I had pulled into a parking lot, proceeded to pull into a spot adjacent to a cinder block building. I had my foot lightly on the gas pedal, then as i made the turn into the spot, my foot was on the brake - the car lurched forward and sped up and the brakes did not stop it. I went right into the concrete building, head on - air bags deployed. The front end crushed and the 2 airbags on the drivers side deployed and were smoking. I read on line that there have been numerous incidences of this happening with the tesla. Spontaneous acceleration with my foot not on the gas pedal. The car would not stop by the brakes! i could have been seriously injured or hit another persona or vehicle. The car hit the building as well as a natural gas pipe that was running along the buildings side at the level of my front bumper. I filed a police report.
Autopilot safety hazard creating a crash with me and my son. Onjuly 25th, 2016 at 4:30am i was involved in a accident with my tesla model x 2016 within 3 weeks of ownership.the road was wide open with only a few cars and great condition for autopilot,we enteredin a construction zone without any construction occurring at the time. Per the picture attached you will see that there is 2 yellow lanes, but the tesla detected on autopilot only the farthest closest to the guardrail making the car driving straight to the have duty construction cones. The road was in good condition and acceptable to activate autopilot. Airbags was not deployed.

Our 5 day old tesla xwhile entering a parking stall suddenly and unexpectedly accelerated at high speed on its own climbing over grass andand crashed into a building.the airbags deployed and my wife's arms have burn marks as a consequence.
On january 28, 2019 i backed out of my driveway in my 2016 tesla x, braked, stopped, put it in drive and barely, slowlystarted down my lane when the car suddenly accelerated at full speed down the lane.i tried the brake twice but it did nothing to slow the acceleration.approximately 75 yards down the lane the tesla entered the cross street below and clipped another car,demolished a large bank of mailboxes across the street, took briefly to the air several feet due to the hillside there, hit a large cedar tree, glanced off and finally stopped nose down,high sided on a large landscape boulder.but for the boulder, i would have continued down into the deep ravine behind the neighbor's house.the airbags had deployed during these events.i was taken to the hospital by ambulance and diagnosed with a t-12 spinal compression fracture, which is still painful a year later.the car was totaled. Police report and medical records available on request.

Main display stops functioning, causing loss of several necessary and safety functions including but not limited to: vehicle control, climate, navigation and vehicle status. This is a known problem for tesla models s and x sold before 2018.
My mcu went out and i had no working horn or blinkers. I was almost side swiped by a truck that was coming into my lane as if i wasn't there and i tried to blow my horn and i had no horn to alert him of presents in the lane he was and did merge and i had to hit my brakes to avoid him from crashing into us! this unsafe for a car that if your mcu goes out you loose some of your safety features that could save your life!
On thursday, september 26, 2019, at 7:40 am, i was parking my car at work and my car went on full speed forward over the median and crashed, luckily no one was hurt. This happened in a split of a second and the vehicle's safety override break that should have prevented from going over an obstruction in front of the car did not engage as claimed by tesla. I requested an analysis of the car's recorded data to tesla and asked to provide me with a copy of the report for better understanding. After weeks of analysis, an employee from tesla called and informed that it was my fault as the gas paddle was fully engaged, i was astonished as i did not hit the gas paddle rather i just let my foot off the break to park my car slowly in the parking space and the car took off with speed as if i put full force to gas paddle. As for getting a copy of the report, he said that he will have to ask the engineers and let me know. It has been four months, and no one reached out to me about the report.some people saw the whole incident and said that i reduced the speed of my car to park and suddenly it took off and went forward fast.at that moment, it felt like the car was controlled by someone else as pressing on a break was not stopping the car and finally, i had to press the park to stop it. I have been driving the car to my work and parking at the same parking lot every day for the last 3 years. My insurance totaled the car and provided the residual value, to this date i am without a car and trying to fix the damages of my car.i almost gave up but saw the news that i am not alone about tesla denying similar claims of hundreds of sudden acceleration crashes.tesla have the data and i am requesting tesla to take full responsibility and compensate for my damage. I am requesting nhtsa to investigate and help bring closure to this matter.
Most times when i take my foot off the gas pedal, the car seems to accelerate on its own.however, when merging onto the highway, acceleration is reduced.as of the past couple of weeks, steering around turns has become a problem.the car operates like it's sliding on an ice patch.
I was in a parking lot waiting on a lunch order. After getting my order i was ready to pull the car out of my parking space. The car was stationary and not moving . As i step on the gas to pull out , the car went into 100% acceleration forward climb the curb and crash into a concrete wall that surround the garbage dumpster . The wall was about 3 to 5feet from my parking space.i contacted tesla and was told i step on the gas 100% in under 2 second , and they would not claim responsibility . As i tell them i am 100% sure i did not step on anything 100%( gas or break ) when i am in a busy parking lot. They still denied and said it was my fault .
The touchscreen become less responsive the past few months, the power-up time becomes longer, the screen would freeze and would have to be rebooted, and on july 24th the mcu unit is a total failure.i couldn't even drive the car until tesla mobile service comes out to cycle the power of 24v.the cost is $85 of diagnostics fee but he couldn't find anything besides that it is not responsive.after that i have to reschedule another visits which is not available until aug 12th. I can drive now with black mcu but i can't control blinkers.my blinkers are always off.very unsafe. This car has only 55k miles.it is right after the warranty expires. The mobile service and phone call with service shop in reno location both suggest me to replace to a new mcu and since it is out of pocket it makes sense to do a mcu upgrade instead.total with part $2500 it would be around $3000.

The vehicle was taken to local dealer tesla (4180 el camino real, palo alto, ca 94306:(650) 681-5800), where it was diagnosed with needing the microchip or the display unit to be replaced.the vehicle had not been repaired as of yet.the manufacturer had been informed of the failure. The center screen keeps going out and having to be restarted.tesla will not cover the repair because the vehicle is over 100,000 miles.
The falcon wing doors of the vehicle would not close. After trying to troubleshoot the vehicle, all the doors of the car would lock and the car door would not open either from the remote or the door handle. Luckily there were no kids or pets in the car and the car was almost near the driveway of the home. This happened after the car was put in the parked mode in a city street after a short 10-mile trip.
My screen goes black.this recovers over time when i reboot, but if i am checking mileage the screen goes black again.people who have the same car have told me that this is a known problem with the computer memory for this vehicle.this has happened when the car is stationary and moving on a city street. I am not able to get a service appointment until august 7, 2020

The contact stated that the gear shifter, windshield wipers, brakes, and accelerator pedals were the only manually operated functions within the vehicle, all of which work as designed. All other functions were operated by a central computer system which would simultaneously become inoperable. The failures lasted between three to five minutes. Additionally, the failure would occur while driving at various speeds or while the vehicle was idle. The computer would reboot itself and function as normal. There was no warning. The contact took the vehicle to the dealer who stated that the issue was normal and would fix itself after an update. The contact stated that updates on the system would occur every few weeks to few months and several had already occurred however, the failure persisted. The manufacturer was made aware of the failure and stated that there were no recalls on the vin. The repairs were no longer covered under warranty. The approximate failure mileage was 30,000.

The mcu is dead. Car cannot be operated safely. No side blinkers, no horn, no ac. At this point my car cannot even turn on and cannot be driven. Contacted tesla, but it is not a safety issue and they sent me home as parts were out of stock. It is not safe to drive as blinkers don't work.
While highway driving the main mcu froze due to a manufacturer known memory corruption defect and would not reboot. This defect failure impacts access and control of the following safety features, turn signals, emergency brake, emergency battery disconnect,fog lights (front and rear), auto high beams, rear pedestrian camera, suspension (inability to disable auto leveling for tire changing or towing), traction control slip start, side mirror adjustments. Batterysafety limits for acceleration and regen based on environmental temperature safety limits are also disabled. This mcu failure also prevents refueling (l3 dcfc stations). As well asthe hvac system from being turned off while parked. Which continued to drain the primary battery for several hours. The odometer is also not available, as well as the hands free bluetooth feature and other non safety features.the initial incident occurred under warranty. The manufacturer switched to a backup memory partition to repair or workaround the defect issue temporarily. Within 3 months, less than 1,000 miles later,the backup partition also became permanently corrupted and required a complete replacement of the unit, out of warranty.
The main computer screen stops working which causes the hvac defroster and other controlling units of the car to stop working while in drive.
Total main computer failure (mcu) - many safety features were unaccessible and not available including but not limited to turn signals and hazard lights stop working, the settings for autopilot and driving are gone, the suspension settings, the ability to turn on headlights manually, ability to manually lock and unlock the vehicle, navigation, a/c, sunroof controls, etc.
Touch screen stayed off for more than 30 minuets on a 2 hour freeway drive with no control of the vehicles defroster, climate, navigation or audio controls. Touch screen does not always respond and glitches. Rear view camera does not always respond nor works immediately when in reverse and goes all black. Defrost would not work on freeway multiple times due to black non responsive screen. Rear driver seat sequels/squeaks a lot when sat in. Car often takes a minute or longer for the dashboard and touchscreen to turn on. Battery does not hold correlating charge to miles driven. Loud whirling/grinding when accelerating...

The vehicle was taken to tesla service center (52 derwood cir, rockville, md 20850, 301-738-6523), but was not diagnosed. The dealer reprogrammed the vehicle. The manufacturer was not notified of the failure. The failure mileage was approximately 17,000.
The vehicle started taking longer to start up about a few months ago. Then the system needed to be rebooted on a regular basis to be functional. At one point the main screen that controls all the safety and visibility features of the vehicle stopped working and the vehicle would take up to 5 minutes to start up. After doing some research this is a common issue with tesla's older than 2018 where they encounter an "emmc failure" of the mcu. The defect of the mcu was addressed when tesla replaced the mcu in its vehicles in 2018. This should be treated as a recall due to the design flaw of the manufacturer. The mcu controls all the safety features of the vehicle and the rear view camera. Without the mcu it makes it extremely difficult to have the visibility needed to go in reverse.
I had blank screen and it turned out to be a memory card problem which was under recall. This was once fixed by tesla and this is second time it happened which i got fixedby tesla under recall.. After fixing the problem i persistently have the same problem where i am driving in screen goes blank all of a sudden. Subsequently a tesla symbol will appearwhich can last up to 5 to 10 minutes minimum. During this time i cannot use any electronic control like radio navigation temperature or a climate control etc.
Tesla refusing to allow me to make a service appointment to rectify an open nhtsa recall.said theyā€™ll contact me when itā€™s available to be fixed.this was two months ago and still havenā€™t heard back from them on when i can have my recall fixed. The center console will become unresponsive, poorly responsive or freeze intermittently. Factory reset has failed to alleviate the problem. The issue is becoming more frequent. A soft reset will take 5-10 minutes to complete (it took less than a minute when the car was new). During that time the hvac is shut off and the backup cameras are not functional. Contact with tesla has been unsuccessful in getting them to replace the mcu. This appears to be a known issue.
Vehicle was stationary, long start up times before i can initiate drive as shown in picture just a t logo for about 10-20 seconds.also stationary and in motion can not pull up some items like vehicle charging or features just get a blank white screen as shown.all have to do since it is on mcu1.
Mcu-touchscreen emmc failure - mcu1 (version 1) unit in tesla, including my 2016 tesla model x has an emmc chip that cause failures resulted in whole central screen going blank, freezes, and as result make car not-operational - no navigation, no climate control, no other controls of the vehicle, since it's all driven through central screen. Considering my car if just 3.5 years old, this is really concerning and tesla should address this technical flaw.

Failure and replacement of mcu touchscreen panel in my model x. Could not safely operate the vehicle because i was unable to extend side mirrors, use back-up camera,turn on/off ac or heating or use navigation. Other safety features requiring using or seeing the touchscreen panelwere not accessible. Vehicle 7 weeks out of 3 year warranty but only had 20,039 miles on it. Tesla said i needed to play for a new touchscreen panel. After a strenuous discussion tesla agreed to discount therepair by 50%. I still had to pay $841.63 for something i felt was a safety issue. Luckily the car was parked and at home at the time of the failure. It could have been catastrophic if it happen while driving. I felt the touchscreen panel failure was caused by a manufacturing defect and should have been replaced at no charge. Also, if tesla found it had a history of these panels failing, they should have done a recall. In no way should such a critical component fail in a vehicle that is 3 years, 7 weeks old with just over 20,000 miles on it.

Main screen computer (mcu)just shut down while driving. This is a very serious safety issue that tesla already knew about and this is a long time issue.
The center display and sometimes even the main display black out during driving. Most times the system requires a reboot which can take a few minutes. During the time where the screen are not visible you have no control over headlights or the ability to defrost the windshield. In addition the camera which is displayed while driving is also unavailable. If the center screen goes black then you can't see how fast your going or if your park, drive or reverse.
May 2020 while my car was still under warranty i continually had issues with the main screen. Issues were reported to tesla who asserted it was the firmware. Tesla kept my car for over a week for repairs, then charged me $400 for new firmware. However within two months the issues began again, the screen going black and multiple errors. In august i was finally given another service appointment. This time they asserted it was the 12v battery (despite recently having that replaced in 2019). After replacing the 12v battery the problems with the mcu still persisted. It takes about 3wks to 1month to even get a service appointment. Now they are saying the issue is with the mcu which needs to be replaced. I do believe the issue was with the mcu all the time. I do believe tesla either misdiagnosed orknowingly did not resolve the issue. Now i am being asked to pay $2500 not including tax and labor to replace the entire mcu for problems which were initially reported while the car was under warranty. Further, tesla is now claiming they no longer have logs of diagnostics on my car when i requested to see the diagnostics that were previously run diagnosing my black screen althout the do agree my car has been continually brought in for a black screen and non-functioning system. Disabled screen functionality causes several other systems of the car not to work: regenerative braking disabled, autopilot disabled, driver and user settings, disabled, ac unit not functioning, stereo/radio capability disabled. Each day a different alarm or not would come on disabling another function while driving. Very dangerous and very unnerving, especially while transporting three young children. I have asked tesla for diagnostics logs on my car and hoping to receive and submit them as support for this complaint.
The main screen failed. This in turn caused other systems to not work correctly. Examples include rear view camera, turn signals, infotainment system which includes maps, navigation, music and the ability to change any settings on the car. The car was stationary when the mcu stopped working.
Main computer system died. All screens stopped functioning (including speedometer). Car wouldnt lock/unlock. Tesla said the mcu had to be replaced and they were going to charge me because it was 5 days out of warranty.
Car was park for a few days. Got in the car and the main control screen did not come on. I restarted the car a few times and noting, car was able to be driven but whit no access to ac, radio, back up camera and most important turn signals, since all the controls are in the center screen. I contacted tesla and was able to drive it, cautiously since the review camera acts as a blind sport monitor for me. Tesla replace the center computer under extend warranty, witch has a $200 deductible.
Center screen (mcu) dies/freezes up while driving and must try to reset all the time. Tesla refuses to accept resposibility and replace with new mcu under warranty.
During charging, the car mcu screen went blank, and the car was not accessible or controllable. Vehicle was unresponsive until tow truck driver bypassed unit by jump starting the 12 volt battery. Limited control was regained. Car was drivable, but the rear doors were inoperative. I am not sure what other systems were inoperative. I am concerned about this happening during driving or getting stranded in a remote place.
Mcu (media control unit) failed on multiple occasions, i was unable to control ac/radio/autopilot system/ or navigation. Also both turn signal were in-operational. Tesla service center refuses to replace the mcu or fix it. Vehicle was in motion, on highway at a speed of about 70 mph.

I have had problems with my tesla1- doors repeatedly malfunctioning (stationary and driving)2- the accelerator was malfunctioning, city streets and freeways3- computer shot down while driving and screen went blank. City streetstomorrow again i am taking the car for repair because of doors malfunctioning i dont find it safe. I left it in the service center. I have only driven it 2000 miles over 13 months.
Vehicle accelerated suddenly and crashed into a tree in front while parking the car in a parking lot. We reported the problem to tesla but they got back to us denying any car malfunction that caused the acceleration. However we received a recall letter today saying that affected parking brake caliper may cause in rollaway and crash.
The mcu (the car's center screen) would not turn on. The center screen controls everything in the car including all the updates. Since the center screen didn't work, and update was not able to pass through, and it essentially made the car not drivable. The car was in the turn off mode and would not turn on.

