I had a single car accidentin my 2004 mazda mpv van with front end sustaining damage with no deployment of front or side airbags.i sustained significant head and facial trauma.

Why does recall only cover the passengers side air bag inflator? what company manufactured the drivers side air bag inflator?i would like the drivers side replaced also. I was told by service department that mazda will not cover replacement of drivers side air bag inflator. I asked parts department if i could purchase drivers side inflator. They said that the part was discontinued. I don't feel safe driving my vehicle. What is the solution to this problem?

Takata recall: i received mazda offer to replace defective part and accommodate for transportation while the car is being repair. The only requirement to have this accommodations is to make an appointment for them to deliver such offer to fix the inconveniences that this brings. I called to make the arrangements but now mazda refuses to give another transportation while fixing the car, even though this is included in their written offer.

I went into the dealership to get my safety recalls done at the mazda dealership on torrance but they said i needed to fill out a form and to wait for them to receive the auto parts. It has been more than 6 months since i went, i have not heard from the at all.

Vehicle was lurching, no warning light was on. It took a while to figure out the rear coil was bad. Wespent $1200 getting a tune up and that didn't fix the problem took a while to figure it out. Also, the transmission failed on this vehicle completely at 55 thousand miles. We had taken it in 3 times before it failed and they said nothing was wrong with it.

Like many other mazda mpv owners, i've experienced problems with the vehicle's ignition coils.in august 2008, the vehicle was riding very rough and hesitating on the highway.the diagnostics for the check engine light showed that one of the ignition coils had misfired.i had the coil replaced and, at the recommendation of the dealership, replaced the spark plugs.at that time, my vehicle with approximately 35,000 miles on it, was out of warranty.i paid $476 for the repair but was ultimately reimbursed by mazda on a customer goodwill basis.one year later, in august 2009, the vehicle again began riding rough and hesitating.when the check engine light came on, i brought the vehicle to the dealership.based on the earlier experience, i suspected an ignition coil as the culprit.my suspicions were realized.the dealership found that cylinder #5 had misfired and that several other coils were weak.the dealer indicated that the other coils were likely to go and recommended that all of them be replaced.i had the work done at a cost of $960.the vehicle at this point had only 46,000 miles on it.both repairing dealerships mentioned that this is a common problem with mpvs.from my research, i can see that is a significant problem for 2002-2006 mpv's.entire message boards are dedicated to this mazda mpv ignition coil defect.most believe that there's a design flaw that prevents the coils from dispersing moisture properly, which over time leads the cylinders to misfire.it seems to me that mazda should have voluntarily recalled the vehicle due to this issue.having previously made a customer accommodation on the earlier repair, mazda has refused to reimburse me for the costs of this latest repair.it's unfortunate that mazda has chosen to deal with the problem on a customer-by-customer basis, rather than having voluntarily recalled the vehicle in order to address this very common problem.

2004 mazda mpv:i have had to replace all6 ignition coils in my van -the van would shake and seem to be stalling and the engine light would come on.this started in july 2007 and continued so far through jan 2010 (when 4 coils and all my spark plugs needed to be replaced at once !).i have also had the car in during oct 2008 because the fan would continue running for several minutes after the van was shut off. My dealer told me this is a "ford" issue. When will mazda be required to pay for all these expensive repairs??

Mazda 2004 mpv with just under 68,000 miles has 3 fuel injectors(cel) replaced by local mechanic- 2 weeks later jerky misfires with cel required #5 coil replaced by dealership, less than a month later #4 coil replaced by local mechanic in town we were vacationing in-no cel, nowsame jerky misfires starting again a month after last replacement. Recent forum entries seem to indicate a growing problem of this type for 2004 mpv. Except for towing charges, expensive repairs, and inconvenience- no injuries.

I purchased a 2004 mpv arch 28, 2005.to date, i have had this van in for warranty serviceto replace ignition coils three times:10/31/06 '' 31,770mi- #4 coil replaced. Transmission range sensor replaced also 12/05/06 '' 33,229mi - #4 coil replaced again 1/08/07 '' 34,708mi - #6 coil replaced. Today as i was driving, the same symptoms '' steering wheel vibration, bucking, lurching '' occurred as before when the othercoils failed.the cel came on, i checked for the code and i discovered that the #5 coil has failed.now this will be the fourth service for the same problem.there has been no effort on the part of country mazda to find the source of the coil failure.replacing the parts obviously is not solving the problem.

We have 2004 mazda mpv which we bought new.while it was still under warranty, we had to have coils replaced.we thought it was odd that we would be having to replace coils on such a new vehicle.after investigation, we found that this is a common occurrence with the year mpv we have.apparently there is a design flaw which allows moisture to get to the coils, causing them to short out.we mentioned this to fitzgerald mazda in annapolis, md where we were having it serviced.they admitted this was a problem but basically thought we should be happy that it was still under warranty..even though it was probably going to be an ongoing problem long after the warranty ran out.since the warranty ran out, we have had to have coils replaced 2 more times at our own expense and are waiting to have yet another coil replaced as i am writing this complaint.each time we have to have a coiled replaced, it is very costly due to the fact that the design of the vehicle makes them very difficult to get to.once we had to have it done while we were traveling on vacation in florida..this was done at the mazda dealership in gainesville, florida.then another time we had a coil replaced by a mechanic in owings, maryland.that mechanic did extensive research for us and came to the same conclusion..this is a problem with moisture getting to the coils and will happen again and again throughout the life of the vehicle.we are now delaying another vacation..paying out of pocket to have yet another coil replaced.we want mazda to stand my their product.there should be a recall and they should be required to correct the problem at their expense.we have spent close to $2,000.00 now. The worst part is, it is only a matter of time beforeanother coil goes out.please help!

