While at a stop light consumer was hit onpassenger's side, butair bags did not deploy. Contacted dealer.
Driving at approximately 55mph when he had to make a sudden stop due to an accident ahead of him, hit vehicle in front of him and the air bags did not deploy.
While driving at approximately 25 mph driver rearend vehicle in front. Upon impact, driver's air bag did not deploy, causing face/neck, and head injuries, even with seatbelt on. Serious damage to front end of vehicle.
Driver air bag did not deploy during an almost centered collision.tt
Clock spring on steering column failed internally, causing steering wheel to lock up. This was during left turn. After rocking steering wheel back and forth, it would turn again with normal input forces, however thehorn, cruise, and airbag are not operational after this incident.
While driving the truck down the highway about 40 mph, the airbag just popped out for no apparent reason, shattering front windshield and causing an injury to the driver's right arm.
Airbags did not deploy during frontal impact.
No deployment of driver's side air bag during a front end collision, resulting in injuries to driver. Vehicle was totaled. The component should be 12111100. Component table does not have it.

Whiledriving at any speed there is some type of noise within the rubber boots. Also,doorlocks/speedometerare not working, and electrical wires shorted out. Dealer can't determinecause of the the problem.

The difference inclimate causedcamshaft in plug to blow ut fromrear driver side cylinder head. This caused all oil from motor to spill onto the exhaust. Consumer's independent mechanic replaced and installed a anti blow out plate to keep the in plug in place from happening again. Plese provide more information.
While drivingvehiclestarted to loose power, shut off and died in the middle of the road. Dealer found the camshaft in the engine to be broken in half. The vehicle is being repaired.

The cylinder head/rocker arm shaft of my engine broke causing almost total loss of engine power.i also had a major fuel leak ( a potential engine fire hazard) and had to have my fuel rail assembly replaced.when the mechanic ordered these parts, there were 160 of the engine heads and even more of the fuel rail assembly on back order. The new engine head came with a special stud in the same area where the old part had been broken.my invoice from the repair shop indicates these repairs in more detail.i also still have the original parts.i contacted chrystler for reimbursement for these repairs and was told that due to the age and milage of my vehicle they were denying my claim. When i asked them about the unusual number of both of these parts being on back order, it didn't seem to be a concern to them nor did the look into the possibility of these items needing to be recalls.

I am sorry that i do not have all of the information necessary.i never knew how to fill out a complaint like this before.a few years ago, my car would not start and when it did it stalled out.it had around 80,000 miles on it at the time.i took it in to be serviced frequently.then it began to smoke when it was driven.finally, i spoke to a dealer concerning these problems and he said that i needed to bring it in for an overhaul. I explained that i was just calling for a price, appt for a tuneup and he said that if i had the plymouth voyager and i was lucky enough to have gotten that far and it was still working then it needed a major overhaul.they all did he said.that's when i started noticing the other minivans on the road.the slightly older ones.all of them have whitish gray smoke coming from the tail pipe.all of them except for the ones that have already had to replace their engines and/or had major overhauls involving the replacement of the valves.now my car is leaking oil and the mechanic won't work on it anymore even a minor tuneup because he says that the engine is going to blow up.i can't afford to replace it.i can't afford to repair it.i've been driving it the way it is because i have no choice.i have made it a point to speak to other van owners that i have seen on the road.they all have the same problem.no matter how well or how badly you maintain the vehicle you have the same result.i recently heard that mitsubishi has been investigated for selling defective automobiles.the plymouth voyager has a mitsubishi engine in it.i believe the dodge caravan has the same engine and the same problem.i don't know how this system works or how long it takes.but i do know that these engines are putting out incredible amounts of exhaust which must violate some sort of regulation. Light from headlights has gradually dimmed over the years although bulbs have been replaced.cause appears to be lack of aging-and/or uv-resistance on the part of the headlamp lense (separate bulb/lense design), which i believe is made of polycarbonate (possibly acrylic or other material), which has substantially yellowed with aging and/or exposure to sunlight.when new, the lenses were completely clear, but are now quite yellowed. This is a serious safety hazard, and i believe the manufacturer should replace these defective safety-related parts.i believe the light output is likely below federal requirements at this time.

1991 voyager with a fuel leak. Consumer states that the leak is at the cross over pipe on the fuel rail. He states that the leak is so bad the vehicle cannot be driven. He was informed that the recall did not apply to this vehicle.