Consumer writes in regards to nhtsa safety recall 20v-062 and 20v-710. *ldthe consumer stated the dealer would not honor the recall stating the vehicle was not experiencing the failure. The consumer stated the screen would black out randomly and the steering assist did not function properly. The vehicle was involved in an accident due to the failure. The vehicle had trim separation that would leak water that the dealer would not repair.
The car doesn't take charge. I drove my car to try different charging spot in the same supercharging station and try in another charging station. The car doesn't charge.it was a weekend when the car has charging issue.eventually the car just out of juice. Won't open it let me in and i have to tow the car to service center in the monday service business day.it needs special towing with dolly.becasue of the car out of juice the 12v battery is dead too after that.charging port is a known issue to tesla but after all the repair. I was charged of $1300
Tesla x model 2016erratic behavior noticed. System will power up normal but some of the safety functions will not be available. Center screen will notrender properly like it is frozen. Very frustrating. Yes tesla is a low maintenance but if the computer modules / monitor that are heart of the car ( engine) there should be extended warranty on it. I am told that mcu unit needs to be replaced like saying your engine need replacement after 4 years. There should be standards on the computer hw devices on electric cars that are more robust and longer lasting. So much for low maintenance compare to cars with engines.
I had my tesla model x mcu replaced per the recall. Prior to having the mcu replaced i had numerous (more than 20) episodes of the car just shutting down and losing all power and would not restart for anywhere from 3 to 20 minutes. Many times after the vehicle would restart the main control screen would remain black, there would be no access to any of the controls usually accessible there. Other times there would be no navigation, radio or hvac available. When reporting these issues i was told to keep trying to reboot that it was a known software issue. When the recall was finally announced i checked frequently for appointments for a replacement chip which were not abundantly available. When i was finally able to get an appointment for the recall, the service center assured me that the replacement mcu would make things better but i also had the option to upgrade to a better chip for $2k but lose many options like fm and xm radio, which i use. Since the mcu replacement i have continued to have these same issues and my phone will no longer pair with the vehicle to provide the hands free use i had previously. I have worked with tesla to try to resolve these issues and i am told by their technicians that this is a known software issue that is happening to "older" model s and model x vehicles and there is no current projection as to when this will get resolved as it is on the back burner. My car is a 2016 with 40k miles! how is this ok? how is this not a safety issue? today the car went completely dark/dead while i was driving!!! luckily i was driving in a parking deck and there was no resulting accident. I have called tesla customer service and messaged the local service center to report these issues multiple times. Even the corporate customer service rep has difficulty reaching them directly. It is so sad to have such a nice vehicle with issues like this. We have talked about trading it in but ethically cannot fathom passing these known issues on to someone else.
The mcu in this 2016 model x does not function properly.it will randomly crash and go completely black while the car is in use which stops all functionality of the hvac, radio/audio, navigation, etc.the car also contains a browser which doesn't work at all.the mcu is extremely laggy when it is working.
The center console went dead, the vehicle could not be moved, would not even go into neutral, had to dragged out of the garage by the tow company and then shipped to the dedham, ma for repair.the issue was a failed emmc module and cost be about $600 in repairs
The screen will go black while driving and then needs a reset, takes about 5 minutes to reboot the system all while still driving on the road. Lag with the screen, put the car in reverse and it takes 30 seconds for rear camera to come on. I can never the the internet to work as it should, unable to pull up the owners manual. This has been happing over the last 12 months. I have many videos to show the issue, but tesla says all is normal when its taken in for service even when i show the video of what happened.

I own 2 teslas (2013 s and 2016 x) and within 6 weeks both center screens went blank. The stated reasons by tesla for the defect for the s is listed as "ms tegra mcu lte prem w-d motherboard na(1004740- 10-a)" and for the x it's listed as "mcu, mdlsx, na premium, remain (64gb)(1098010-99-h)". I'm convinced it's a built-in flaw that should be covered by tesla vs the owners. Also, upon my request to send me the diagnostic logs that would allow me to verify wether it's truly the motherboard or the sc card that is defect, tesla refused to do so. It's a critical piece of information for any owner to be able to see and read.
Touchscreen died completely affecting ability to drive vehicle.no ability to control defrost or air conditioning for windshield visibility. Turn signals produce no audible noise. My center screen keeps on turning black without any controls of some important functions on the car. I called tesla regarding the recall and they said they just cleared the memory and does not qualify to have it replaced. Yesterday it turned black again and could not drive the car at all.tesla is trying to get away with recall. The other recall for the bolt in the steering has not been done at all.
On a weekly basis, my center console that has controls for lights, defrosters, a/c will freeze and go blank for an approximate 2 minute reboot. This is while driving the vehicle. The ability to adjust the windshield front and rear, are inaccessible at this time. This will happen in motion whether on a city street or highway.

My 2016 tesla model x's mcu is often freezing, restarting, or going completely black while driving and while parked. When this happens, i'm unable to access any of the features of the car but more importantly, the rearview camera does not work when the mcu is having an issue. With the small rear window, i cannot see any children or people behind my vehicle. Reversing with no rear view camera is dangerous. I'm also unable to turn on the fog lights when needed. I've brought it into tesla twice already and each time, they have said that they cleared the memory and disconnected the battery for a moment and reconnected. This has not fixed the issue. Just today, the mcu screen and my icc screen shut down for a few minutes without any notice. I recorded the incident. I've spoken to other owners and it seems to be a common issue.

While driving the car on july 31 at about 7pm, the center touchscreen went black and would not reset using tesla instructions.vehicle was parked.the a/c and climate controls were therefore stuck in the on position with no recourse.all other functions handled by touchscreen were inoperative. Tesla roadside assistance was called on morning of aug 1 for oiptions, but reset protocols offerred did nothing to help solve the problem.car taken in to tesla service center on aug 2, with inoperative mcu identified as the cause on aug 3.repair cost estimated to be about $1500. I then found out the repair was out of warranty only because tesla had shortened the warranty by 2 years after i purchased the vehicle.if the warranty was as stated when the car was purchased, this repair would have been covered by tesla.sorry, no useful pics or docs appy at this point, as repair has not yet been made.
The mcu screen on my tesla went out after returning to the vehicle from it being parked. Screen would not turn on even after multiple reboots. Was able to drive the vehicle but lost all controls that the mcu screen controls, luckily it was a cooler day and i didn't need the ac but hvac is inaccessible when this goes out among other safety items. Prior to this there were times when the screen would be out and then come on after a few minutes. This is a known issue with tesla model s/x vehicles built before april 2018. They seem to always fail outside warranty but this shouldn't be an issue and can cause harm because of what the mcu controls.
My touchscreen went black and systems became disabled (air control, autopilot, music, navigation). Within a week the readings on my phone app were not correct and it took some time for the power to engage the gears. My backup camera went dark. Finally, i could not put my car in gear and it had to be towed by a specialty towing service. Tesla tells me it is a software issue, but i think a memory chip has gone bad. I think this relates to an existing investigation on this issue by nhtsa.
Yellowing of the screen and the screenclicks in multiple places during a single click of a button -car was on some kind of uv treatment for 73 hours -not better at all -also the navigation unit resets all the time sometimes leaving you unable to activate air conditioner.tesla tried to push me to buy some and of unit update 2k - not acceptable -the car is in perfect condition but the screen'. Can you point me out to someone that can provide a solution or any active class action lawsuit so i can join in
Chip failure in mcu caused turn signals to stop working at approx 65,000 miles.known defect to tesla and should be replaced as a recall as they changed the chip type for vehicles made in 2018 or newer due to this defect.the failure of this chip is a safety issue and they are currently charging owners $1500-$2500 to fix.
Having many issues that seem to follow the reported symptoms of failing emmc memory, however tesla told me that it is not the issue and that the emmc just needs "refreshed" for $95.59.it sounds like a way for tesla to delay the inevitable replacement or avoid replacing the module.also, i think it should be illegal for an automaker to leave a feature such as autopilot in "beta" form indefinitely in a production vehicle.i suspect the feature (that was quite expensive) will never be actually finished since development is probably no longer happening on older vehicles.
My vehicle's center display (mcu) is starting to show signs of failure on account of a flawed design that depends on an emmc memory component that is known to fail catastrophically after several thousand write cycles, causing loss of critical adas and rear view camera functions, hvac controls, and other basic vehicle functions. Now, i am facing the prospect of having to pay tesla thousands of dollars to replace the entire mcu in my just-out-of-warranty vehicle. Tesla has tacitly acknowledged the safety implications of their original design by modifying it to use a more robust memory component in 2018, and they should be made to recall pre-2018 models to ensure that nobody else has to endure what i and many hundreds of others have experienced.
On thursday, september 26, 2019, at 7:40 am, i was parking my car at work and my car went on full speed forward over the median and crashed, luckily no one was hurt. This happened in a split of a second and the vehicle's safety override break that should have prevented from going over an obstruction in front of the car did not engage as claimed by tesla. I requested an analysis of the car's recorded data to tesla and asked to provide me with a copy of the report for better understanding. After weeks of analysis, an employee from tesla called and informed that it was my fault as the gas paddle was fully engaged, i was astonished as i did not hit the gas paddle rather i just let my foot off the break to park my car slowly in the parking space and the car took off with speed as if i put full force to gas paddle. As for getting a copy of the report, he said that he will have to ask the engineers and let me know. It has been four months, and no one reached out to me about the report.some people saw the whole incident and said that i reduced the speed of my car to park and suddenly it took off and went forward fast.at that moment, it felt like the car was controlled by someone else as pressing on a break was not stopping the car and finally, i had to press the park to stop it. I have been driving the car to my work and parking at the same parking lot every day for the last 3 years. My insurance totaled the car and provided the residual value, to this date i am without a car and trying to fix the damages of my car.i almost gave up but saw the news that i am not alone about tesla denying similar claims of hundreds of sudden acceleration crashes.tesla have the data and i am requesting tesla to take full responsibility and compensate for my damage. I am requesting nhtsa to investigate and help bring closure to this matter.
I was driving my tesla model x ev home and there were no problems. When i pulled into the driveway the self driving features accelerated the vehicle causing an accident.
The mcu mdlsx,na premium,reman part #: 1098010-99-h is faulty and needed to be replaced.there are many people complaining of the failure in various forums and it is all happening at the same time.the options from tesla is to repair/replace for $1200 and back ordered for 1+ month or to order an upgrade mcu intel upgrade-for tegraap1.0 - north americapart #: 9874564-00-a for $2500. This should be a recall replacment.
I was in a parking lot waiting on a lunch order. After getting my order i was ready to pull the car out of my parking space. The car was stationary and not moving . As i step on the gas to pull out , the car went into 100% acceleration forward climb the curb and crash into a concrete wall that surround the garbage dumpster . The wall was about 3 to 5feet from my parking space.i contacted tesla and was told i step on the gas 100% in under 2 second , and they would not claim responsibility . As i tell them i am 100% sure i did not step on anything 100%( gas or break ) when i am in a busy parking lot. They still denied and said it was my fault .
Main screen computer (mcu)just shut down while driving. This is a very serious safety issue that tesla already knew about and this is a long time issue.
Starting in 2013 tesla cars the emmc memory chip connected to the mcu that controls the center console display and most electronics fails after about 4 years of usage due to writing to the chip over and over again. When the memory chip fails you loose control of the heating/cooling, backup view, etc. Every tesla car manufactured between 2013 and 2018 will have the problem.

Autopilot safety hazard creating a crash with me and my son. Onjuly 25th, 2016 at 4:30am i was involved in a accident with my tesla model x 2016 within 3 weeks of ownership.the road was wide open with only a few cars and great condition for autopilot,we enteredin a construction zone without any construction occurring at the time. Per the picture attached you will see that there is 2 yellow lanes, but the tesla detected on autopilot only the farthest closest to the guardrail making the car driving straight to the have duty construction cones. The road was in good condition and acceptable to activate autopilot. Airbags was not deployed.
I went to start my car and the screen was black. My car hasn't been driven for 5 months since i was traveling. I had cleared all of the memory (navigation, addresses, music stations, etc.), prior to leaving. I was in traffic and the turning signals didn't work. The safety of the car felt compromised since the turning signals,rear backup camera, suspension -basically most of the car controls were in-operatable.
On january 28, 2019 i backed out of my driveway in my 2016 tesla x, braked, stopped, put it in drive and barely, slowlystarted down my lane when the car suddenly accelerated at full speed down the lane.i tried the brake twice but it did nothing to slow the acceleration.approximately 75 yards down the lane the tesla entered the cross street below and clipped another car,demolished a large bank of mailboxes across the street, took briefly to the air several feet due to the hillside there, hit a large cedar tree, glanced off and finally stopped nose down,high sided on a large landscape boulder.but for the boulder, i would have continued down into the deep ravine behind the neighbor's house.the airbags had deployed during these events.i was taken to the hospital by ambulance and diagnosed with a t-12 spinal compression fracture, which is still painful a year later.the car was totaled. Police report and medical records available on request.

During charging, the car mcu screen went blank, and the car was not accessible or controllable. Vehicle was unresponsive until tow truck driver bypassed unit by jump starting the 12 volt battery. Limited control was regained. Car was drivable, but the rear doors were inoperative. I am not sure what other systems were inoperative. I am concerned about this happening during driving or getting stranded in a remote place.
The mcu is dead. Car cannot be operated safely. No side blinkers, no horn, no ac. At this point my car cannot even turn on and cannot be driven. Contacted tesla, but it is not a safety issue and they sent me home as parts were out of stock. It is not safe to drive as blinkers don't work.
In june of 2020, the entire power/electronic system in my 2016 model x failed (at 73k miles), leaving the car undrivable (and even unable to be put into neutral for towing). I had it dragged onto a truck and taken to a tesla service center, where they diagnosed a failed mcu. The failure, i was told, in rare cases caused a failure across the entire system. They replaced the mcu and charged me $1400. Tesla has supposedly offered a refund on the repair to customers who had the part fail (while refusing to acknowledge the safety issues associated with failure), but has not included my vin in their database of impacted vehicles. The company is not acknowledging my car was one of the vehicles in the recall, but the failure clearly happened. How can i hold them accountable for that? is there any way the nhtsa can help?

Attempted to merge onto a freeway from a stop signed parallel entrance. After a vehicle in the freeway lane had just passed and there was adequate space before the next vehicle, i attempted to accelerate rapidly from the full stop. The vehicle accelerated for only a very brief period, enough to get it into the lane at maybe 10mph before the acceleration cut out completely and a vehicle collision warning and audible alert appeared on the screen in front of the driver. There was not actual collision threat present and i tried pressing the accelerator pedal repeatedly to avoid being hit in the rear by the next vehicle coming from behind at full freeway speed. The vehicle behind me changed lanes and went around me to avoid an accident. After a few seconds the warning went away and i was able to accelerate normally to freeway speed.this safety feature seems extremely dangerous for several reasons:* it incorrectly senses that a collision will occur with acceleration in some instances* it disables user control of the vehicle (acceleration)* it disables that control for an extended period of time even after the false threat is no longer detected* it allows some movement before disablement creating a situation where the vehicle is put in harm's way without any ability to safely accelerate away from an impending accident. Had the vehicle not moved at all, there would have been inconvenience but no risk* there is no ability in the current software to disable what i believe to be unsafe functionality intended for safety* there is no documentation to indicate that the car has this unusual functionalityi have discussed the above issues with tesla motors support and they have reviewed the data logs for the vehicle and believe it is functioning normally and that this behavior is as intended by design.my initial vehicle position is in yellow in the image.
The eye bolt the 2016 tesla model x that is use to load a tesla on to a tow truck is defective.the loading manualis wrong as well.08/24/2018 around 10 am mountain time in utah. The location was at service king in salt lake city, utah.vehicle vin # [xxx] 2016 model x. The eye bolt used to load the vehicle on to a wrecker fortransport is defective. The loading manual does not illustrate a safe method to load vehicle using this eye bolt. Iwas loading the vehicle via the instructions of the tesla model x manual and as soon as i got the vehicle to the spot to put a chalk behind wheel, the bolt broke and the vehicle came off the deck and almost ran over a kidworking in the body shop. I got slightly injured trying to stop vehicle from hitting the wall. The body shoprefuses to report this safety issue to tesla. When i tried to report this to tesla i was hung up on 2 times. When itried to contact teslas corporate office i got redirected back to the salt lake city service department 2 timeswhere i got hung up on 2 time again. This a major safety issue that can and will cause a loss of life or seriousinjury. But yet tesla has customer service personnel acting like this is a game. Model x's mcu touchscreen completely gone dark. This basically disable all back-up camera, turn signal sounds, gps, navigation, vehicle controls: door locks/unlocks, lighting, ac, etc.. In my instance, the vehicle's screen did not boot upon start up of the vehicle. The car is drivable but unsafe because of all the above mentioned disablement. Rear view camera is too dark in low light conditions (particularly night time). Only occurs on cars with updated infotainment systems (referred to as "mcu2"). Image is very hard to make out, exhibiting high levels of black crush, making it very unsafe when parking or driving.

