Was involved in a head on collision and airbag did not deploy.approx. $14,000 in frontal damage to my car and approx. $4,000 frontal damage to other vehicle.

The contact owns a 1993 mazda rx-7. The contact heard a loud noise coming from the vehicle while it was parked. The contact discovered that the air bags deployed independently. The vehicle was taken to an independent mechanic; however, the mechanic refused to diagnosis it and advised the contact to go to a dealer. The manufacturer was notified of the failure. The approximate failure mileage was 51,976.

The contact owns a 1993mazda rx7.the contact stated that the vehicle was parked when it caught on fire.the local fire department was notified.the contactwas unsure as to what caused the fire, but stated that he smelled gas fumes before the fire occurred.the current and failure mileages were 50,000.

Battery replaced due to battery acid leaking onto engine. *sd

Battery exploded, causing electrical system failure, including power door locks which caused driver to belocked inside of vehicle.*skd

Leaking battery acid causes a short in a cooling fan motor located directly below the battery causes fires in 1993 and older model mazda rx-7s.this will not show in a fire investigation, as the insulation will be burned away.this can explain the multiple fires in parked, cool vehicles.

If not please forgive me.i have the occasion to drive many different types of vehicles made by many manufacturers, and feel i have found a major safety shortcoming. On more than one occasion, i have had the need to sound the horn.the problem is that the horn button is in a different location on each car.it is often hard to find as well which can be a problem when you have to react swiftly.i believe the ntsa should standardize the horn location and design, and require manufacturer's of automobiles to comply.the horn button should be large ( like a ford taurus not like a jeep grand cherokee), easy to find and in the same location on all vehicles.i believe this safety feature would reduce accidents and possibly save lives.thank you for your consideration........brian slocum, rt2 box163, millbrook, ny 12545.

Intermittent stalling.

The vehicle starts bucking whendriving at any speeed/any rpm and in any gear,most noticeableat 35-40 mph, about 2500 rpms.in the shop (9) times for this condition in the past (3) months.

Failed electronic control unit caused sudden acceleration/accident. *dh

When air conditoning is used vehicle stalls.also there is a loss steering power. Computer module has been replaced, but problem remains.

At stop light noticed brownish/gray smoke coming through vents & from under the hood. Pulled to side & lifted hood. Noticed flames in area of the intake manifold (engine). Fire department called & put flames out. Vehicle was totaled. Reported to mechanic at the dealership that there was a strong odor of gas since jan. 19, 1995, however they were unable to indicate source or find any problem prior to fire. *mjs

Drove vehicle about 3 miles, engine began running rough. Went back home and cut vehicle off, saw smoke and raised the hood and fire was coming from underneath the hood of vehicle.

Drove on highway and vehicle started to smoke.pulled off an exit, opened hood and flames were coming out from engine compartment. Car is sitting at a garage waiting to be repaired.this is the the 3rd engine in the car, all resulting from overheating or fire.6/2001- antifreeze fire- issued a used engine5/2005- antifreeze fire- bought a used engine3/2006-antifreeze and oil leak- replace all associated hoses/o-rings2/2012- engine fire- waiting for repairs.

The rear oil cooling jet on the engine eccentric shaft came loose and fell into the oil pan.this caused oil pressure to drop.

The engine had and overheating problem which lead to excessive heat deteriorating the engine seals, a new engine was installed, the new engine developed an oil leak, the consumer requests reimbursement.nlm

Engine malfunction.

Failure of cooling system to draw back coolant from the overflow bottle when the engine cools causing low coolant level in engine upon subsequent startup. Large puddle of coolant forms under vehicle.

Engine cooling system overheated prior to recall, repairs refusedbecause defective part does not meet the specific parts enumerated in recall (94v-094).

Recall site shows 3 recalls for 93 mazda rx7.engine cooling system recall says "manufactured from dec 1991 to jan 1994".my rx7 was manu feb 1992 and i feel should be included in this recall.mazda however states that there is only 2 recalls on my car.this is my 2nd rx7.the first one i had was manufactured march 1992, one month after this current one.it was exact same car.it was recalled for all 3 showing in the recall database.so what do i do?mazda says the cooling system is not a recall item for my car.i disagree.please let me know if i'm writing to wrong place for what i need.thanks.

