaker will be silent, no turn light signal clicking is audible.the manufacturer refused to fix the problem, giving all types of excuses.
The car suddenly accelerated on its own as i was slowly entering a parking space in a supermarket parking lot and hit the car that was parked in the space in front of me.
Was parking my 2018 model x on feb 1, 2019, in my driveway. The vehicle suddenly accelerated, hit the garage door(closed. No warning from sensor), went through garage door( vehicle did not stop after hitting the garage door), did not sense things(like washer, dryer, water heater, wall to my kitchen) in the garage door and finally smashed the washer/dryer before coming to stop. Washer/dryer hit the wall and damage the entire kitchen. No airbags came out. My wife was in the front seat, my daughter was in the backseat and my son was inside the home when it happened.
Touch screen and main display unit and controller problems. It goes black very often, it becomes slow many times, restart takes a long time, sometimes bluetooth stop working for all devices, no sounds chimes, no audio sometimes, the blank screen stays for long time sometimes during the drive. This is big safety concern. I went to service few times and most recently today and tesla service denied the service or fix siting everything works. They asked me to restart the screen. They told if screen goes blank weekly it's acceptable.
Cannot open doors. Locked out with no way to enter with keys or manually. Car is only four months old and had plenty of charge. Tesla roadside assistance has needed to send someone out to try and fix
I purchased this vehicle from tesla on march 31 2021.the vin sticker affixed to vehicle inside driver's door shows a manufacturer date of july 2018.after i got it home and dug deeper into the main control unit, i discovered that the main control unit was from a vehicle built prior to march of 2018. All tesla vehicle built march of 2018 and on, have an updated main control unit. The contact owned a 2018 tesla model x.the contact stated that as her husband was parking in a space, the vehicle made a loud noise and accelerated independently. As a result, the vehicle crashed into a tree and a cement planter, causing the planter to move about 10 feet away. There was no warning light or chimes. The contact was asked about air bag deployment and confirmed that no air bags deployed. The contact who was the front passenger and at the time of the failure was bending over to grab her purse from the floor mat sustained a bruised rib and later drove herself to the doctor for medical attention. The police was called to the scene but the contact was not sure if a police report was filed. The vehicle was totaled and towed to a salvage lot. The contact called the tesla motors dealer (650-681-5800, located at 4180 el camino real, palo alto, ca 94306) and was told to email the information. The contact emailed the information but did not get a response. The failure mileage was approximately 8,000.

I found more than 60 complaints for spontaneous sudden acceleration for tesla vehicles that have been filed to nhtsa. What these complaints have in common are that vehicles are traveling slowly trying to park or make turns at low speed when sudden spontaneous full acceleration was triggered.
On march 1, 2021, wife and i was driving east bound on route 80 highway in ohio.twice the vehicle braked while driving in cruise control.it was a clear day.there were no cars around.no bridges or obstacles.and the road lines where clear.the first braking occurred while we were on right lane on a two-lane highway.the second braking occurred (around 5:15pm) while we were in the middle lane of 3-lane highway.we were driving approximate 65mph.at both braking, my wife was driving.when i happen, we both confirmed there was no cars or obstacles around us.we contacted tesla, who after reviewing their logs says it was working in "normal".that the vehicle saw something and braked.even though both myself and wife can confirm there was nothing around us, particularly since covid has significantly reduced any traffic.there has been a history of issues.on other occasion, one of my side doors indicated there was something there, even though i was the only vehicle in completely empty parking lot.only after many complaints an attempts did they finally replace the sensor.it seems that they are repeating their denial of claims by two eye-witnesses of the issues.