Main computer system died. All screens stopped functioning (including speedometer). Car wouldnt lock/unlock. Tesla said the mcu had to be replaced and they were going to charge me because it was 5 days out of warranty.

While highway driving the main mcu froze due to a manufacturer known memory corruption defect and would not reboot. This defect failure impacts access and control of the following safety features, turn signals, emergency brake, emergency battery disconnect,fog lights (front and rear), auto high beams, rear pedestrian camera, suspension (inability to disable auto leveling for tire changing or towing), traction control slip start, side mirror adjustments. Batterysafety limits for acceleration and regen based on environmental temperature safety limits are also disabled. This mcu failure also prevents refueling (l3 dcfc stations). As well asthe hvac system from being turned off while parked. Which continued to drain the primary battery for several hours. The odometer is also not available, as well as the hands free bluetooth feature and other non safety features.the initial incident occurred under warranty. The manufacturer switched to a backup memory partition to repair or workaround the defect issue temporarily. Within 3 months, less than 1,000 miles later,the backup partition also became permanently corrupted and required a complete replacement of the unit, out of warranty.

Touch screen stayed off for more than 30 minuets on a 2 hour freeway drive with no control of the vehicles defroster, climate, navigation or audio controls. Touch screen does not always respond and glitches. Rear view camera does not always respond nor works immediately when in reverse and goes all black. Defrost would not work on freeway multiple times due to black non responsive screen. Rear driver seat sequels/squeaks a lot when sat in. Car often takes a minute or longer for the dashboard and touchscreen to turn on. Battery does not hold correlating charge to miles driven. Loud whirling/grinding when accelerating...
Mcu failure while driving on the freeway.i was driving with autopilot on the freeway.the mcu failed and disengage the autopilot but there was no audio warning.almost crashed into the wall of the hov lane.

On july 8th 2016, at 9:37 a.m., while slowly pulling into a parking space at creekwood dog park in bradenton florida, my tesla model x suddenly accelerated under its own volition, drove over a parking stop, over a five inch curb, and then hit and knocked over a concrete light pole.all this happened in a distance of less than twenty feet.tesla was notified immediately and the car was taken to dimmitt collision center in clearwater, florida.the service manager at tesla of tampa, told me verbally the log from the edr says the car was traveling at 6 mph, then the accelerator was advanced to over 50% and then to 87%.the car accelerated to 20 mph and abruptly stopped.i denied this scenario and asked for a supervisor.tesla's southeast regional manager met us at the body shop. He handed me a letter that had different edr results-vehicle speed was 7 mph, pedal position went from 3.2% to 15.6% to 100% and car went to 14 mph.the first repair estimate showed actual mileage as 205 and a subsequent repair estimate shows the actual mileage as 1425.these figures are inaccurate since i had looked at the odometer several days before the accident and the mileage was over 1800. I informed tesla that i am positive beyond a shadow of doubt that the car's electronic throttle computer was responsible for the accident which they deny.this appears to be the industry standard since experts will testify that although a car is responsible for unintended acceleration there will be no traceable evidence of that responsibility and therefore the manufacturer has plausible deniability.teslas are undergoing unintended acceleration at a rate more frequent than 1/5,000 vehicles manufactured.this is way more frequent than the industry standard. General motors has an extremely good rate of 1/123,000 vehicles.
While turning left into a parking spot at a very slow speed, the car suddenly accelerated with extreme force.it ran over a curb and collided with a tree and a truck.there was only light pressure on the accelerator.the automatic braking and the airbags did not deploy.there was over $18 000 damage to the two vehicles and the tesla model x is not driveable without repairs.
The mcu is dead. Car cannot be operated safely. No side blinkers, no horn, no ac. At this point my car cannot even turn on and cannot be driven. Contacted tesla, but it is not a safety issue and they sent me home as parts were out of stock. It is not safe to drive as blinkers don't work.

In june of 2020, the entire power/electronic system in my 2016 model x failed (at 73k miles), leaving the car undrivable (and even unable to be put into neutral for towing). I had it dragged onto a truck and taken to a tesla service center, where they diagnosed a failed mcu. The failure, i was told, in rare cases caused a failure across the entire system. They replaced the mcu and charged me $1400. Tesla has supposedly offered a refund on the repair to customers who had the part fail (while refusing to acknowledge the safety issues associated with failure), but has not included my vin in their database of impacted vehicles. The company is not acknowledging my car was one of the vehicles in the recall, but the failure clearly happened. How can i hold them accountable for that? is there any way the nhtsa can help?
Having many issues that seem to follow the reported symptoms of failing emmc memory, however tesla told me that it is not the issue and that the emmc just needs "refreshed" for $95.59.it sounds like a way for tesla to delay the inevitable replacement or avoid replacing the module.also, i think it should be illegal for an automaker to leave a feature such as autopilot in "beta" form indefinitely in a production vehicle.i suspect the feature (that was quite expensive) will never be actually finished since development is probably no longer happening on older vehicles.
After software update blind spot monitoring no longer exists. This resulted in a few near miss accidents on the highway. When i raised the issue with service they said it is operating as designed.

Also, a gel-like substance was leaking from the monitor display unit onto the monitor screen posing a visibility hazard while using the backup camera. The cause of the failure was not yet determined. The local service center and manufacturer were notified of the failure. The failure mileage was 31,570.

The mcu (the car's center screen) would not turn on. The center screen controls everything in the car including all the updates. Since the center screen didn't work, and update was not able to pass through, and it essentially made the car not drivable. The car was in the turn off mode and would not turn on.
The vehicle started taking longer to start up about a few months ago. Then the system needed to be rebooted on a regular basis to be functional. At one point the main screen that controls all the safety and visibility features of the vehicle stopped working and the vehicle would take up to 5 minutes to start up. After doing some research this is a common issue with tesla's older than 2018 where they encounter an "emmc failure" of the mcu. The defect of the mcu was addressed when tesla replaced the mcu in its vehicles in 2018. This should be treated as a recall due to the design flaw of the manufacturer. The mcu controls all the safety features of the vehicle and the rear view camera. Without the mcu it makes it extremely difficult to have the visibility needed to go in reverse.
While coming to a stop in a parking garage going 3 mph the auto suddenly projected forward at full speed into a concrete wall destroying the entire front end of the car and injuring the passengers by deployment of the air bags08102016

Tesla model x.consumer writes in regards to no warning beeps from vehicle autopilot before acceleration accident. The consumer stated that while driving on autopilot 55-60 mph, the vehicle suddenly began to veer to the right after about a mile. It crashed into 12 safety post before coming to a stop. The vehicle made an abnormally loud sound. The engine was still running at high speed. The loud sound stopped after being put into park. Attempted to merge onto a freeway from a stop signed parallel entrance. After a vehicle in the freeway lane had just passed and there was adequate space before the next vehicle, i attempted to accelerate rapidly from the full stop. The vehicle accelerated for only a very brief period, enough to get it into the lane at maybe 10mph before the acceleration cut out completely and a vehicle collision warning and audible alert appeared on the screen in front of the driver. There was not actual collision threat present and i tried pressing the accelerator pedal repeatedly to avoid being hit in the rear by the next vehicle coming from behind at full freeway speed. The vehicle behind me changed lanes and went around me to avoid an accident. After a few seconds the warning went away and i was able to accelerate normally to freeway speed.this safety feature seems extremely dangerous for several reasons:* it incorrectly senses that a collision will occur with acceleration in some instances* it disables user control of the vehicle (acceleration)* it disables that control for an extended period of time even after the false threat is no longer detected* it allows some movement before disablement creating a situation where the vehicle is put in harm's way without any ability to safely accelerate away from an impending accident. Had the vehicle not moved at all, there would have been inconvenience but no risk* there is no ability in the current software to disable what i believe to be unsafe functionality intended for safety* there is no documentation to indicate that the car has this unusual functionalityi have discussed the above issues with tesla motors support and they have reviewed the data logs for the vehicle and believe it is functioning normally and that this behavior is as intended by design.my initial vehicle position is in yellow in the image.
The seatbelt has stopped retracting fully and became twisted. Although tesla repair personnel have repeatedly untwisted the belt, it immediately becomes twisted again and stops retracting.

Received a recall notice with nhtsa recall code 17v-260.there is a problem with the parking brake caliper that "can result in an inability to move a parked vehicle" or an "increased risk of a crash".i received this notice today, june 10, but tesla doesn't expect to have the necessary parts until october!!this to me seems unreasonable.

The rear driver side lower control arm snapped off detaching the rear wheel/tire, brake assembly and strut/axel rendering the car immobile.fortunately this did not happen 20 minutes sooner as my gf and i were traveling on the highway at normal highway speeds - this would have caused a life threatening, catastrophic multiyear crash being unable to control the vehicle...we were backing out >1mph of a parking space in a very shallow curve when it snapped - we stopped immediately and i think the electronic braking tried to stop the wheel from rolling jamming it further at angle into the wheelwell.there was nor had there been any unusual impacts on this tire nor the vehicle from potholes/sudden stops or any accidents of any kind.

Sudden unintended acceleration. The car suddenly accelerated instead of stopping when i applied the brakes while pulling into a parking space. I was going approximately 5 mph. Instead of stopping, the car jumped a concrete parking block, went over a curb, climbed an embankment, and struck a tree before it came to a stop. The car suffered damage to the front driver's side wheel well and the undercarriage. The driver was not injured and the airbags did not deploy. Video surveillance captured the incident in its entirety. Photos of the scene and the resulting damage to the car are also attached. This incident occurred during a routine trip that i drive about 4 times each week. Weather, traffic, and road conditions were not a factor in this incident. The car does not contain any aftermarket equipment.
While highway driving the main mcu froze due to a manufacturer known memory corruption defect and would not reboot. This defect failure impacts access and control of the following safety features, turn signals, emergency brake, emergency battery disconnect,fog lights (front and rear), auto high beams, rear pedestrian camera, suspension (inability to disable auto leveling for tire changing or towing), traction control slip start, side mirror adjustments. Batterysafety limits for acceleration and regen based on environmental temperature safety limits are also disabled. This mcu failure also prevents refueling (l3 dcfc stations). As well asthe hvac system from being turned off while parked. Which continued to drain the primary battery for several hours. The odometer is also not available, as well as the hands free bluetooth feature and other non safety features.the initial incident occurred under warranty. The manufacturer switched to a backup memory partition to repair or workaround the defect issue temporarily. Within 3 months, less than 1,000 miles later,the backup partition also became permanently corrupted and required a complete replacement of the unit, out of warranty.
2016 tesla model x. Consumer writes in regards to nhtsa safety recall 20v-062 and 20v-710. *ldthe consumer stated the dealer would not honor the recall stating the vehicle was not experiencing the failure. The consumer stated the screen would black out randomly and the steering assist did not function properly. The vehicle was involved in an accident due to the failure. The vehicle had trim separation that would leak water that the dealer would not repair.
Chip failure in mcu caused turn signals to stop working at approx 65,000 miles.known defect to tesla and should be replaced as a recall as they changed the chip type for vehicles made in 2018 or newer due to this defect.the failure of this chip is a safety issue and they are currently charging owners $1500-$2500 to fix.
My center screen keeps on turning black without any controls of some important functions on the car. I called tesla regarding the recall and they said they just cleared the memory and does not qualify to have it replaced. Yesterday it turned black again and could not drive the car at all.tesla is trying to get away with recall. The other recall for the bolt in the steering has not been done at all.

In summer of 2020 my screen for odometer, speed and auto pilot status went out. The driver screen went black and never came back on. Tesla service request a replacement which i had to pay $1500+, few month later my steering wheel was very loose and offbeat. Again, i had to pay out of pocket. Which i believe was dangerous to drive because i could not turn very well. Now, my car takes very long time to turn on and the driver side window does not close all the way.

Left front suspension fore link cracked and fell off.vehicle was only backing up out of garage and steering wheel was turned to the left when a loud crack was heard from under the vehicle.the back up speed was only 1-2 mph.luckily, i was still in my driveway taking my kids to school.i can't imagine what the outcome would have been if it broken 5 minutes later on the highway with the kids inside the car.
Most times when i take my foot off the gas pedal, the car seems to accelerate on its own.however, when merging onto the highway, acceleration is reduced.as of the past couple of weeks, steering around turns has become a problem.the car operates like it's sliding on an ice patch.

While driving slowly, making parking maneuvers, i heard a clunk sound from the right front wheel.afterwards, sometimes when i turned right, i can hear screeching sounds.after bringing the vehicle to the dublin, ca tesla service center, the initial inspection revealed a broken control arm.repairs call for the replacement of both right and left fore control arms.the repair estimate is attached below.
Was driving at 19 mph in city street and had to make a panic stop due to a car about to cut me off. Heard a loud bang. I got out of the car and the driver side front wheel is displaced. Further investigation i can see the lowerdriver side control arm had broken . Car was not drivable without a loud crunching sound. I had it towed to the tesla service center.
As i was driving, i heard loud grinding noise when i turned the steering in either direction and i lost power to my steering and a message "power steering failure, may need extra effort to steer" was on the dashboard. Luckily i was not too far from my house and was able to go back home with lots of extra effort turning the steering. I don't know what would have happened if i was going full speed on a busy highway? i called tesla service and had my car towed to tesla latham, ny service center. I have 60,000 miles. So i was out of manufactures warranty. I was told that i need new rack and pinion for my car and the cost would be $3863.40 parts & labor. To say the least, i was shocked. I was told that three aluminum bolts that holds the electric motor attached to rack and pinion, two were completely broke off and the third was barley holding the motor hanging. Please see attached photos of the damaged parts. I did notice on the repair invoice that new rack and pinion now is updated with steel bolts.
Entering the intersection from a four way stop, the driver side lower control arm broke when brake applied firmly at about 10 mph while turning left.the vehicle made a few creaking sounds when parking for two days prior to the total failure. After failure driver side front wheel moved forward and aft when vehicle was moved and the tire impacted the fender liner and ripped the rear half out.

While reversing into my driveway, we heard a loud bang sound as if i ran over something, i stopped the car to make sure i didn't run over anything, which i didn't.i put the car back in reverse and can hear big scraping sound, it was the tire hitting wheel well.after further inspection, i found the right fore link was completely off the joint and hanging there.

