It was on friday morning on january 11th 2020 at 8.30am. I was slowly pulling into a parking space in front of my medical office where i have practiced for 21 years. I was in an outdoor parking deck. I touched the brakes to stop the car and the car suddenly accelerated forwards and hit a concrete wall in front of it. I know that i did not hit the accelerator. I was slammed into my steering wheel. My airbag did not deploy.i have been driving for 30 years and have driven a tesla for 5 years. I know the difference between the accelerator and the brake. Tesla claims that i put my foot on the accelerator and drove into the concrete wall. That is not what happened. I have serious safety concerns about what happened. The car was out of my control.

The vehicle downloaded tesla update on 12/27/20202. The firmware version is 2020.48.26. After the update was installed, the instrument panel above the steering wheel had very small illegible font. For example, the transmission shift indicator letters (p, d, r) were too small to read which is critical. Mph, and percentage of power remaining were also near illegible as well.

I dropped my personal tesla at tesla service to getan mcu/center screen failure repaired. As i left the lot, the screen in the loaner car rebooted itself. Apparently that is the precursor to failure. It feels weird knowing this car is probably about to fail the way mine did.

Touch screen goes disabling all visual communication, including air conditioning, rear camera, gps. Occurs while driving, and while parked (several times on start up). Have notified tesla several months ago,but have been told not to schedule replacement as they are short on parts.

The main console center screen in my would stop responding and will basically freeze up. The indicator clicker sound would stop working. The navigation system would remain stuck. The air conditioner would stop working. All these are happening while the driving. I have to reboot the screen by holding the 2 scroll buttons down and then the screen would go dark for 10 min before it starts working. I have bought this to service few times and they don't do anything saying everything is just fine.

This relates to a tesla warranty adjustment program notice issued november 09, 2020. There is a malfunctioning of a memory chip (embedded multimediacard which causes the screen to freeze or blackout or get fuzzy and does not allow you to make or save changes to the vehicle control touch screen. My car was reset to faculty default on november 23, 2020, without changing the chip, and the next day it failed again. I have scheduled a service visit for december 02 2020 with the expectation that the faulty chip will be replaced. Previously my screen had a yellow border that appeared from nowhere and the tesla service team used some sort of uv light to remedy the situation in june of 2020. At that time they told me if the yellow border reappears then the screen will need to be changed. At the june 2020 service visit they apparently also replaced, at no charge, a computer component. The screen failure i am reporting today began shortly after the june 2020 components were changed, approximately first noticed august 1, 2020. My ongoing concern is that this problem needs to be fully vetted and repaired during the warranty period as these components are quite expensive. Tesla owes its customers a definite fix of the screen and the onboard computer problems, and an assurance that the clock will restart on any repairs made under warranty (what i am proposing is that tesla does a good faith extension of certain of the vehicle's components so that customers are covered for problems not related to normal wear and tear for a reasonable period of time). My september 2018 build tesla model s experiences the same screen failures (both the dash + media control unit) as the recalled versions, but mine falls outside of the recall period. The latest incident occurred while i was driving on a desolate highway traveling at 85 mph. I lose my speedometer as well as features such gps which occurred once in an unfamiliar area.

When i pulled over, the entire car shut down. Both instances required a "hard stop" to re-start the car. The dealer informed me that this can happen when the software has not been recently updated. However, you need wi-fi to update the software and it is not readily available where i reside and park my car. I consider this situation extremely dangerous in that the car can spontaneously shut down while in use. I am taking the car into the dealer today-probably so they can upgrade the software on site. However, this does not address future potential issues with the car. I do not have the ability to routinely take the car into the dealership for upgrades. Further, there is no message from tesla when the upgrades are available. Please help. I really have no desire to die when the car stops working while driving it!

Twice recently while driving on highway at night, the car computer started failing.lights went out, turn signals didn't work, eventually the whole screen and dashboard that let you drive turned blank. I had to find a way to quickly pull to the side of the road and reset the car by pressing and holding the two buttons on steering wheel. This is a very unsafe condition.