A tesla model x was able to be started and shifted into gear by a 2 year old who was then able to drive the vehicle forward and pin his pregnant mother between the vehicle and garage wall. Apparently there are no safety-interlocks preventing starting of the engine, such as transmission in 'p' or 'n'. Further, there is a wand shifter, but gear selection is completely under software control. There is no interlock such as with a traditional prndl selector. This is an inherently unsafe design.civil docket describing the accident and tesla's response are attached.note in tesla's response: 1) they do not state how many minutes elapsed between driver exiting the vehicle and toddler entering it. They use the vague term "moments." it could well have been 10 or 15 minutes. 2) they claim it is not reasonably foreseeable that a child would manage to shift a vehicle into gear, which is patently false. Safe vehicle design absolutely includes consideration of unsupervised children playing at the controls.
We had an accident 2days ago. We were in the school loop to drop off our son to school. My husband was driving and was heading towards the drop off area when everything just happened so fast, the next thing i knew is that we hit one of the in-ground installed basketball hoops. The impact was hard and deployed all the airbags on the passenger seats and the side airbag of the driver and the 2nd row passenger seats where my son was seated and also saw some smoke from the front where i was seated and i do not know where it was coming from that i thought that the car was on fire but my husband informed me there was none. Recalling now, it has been a mystery to us as to why and what really happened. I was seated on the front passenger seat and sustained a fracture in the right scapula and generalized contusions in my body area. My son who was seated on the 2nd row also sustained contusions on his face and a few lacerations. We were coming into the school loop and was running on a somewhat 20mph speed, i was wondering if the vehicle did accelerate it's speed cause why would we have such a hard impact and sustained those injuries that it even deployed almost all the airbags? hope someone can look into this so as not to have anyone be on our same fate and for everyone's future safety. When slowly pulling into a parking spot, the car accelerated on it's own jumping the curb and slammed into a chain link fence. I pressed the brake pedal but it didn't respond. My ten year old child was in the back seat and thankfully we are both unharmed. This is the second incident of the car accelerating on it's own. The first time this occurred was during my first week with the car and when i was slowly driving around a traffic circle in a residential area and the car took off on it's own. No damage other than misalignment that time but i was very scared and called tesla service. It took several weeks for them to respond with the data report from the car and said that the vehicle increased speed due to the driver pressing the accelerator.i accepted their explanation and assumed that i was indeed at fault even though i was positive that i didn't press the accelerator. I'm incredibly saddened by this situation as i truly loved this car but i'm too frightened to drive it again. It's currently at a body shop.
My model xwas traveling at low speed as it was near home. Before entering the curve, the x suddenly accelerated out of control. It first crashed into my other parked car in front of my house, pushing the parked car into my garage. Then it continued to hit a city light pole, breaking it at the base. The x continued to hit my next door neighbor's two cars parked outside of their home, damaging their garage.the car finally stop when it hit the wall of third neighbor' that divide up her driveway.tesla roadside support contacted me on the day of the incident because they were able to see my airbag was deployed and may require assistance. I requested tesla to review the log of the car and explain why the car suddenly accelerated out of control. When i followed up that first week they acknowledged my request in an emailand told me that the request is with theirengineering teamand it may take a while before they will have any response.however, as of today, i haven't heard from their engineering team.
As the vehicle self driving system was active it came to a full stop in a red light. The vehicle stopped behind an 18 wheeler. As i was in a full stop, i stopped looking to the front of the car and looked for a document and checked messages. Suddenly the vehicle accelerated fully, impacting the 18 wheeler in front of me. My reaction was not fast enough as i was very confused, eventually i hit the breaks and activated abs system but not with enough time to stop before impact. This has been confirmed by the tesla report that the vehicle acceleration occurred with out drive input. They seem to ignore the fact that i was at a red light and state it is operator error not to be attentive, but regardless, accelerating at full speed with no input towards another vehicle is a hazard to the population. They have not shared the report from engineering, only read the statement over the phone once and have not been able to ask for the written report. Significant avoidance in occurring from the company.