Here is yet another tesla in the junkyard.https://www.iaai.com/vehicledetails/39245824?tenant=us&rownumber=10the front suspension has snapped off completely.this does not look like damage caused by a collision.the wheel and tire are almost undamaged so it is unlikely that a violent force was applied to the suspension through an impact to the wheel.the steel stems of both the steering arm and lower suspension ball-joints have sheared off.these ball-joints are attached to soft alloy parts of the steering knuckle and it should be the case that the alloy would yield before the steel bolts do.you can see that the ball-joint stems have brittle fractures with zero knecking of the bolt.a normal bolt would not be expected to crack in such a manner.these tesla suspension failures are so commonplace that it is obvious the manufacturing, quality control and testing are deficient.please take immediate action.people are getting injured and killed by these defects.thank you.citizen keef
Here is a tesla model x in the salvage yard.it has been in a major crashthe front lh suspension top link is torn away which could have happened in the crash. However, the tire has had the tread severely worn away over the entire circumference. This could not have happened after the impact unless full power was still applied to the motor post impact. It is far more probable that the tire damage was caused prior to the crash by a mechanical fault in the suspension.several other model x cars have been reported with broken suspensions.please investigate this matter urgently.please note that the date of crash, details of injury are only estimates.
Autopilot safety hazard creating a crash with me and my son. Onjuly 25th, 2016 at 4:30am i was involved in a accident with my tesla model x 2016 within 3 weeks of ownership.the road was wide open with only a few cars and great condition for autopilot,we enteredin a construction zone without any construction occurring at the time. Per the picture attached you will see that there is 2 yellow lanes, but the tesla detected on autopilot only the farthest closest to the guardrail making the car driving straight to the have duty construction cones. The road was in good condition and acceptable to activate autopilot. Airbags was not deployed.
Lower u joint from steering shaft to steering rack is directly exposed to all road elements and debris that may be raised, kicked up, or splashed up from the front drivers side tire. As such it's subject to excessive corrosion, wash out of lubrication, rust and other damage. Recently steering had started to stiffen up, but was not too bad. Until my wife was driving and she could no longer turn the steering wheel. The wheel locks up about every 90 degrees. It will go smoothly, then as you turn it will find "lock up" or extremely stiff points every ~90 degrees it is turned. Tesla service told me it was the lower steering shaft u joint, and that corrosion and the locking up is a common problem. If it is common, it should be recalled or have some sort of shield on it to protect from the elements. Service appointment for it is about 3 weeks to a month out. Vehicle is not safely drivable at this time. I did some inspection further. I've attached some photos of the specific part. When i first saw the part (sorry, i do not have before hand pictures) the u-joint was completely caked in grease, grime, even salt from last winter, sand, dirt, anything you could imagine being kicked up off the road. After i thoroughly degreasing and cleaning the lower steering u-joint, which removed all of that crud, then blew it out with a air gun, you can see in the attached photos that it is very rusted and corroded even after a deep cleaning. After this deep cleaning and relubrication, the steering wheel fully turns smoothly with minor binding in places. While no major accident, when my wife could not turn the wheel at all, she did impact the curb.
The enhanced autopilot software drives the car over lane lines and inconsistently maintains speed and braking. It is unsafe.
The control arm broke while backing out of parking space. I did not run over anything when it happened. When i pulled back forward the front end lifted up. I hate to think what could have happened at high speed,

Failure of the left front suspension fore link, while driving at low speed
2016 tesla model x. Consumer writes in regards to nhtsa safety recall 20v-062 and 20v-710. *ldthe consumer stated the dealer would not honor the recall stating the vehicle was not experiencing the failure. The consumer stated the screen would black out randomly and the steering assist did not function properly. The vehicle was involved in an accident due to the failure. The vehicle had trim separation that would leak water that the dealer would not repair.
The eye bolt the 2016 tesla model x that is use to load a tesla on to a tow truck is defective.the loading manualis wrong as well.08/24/2018 around 10 am mountain time in utah. The location was at service king in salt lake city, utah.vehicle vin # [xxx] 2016 model x. The eye bolt used to load the vehicle on to a wrecker fortransport is defective. The loading manual does not illustrate a safe method to load vehicle using this eye bolt. Iwas loading the vehicle via the instructions of the tesla model x manual and as soon as i got the vehicle to the spot to put a chalk behind wheel, the bolt broke and the vehicle came off the deck and almost ran over a kidworking in the body shop. I got slightly injured trying to stop vehicle from hitting the wall. The body shoprefuses to report this safety issue to tesla. When i tried to report this to tesla i was hung up on 2 times. When itried to contact teslas corporate office i got redirected back to the salt lake city service department 2 timeswhere i got hung up on 2 time again. This a major safety issue that can and will cause a loss of life or seriousinjury. But yet tesla has customer service personnel acting like this is a game. On july 8th 2016, at 9:37 a.m., while slowly pulling into a parking space at creekwood dog park in bradenton florida, my tesla model x suddenly accelerated under its own volition, drove over a parking stop, over a five inch curb, and then hit and knocked over a concrete light pole.all this happened in a distance of less than twenty feet.tesla was notified immediately and the car was taken to dimmitt collision center in clearwater, florida.the service manager at tesla of tampa, told me verbally the log from the edr says the car was traveling at 6 mph, then the accelerator was advanced to over 50% and then to 87%.the car accelerated to 20 mph and abruptly stopped.i denied this scenario and asked for a supervisor.tesla's southeast regional manager met us at the body shop. He handed me a letter that had different edr results-vehicle speed was 7 mph, pedal position went from 3.2% to 15.6% to 100% and car went to 14 mph.the first repair estimate showed actual mileage as 205 and a subsequent repair estimate shows the actual mileage as 1425.these figures are inaccurate since i had looked at the odometer several days before the accident and the mileage was over 1800. I informed tesla that i am positive beyond a shadow of doubt that the car's electronic throttle computer was responsible for the accident which they deny.this appears to be the industry standard since experts will testify that although a car is responsible for unintended acceleration there will be no traceable evidence of that responsibility and therefore the manufacturer has plausible deniability.teslas are undergoing unintended acceleration at a rate more frequent than 1/5,000 vehicles manufactured.this is way more frequent than the industry standard. General motors has an extremely good rate of 1/123,000 vehicles.
I was driving on the highway in the passing lane.suddenly i felt something smashing over and over on the roof of the car.i looked up but could not see what the cause was.i put on my hazard lights and made my way over the shoulder.i got out and inspected the roof.the portion of hard plastic that separates the windshield glass from the falcon door glass was torn off.there was one little corner portion attached.i tried to remove it manually but could not.i had to continue driving onto a suspension bridge as i had no other choice.once i crossed the bridge, i exited and headed to an auto body shop.as you can see from the photots, the plastic was smashing around with so much force that it cracked the falcon door glass.the mechanic there was kind enough to remove the dangling plastic and weatherstrip so that i could get home.i parked in the town nearby to calm my nerves as i was shaking and upset from the whole incident.i called tesla service and they suggested i drive the car to a service center 20 miles away.i did not feel safe driving the car there so they agreed to tow instead.it is currently at the tesla dealer and i've notified my insurance company.i am waiting to hear from both.
While driving at highway speeds on an interstate, a glass plate approximately 8 i inches x 18 inches on the car roof situated between the falcon wing doors and covering the door lift henges was blown off by the wind force.i was unaware that this had happened until arrival at my destination.fortunately, there were no other cars near me at the time.so, i am unaware of any damages to other vehicles.the possibility of severe damage or injury had there been another car directly behind me should be evident.from what i understand, the glass plate is only attached with glue.according to the tesla model x owners forum, this has happened to other owners and is related to deteriorating rubber seals around the falcon wing doors that tesla has claimed to be only cosmetic.i believe that the deterioration of that rubber seal allowed the wind to get under the glass plate and allow it come loose.

While i was driving on the freeway, the roof tap flew off the car in mid-june 2020. I made an appointment and had it fixed, and complained to the manager at burbank service center in los angeles. They ignored my complaint and the safety harzard posted to the safety of the freeway. I requested they repaired it under the warranty. They refused, and charged me over $1400 for the replacement of the new roof top. Now, i saw the recall, and i tried to get refunds for the replacement. They refused.
Takata recalli drove my tesla model x 2016 today to work and when i was about to park the car in the parking lot (around 6 miles per hour may be) it suddenly accelerated and hit the concrete wall and bounced back around 8 feet. Since it was for parking i can surely say that i did not accelerate the car.the steering air bags and knee air bags came off and also the passenger side knee air bags came off as well. I searched online and there seems to be a class action suite on this issue but tesla is not accepting it as the glitch in their software or some other component. I felt like the accelerator got pressed the way when the car was in cruise mode. Unless i wanted to hit the wall intentionally there was no need for me to press the accelerator to speed from almost zero to whatever the high speed it attained at the time of hitting the wall.
Corrosion on metal body parts.
The falcon wing doors of the vehicle would not close. After trying to troubleshoot the vehicle, all the doors of the car would lock and the car door would not open either from the remote or the door handle. Luckily there were no kids or pets in the car and the car was almost near the driveway of the home. This happened after the car was put in the parked mode in a city street after a short 10-mile trip.
There are about 4-5 bolts which secure the bottom plastic cover in between the rear wheels, see photos, the cover will come loose and fall out even at normal used. If the car continues travel on the freeway, without the bolts, the covers can easily fall and hit the behind vehicles.that might result with serious accidents especially on the freeway with high speed. The car only 18k miles from 2016- 2019, low mileage, had been service twice in last two years. Tesla needs to provide schedule annual inspection during the regular maintenance to inspect the bolts (screws which secured these bottom panels/covers) to make sure the bolts (screws) are tighten and the covers are secured. That also should be covered under the 4 years vehicle warranty, not just blame the clients, and try to send the client to contact local body shop. It is their responsibility for the manufacturer to make sure their vehicles are safe to drive and to be on the road within the warranty period under reasonable used. They also need to prevent future accident with their products.

On thursday, september 26, 2019, at 7:40 am, i was parking my car at work and my car went on full speed forward over the median and crashed, luckily no one was hurt. This happened in a split of a second and the vehicle's safety override break that should have prevented from going over an obstruction in front of the car did not engage as claimed by tesla. I requested an analysis of the car's recorded data to tesla and asked to provide me with a copy of the report for better understanding. After weeks of analysis, an employee from tesla called and informed that it was my fault as the gas paddle was fully engaged, i was astonished as i did not hit the gas paddle rather i just let my foot off the break to park my car slowly in the parking space and the car took off with speed as if i put full force to gas paddle. As for getting a copy of the report, he said that he will have to ask the engineers and let me know. It has been four months, and no one reached out to me about the report.some people saw the whole incident and said that i reduced the speed of my car to park and suddenly it took off and went forward fast.at that moment, it felt like the car was controlled by someone else as pressing on a break was not stopping the car and finally, i had to press the park to stop it. I have been driving the car to my work and parking at the same parking lot every day for the last 3 years. My insurance totaled the car and provided the residual value, to this date i am without a car and trying to fix the damages of my car.i almost gave up but saw the news that i am not alone about tesla denying similar claims of hundreds of sudden acceleration crashes.tesla have the data and i am requesting tesla to take full responsibility and compensate for my damage. I am requesting nhtsa to investigate and help bring closure to this matter.

The car was on autopilot. My model x autopilot ran over a recliner at 60m/hthat was in the middle of the highway. It was around 6 pm, weather was nice and clear.i tried to avoid it by moving to right lane but it hit the driver side of the front of car causing minor damages to car. I tried to reach out to tesla customer service but they told me [4 or 5 times]that someone will contact me and no one ever did. Everytime i talked to tesla [by phone and email] they ask me for more information and then tell me someone will call you back. The latch on the falcon wing door (driver side) cracked from one side on its own. Doubt door is securing properly.
The right rear door (known as "falcon wing doors") came out of alignment and began to impact the body of the car.not only did this pose a risk for breaking the glass on the door while it drags against the body of the vehicle, but there appears to be no "pinch detection" in this scenario as the door continued to close while grinding the glass against the door jamb.if a hand or other body part where to get caught, this mechanism would clearly sever it under the force of the motors driving the heavy door.it is a much higher risk design than a regular door due to the high forces involved given the lack of a proper pinch detection mechanism.

Sudden unintended acceleration. The car suddenly accelerated instead of stopping when i applied the brakes while pulling into a parking space. I was going approximately 5 mph. Instead of stopping, the car jumped a concrete parking block, went over a curb, climbed an embankment, and struck a tree before it came to a stop. The car suffered damage to the front driver's side wheel well and the undercarriage. The driver was not injured and the airbags did not deploy. Video surveillance captured the incident in its entirety. Photos of the scene and the resulting damage to the car are also attached. This incident occurred during a routine trip that i drive about 4 times each week. Weather, traffic, and road conditions were not a factor in this incident. The car does not contain any aftermarket equipment.
Falcon doors do not always open correctly.sometimes they sense an obstacle that is not present and so will not open.this failure happens more often when the falcon doors are hot, for example after being in the sun.tesla service center has replaced the sensors in both of my falcon doors, but the problem remains.
Driver side falcon door opened while driving on highway with my 6 y.o. Sitting next to door.on june 2, 2017 we were at toy r us with our parents and our 6 year old son, a total of 5 people.for context, my son sits next to the driver side falcon door, me in the center and my mom on the right. My husband and father are in the front.as we were leaving, the left side falcon door was stuck again.we immediately called the service center due to it being friday but since it was late in the day we left a message as they were already closed. We started to drive home because through our previous experience we were always advised to drive the car back to the service center as the car is still drivable.during the ride, i was calling the roadside assistance to write up a ticket to reinforce to the service center to call us to schedule service first thing in the morning. While on the phone with roadside assistance, i head a click in the door and the motor as the door started to lift open.i grabbed onto the door with my left hand and held on and the door had about a 2-3 inch gap.we had been on the road for approximately 15 minutes at this time and were on the 5 north freeway right before the tustin ranch exit.we immediately exited and parked in the tustin auto center.the door opened once we parked and clicked closed once the button was pressed. As we were close to home and can drive home via local streets, we drove home.we informed roadside assistance to immediately set up a tow first thing in the morning on saturday.this is the fourth time the door has malfunctioned,the other three times occurred on jan .28. April 23, and april 25 and their attempts to fix door has been unsuccessful but this last incident added on the door opening on highway would lead to serious bodily injury had i not grabbed onto the door. My car had a bolt brake from its thread. They told me it was a critical bolt that was marked with paint. This bolt is to keep the subframe attached securely to the frame.
Driver side door will no longer latch properly - constant cycling noise coming from latch.cycling can be stopped by manually moving the striker latch with a screwdriver to fool the car into thinking the door is closed.car can be driven by placing an arm out the window and holding the door closed manually.if the door is forced closed during the constant cycling, the door cannot be opened from the outside, you need to climb in to the front from the passenger side or the rear seats to open with the inside handle.the faulty latching mechanism raises concern for not only whether the door can be closed securely and not open while driving, but the failure of being able to open the door from the outside is a safety risk in the event someone needs to be removed from the vehicle in an emergency and the front door cannot be opened from the outside.

I had pulled into a parking lot, proceeded to pull into a spot adjacent to a cinder block building. I had my foot lightly on the gas pedal, then as i made the turn into the spot, my foot was on the brake - the car lurched forward and sped up and the brakes did not stop it. I went right into the concrete building, head on - air bags deployed. The front end crushed and the 2 airbags on the drivers side deployed and were smoking. I read on line that there have been numerous incidences of this happening with the tesla. Spontaneous acceleration with my foot not on the gas pedal. The car would not stop by the brakes! i could have been seriously injured or hit another persona or vehicle. The car hit the building as well as a natural gas pipe that was running along the buildings side at the level of my front bumper. I filed a police report.
I have had problems with my tesla1- doors repeatedly malfunctioning (stationary and driving)2- the accelerator was malfunctioning, city streets and freeways3- computer shot down while driving and screen went blank. Tesla deactivated safety features on the falcon wing rear doors.safety features were active when vehicle was sold, now they have been deactivated due to chronic problems with the doors closing reliably.instead of fixing the root problem witht he doors, tesla chose to deactivate the safety feature designed to prevent the doors from injuring hands, fingers, or arms that may damaged by the mechanically closing doors.tesla did not notify owners of the change.tesla sales reps described the safety features, but the company has not notified owners that those safety features advertised by their sales reps no longer work.

The control arm for the front driver side wheel broke all of a sudden. There was no impact that caused this. I heard a loud metal grinding noise while reversing the car out of garage, while the car was resisting to move back. The wheel also started rubbing against the walls & damaging it.called the towing service & they found the control arm broken & grinding against the wheel. The front suspension could have collapsed if i had driven even a little bit.finally towed back to service center, awaiting for a response from tesla service team

A suspension arm (tesla called it a 'front left fore link') broke on february 2nd, 2021. I was drive about 5 mph in a straight line to stop at a lighted intersection. There was not any undue strain. The car had less than 34k miles at the time. Towed the vehicle to tesla and paid $2269 for the repair plus $100 to tow.now the car wobbles (some call it 'whompy wheel') at low speeds (under 30 mph). Have an appointment to diagnose on august 6, 2021. Tesla wants $210 to diagnose.