I got a strong odor of gas in the cab, took it to my mechanic who determined the fuel injector seals and rail were leaking.when he ordered the replacement part, we found out that the part is on a nationwide backorder.chrysler tells me that this is not a recall for my vehicle.i have found out that the rail is a recalled part for the '93 - '97 voyagers.my question is should it be a recall, and why is the entire country out of this part?they are telling me they will ship some out on 1/21, but do not have enough to meet all the orders!!!!!what's going on?also, a few months ago, i was going along down the road and the van just "locked up" and stopped. --luckily i was traveling fairly slowl when i ran off the road and there were no injuries.but there were no warning signs, no noises, nothing.the transmission just went out and locked up the vehicle.any ideas what's going on with these two things?i'm very interested in learning why the entire country is waiting on this one part.

I was heading home from work, when i felt my minivan start to lurch and slow down at the same time. I barely made it home. I had it looked at the next day and the repair shop said the clutch plates were frozen together and i was lucky that the transmission didn't explode or freeze-up(causung accidents) like some others that they have seen. I looked into this and there are more than a few horror stories on the internet concerning plymouth automatic transmissions exploding, some with less than 300 miles. This seems like an extremely hazardous condition, especially concerning minivans which are mostly a family vehicle. This first episode cost me $2100 and the second episode the same at 167000 miles.shouldn't this problem, which i understand is inherent to all chrysler/plymouth/dodge automatic tranmissions, be addressed at least from a safety standpoint, if not from a defective standpoint?

The contact owns a 1991 plymouth voyager. The contact stated while driving at unknown speeds, the anti-lock braking system warning light illuminated on the dashboard. The dealer advised the abs system was defective and would need to be replaced, the repair was performed on the vehicle, however, the failure recurred. The manufacturer advised they would contact the dealer but would not provide any assistance. The failure mileage was 79,000 and the current mileage was 80,000.
The problem is that dimeler chrysler cannot preform recall #685 (abs piston pump mtr assy) because a key component of the abs system (front sensor exhauster ring )has fallen off of the axle mounting point, in turn, not allowing the front wheel sensors to sense movement. Hence, the abs system is disabled by the on board computer, and the technician is unable to preform the recall. The repairs fall into the (extented warrenty) section of the letter sent along with recall notice #685. Thank you for your attention in this matter . Happy hollidays!

The problem here, is the re placement of calipers during a rebuild sequence allows for the rebuilder, the right to replace a metal cylinder with a "phenolic" like cylinder, which has failed twice within a 5 month period on both front wheels. While on occasion a caliper or wheel cylinder may go bad-this design is seriously flawed in it's engineering concept. The phenolic pistons do not dissipate heat properly, and because of the design flaw of where the cylinder protracts when it meets the steel backing of the brake pad leaves fully 1/3 of the piston fully exposed to unnecessary hydraulic pressures. It is exactly at this juncture where a serious fracture occurs on the piston, rendering the brake dangerously inoperable. Moreover, the possibility of the ruptured particles can fall in between the rotors and the assembly, which would cause a vehicle to pitch and overturn due to the principles of physics. Chrysler has many of these deployed on the road in various models of autos an vans. This is far too dangerous an aftermarket product to allow the producers to continue flooding these markets with flawed design to begin with, and a seriously flawed replacement cylinder. As a pilot and aircraft owner i would tell you that the faa, would never permit such a desing on an aircraft. Having had many extensive years of expertise on both aircraft and automotive rebuilding-these brake units should be taken off the market and a suitable replacement should be mandated by the dot. The changes necessary to accomplish this are quite simple and would be effective. I would like to see the engineering documents showing the required tests performed by these aftermarket brake assembly producer. My suspicion is that there is little or no information other than a "white sheet" introduced in this matter. I can assure you that considering the sheer volume of these innefficient and flawed systems that the possibility that many may have.

The contact owns a 1991 plymouth voyager.the contact stated the bolt that held the strut on the passenger side of the liftgate fractured as it was closing. The contact stated that the liftgate was no longer operable. The contact received notification of nhtsa campaign id number: 99v213000 (structure:body:hatchback/liftgate:hinge and attachments) in 1999 but had never received the recall repair. The contact took the vehicle to the dealer and the repair was refused.the manufacturer advised that due to the age of the recall, the contact would be liable for one half of the repair costs. The contact stated that the manufacturer should stand behind the recall and repair the liftgate.the vehicle was not repaired. The failure mileage was 185,102.

Pe-92053; the vehicle was parked for about 6 1/2 hours,windows were cracked, upon checking the vehicle the next morning, found that the rear window had shattered for the second time in less that six months. Dealer was contacted and stated that it was a common occurrence because the weather was cold, and the vehicle was parked in a sunny area. Healso stated that the sun heated the glass, causing an inflow of the glass, no injuries in either case of the glass shattering.