Using the self-park system, from a stationary position and pressing the touch screen prompts to use the self-park, and having hands and feet away from the gas, brake and steering wheel, and parking between two cars, the car accelerated backward on its own and i had to slam my foot on the brake to stop it but it still hit a pole going in reverse and damaged the body of the car. Had i not hit the brake my son and i would have been injured. Tesla has denied responsibility.
The contact owns a 2018 tesla model x. While pulling into a parking space, the brake pedal was depressed but the vehicle inadvertently accelerated over a curb before crashing into a cement light pole. The front end of the vehicle was severely damaged, and the air bags did not deploy. The driver sustained neck, shoulder, and chest injuries that required medical attention. A police report was filed. The vehicle was towed to a tow yard. On several occasions, prior to the failure, an erroneous warning message displayed while driving, indicating that the vehicle was driving too closely to the preceding vehicle, even though another vehicle was nowhere in the vicinity. The contact indicated that the failure occurred on multiple occasions, including shortly before the crash. The manufacturer was notified of the failures. The local dealer was not contacted for diagnostic testing or repairs. The failure mileage was 1,500.
While in a parking lot at low speed and coming around a turn to park, tesla suddenly accelerated from low speed to high speed hitting a pole. Airbags deployed. Sudden acceleration was without warning. Severe damage to car. Luckily no pedestrians were nearby.
All screens in the car shut down while driving and there is no access to your speed, blinkers, control of lights, control of car safety features, wipers, etc.tesla was notified multiple times and each time they tried to correct the issue, it was not resolved.it has been a year since the issue started and has happened over a dozen times, resulting in unsafe driving conditions.tesla keeps stating they "remotely diagnose" the problem and told me to keep an eye on it and see if it happens again, although they told me less than 2 weeks prior to call immediately if it happened again.they have refused to fix the problem and it has become a safety issue with driving the vehicle.
Edit/additional note to my earlier complaint number: 11300604,this sudden acceleration with tesla happen when using with/without auto pilot, i observed many times during(a).this unintended acceleration happen even when driving normally without auto pilot, though i am not pressing gas pedal, especially observed in the city road speeds around 30-45 mph, whn using auto pilot on freeways, this happened arounbd 55-70mph speeds too.(b). Also this unintended acceleration happen when battery is at 20-40% of range.(c). Also happen whenver there is a new software update, this unitended acceleration popped up many occasions(don't remember exact times) as if some software bug resurfaced and i used to take control of the vehcile having been alert all the time.reported this over email, talked to service center, but no concrete action from tesla end.
Note: i am not a tesla short seller, i am real owner, driver of tesla for the last 19 + months.1.sudden acceleration happened many times during autopilot, vehicle doesn't decelerate even when another vehicle is in front, i had to take control from autropilot with this sudden acceleration and i apply brake. This has been many many times at different locations (freeways ~65mph, expressways ~45/50mph speeds). I did not apply any gas pedal/other techniques to increase the speed, but the vehicle suddenly without any intervention speeds up, i was terrified with this and took control from autopilot. Contacted tesla earlier regarding this, but their poor customer service, service center did not help much.2.automatic emergency braking...which is supposed to stop if any objects infront, does not stop. Its just the visible clear objects, vehcile does not stop even at lower, higher speeds.i request nhtsa to kindly take note of these issues with teslas and appreciate your action in this regard.