I drove to work on may 2, 2019, and was trying to park in my allotted parking stall on the 3rd level of my office parking garage. My parking stall was along the outer edge of the garage and the only barrier to the outside was a set of three (about" diameter) cables strung between outside columns of the garage, besides a thin perforated corrugated metal sheet outside the cables, acting as a rain stop. I was trying to slowly turn right from the parking aisle to park between two pickup trucks on either side of my stall. When i had turned partway into my stall, my tesla suddenly accelerated into my parking stall at high speed, hit the outside cables, rebounded backwards and stopped about 10 feet from the outside cables. The cables saved me from plunging 3 floors down from the garage. There was extensive damage to the hood and front portions of my tesla. There was also some damage to the metal corrugated sheets outside the cables and a slight dent on the corner of the vehicle parked to my right.i have been parking in my office parking stall for about the past six months without any issue like this. I want to know why my car accelerated like that without warning and why the collision warning system was not triggered by the outside corrugated metal sheet. Backup lights are inadequately bright at night, in dark situations allowing no ability to make out ground features when backing up.tesla has updated firmware and replaced camera with no improvement.safety defect of being unable to see behind car when backing up at night not fixed, seems to be a design problem with the car.

The car software has been modified and appears to be compromised. Settings related to safety and autopilot have been unstable. Driver profile does not work properly. Factory reset performed by tesla paramus and by switching email account doesn't not remove the malware. Personal data and malfunctioning of the car continue despite several complaints to tesla.

I had a 2018 tesla s.i was driving north on us 395 in lane 2 of a 4 lane divided highway at 0030 about 20 miles south of lone pine, ca.it was pitch black out.i was the driver of a tesla s in semiautonomous driver-assist mode.the road was going up hill and i clicked my turn signal to pass a semi trailer and my car went into the left lane and passed the truck.after going a significant distance in front of the truck and starting down hill and i clicked my turn signal to return to the right lane.the car went into the right lane and continued onto the right lane but continued abruptly onto the shoulder.it felt like the steering wheel turn to the right and we went off the road into a ditch 15-20 feet below.my hands were on the steering weal, a fact i can prove because i suffered a fracture of the third metacarpal head of my left hand, a fracture an orthopedist called an "airbag fracture".also there was busing of my distal digits of the fingers of my left hand.the car was a total loss.my wife who was sleeping at the time was in the front passenger seat and suffered significant trauma from her seat belt, a fracture of the lateral spine of c-7, bilateral first rib fractures, sternum fracture and a t-12 blowout fracture.i have copies of my x-ray of my hand fracture and a picture of the echymosis of my fingers.i reported the accident to tesla with the date and time of the accident and was told they would get back to me after investigating, but they haven't.

Multiple doorhandles are failing to be able to open the door from the outside when the door is unlocked i have a 2018 model s tesla and two of the doorhandles have failed already although the vehicle is unlocked when i pull on the door handle it's bears to open the door only way to open it is from the inside this could pose a potential safety hazard should i need to open the door from the outside

I was using the autosummon on my tesla.i had used the car to park itself so figured that it would be safe to try.car was close to a wall.car immediately moved forward and hit the wall.there was no time to stop it as it turned immediately.

1. Suspension problem, ongoing concern. 2. Excessive tire wear of unknown cause. 3. Driver screen for managing vehicle controls is faulty and hopefully being fixed via a warranty alert dated november 09, 2020. See attached document. My concern is that tesla is not forthcoming in solving this suspension problem. I would encourage your agency to investigate claims into model s vehicles produced through 2018 as well.

Design flaw on the rear suspension set up causes excessive tire wear on the inside of the tires. Very likely goes without being detected even by the service centers and can cause serious accidents due to tire blow outs specially on freeway drives.

There is loud clanking noise from the front of the car. Tesla service center said they must replace front sway bar and links. That was in december 2020. They still do not have those parts. Driving with damaged sway bars requires being super alert when changing lanes or turning. However, one cannot guarantee it 100% of the time. Tesla says that this is a minor noise issue. I would like them to fix the issue soon (not have to wait 6+ months).

1. Suspension problem, ongoing concern. 2. Excessive tire wear of unknown cause. 3. Driver screen for managing vehicle controls is faulty and hopefully being fixed via a warranty alert dated november 09, 2020. See attached document. My concern is that tesla is not forthcoming in solving this suspension problem. I would encourage your agency to investigate claims into model s vehicles produced through 2018 as well.

I was driving home on the freeway. On my right was an exit ramp and in the left lane there was not another vehicle. Suddenly the warning buzzer went off and the car pulled sharply to the right. I quickly corrected to the right. It scared me. In addition i came home and drove into my garage like i always do. The buzzer went off again telling something was in the way and shifted itself into park. I got out of car and checked to see if anything was in the way. There was not.i called tesla stating these things were safety issues. They wanted me to drive the car in to the service center. I told them my 3 and 6 year old grandkids where staying with me and i did not feel safe doing that. After calling continuously for 2 days, a loaner was brought to my house and my tesla was driven (not towed) to costa mesa service center.