My 2016 tesla model x has a serious shimmy in the front suspension as you approach 35 mph.the dealer has stated that the vehicle is operating as designed, and has no fix suggested for the shaking of the front suspension at speeds between 35 and 45 mph
Driver side upper control arm failed causing binding and creaking, further failure would have resulted in it leaving the vehicle un-drivable. I was told further failure while driving could have resulted in accident. Tesla service told me that it's an known issue.

The contact owns a 2016 tesla model x. While driving into the garage, the rear driver side wheel detached from the vehicle. The vehicle was not diagnosed or repaired. The manufacturer was made aware of the failure. The approximate failure mileage was 1,118.
Upper control arms on both driver and passenger side had to be replaced because of loud noises when going over even small bumps and also while attempting to steer car at slow speeds.each side was replaced in quick succession, about a month or two apart and was outside of the 5 yr/100000 mile warranty and service and part charges incurred.service date identified below approximate, dont have the actual date handy

Leaving child's soccer game, backing out of parking space lower front control arm on the front driver side occurred.my family lives about 30 mins from this and the vehicle seemed fine driving to our son's game.thankfully this did not happen on the tollway and highway that is used during this commute.back in april i had the tesla serviced to get a 4 wheel alignment.nothing was done then to inspect vehicle, even when a tsb was put out back in feb. 2017 for this issue.again, thankful for no injures, tesla is now wanting me to pay for the damage and then a new alignment, close to $1200.this when my family could have been fatally injured.

The contact owns a 2016 tesla model x. The contact was driving at 25 mph, when the brakes were applied before making a turn and the front driver's side wheel moved back to the fender which caused the vehicle to move up and down. The vehicle was towed to tesla (17731 pacific hwy, lynnwood, wa 98037) and diagnosed that two stabilizer links failed and two other links were failing. The left and fore links assembly needed to be replaced. The vehicle was not yet repaired and remained at the dealer. The manufacturer was made aware of the failure and stated that they could not speculate on the failure. The failure mileage was 23,125.

Upper control arm becomes issue prematurely. The issue needs to be looked at. I had to replace both of my upper control arm so far. Talking to mechanic, it was the design and material choice for this part that seems to be an issue. The control arm can break during highway speed pothole.

One of the arms connected to the driver's side front wheel completely broke off the wheel assembly while parallel parking at a normal speed.the entire weight of the front left of the car was then resting on the wheel.

In 2019 air suspension compressor failed- required 3500 dollars to fix, was told its wear and tear. Again on nov 20th air suspension failed sign coming up. Tires very close to body, car sitting low on road.

Was driving at 19 mph in city street and had to make a panic stop due to a car about to cut me off. Heard a loud bang. I got out of the car and the driver side front wheel is displaced. Further investigation i can see the lowerdriver side control arm had broken . Car was not drivable without a loud crunching sound. I had it towed to the tesla service center.

I refrained from making any further movement as the wheel well was acting as a giant brake on the tire.this morning, got a call from the service center, they confirmed that the driver side control arm had cracked and was broken. . as a precaution they are replacing the control arm on the passenger side too..i am surprised that the control arm cracked as the fatigue cycles on the part would have been low. I think when one is making an extreme turn, the control arm will be most stressed and it appears the stress analysis done may not have fully captured the loads during the extreme turn. I was driving at less than 5 mph while reverse parking when it failed. I shudder to think what could have happened if this failure occurred as i was on an expressway at speed, taking a turn/on a curved exit ramp, possibly made worse if i was on autopilot.please do not give tesla any more benefit of the doubt but instead take immediate action to prevent loss of life.

The front left suspension fore link failed, making the car undrivable. We were leaving our driveway at low speed, and the wheel collapsed, dragging against the skirt and creating loud noises and lurching. The steering wheel could not be turned. No collision or other unusual force applied. Car at 36k miles.tesla repaired, confirmed there was no collision. They said they replaced with an updated part indicating to us the original post is inadequate for safe driving. Since out of warranty we paid the $900+ to replace the inadequate part out of pocket.

I was getting home, driving into my driveway at a speed of 5 miles an hr. I braked to park the car and i heard a jerk and the car made a big noise and then hit the front of my home door. I reversed and braked to see if it would happen and it did and the car felt like it was very heavy. We then parked on the driveway and called tesla roadside assistance and they informed us to get it towed to a tesla dealership. Tesla, refused to pay for the towing or the repairs or provide a loaner car.we requested them to replace it as part of the warranty, which they refused to do. We understand there are many people who are dealing with a similar issue. This is very concerning that tesla is knowingly putting our lives at risk, instead of addressing a known issue.

The contact owns a 2016 tesla model x. Equipped with tire brand: bridgestone tires, tire line to1027257-00-8, tire size: 275/45r/20 cclxs, dot #unknown. The contact stated that the vehicle's steeringhad been shaking in the contact's hand while driving. The contact's daughter was driving the vehicle and she felt a boom in the vehicle. The two tires on the passenger side of the vehicle hadflattened.the vehicle was towed to the dealer tesla 52 derwood cir, rockville, md 20850 (301) 738-6523 . The dealer said that the entire suspension system had failed. The contact had the repairs completed at the dealer at a cost of $8,000. The manufacturer stated that the warranty had expired and would not assist with the repair costs. The failure mileage was 34,000.
While driving straight at about 30mph on the parkway, the connecting rods for the front left wheel suddenly failed. This caused a dangerous grinding of the front left wheel against the wheel well, damaging the tire and wheel well, while pulling dangerously to the left. The car was not drivable after the failure. The car had about 60,000 miles.
The car front suspension rattle a lot when accelerating. This was brought to tesla's attention but at the time, they did not have a permanent fix for it. This was dated back before the vehicle lost warranty.
I have been noticing my 2016 tesla model x 90d has been making a strange rattling noise from the front right tire/suspension area.it only makes that noise at lower speeds of 30mph or less.when i took it in for service on october 27, 2020, they shared that it was likely the air suspension system that needed to be replaced, but they weren't sure and they recommended we not replace it until it failed (i.e. Would no longer adapt to going up or down when we pulled into our drive-way, etc...).after reading the investigation that was recently announced, i thought it was good to share this information with your team as it could be a similar issue.the noise, which sounds like a rattling or something is loose, began around may of 2020.in addition, i have found it very odd that i have had to replace tires within one year of purchasing them because the inside area of both rear tires have worn down completely to the tread.during my last service on october 27, 2020, they said they would adjust the rear suspension so the tires would not wear out like that as it was likely due to an alignment/camber issue with how the suspension was riding.
The rear driver side lower control arm snapped off detaching the rear wheel/tire, brake assembly and strut/axel rendering the car immobile.fortunately this did not happen 20 minutes sooner as my gf and i were traveling on the highway at normal highway speeds - this would have caused a life threatening, catastrophic multiyear crash being unable to control the vehicle...we were backing out >1mph of a parking space in a very shallow curve when it snapped - we stopped immediately and i think the electronic braking tried to stop the wheel from rolling jamming it further at angle into the wheelwell.there was nor had there been any unusual impacts on this tire nor the vehicle from potholes/sudden stops or any accidents of any kind.
Ever since i bought my 2016 tesla model x i have been having problems with the mcu (main screen) it keeps freezing up, it won't respond to my touch commands, i can not change the suspension settings, driving settings, climate control, disabling pin and everything that is related to the screen which is basically all functions of the vehicle. My tesla is under full factory warranty and i have taken it to service for the same problem along with some other issues 4 or 5 times, every time tesla service center tells me that the screen problem is a known issue and engineers are working on a software fix for it. I have never been able to fully get a use out of all features due to this problem. Tesla has not been able to give me a timeframe on when they are going to send the update to fix the issue with the screen. Meanwhile me and my family are at the risk of not being able to start and drive the vehicle if screen is frozen and we would have to reset the system which usually takes several minutes. Backup camera doesn't show the picture, screen gets distorted. I noticed uneven tire wear which i was told by tesla that it was due to suspension settings and i was driving it in low mode. I can't change the settings due to screen not functioning. I have given tesla several opportunities to fix the issue and they failed. My last service they wanted to charge me around $1700 (50% off) to relapse the screen which should be fully covered under warranty. Problem happens during drive or while stationary.
My air compressor on the suspension broke. The car is so low to the ground now i could not clear a speed bump without getting stuck on it.in addition, the car bounces quite dangerously on even the smallest changes in road.i was told it was safe to drive by tesla as i am awaiting parts.at one point, the car went slightly airborne on the highway going below the speed limit.it's also very difficult on your back bouncing.i think it would be very easy to lose control of the car and get in an accident, especially if someone were speeding.
There is major rattling that happens in the front suspension and when notified this to tesla, they mentioned that they do not have a permanent fix for it yet. They have replaced the upper control arm in the past but the problem seems to come back.

While backing out of driveway we felt a clunk and heard grinding. We were on a local residential street. I let the car creep forward at no more than a walking pace and applied brakes. When the car was moving forward and brakes were applied an extremely loud grinding and groaning was heard. I had another person drive the car while i observed from the outside and observed the entire wheel shift when brakes were applied.

It has a ca salvage certificate.there is no damage to the bodywork to suggest that it has been in a collision but thesuspension has collapsed and the front rh wheel has jammed up into the wheel well causing some minor damage to the wheel arch.the air-bags have deployed which showsthat car stopped very rapidly due to the wheels locking up.the fact that this car is being sold as a salvage wreck suggests that it cannot be restored to a safe condition at a reasonable cost.there are enough similar cases to prove that there is a dangerous weakness in the tesla suspension.it is only through good fortune that another car has not rammed into the back of the car andslaughtered a child in the rearward facing trunk seats.in proportion to the small number of these tesla cars on the roads the number of written off cars in the junkyards is astronomical.please do not allow the alleged green credentials of these cars to override the safety issues that are placing lives at risk.not an owner.concerned citizen etc. Etc.

Car lost control at low speed on city street going straight after hearing creaking (under 35mph) therefore making me run into curb. Thought it was due to tire as i saw it damaged, but tesla service center said the control arm is broken also leading me to believe the control arm broke leading me to lose control of the car and causing damage to other vehicle parts.

I am not owner but am filing on behalf of chinese owner who had accident in montana at 2am 7/09/2016.driver not sure what caused car to veer to the right off the road but front right wheel is off. Tesla model x front left fore link failed while braking at an intersection on city streets 1/2 mile from home.accident occurred at 5 mph or less. Vehicle was driven back to home.any application of the brake caused lf wheel to move rearward and contact rear of wheel well.liner was ripped off, retrieved off the ground, and placed in trunk of car. Tire rubbed against wheel arch liner (wheel liner) ripping it off and destroying it.electrical lines behind liner had insulation damaged.hydraulic lines behind liner were loosened.disc brake dust shield was damaged.vehicle was towed to a tesla repair facility. While there were scratches on the leading edge of the fore link, indicating contact with an object, driver has no recollection of this event and if it occurred, it had to months (or years) before the failure at the knuckle ball joint ring.part was replaced at owner's expense.damaged part was requested but not provided for inspection.tesla took photos of the link prior to replacement (attached). This entry should be filed with nhtsa investigation pe 20-020.

Entering the intersection from a four way stop, the driver side lower control arm broke when brake applied firmly at about 10 mph while turning left.the vehicle made a few creaking sounds when parking for two days prior to the total failure. After failure driver side front wheel moved forward and aft when vehicle was moved and the tire impacted the fender liner and ripped the rear half out.
I was leaving my gym parking lot, backing out of my parking space. I tried to turn my car and i heard a scraping noise, and the car was difficult to turn. I was able to make it the half a block to tesla. (in the past i had this scraping noise/issue before and the car was serviced in jan 2019 when a tech rode with me to hear the scraping noise i was complaining about, and again in march 2021. At that time tesla stated the issue was tire related and the tires were checked with no issues observed.)on july 30, 2021 my car was entered into service at tesla service center- 6701 amador plaza rd., dublin, ca. 94568 at 8:30am. I let tesla know i was moving out of state august 8th, 2021. From 7/30- 8/16 tesla would set a pickup date and then call me to push back the date . This happened several times. I had to rent a minivan out-of pocket in order to complete my drive from california to colorado. Now on 8/18/21 my car is still in california, and i had to pay a shipping service to have my car shipped to me- out of my pocket. The issue with my 2016 model x was broken left and right forelinks as stated on the service invoice. Tesla has stated that these parts were replaced. In researching this, i found that this was a problem with other vehicles of my particular year/model. There was no warning when this happened. I could easily have been driving with my family on a crowded freeway during our move, but i was thankfully in a parking lot. I have paid the service invoice $878.10 out of pocket since my car is out of warranty. I feel tesla knew about this problem. They failed to diagnose/fix this issue the 2 previous times i took it in. My car was under warranty the 1st time. My car is available for inspection to the nhtsa, and i am planning on having it inspected on it's arrival to colorado next week.

I was driving my model x p90d on a local street 2 days ago. When i made a v-shaped right turn at the slow speed of 5 to 10 mi/h, i hit a very small pothole and heard a bang. There was no vibration at all, so i didn't pay attention at the time.the car was driving perfectly normal for the rest of that day. I drove another 40 miles or so, including 20 miles on the highway with 60+ mi/h speed.the issue came up the next day when i was pulling my car out of the garage. I heard another bang when the car was backing, and my steering became stiff. The car wouldn't move anymore. Then the "suspension needs service" warning came out the instrument panel. I came out and check the car, found out that the entire front wheel snapped off!i'm glad that it only happened to me at my garage but not on the highway.

While driving on the second floor of a parking garage at the local mall with the entire family in the model x (2 adults & 3 kids) to see a family movie, the rear driver side wheel "fell-off" (see attached photo's).it appears as though the control arm disengaged from the chassis of the car vis-a-vis a missing bolt or pin.the vehicle is brand-new (i.e., less than 1,000 mi), purchased directly from tesla, is driven by a mother of three, and is accident free.the car has not been subject to any extreme wear and tear or off-road driving conditions.tesla service manager is familiar with vehicle as it has been recently serviced for door/window issues - no frame or wheel issues prior to the incident.the vehicle was in motion, however as mentioned, we were driving in a crowded parking garage during holiday season.tesla road-side service was extremely helpful and arranged to have the car towed to the local service center - the car was obviously not drivable.tow truck driver had mentioned he had never witnessed anything like what he had seen with our vehicle.moreover, he did express to us that we were very lucky that the wheel hadn't disengaged from the vehicle while we were driving at a high speed (e.g., on the highway).the model x has been at the tesla service center for close to 3 weeks and despite our inquiries, we have not been expressly told what caused the incident or whether or not the issue is a more serious/systemic problem with the vehicle or tesla quality control.consequently, my wife is concerned for the well being of the other mothers/families she routinely see's driving the model x in and around our neighborhood.should a wheel disengage from the car at a high speed, the outcome is likely to be much different and much less favorable.
Left front suspension fore link cracked and fell off.vehicle was only backing up out of garage and steering wheel was turned to the left when a loud crack was heard from under the vehicle.the back up speed was only 1-2 mph.luckily, i was still in my driveway taking my kids to school.i can't imagine what the outcome would have been if it broken 5 minutes later on the highway with the kids inside the car.

The top ball-joint has popped apart.even in a crash it is unusual for a ball-joint to be popped apart like this.it is highly probable that the ball joint popping caused loss of control which then resulted in this crash. This car has done 95,000 miles. It is highly likely that the ball-joint was worn down sufficiently to allow the ball to pop out.many of these ball joint separations have already been reported to nhtsa and to the chinese equivalent organisationsamr/dpac.it would be a very good idea for you to demand a tesla recall in the same way that china has done.thank you.your friend keef

My car had a bolt brake from its thread. They told me it was a critical bolt that was marked with paint. This bolt is to keep the subframe attached securely to the frame.