Thank you in advance for any assistance or guidance you can provide.' hi there are no time stamps, as it has for the past 6 months, the screen is perpetually slow. I assume it is in the records, we have reached out to service many times. We've had the screen replaced, we've cleared out the memory, gone through reboots and we still have issues. We can rarely get to our reverse camera in a timely manner, and all features accessible via the touchscreen are unusable. Our vehicle is no longer safe to drive. We've recently gone online and recognize that we are having the exact same issue as other vehicles. We've gone from the biggest tesla fan and turned out 50+ people to the vehicle, to have lost faith in the brand. Especially with the safety issues this is causing us. We have gone through months or troubleshooting and repairs and it is still not resolved. What do you suggest for our path forward? how do we get this in front of tesla's leadership team?'lee schor
Central console screen stopped working for a few minutes. During this time frame, there is no way to open the rear doors of the vehicle. The only recommended option from the manufacturer is to remove the speakers on the rear door panel and then pull on emergency release. There is no way a passenger can remove the speakers without tools. This can lead to a safety issue, were the rear passengers have no mechanical way to open the door to escape the vehicle. Similarly when the screen is not responsive, there is no way to activate the rear defogger or defog the front windscreen. This situation drastically reduces visibility and can lead to an unsafe condition on the road. When the front display is blank; the driver gets no positive feedback whether a turn signal has been engaged - no visual indication or audible clicking noise. When this happens the driver is unable to change lanes safely; not knowing whether blinkers are on or off. When these conditions happen vehicle was in motion in both freeway driving conditions.
Tires are separating internally after 27k miles. Continental tires. This tire should be good for 45k miles. It has no other problems. Tesla says they have lots of problems with those tires. When the tire has problems it faults all the safety issues of car and computer. Big problem. Only answer is to replace all of tires and see what works.
For 2-3 weeks, the vehicle's ride has shuddered and wobbled, from 1-70 mph. The wobble affect is particularly noticeable when making turns, from 5+ degrees. The vehicle feels as though a tire has been blown, but the tire sensors do not indicate any tire pressure issues. We have taken the vehicle to the manufacturer's service center, thinking initially that the problem was drivetrain related. However, the service center has notified us that the problem lies with both rear tires having issues, both internal separation and internal bubbles. The service center has notified us that we must pay for replacement of these tires, which are the vehicle's original tires with aprox. 25,000 miles. All research we can find on internal separation and internal bubbles indicates a manufacturing defect. We are grateful the issue was discovered before an accident occured, but wonder why the consumer is asked to pay for what appears by all of our research to be a manufacturer's defect. Shouldn't the vehicle's manufacturer also be aware of this? and if so, why would they ask the purchaser of the vehicle to pay for the replacement of a defective part, whether it be the vehicle manufacturer's or original components provided by a 3rd party, such as the tires? we have not yet finalized a resolution, but wanted to file this in order to make sure there is a record of such tire defects in case it is a common defect for said tire.
Sudden accelerationpulling into a parking space head first with foot on the break.next thing my wife knew was that she had jumped the curb and hit 2 other cars.her foot was still on the brake.tesla downloaded the black box.tesla told us that the car was traveling at 3 mphand that the accelerator had bee depressed.tesla refused to share the black box print out due to "proprietary information".3 mph in a parking lot, dry pavement, clear sky, 10:00 am.repair estimate was low by $10,000.
Defective tire:on 2018 tesla model x - 4201 miles, 4 months oldrear tire:continental cross contact lx sport csi - 275 /45 r20 110v xl bsw tecar reported loss in tire pressure, took it to costco for air refill, technician discovered tire had a bulge in it side wall.refer to image attached.tire or rim shows no sign of side wall hit or impact.this is a defective tire, would like tesla and continental to investigate and replace.
It happened at 7/21/2019, 1pm. The place is a parking lot in front of a store. I was driving my tesla x. Before i reached there, i was already in that parking lots for several minutes; the speed was very low, about 3 miles/hour. When i found the store, i wanted to turn left and park next to a car in front of the store. At that time, i loosed the accelerator panel. Since my tesla has the default regenerative brake setting, the car speed is almost zero. As usual, i touched a little bit of the accelerator panel to park into the parking space. However, the tesla had a big acceleration during the turn. I realized something was wrong, then elevated my feet and touched nothing at that time. My tesla still forwarded and jumped the curb and hit the concrete garbage bin in front of the store. After tesla crashed the concrete garbage bin, it still has acceleration and moved forward. It scratched a car, which was parking in the next parking space, then moved forward until it hit the wall of store. I'm 100% sure my feet touched nothing before my tesla jumped the curb.