Engine coolant tank failed, causing fluid leak.

Vehicle caught fire due to leaking engine coolant and gas line.

The consumer received a recall nhtsa#94-v-094-000on may 1997on the radiator cap pressure relief valve.the release pressure was set too high which could cause the coolant to reach temperatures that exceed thermal design limits of some components of the coolant system.should this occur the engine coolant could leak directly on top of the engine.the consumer stated this recall failed.*nlmthe recall was partially completed.both required replacement of the cooling system including the water pump which was not done because the vehicle lost the water pump on the road. *scc

Failure of internal water seal in engine, resulting in loss of coolant with subsequent overheating of engine.

Engine coolant leaked on top of engine, could have resulted in a fireprior to recall, recall repairs refused by dealer (94v-094).

On 11/16/99 i was driving about 40 mph and my engine cut out.i drove the car to classic motor cars.they contacted me to let me know that my car had lost compression and that the engine would have to be replaced.i contacted mazda customer relations to determine what recalls had been applied to my car.they confirmed that mazda recall #60504 and #65609 had been applied.i asked about mazda recall #54407 and [xxx]stated that i was not notified of that recall because according to the computer, my car did not need the recall.upon further investigation he identified the vin #jm1fd33**p0200001 through jm1fd33**r0302076 should have been notified of that recall.since my vin #[xxx] falls within that range, i asked why my car shouldn't be considered to have the recall.he said he didn't know.it seems that there is a computer problem.i should have been notified and had the recall work done.in specific, i understand that this recall work prevents the engine compartment from catching fire.since my vehicle was not included in this recall, in error, i would suspect there are many others that should have been recalled, that were not.this negligence is a potential threat to me, my family and my property.also, i believe that since my car wasn't identified to have the recall work done, that this contributed to the deterioration of the apex seals in my engine.any engine should last longer than 49,800 miles.upon contacting mazda, the dm has agreed to offer $1,000 towards the replacement of the engine, but they do not feel that this is a defect.i feel that since we are not identified in their computer as recall candidates, and since we were not notified to have the recall work done, and since we did not have the recall work done, the apex seals have deteriorated as a result of the heat generated inside the engine that causes fires in the engine compartment without warning.i would like for your agency to assist me by confirming this.information redacted pursuant to the freedom of information act

Oil line to turbo's collapsed causing the inner coolers to be flooded with oil. This was caused by the apex seal coming off in the rotary tube which caused the compression levels to drop. This damage costs me in excess $10000 and happened at 30000 miles. An investigation is currently under way.

The mechanic told me that these fires happen quite frequently in 1993-1996 rx-7's as well as in the prior body style.i could have been seriously injured from this incident, and my car now needs over $3,000 worth of repairs, all due to a known problem that has never been recalled. Vehicle caught fire due to leaking engine coolant and gas line.

While fuel system is pressurized fuel leaked out of the top of pulsation damper prior to/after recall repairs, could lead to a fire (95v-069).

Fuel line , non-metallic failed prior to/after recall repairs (95v-069).

Fuel lines/hoses; non-metallic leaked fuel on hot engine, resulting in a fire prior to recall (95v-069).

Recall related, 95v069, fuel line fittings in for repair four times, the problem has not been resolved.

Recall related in for repair, did not preform the corrected action properly, vehicle has be in dealership for a period of 2 months since may 16 (95v-069).

The lower intake manifolds supplied oem on these cars consistently fail.the material used by the manufacture has since changed to compensate for their mistake however, all of the owners of these cars are forced to pay the cost of repair for their poor design.at least 90% of these cars have had to have this part replaced and i am positive that that number will increase to 100% of the vehicles on the road.some compensation should be made.

Pulsation dampers fail on almost all fi mazda rx-7s.

There was a fuel leak that led to a fire, mechanic saida screw came loose,allowing gas to leak ontomanifold,which a caused fire.