While backing into driveway at low speed (<5mph), i turned the wheel sharply and heard loud clunk from front end followed by large noise of dragging. Exited car to see that front drivers side suspension was broken and dragging on top of wheel rendering car undrivable. Could have resulted in serious loss of control had it occurred at higher speed. Called tesla service who dispatched tow truck. Paid $450 to tow to nearest tesla service center where they diagnosed failed lower fore link control arm. Per tesla service bulletin sb-17-31-001 this is a known defect where the installed part did not meet tesla specs. Vehicle has under 28k miles, but was charged >$1100 for out of warranty repair. Seems bizarre that they acknowledged a defective part since 2017 but never replaced during any service visits and now will not replace because vehicle is out of warranty.seems to be a very common issue, so why hasn't there been a recall?

However, the tire has had the tread severely worn away over the entire circumference. This could not have happened after the impact unless full power was still applied to the motor post impact. I went to start my car and the screen was black. My car hasn't been driven for 5 months since i was traveling. I had cleared all of the memory (navigation, addresses, music stations, etc.), prior to leaving. I was in traffic and the turning signals didn't work. The safety of the car felt compromised since the turning signals,rear backup camera, suspension -basically most of the car controls were in-operatable.

I am simply adding more data points that may possibly have been overlooked.when a wheel contacts the inside of the well and locks up it will cause the wheel on the opposite side to instantly double its rotation speed due to the way in which the cars differential operates.even if the accelerator pedal is lifted the kinetic energy stored in the fast spinning tesla motor will still give a surge in power to the opposite.wheel.i'm not saying this is 100% proof of what happened but i do believe it is a possibility. I hear a loud noise.the vehicle started making a loud noise.i determined that some sort of catastrophic failure had happened in the front passenger side suspension.the wheel well liner was torn from the front wheel well.the tow truck driver visually confirmed that the front passenger suspension had failed (they mentioned a part by name, but i don't recall what they said).the vehicle was towed to tesla service watertown, where a few days later they confirmed, in their words "found rh front fore link, wheel arch liner, and fender garnish require replacement, and vehicle requires alignment".they denied the service under warranty, stating that in the view of their service technicians, i had struck some sort of obstacle causing the failure.for the record: i did not.there was no notable coliision or road hazard strike in the period leading up to this suspension failure.i paid $1319 out of pocket (and lost the use of the vehicle for 5 days waiting on parts) to restore the vehicle to service, and there's still a deferred item from the service item i haven't addressed yet: "front camber was not able to be adjusted due to hardware, and will require replacement in the future.also noted vehicle has vibration under acceleration.recommend scheduling follow up for axles and motor mount."when i read today that the nhtsa was initiating an investigation into something similar, i wanted to add my data to the complaint.attached is the service invoice from tesla related to this issue.

It looks highly probable that the wheel was sliced in half whilst it was still rotating and that almost certainly happened before the crash.there is no other make of car in which near new cars are showing up in the junkyards with broken suspensions and sliced wheels at anywhere near the frequency that tesla cars do.please investigate the reasons for all these failures.thank you.please note that i am filing these reports as a concerned citizen.i am not a tesla owner and i am making these reports because i believe there is a clear pattern of repeated failures in the tesla suspensions.date of crash is an approximation.
The contact owns a 2016 tesla model x. The contact stated while driving 5 mph, the contact heard a squeaking sound coming from the front of the vehicle. The contact continued driving to the residence. There was no warning light illuminated. The vehicle was taken to a local dealer to be diagnosed. The contact was informed that the driverā€™s side upper control arm needed to be replaced. The vehicle was repaired however, the failure recurred. The vehicle was taken back to the same dealer and the passengerā€™s side upper control arm was replaced. The contact stated that a year later the failure recurred on the passengerā€™s side upper control arm and the vehicle was repaired. The contact stated that the front and rear suspension were leaning to the passengerā€™s side. The vehicle was not repaired. The manufacturer was not made aware of the failure. The failure mileage was 49,451.
Tesla has an active recall on their mmc chips but says my screen has to be totally blacked out before they will replace. I live 2 hours from the nearest dealership and i'm sure this will happen at the worst time. It takes sometimes over a minute for my car to even turn on now and warm up
My vehicle's center display (mcu) is starting to show signs of failure on account of a flawed design that depends on an emmc memory component that is known to fail catastrophically after several thousand write cycles, causing loss of critical adas and rear view camera functions, hvac controls, and other basic vehicle functions. Now, i am facing the prospect of having to pay tesla thousands of dollars to replace the entire mcu in my just-out-of-warranty vehicle. Tesla has tacitly acknowledged the safety implications of their original design by modifying it to use a more robust memory component in 2018, and they should be made to recall pre-2018 models to ensure that nobody else has to endure what i and many hundreds of others have experienced.

I was driving my model x p90d on a local street 2 days ago. When i made a v-shaped right turn at the slow speed of 5 to 10 mi/h, i hit a very small pothole and heard a bang. There was no vibration at all, so i didn't pay attention at the time.the car was driving perfectly normal for the rest of that day. I drove another 40 miles or so, including 20 miles on the highway with 60+ mi/h speed.the issue came up the next day when i was pulling my car out of the garage. I heard another bang when the car was backing, and my steering became stiff. The car wouldn't move anymore. Then the "suspension needs service" warning came out the instrument panel. I came out and check the car, found out that the entire front wheel snapped off!i'm glad that it only happened to me at my garage but not on the highway.
In summer of 2020 my screen for odometer, speed and auto pilot status went out. The driver screen went black and never came back on. Tesla service request a replacement which i had to pay $1500+, few month later my steering wheel was very loose and offbeat. Again, i had to pay out of pocket. Which i believe was dangerous to drive because i could not turn very well. Now, my car takes very long time to turn on and the driver side window does not close all the way.
2 months after my 4-year warranty expired, my car's main computer (mcu) "failed" by going black, apparently during the course of an over-the-air software update by tesla.i lost all control of my climate, charging ability, adjustments to suspension, safety sensors (door sensors, lane sensors, other vehicle sensors), remote access via my cell phone, and most importantly my turn signal indicators.this makes my car illegal to operate.tesla "recommended" an "upgrade" to a new computer (mcu), but now appears to have forced me to upgrade by disabling most of the safety, convenience, and comfort features of my automobile.coincidentally (or not) my mothers tesla automobile experienced the same mcu failure on the same day (she will file a separate report).
On the first used of the day, the instrument cluster takes about 45-60 seconds to boot up.this makes the car underivable until the screen is active.i have had the issue since july, tesla has investigated the logs numerous times to no avail.i am now being told it is a firmware issue they are aware of.but, there is no timeline for a fix.i find this very dangerous and annoying.if i had to move the car quickly in any kind of an emergency, i cannot do so.the car is completely underivable until the instrument cluster boots up.other than waiting, the only option i have been given is to pay $2,500 for a newer version of the main computer.this is totally unacceptable to me, as the car is still under warranty.as per your note below, the car is always stationary.as said above, it is underivable until the system boots up.
Falcon doors not closing, rear driver seat electrical dead, warning light blocking backup camera reading rear seats not in place, all insulation peeling off above tires, bubbling paint, trunk and side panels misaligned so tilted trunk hood and finger goes in one gap but fingernail only barely in other side gap, wavy insulation along rear, front passenger door motor spins.permanent seat depression in rear. On july 3rd, 2020 we observed that our center console was completely dark.this impacted many components and features of the vehicle including but not limited to the heating/cooling, rear camera, audio, bluetooth, gps, side mirrors, etc.we were notified by tesla that an mcu was ordered and we would be notified when it arrives.as of 7/29 we still do not have a date for when the new mcu will arrive.

I was driving my tesla model x ev home and there were no problems. When i pulled into the driveway the self driving features accelerated the vehicle causing an accident.
The car was on autopilot. My model x autopilot ran over a recliner at 60m/hthat was in the middle of the highway. It was around 6 pm, weather was nice and clear.i tried to avoid it by moving to right lane but it hit the driver side of the front of car causing minor damages to car. I tried to reach out to tesla customer service but they told me [4 or 5 times]that someone will contact me and no one ever did. Everytime i talked to tesla [by phone and email] they ask me for more information and then tell me someone will call you back. I wanted to know why didn't the car slow down or even change lanes and how can i avoid this in future specially that i use this car for my 2 kids. I had been trying to reach out to tesla since october 2016.
The vehicle started taking longer to start up about a few months ago. Then the system needed to be rebooted on a regular basis to be functional. At one point the main screen that controls all the safety and visibility features of the vehicle stopped working and the vehicle would take up to 5 minutes to start up. After doing some research this is a common issue with tesla's older than 2018 where they encounter an "emmc failure" of the mcu. The defect of the mcu was addressed when tesla replaced the mcu in its vehicles in 2018. This should be treated as a recall due to the design flaw of the manufacturer. The mcu controls all the safety features of the vehicle and the rear view camera. Without the mcu it makes it extremely difficult to have the visibility needed to go in reverse.
I had my tesla model x mcu replaced per the recall. Prior to having the mcu replaced i had numerous (more than 20) episodes of the car just shutting down and losing all power and would not restart for anywhere from 3 to 20 minutes. Many times after the vehicle would restart the main control screen would remain black, there would be no access to any of the controls usually accessible there. Other times there would be no navigation, radio or hvac available. When reporting these issues i was told to keep trying to reboot that it was a known software issue. When the recall was finally announced i checked frequently for appointments for a replacement chip which were not abundantly available. When i was finally able to get an appointment for the recall, the service center assured me that the replacement mcu would make things better but i also had the option to upgrade to a better chip for $2k but lose many options like fm and xm radio, which i use. Since the mcu replacement i have continued to have these same issues and my phone will no longer pair with the vehicle to provide the hands free use i had previously. I have worked with tesla to try to resolve these issues and i am told by their technicians that this is a known software issue that is happening to "older" model s and model x vehicles and there is no current projection as to when this will get resolved as it is on the back burner. My car is a 2016 with 40k miles! how is this ok? how is this not a safety issue? today the car went completely dark/dead while i was driving!!! luckily i was driving in a parking deck and there was no resulting accident. I have called tesla customer service and messaged the local service center to report these issues multiple times. Even the corporate customer service rep has difficulty reaching them directly. It is so sad to have such a nice vehicle with issues like this. We have talked about trading it in but ethically cannot fathom passing these known issues on to someone else.

Mcu (media control unit) failed on multiple occasions, i was unable to control ac/radio/autopilot system/ or navigation. Also both turn signal were in-operational. Tesla service center refuses to replace the mcu or fix it. Vehicle was in motion, on highway at a speed of about 70 mph.
Mcu-touchscreen emmc failure - mcu1 (version 1) unit in tesla, including my 2016 tesla model x has an emmc chip that cause failures resulted in whole central screen going blank, freezes, and as result make car not-operational - no navigation, no climate control, no other controls of the vehicle, since it's all driven through central screen. Considering my car if just 3.5 years old, this is really concerning and tesla should address this technical flaw.
Center screen (mcu) dies/freezes up while driving and must try to reset all the time. Tesla refuses to accept resposibility and replace with new mcu under warranty.

The mcu has failed completely rendering the vehicle undriveable.it was in the parking lot and when i went to drive it, the computer would not come on.the computer had been having problems for quite some time but tesla supposedly fixed it previously.
While i was driving on the freeway, the roof tap flew off the car in mid-june 2020. I made an appointment and had it fixed, and complained to the manager at burbank service center in los angeles. They ignored my complaint and the safety harzard posted to the safety of the freeway. I requested they repaired it under the warranty. They refused, and charged me over $1400 for the replacement of the new roof top. Now, i saw the recall, and i tried to get refunds for the replacement. They refused.
First encountered while driving to town and it just kept getting worse over time,almost did not make it home.had to pay about $700 to have it towed to the tesla repair center in denver, colorado. Yellowing of the screen and the screenclicks in multiple places during a single click of a button -car was on some kind of uv treatment for 73 hours -not better at all -also the navigation unit resets all the time sometimes leaving you unable to activate air conditioner.tesla tried to push me to buy some and of unit update 2k - not acceptable -the car is in perfect condition but the screen'. The tegra daughter board failed inside the mcu, which is the "brain" of the car.tesla had known this would happen and yet they did not do anything about it beforehand.the central screen of the car would not turn on despite me trying reboot the computer 15 times. The air conditioning would not work.i had to drive my tesla 30 miles without the turn signal working and the falcon door half open (falcon door could not close completely because the central computer was not working). I am a widow with 3 young kids less than 10. I cannot imagine what would happen if the kids were in the car with me.i paid $$791.70 for the replacement of the daughter board. Several months later i received an email saying that i might be eligible for reimbursement. However, when i logged into my tesla account, it says my car is not eligible for reimbursement without providing a reason. I reached out to tesla support through email. But the email offered no help. It just says the team that reviews emmc related issues will not receive forwarded cases from tesla support and that i will not receive any replies from them if i send them further emails.i did reply to the email and never receive any reply from them. I believe that i'm eligible for reimbursement and i would like the emmc team to review my model x information.i would like to get in contact with the emmc team and be provided a reason for my ineligibility for reimbursement.
While turning left to enter a very narrow garage entrance i needed to determine whether or not i had to straighten out before pulling in or if my left turn was tight enough to pull in without reversing to straighten out. I saw that i was in the position that i could continue into the garage and lightly pressed the accelerator to finish my turn into the garage. It was at this point that the car accelerated with extreme force and within a second slammed into a large concrete pole that was just inside the garage to the left.i never felt the car slow in that moment, only speed up and i believe the car slammed into the pole while accelerating and would have continued to accelerate if not for the large pole. I did not have either foot depressed on either pedal at the moment of collision. The air bags did not deploy, but there was very severe damage to the front end of the car that will be at least $25k.i was not on the phone or distracted in any way. I was driving carefully and paying full attention. This is not a case of mistaken pedal because i was intending to accelerate. At first tesla told us over the phone that their logs show that the driver pressed the pedal 100% and then tapped the brake before impact. This explanation sounded physically impossible because the distance covered was less than 3 car lengths. A month later tesla sent a letter stating the driver pressed the accelerator 100% until the vehicle sensed a crash. Tesla did not respond to our query about why their log story had changed. Tesla also refused to provide data about accelerator/brake percentage and car speed for the car earlier in the day. If a driver is pressing the pedal 100% it is a very deliberate action. This is a failure of the accelerator and the automatic braking. The car accelerated on its own and crashed full force into a large concrete pole.
My screen goes black.this recovers over time when i reboot, but if i am checking mileage the screen goes black again.people who have the same car have told me that this is a known problem with the computer memory for this vehicle.this has happened when the car is stationary and moving on a city street. I am not able to get a service appointment until august 7, 2020

Tesla continues to make thousands of new cars but it has yet to resolve the recall (21v-035) claiming that it lacks parts.my tesla has had screen blackouts and has lost important functions while driving due to the inadequate chip capacity which needs replacing under the recal.
The mcu (main graphical display) will lose all sound (loss of warning sounds, blinker sounds, loss of radio), freeze (requiring a manual reset/reboot, or take a long time to boot before the car will start.this happens frequently (many times a month for the last 6-9 months).it has been somewhat less often after a recent software up from tesla.one should not have to reboot their main display to return functionality while driving (driver distraction potential).
On saturday october 24. My touch screen when blank.for several hours with 1:20min while driving.happened between 9am to noon.i won't be able to use any control beside driving .after i read an article online which mentioned tesla willfix it for free and scheduled a repair on nov12. They then cancelled my appointment by saying my problem caused by software glitch not hardware. Please see attached screenshot of their response on nov 14!

