On april 11, 2018, while operating at a two alarm fire at a three story residential building, a 95 foot mid mount aerial platform failed without warning. The bucket truck was stationary and ladder was extended to roof. The bucket was unmanned and no one was at the turntable. During operations, the ladder dropped onto the roof, and then buckled.