I dropped my personal tesla at tesla service to getan mcu/center screen failure repaired. As i left the lot, the screen in the loaner car rebooted itself. Apparently that is the precursor to failure. It feels weird knowing this car is probably about to fail the way mine did.

The touch screen has become completely unresponsive/went dark. I first discovered the problem while the car was parked in the garage, infrequently used. This has been the case for the last 4 weeks, also rendering the mobile app unresponsive/unable to connect to my car. I am the owner of a 2018 tesla model p100d. On dec 17 at approximately 11:40 am i and a business associate were driving to my home at 804 duke street, alexandria, va.my associate was driving approximately 1-2 miles/hour and starting to park.the parking location isin front of a brick wall.while coming to our final stop the tesla began to accelerate on its own as though the gas was punched (it was note). The car thrust forward and slammed into the wall, destroying a part of the wall and damaging the front of the tesla.the air bags did not engage and we were not injured.the car simply accelerated on its own.i reported the incident to tesla but they have not responded.

It also reminds me of saddam hossein using chemical weapons on kurdish people and on iranian soldiers during iran iraq war. However, saddam hossein did so and killed so many iranian soldiers during the 8 year war between iran and iraq. I saw pictures of so many iranian severely injured and burned soldiers being air lifted to germany from the war zone, because iran didn't have the medical capability to handle this situation. My september 2018 build tesla model s experiences the same screen failures (both the dash + media control unit) as the recalled versions, but mine falls outside of the recall period. The latest incident occurred while i was driving on a desolate highway traveling at 85 mph. I lose my speedometer as well as features such gps which occurred once in an unfamiliar area.

The latest model s software update shrank the size of the font of several critical pieces of information on the digital dashboard. These include the gear selection (park, drive, etc), the remaining battery charge (fuel tank equivalent), exterior temperature and time. I am no longer able to rapidly identify this key information without extra effort or putting on reading glasses. Last week, while at an entrance to the parking garage at work, i advancedbecause i thought my access badge had opened the gate. Unfortunately, it (the gate) came down before i had a chance to enter the garage. I backed up, reapplied my key card, but failed to identify i was still in reverse. When i started to attempt to go forward, i failed to notice due to the small font that i had not successfully returned to forward drive and almost backed up into the car behind me waiting their turn to enter the garage. I also now have difficulty in parking lots confirming whether i am in drive, park or reverse when attempting to maneuver. The tiny font is a major safety concern!! i have checked on the tesla website owner forums and this problem is that widespread for 'middle aged and older' owners. There was even a post by an opthalmologist (md) that this was dangerous and unacceptable.

On april 17, 2020 around noon while driving on the far left lane of interstate 495 at about 55 mph, my right rear window shattered unexpectedly. There were no indication of anything hitting my car as no cars were beside, in front, or behind me at the time of incident. After doing some research on the incident, i learned that other tesla users have had the same incident happen to them involving the same right rear back window.

As i was turning right into a parking space going at a very slow speed, the tesla suddenly accelerated to full speed.i did not press the gas or commit any other user error.i tried to apply the brake.however, because of this sudden unintended acceleration, i crashed into the building in front of me.

I drove to work on may 2, 2019, and was trying to park in my allotted parking stall on the 3rd level of my office parking garage. My parking stall was along the outer edge of the garage and the only barrier to the outside was a set of three (about" diameter) cables strung between outside columns of the garage, besides a thin perforated corrugated metal sheet outside the cables, acting as a rain stop. I was trying to slowly turn right from the parking aisle to park between two pickup trucks on either side of my stall. When i had turned partway into my stall, my tesla suddenly accelerated into my parking stall at high speed, hit the outside cables, rebounded backwards and stopped about 10 feet from the outside cables. I was at a parking lot and i was making my way into a slot when i first had to reverse them when i changed the gear the drive and released the breaks to move forward into the slot i suddenly accelerated and had to press on the breaks harder than ever because i thought at first i was releasing it too fast. I entered the stationary car and turned it on. The windshield and all interior glass immediately fogged making visibility nearly zero. The inside of the windshield became extremely wet. Wiping the moisture only resulted in immediate fogging of the windshield. I set the front and rear defrosters on maximum and only a 8 inch semi circle at the bottom of the windshield in-front of the driver and passenger would defog. I reset the computer and nothing changed. After 20 minutes i called the emergency service number for tesla. They could not help me because i had already done everything. The the tesla parked next to me had a similar experience only resetting the computer corrected the problem. Ipreviously owned the same model tesla and had the same problem several times and the service department located in cincinnati could not find a problem or solution.