Unless i had taken over i believe would have hit the car adjacent to me in the next lane over.after adjusting and regaining composure, i quickly looked at screen and saw that road striping was dimmed and i was unable to re-engage ap. It appears that the white concrete roadway coupled with intense sun facing car was impacting the x's ability to traverse the road properly."note having hands near the wheel or lightly resting on the wheel (coupled with autopilot induced inattention) is not good enough to prevent a crashplease bring this uncontrolled experiment to an end.
My tesla model x has severe windshield secondary image separation (ghosting).the lights of oncoming cars have triple images that are radiate several meters above the origin of light. This makes driving at night a dangerous and exhausting experience as i try to differentiate between the real lights and the secondary images. Additionally, the tail lights of autos are also doubled making the cars in front of me blurry. Lastly, traffic lights are also displaying secondary image separation making it difficult to accurately read traffic lights. Currently, i am avoiding driving at night to avoid the danger of seeing multiple images. May 2020 while my car was still under warranty i continually had issues with the main screen. Issues were reported to tesla who asserted it was the firmware. Tesla kept my car for over a week for repairs, then charged me $400 for new firmware. However within two months the issues began again, the screen going black and multiple errors. In august i was finally given another service appointment. This time they asserted it was the 12v battery (despite recently having that replaced in 2019). After replacing the 12v battery the problems with the mcu still persisted. It takes about 3wks to 1month to even get a service appointment. Now they are saying the issue is with the mcu which needs to be replaced. I do believe the issue was with the mcu all the time. I do believe tesla either misdiagnosed orknowingly did not resolve the issue. Now i am being asked to pay $2500 not including tax and labor to replace the entire mcu for problems which were initially reported while the car was under warranty. Further, tesla is now claiming they no longer have logs of diagnostics on my car when i requested to see the diagnostics that were previously run diagnosing my black screen althout the do agree my car has been continually brought in for a black screen and non-functioning system. Disabled screen functionality causes several other systems of the car not to work: regenerative braking disabled, autopilot disabled, driver and user settings, disabled, ac unit not functioning, stereo/radio capability disabled. Each day a different alarm or not would come on disabling another function while driving. Very dangerous and very unnerving, especially while transporting three young children. I have asked tesla for diagnostics logs on my car and hoping to receive and submit them as support for this complaint.
Sudden unexpected failure of mcu1-the main control touch screen of the car. Limiting access to key safety features such as back up camera and other vehicle controls.the vehicle had an unusual display behavior and took a prolonged time to wake up, then the center display failed to turn on at all. Manual reset attempts failed.
My touchscreen went black and systems became disabled (air control, autopilot, music, navigation). Within a week the readings on my phone app were not correct and it took some time for the power to engage the gears. My backup camera went dark. Finally, i could not put my car in gear and it had to be towed by a specialty towing service. Tesla tells me it is a software issue, but i think a memory chip has gone bad. I think this relates to an existing investigation on this issue by nhtsa.
My tesla model x 2016 accelerated on its own after i come to a complete stop at a parking lot, crashed into the front of a pediatrics building. 2 injuries. Airbag was inflated.
An accident occurred in my tesla model x 90d that was due to the fault of the car. We have owned the car for 3+ years. What occurred today in the car is something that has never happened before. The car was functioning perfectly fine during my whole 20 minute commute. As i pulled into my office's parking lot, the car seemed fine. However, as i was finishing parking the car in the parking space, the car unexpectedly started accelerating at a frightening speed. More clearly put i was almost done parking and my foot was partially on and off the brake. While my foot was slightly off the brake the car accelerated so fast that it ran over a curb and climbed a grass slope and ran over and tore down a small tree. The car accelerated so quickly that it ran over the tree before i could even react. After this experience i no longer feel that it is safe to drive a tesla. I'm just happy that there weren't any people or cars in front of me. This is a major safety issue and needs to be investigated.
The door seals on the falcon wing doors leak anytime there is any amount of water on them. Tesla knows about the faulty seals and used improved seals for newer models.
I was driving on the freeway on 'auto pilot', and the car suddenly braked for no apparent reason. The truck behind me was driving close and rear ended me destroying the car's rear portion ( insurance is declaring it a total loss ), and i had a whip lash injury but survived. This had happened before also but there was nobody behind to collide. I had complained about this twice to their service center but they said 'they could not find anything wrong', so continue using the auto pilot .
Model x's mcu touchscreen completely gone dark. This basically disable all back-up camera, turn signal sounds, gps, navigation, vehicle controls: door locks/unlocks, lighting, ac, etc.. In my instance, the vehicle's screen did not boot upon start up of the vehicle. The car is drivable but unsafe because of all the above mentioned disablement. The touchscreen in this car, which tesla misleadingly calls the infotainment system, has failed repeatedly, leaving the car undrivable and even unable to be placed into neutral. In the first instance, all the car's computer systems went dead and the car locked up (parked at home) and had to be dragged onto a tow truck. Tesla replaced the entire center screen (at my expense) in july 2020. In another instance about a month later, the screens died while in a grocery parking lot. Eventually the power screen came back, which allowed the car to be driven home, but with no control over lights, climate systems, rear camera, etc. In that case tesla repartitioned the hard drive, which made the car operable again for a couple of months. Both screens are now failing again, which makes the car unsafe to take anywhere. Just to be clear, despite calling the center screen an "infotainment" system, that screen is necessary to operate the car safely. Without the backup camera, there is no visibility behind the car. It also controls headlights, climate, interior lights, and other essential systems. Based on anecdotal reports in tesla forums, similar problems seem to be happening to many 2016 model xs, which suggests that the cars should be recalled.
Although the nhtsa had pushed tesla on the refresh rate of the memory and tesla had remedied the memory on my car that is only one issue.the computer has slowed to a point that the car no longer operates consistently and is there a risk.there needs to be mandatory testing that software upgrades do not make any of the processes that the car is supposed to do inoperable or inconsistent. I had recognized many issues over the last 1.5 years that my car was not working or was extremely slow in reacting and have had my car brought into tesla because of it many many times. I then heard about the memory issue and pushed tesla to refresh it but that still has not resolved the regular issues.this is currently impacting tesla but will impact all cars in the future as computers are the main inner working for vehicles such as evā€™s as far as my wife is concerned about her car she has a $125k anchor.teslaā€™s reply to me is to spend over $2,000 and upgrade the computer.

Ever since i bought my 2016 tesla model x i have been having problems with the mcu (main screen) it keeps freezing up, it won't respond to my touch commands, i can not change the suspension settings, driving settings, climate control, disabling pin and everything that is related to the screen which is basically all functions of the vehicle. My tesla is under full factory warranty and i have taken it to service for the same problem along with some other issues 4 or 5 times, every time tesla service center tells me that the screen problem is a known issue and engineers are working on a software fix for it. I have never been able to fully get a use out of all features due to this problem. Tesla has not been able to give me a timeframe on when they are going to send the update to fix the issue with the screen. Meanwhile me and my family are at the risk of not being able to start and drive the vehicle if screen is frozen and we would have to reset the system which usually takes several minutes. Backup camera doesn't show the picture, screen gets distorted. I noticed uneven tire wear which i was told by tesla that it was due to suspension settings and i was driving it in low mode. I can't change the settings due to screen not functioning. I have given tesla several opportunities to fix the issue and they failed. My last service they wanted to charge me around $1700 (50% off) to relapse the screen which should be fully covered under warranty. Problem happens during drive or while stationary.
While driving the car on july 31 at about 7pm, the center touchscreen went black and would not reset using tesla instructions.vehicle was parked.the a/c and climate controls were therefore stuck in the on position with no recourse.all other functions handled by touchscreen were inoperative. Tesla roadside assistance was called on morning of aug 1 for oiptions, but reset protocols offerred did nothing to help solve the problem.car taken in to tesla service center on aug 2, with inoperative mcu identified as the cause on aug 3.repair cost estimated to be about $1500. I then found out the repair was out of warranty only because tesla had shortened the warranty by 2 years after i purchased the vehicle.if the warranty was as stated when the car was purchased, this repair would have been covered by tesla.sorry, no useful pics or docs appy at this point, as repair has not yet been made.

I am complaining because tesla has failed to remedy safety recall # 21v035for my vehicle in a timely manner.soon after i bought my car in 2016, i started experiencing issues with the center display, which tesla was unwilling to address. Finally in 2021, nhtsa recall # 21v035 was created that would address my safety issues. In this recall dated february 5 2021, tesla agreed to "tesla will replace the vcm daughterboard that has the hynix 8gb emmc with a vcm daughterboard that has an enhanced 64gb micron emmc, free of charge."on february 2 2021, tesla emailed me: the email says in part: "tesla has decided to voluntarily recall certain model s and model x vehicles built before march 2018 that are equipped with an 8gb embedded multimediacard (emmc) in the media control unit because the emmc may malfunction due to accumulated wear. Our records show that you own a model s or model x affected by this recall. ...additionally, tesla will upgrade, free of charge, the emmc on your vehicle with an enhanced 64gb emmc.there is no need at this time to contact us as we will notify you when the parts become available. "on march 5 2021, tesla emailed me again saying ""your vehicle is subject to conditions covered under the 8gb embedded multimediacard (emmc) recall that addresses the malfunction of the emmc memory chip due to accumulated wear. "every time i have contacted tesla about this recall, tesla has given me the runaround stating they are "still waiting for parts to become available", by which they presumably mean the "vcm daughterboard with the enhanced 64gb emmc". This is a very effective way for tesla to avoid the responsibility and cost while my safety problem persists. As a reminder, the recall is now nearly 6 months old, and the underlying safety problems have persisted for years prior to the recall being initiated.tesla must fix my car promptly by either: a. On april 30, my husband was driving my new model x on the freeway. It was a sunny day. He disengaged autopilot to change freeways. After he changed freeways, he re-engaged autopilot. The car followed the curve on the new freeway, turning to the right, but when the freeway straightened out, the car kept turning and hit a guardrail on the right side. No warning, no braking, no stopping. Tesla tells us that the autopilot was not re-engaged and was just in cruise control (i don't know how the car would be following the curve in the road then). Also, that it's algorithm correctly instructed the car to hit the guardrail at full speed rather than to stop or beep a warning. At best, tesla suffers from a design flaw (how could my husband incorrectly think he was in autopilot mode; how could the algorithm make us crash when the freeway was pretty empty). At worst, tesla is covering up their mistakes.
The center display and sometimes even the main display black out during driving. Most times the system requires a reboot which can take a few minutes. During the time where the screen are not visible you have no control over headlights or the ability to defrost the windshield. In addition the camera which is displayed while driving is also unavailable. If the center screen goes black then you can't see how fast your going or if your park, drive or reverse.
The mcu mdlsx,na premium,reman part #: 1098010-99-h is faulty and needed to be replaced.there are many people complaining of the failure in various forums and it is all happening at the same time.the options from tesla is to repair/replace for $1200 and back ordered for 1+ month or to order an upgrade mcu intel upgrade-for tegraap1.0 - north americapart #: 9874564-00-a for $2500. Vehicle was stationary, long start up times before i can initiate drive as shown in picture just a t logo for about 10-20 seconds.also stationary and in motion can not pull up some items like vehicle charging or features just get a blank white screen as shown.all have to do since it is on mcu1.
Main touch screen failed to load. Same screen control almost everything from ac to suspension, navigation etc. Car was parked when screen failed to load . Same issue happened few times before in span of one week , however eventually screen booted while driving causing me to get even more nervous what if screen fail again while i'm towing trailer ( towing mode is also controlled in same scree as well as rear camera ).
There are about 4-5 bolts which secure the bottom plastic cover in between the rear wheels, see photos, the cover will come loose and fall out even at normal used. If the car continues travel on the freeway, without the bolts, the covers can easily fall and hit the behind vehicles.that might result with serious accidents especially on the freeway with high speed. The car only 18k miles from 2016- 2019, low mileage, had been service twice in last two years. Tesla needs to provide schedule annual inspection during the regular maintenance to inspect the bolts (screws which secured these bottom panels/covers) to make sure the bolts (screws) are tighten and the covers are secured. That also should be covered under the 4 years vehicle warranty, not just blame the clients, and try to send the client to contact local body shop. It is their responsibility for the manufacturer to make sure their vehicles are safe to drive and to be on the road within the warranty period under reasonable used. They also need to prevent future accident with their products.
My mcu failed which ended up immobilizing the car which required a flat bed tow to a nearby tesla service center. At that time, tesla replaced the mcu emmc in sept 2020. Since this part has been replaced, the backup safety camera and other functionality (e.g. Navigation, user interface access) will become non-functioning frequently. This can be non-functioning for a few seconds to a few minutes. Sometimes the backup camera will freeze the object on screen and not move or be solid black. Overall the hardware "fix" has caused more issues than before the failure. I also have an extended tesla warranty, and tesla will not replace this more problematic mcu now with a newer mcu since tesla classifies this as a "performance" related issue. Tesla service wants me to spend $2500 to fix my backup safety camera issue or wait an unknown period for an unknown software. It has been almost 4 months and there's been no fix. I want tesla to replace my mcu with another mcu1 or a mcu2 which tesla confirms fixes the camera issue.

My car had a bolt brake from its thread. They told me it was a critical bolt that was marked with paint. This bolt is to keep the subframe attached securely to the frame.
My mcu has begun to fail. Not allowing me to have total control of the car. Examples are... Safety issues like loss of backup camera, loss of turning signals, loss of emergency hazard lights, loss of headlight controls, unlocking doors from inside of the vehicle, and opening electronic doors.
The mcu (the car's center screen) would not turn on. The center screen controls everything in the car including all the updates. Since the center screen didn't work, and update was not able to pass through, and it essentially made the car not drivable. The car was in the turn off mode and would not turn on.
On 10/31/2019, my wife was waiting in line at a carwash. While waiting for the car in front of her to move out of the carwash tunnel, she had her foot fully pressed on the brake pedal and the 'hold' sign was showing on the instrument penal screen.approximately 2 minutes later, she released the break paddle, but before she could press the gas peddle,the car suddenly moved forward by itself at almost full acceleration.my wife immediately attempted to stop the car by pressing the break pedal in full, the car slowed down, but did not fully stop and kept moving forward until it hit the car in front of it. Before and during the entire accident,sophie kept full humanly possible control over the car,but again, the car abruptly accelerated by itself and did not come to a full stop when break pedal was pressed.fortunately there was no injury to anyone, but the front bumper of the car is dented.

I was going up by driveway waiting for my garage door to open. I took my foot off the accelerator and was slowing down without hitting the breaks waiting for the garage door to open. The car took off through the garage door and hit my husbands car sitting in the garage.
While turning left into a parking spot at a very slow speed, the car suddenly accelerated with extreme force.it ran over a curb and collided with a tree and a truck.there was only light pressure on the accelerator.the automatic braking and the airbags did not deploy.there was over $18 000 damage to the two vehicles and the tesla model x is not driveable without repairs.
Takata recalli drove my tesla model x 2016 today to work and when i was about to park the car in the parking lot (around 6 miles per hour may be) it suddenly accelerated and hit the concrete wall and bounced back around 8 feet. Since it was for parking i can surely say that i did not accelerate the car.the steering air bags and knee air bags came off and also the passenger side knee air bags came off as well. I searched online and there seems to be a class action suite on this issue but tesla is not accepting it as the glitch in their software or some other component. I felt like the accelerator got pressed the way when the car was in cruise mode. Unless i wanted to hit the wall intentionally there was no need for me to press the accelerator to speed from almost zero to whatever the high speed it attained at the time of hitting the wall.
While turning left to enter a very narrow garage entrance i needed to determine whether or not i had to straighten out before pulling in or if my left turn was tight enough to pull in without reversing to straighten out. I saw that i was in the position that i could continue into the garage and lightly pressed the accelerator to finish my turn into the garage. It was at this point that the car accelerated with extreme force and within a second slammed into a large concrete pole that was just inside the garage to the left.i never felt the car slow in that moment, only speed up and i believe the car slammed into the pole while accelerating and would have continued to accelerate if not for the large pole. I did not have either foot depressed on either pedal at the moment of collision. The air bags did not deploy, but there was very severe damage to the front end of the car that will be at least $25k.i was not on the phone or distracted in any way. I was driving carefully and paying full attention. This is not a case of mistaken pedal because i was intending to accelerate. At first tesla told us over the phone that their logs show that the driver pressed the pedal 100% and then tapped the brake before impact. This explanation sounded physically impossible because the distance covered was less than 3 car lengths. A month later tesla sent a letter stating the driver pressed the accelerator 100% until the vehicle sensed a crash. Tesla did not respond to our query about why their log story had changed. Tesla also refused to provide data about accelerator/brake percentage and car speed for the car earlier in the day. If a driver is pressing the pedal 100% it is a very deliberate action. This is a failure of the accelerator and the automatic braking. The car accelerated on its own and crashed full force into a large concrete pole.

I'm certain that i didn't accidentally activate cruise control/ap, so there's no way that could've caused it. My theory is that the regenerative brakes may have given me a sudden kick of acceleration? i'm kind of worried now, because this is actually the second time something like this has happened to me, except the first time wasn't nearly as bad, so i didn't ask you guys about it. Has anyone else had this happen to them? do you guys think i need to ask my tesla team about this?this hasn't only happened to me while getting ready to park, the first time i was just accelerating up to 25 in my neighborhood and it suddenly went to 35 in a second but i wasn't too bothered about that, because it was just a 10mph burst, but this one that happened to me today was the car jumping 30 mph... I've driven this car for 2000 miles now and it's the only car i've been driving really over the past month.
This is a complaint about the tesla model x and model s unintended acceleration incidents.it appears that tesla is still using an accelerator pedal that has only a single sensor.this pedal is supplied by ford who have a history of ua incidents involving electronic throttle control.some manufactureres have now swapped to a dual sensor 6 wire system.in the event of a bad input from one sensor this triggers a fault code and disables the accelerator.tesla has repeatedly claimed that in all these incidents the logs show 100% throttle.this can easily happen due to dirt or whiskers on the sensor.the log would be unable to show whether it was a genuine case of pedal pushed to the floor or an electrical glitch.i should not need to remind you of the history of toyata and ford ua cases and the nhtsa.tesla nd its supporters constantly repeat the old claim that all cases of ua are caused by driver error.that was true for mechanical throttles but it has been proven that electronic engine controls are susceptible to emi or spuriously induced glitches.with the tesla's insane and ludicrous acceleration the results are even more dangerous than for a less powerfull vehicle.please investigate as to whether tesla is still using a throttle sensor that does not have dual circuit safety protection
An accident occurred in my tesla model x 90d that was due to the fault of the car. We have owned the car for 3+ years. What occurred today in the car is something that has never happened before. The car was functioning perfectly fine during my whole 20 minute commute. As i pulled into my office's parking lot, the car seemed fine. However, as i was finishing parking the car in the parking space, the car unexpectedly started accelerating at a frightening speed. More clearly put i was almost done parking and my foot was partially on and off the brake. While my foot was slightly off the brake the car accelerated so fast that it ran over a curb and climbed a grass slope and ran over and tore down a small tree. The car accelerated so quickly that it ran over the tree before i could even react. After this experience i no longer feel that it is safe to drive a tesla. I'm just happy that there weren't any people or cars in front of me. This is a major safety issue and needs to be investigated.
Main computer system died. All screens stopped functioning (including speedometer). Car wouldnt lock/unlock. Tesla said the mcu had to be replaced and they were going to charge me because it was 5 days out of warranty.
Attempted to merge onto a freeway from a stop signed parallel entrance. After a vehicle in the freeway lane had just passed and there was adequate space before the next vehicle, i attempted to accelerate rapidly from the full stop. The vehicle accelerated for only a very brief period, enough to get it into the lane at maybe 10mph before the acceleration cut out completely and a vehicle collision warning and audible alert appeared on the screen in front of the driver. There was not actual collision threat present and i tried pressing the accelerator pedal repeatedly to avoid being hit in the rear by the next vehicle coming from behind at full freeway speed. The vehicle behind me changed lanes and went around me to avoid an accident. After a few seconds the warning went away and i was able to accelerate normally to freeway speed.this safety feature seems extremely dangerous for several reasons:* it incorrectly senses that a collision will occur with acceleration in some instances* it disables user control of the vehicle (acceleration)* it disables that control for an extended period of time even after the false threat is no longer detected* it allows some movement before disablement creating a situation where the vehicle is put in harm's way without any ability to safely accelerate away from an impending accident. Had the vehicle not moved at all, there would have been inconvenience but no risk* there is no ability in the current software to disable what i believe to be unsafe functionality intended for safety* there is no documentation to indicate that the car has this unusual functionalityi have discussed the above issues with tesla motors support and they have reviewed the data logs for the vehicle and believe it is functioning normally and that this behavior is as intended by design.my initial vehicle position is in yellow in the image.
While visiting a funeral home in melrose, ma., i needed to back into a spot to be able to take part in the funeral procession.i pulled out of the spot i was in and proceeded to put the car in reverse to back into the same parking spot.at the same moment that i started to back in the car surged and made an arc in the parking lot only stopping after i hit a parked car with such force that it caused two more cars to be hit by the impact.i attempted to stop the car by stepping on the break which had no effect and even though i wasn't hurt, my arms ached for days because i was trying to control the wheel to no avail.i have a clean driving record and was in reverse at the time which meant i had my foot on the gas to back in slowly.i have requested that tesla check the the computer to see if i am of fault or not and they have refused.my tesla is being repaired and i need to know if this was a technical error .i was told it could be the auto pilot system that could be a fault even though i have never used it. Please help because i am concerned that we should be aware of how this happened.

The local tesla service center located at 52 derwood cir, rockville, md 20850 was notified of the incident. The manufacturer was not contacted. The failure mileage was 25,628.the consumer stated the local dealer escalated the complaint to tesla hq and tesla couldn't download the data at the time of the incident.

Sudden unintended acceleration. The car suddenly accelerated instead of stopping when i applied the brakes while pulling into a parking space. I was going approximately 5 mph. Instead of stopping, the car jumped a concrete parking block, went over a curb, climbed an embankment, and struck a tree before it came to a stop. The car suffered damage to the front driver's side wheel well and the undercarriage. The driver was not injured and the airbags did not deploy. Video surveillance captured the incident in its entirety. Photos of the scene and the resulting damage to the car are also attached. This incident occurred during a routine trip that i drive about 4 times each week. Weather, traffic, and road conditions were not a factor in this incident. The car does not contain any aftermarket equipment.
The enhanced autopilot software drives the car over lane lines and inconsistently maintains speed and braking. It is unsafe.
I had pulled into a parking lot, proceeded to pull into a spot adjacent to a cinder block building. I had my foot lightly on the gas pedal, then as i made the turn into the spot, my foot was on the brake - the car lurched forward and sped up and the brakes did not stop it. I went right into the concrete building, head on - air bags deployed. The front end crushed and the 2 airbags on the drivers side deployed and were smoking. I read on line that there have been numerous incidences of this happening with the tesla. Spontaneous acceleration with my foot not on the gas pedal. The car would not stop by the brakes! i could have been seriously injured or hit another persona or vehicle. The car hit the building as well as a natural gas pipe that was running along the buildings side at the level of my front bumper. I filed a police report.

On july 8th 2016, at 9:37 a.m., while slowly pulling into a parking space at creekwood dog park in bradenton florida, my tesla model x suddenly accelerated under its own volition, drove over a parking stop, over a five inch curb, and then hit and knocked over a concrete light pole.all this happened in a distance of less than twenty feet.tesla was notified immediately and the car was taken to dimmitt collision center in clearwater, florida.the service manager at tesla of tampa, told me verbally the log from the edr says the car was traveling at 6 mph, then the accelerator was advanced to over 50% and then to 87%.the car accelerated to 20 mph and abruptly stopped.i denied this scenario and asked for a supervisor.tesla's southeast regional manager met us at the body shop. He handed me a letter that had different edr results-vehicle speed was 7 mph, pedal position went from 3.2% to 15.6% to 100% and car went to 14 mph.the first repair estimate showed actual mileage as 205 and a subsequent repair estimate shows the actual mileage as 1425.these figures are inaccurate since i had looked at the odometer several days before the accident and the mileage was over 1800. I informed tesla that i am positive beyond a shadow of doubt that the car's electronic throttle computer was responsible for the accident which they deny.this appears to be the industry standard since experts will testify that although a car is responsible for unintended acceleration there will be no traceable evidence of that responsibility and therefore the manufacturer has plausible deniability.teslas are undergoing unintended acceleration at a rate more frequent than 1/5,000 vehicles manufactured.this is way more frequent than the industry standard. My vehicle's center display (mcu) is starting to show signs of failure on account of a flawed design that depends on an emmc memory component that is known to fail catastrophically after several thousand write cycles, causing loss of critical adas and rear view camera functions, hvac controls, and other basic vehicle functions. Now, i am facing the prospect of having to pay tesla thousands of dollars to replace the entire mcu in my just-out-of-warranty vehicle. Tesla has tacitly acknowledged the safety implications of their original design by modifying it to use a more robust memory component in 2018, and they should be made to recall pre-2018 models to ensure that nobody else has to endure what i and many hundreds of others have experienced.
Vehicle accelerated suddenly and crashed into a tree in front while parking the car in a parking lot. We reported the problem to tesla but they got back to us denying any car malfunction that caused the acceleration. However we received a recall letter today saying that affected parking brake caliper may cause in rollaway and crash.

The tesla models x comes with a tinted rear windshield. This is very bad a night, limiting the visibility through the read windshield.telsa has clear rear windheils for the models sw and e. The tesla model x only comes with a tinted rear windshield. No option is available for a clear rear win shield. Rear view camera is too dark in low light conditions (particularly night time). Only occurs on cars with updated infotainment systems (referred to as "mcu2"). Image is very hard to make out, exhibiting high levels of black crush, making it very unsafe when parking or driving.
Touch screen stayed off for more than 30 minuets on a 2 hour freeway drive with no control of the vehicles defroster, climate, navigation or audio controls. Touch screen does not always respond and glitches. Rear view camera does not always respond nor works immediately when in reverse and goes all black. Defrost would not work on freeway multiple times due to black non responsive screen. Rear driver seat sequels/squeaks a lot when sat in. Car often takes a minute or longer for the dashboard and touchscreen to turn on. Battery does not hold correlating charge to miles driven. Loud whirling/grinding when accelerating...
I went to start my car and the screen was black. My car hasn't been driven for 5 months since i was traveling. I had cleared all of the memory (navigation, addresses, music stations, etc.), prior to leaving. I was in traffic and the turning signals didn't work. The safety of the car felt compromised since the turning signals,rear backup camera, suspension -basically most of the car controls were in-operatable.

The center display and sometimes even the main display black out during driving. Most times the system requires a reboot which can take a few minutes. During the time where the screen are not visible you have no control over headlights or the ability to defrost the windshield. In addition the camera which is displayed while driving is also unavailable. If the center screen goes black then you can't see how fast your going or if your park, drive or reverse.

Late 2016 tesla mx still suffer from ghosting/secondary images/double images. In my case it is that bad that is hard to tell how many cars are driving on the road at night. The secondary images appear above the car and it's hard to say if there is a car behind the car producing the ghosting. The secondary light is also very intense. I have driven other teslas and the issue is not that bad for all but still bad for a hi end car and compared to 20.000 cars. Tesla fails to recognize the issue and they say their windshield pass their tests. Either they lie or the tests are wrong.in other cars teslas i've tested the ghosting is not that intense but still bad and i noticed 2 secondary images, one under the car and one above the car.
The mcu died, preventing use of the backup camera creating a hazard while backing up with low visibility.
Ever since i bought my 2016 tesla model x i have been having problems with the mcu (main screen) it keeps freezing up, it won't respond to my touch commands, i can not change the suspension settings, driving settings, climate control, disabling pin and everything that is related to the screen which is basically all functions of the vehicle. My tesla is under full factory warranty and i have taken it to service for the same problem along with some other issues 4 or 5 times, every time tesla service center tells me that the screen problem is a known issue and engineers are working on a software fix for it. I have never been able to fully get a use out of all features due to this problem. Tesla has not been able to give me a timeframe on when they are going to send the update to fix the issue with the screen. Meanwhile me and my family are at the risk of not being able to start and drive the vehicle if screen is frozen and we would have to reset the system which usually takes several minutes. Backup camera doesn't show the picture, screen gets distorted. I noticed uneven tire wear which i was told by tesla that it was due to suspension settings and i was driving it in low mode. I can't change the settings due to screen not functioning. I have given tesla several opportunities to fix the issue and they failed. My last service they wanted to charge me around $1700 (50% off) to relapse the screen which should be fully covered under warranty. Problem happens during drive or while stationary.

My 2016 tesla model x's mcu is often freezing, restarting, or going completely black while driving and while parked. When this happens, i'm unable to access any of the features of the car but more importantly, the rearview camera does not work when the mcu is having an issue. With the small rear window, i cannot see any children or people behind my vehicle. Reversing with no rear view camera is dangerous. I'm also unable to turn on the fog lights when needed. I've brought it into tesla twice already and each time, they have said that they cleared the memory and disconnected the battery for a moment and reconnected. This has not fixed the issue. Just today, the mcu screen and my icc screen shut down for a few minutes without any notice. I recorded the incident. I've spoken to other owners and it seems to be a common issue.

On a weekly basis, my center console that has controls for lights, defrosters, a/c will freeze and go blank for an approximate 2 minute reboot. This is while driving the vehicle. The ability to adjust the windshield front and rear, are inaccessible at this time. This will happen in motion whether on a city street or highway.
Double and triple ghosting of lights through the windshield. I will see false images of headlights, streetlights, traffic lights, reflections of the sun on cars, etc. These false images float above their source at varying heights as my car approaches the source. In heavy traffic with a lot of lights, it is impossible accurately to discern reality.

My tesla model x has severe windshield secondary image separation (ghosting).the lights of oncoming cars have triple images that are radiate several meters above the origin of light. This makes driving at night a dangerous and exhausting experience as i try to differentiate between the real lights and the secondary images. Additionally, the tail lights of autos are also doubled making the cars in front of me blurry. Lastly, traffic lights are also displaying secondary image separation making it difficult to accurately read traffic lights. Currently, i am avoiding driving at night to avoid the danger of seeing multiple images. While driving on the second floor of a parking garage at the local mall with the entire family in the model x (2 adults & 3 kids) to see a family movie, the rear driver side wheel "fell-off" (see attached photo's).it appears as though the control arm disengaged from the chassis of the car vis-a-vis a missing bolt or pin.the vehicle is brand-new (i.e., less than 1,000 mi), purchased directly from tesla, is driven by a mother of three, and is accident free.the car has not been subject to any extreme wear and tear or off-road driving conditions.tesla service manager is familiar with vehicle as it has been recently serviced for door/window issues - no frame or wheel issues prior to the incident.the vehicle was in motion, however as mentioned, we were driving in a crowded parking garage during holiday season.tesla road-side service was extremely helpful and arranged to have the car towed to the local service center - the car was obviously not drivable.tow truck driver had mentioned he had never witnessed anything like what he had seen with our vehicle.moreover, he did express to us that we were very lucky that the wheel hadn't disengaged from the vehicle while we were driving at a high speed (e.g., on the highway).the model x has been at the tesla service center for close to 3 weeks and despite our inquiries, we have not been expressly told what caused the incident or whether or not the issue is a more serious/systemic problem with the vehicle or tesla quality control.consequently, my wife is concerned for the well being of the other mothers/families she routinely see's driving the model x in and around our neighborhood.should a wheel disengage from the car at a high speed, the outcome is likely to be much different and much less favorable.
The rear driver side lower control arm snapped off detaching the rear wheel/tire, brake assembly and strut/axel rendering the car immobile.fortunately this did not happen 20 minutes sooner as my gf and i were traveling on the highway at normal highway speeds - this would have caused a life threatening, catastrophic multiyear crash being unable to control the vehicle...we were backing out >1mph of a parking space in a very shallow curve when it snapped - we stopped immediately and i think the electronic braking tried to stop the wheel from rolling jamming it further at angle into the wheelwell.there was nor had there been any unusual impacts on this tire nor the vehicle from potholes/sudden stops or any accidents of any kind.

The car was on autopilot. My model x autopilot ran over a recliner at 60m/hthat was in the middle of the highway. It was around 6 pm, weather was nice and clear.i tried to avoid it by moving to right lane but it hit the driver side of the front of car causing minor damages to car. I tried to reach out to tesla customer service but they told me [4 or 5 times]that someone will contact me and no one ever did. Everytime i talked to tesla [by phone and email] they ask me for more information and then tell me someone will call you back. I wanted to know why didn't the car slow down or even change lanes and how can i avoid this in future specially that i use this car for my 2 kids. I was going up by driveway waiting for my garage door to open. I took my foot off the accelerator and was slowing down without hitting the breaks waiting for the garage door to open. The car took off through the garage door and hit my husbands car sitting in the garage.