Ignition coils retain moisture and cause cylinders to misfire.this causes vehicles to stall or lose power -- usually when raining.in our case, this has happened repeatedly on the interstate during rain.we early caused an accidents by our inability to get out of traffic with our suddenly-slowed vehicle.replacement of a single coils cost us over $200.there are six ignition coils (over $1000).many internet forums indicate that this problem is systemic in this make/model.we understand that the manufacturer redesigned the coil for subsequent models.they obviously recognized the flaw.

My 2004 mazda mpv suffered the same problem as many other consumers.on two different occasions i had to replace all of the coils and plugs on my vehicle.the first instance was just after the warranty expired and involved one of the rear coils but the said i should replace all three rear coils.the second incident occurred two months later to the front coil and i replaced all of those - total cost $900.i complained to mazda but they wouldn't do anything for me.this should be recalled and owners compensated.

2004 mazda mpv. Consumer states that while driving speeds of 70 mph the vehicle just lost all acceleration dropping to below 5 mph. She also states that the vehicle just shut off.the mechanics were unable to find a problem.

The contact owns a 2004 mazda mpv. While driving at speeds of 30 mph, the vehicle began hesitating as if it were attempting to stall. The vehicle was taken to the dealer where they replaced the electrical coil to the cylinder. The repairs were performed under factory warranty. The failure recurred within a year and the vehicle was taken to an independent mechanic. The mechanic placed insulating materials around the coil to prevent moisture from entering the coil. The mechanic advised when moisture would enter the electrical coils; it would cause a shortage contributing to the failure. In addition, when the engine was shut off, the cooling fan would continue operating. The failure persisted. There were no recalls for the failure. The failure mileage was 23,000 and the current mileage was 100,000.

2004 mazda mini van the ignition coils are continually having to be replaced-i am on the 4th time having one of them replaced over the pass 4 years.

Bought the 2004 mazda mpv es in 2005.currently it has about 64,000 km (40k miles).problem:recently 4 ignition coils malfunctioned ..had 3 replaced right after warranty had expired.apparently, this is not an uncommon problem for this 2004 model.watch out!

The mini van caught on fire.

The contact owns a 2004 mazda mpv. The contact stated while driving various speeds the vehicle would intermittently stall without warning. Immediately after the failure occurred all of the warning indicator lights illuminated on the contacts instrument panel. Both the manufacturer and dealer was not made aware of the failure. The vehicle was not repaired. The failure mileage was 100,000. The vin was not available. Rk

Persistent ticking sound from engine, which has become louder with passing of time.tsb re ticking sound has been performed by showcase mazda, and on follow-up visit they advised situation is normal, the ticking sound notwithstanding.a respected independent auto repair service, combs automotive, advised the engine absolutely should not be making the ticking sound with only 30k miles on it.please note that below i show incident date as this month, but it has become more pronounce over past few weeks.

The contact owns a 2004 mazda mpv.while driving approximately 35 mph, the vehicle independently decelerated. The contact applied pressure to the accelerator pedal but the vehicle would not respond before stalling.the emergency flashers were engaged but to no avail. After ten minutes, the contact was able to restart the vehicle. The vehicle was neither diagnosed for the failure nor repaired.the failure mileage was 42,000 and the current mileage was 42,400.

With our mazda 2004 mpv, we have had four separate incidents in 2 1/4 years from mar 2008 through june 2010 where the check engine light has come on to indicate an engine problem.in each incident, the vehicle runs very rough and very slow.each incident resulted in us taking the vehicle to the mazda dealership where some or all of the ignition coils were replaced.for some of theincidents, the vehicle was out of warranty but in each incident all or some of the costs were covered by mazda.in 2 of the incidents, we were out of town and it was dark.in another incident, we were leaving town, and the other, we were in town.now, we will not take the vehicle out of town for fear of completely breaking down at night or in unsafe conditions.on the internet, i find very many similar incidents for the mazda mpv.

My radiator fan constantly runs intermittently but with increasing lengths of time, regardless of the weather (hot, cold.)the engine does not overheat.but the lights dim at night when the fan kicks in and creates a loud humming sound that is becoming more irritating.i read on the web that many earlier models of the mazda had a recall for the fan control module up to 2003.my vehicle is not in the recall, but i noticed a lot of people on the web have models after the recall period and we all have the same problem as the recalled modules.(sp-60 i think is the recall notice.)i'm not sure i am contacting the right office, but how do you get this on the recall program or determine that the faulty modules aren't creating the same problem for models made after the recalled versions?can you help?

Prolong off and onticking sound in the engine , finally thought it was a belt took it to a shop to have belts changed or checked , mechanic said it is not your belts it is your engine, so we had to have it towed at our expense to the dealer we bought it from to have engine checked or replaced , and now mazda won't warranty it !!! the mechanic from where we bought it from ,got kind of nasty and told us to get our dam car out of hisf-ing shop , we were upsetbecause mazda won't replace engine , now i am stuck with a 15,000 dollar car no engine !!!!just turned 51,000, and i believe that is not right either , i left the lights on by accident one day for 30 minutes maxand my battery died , when we bought new battery i think mileage jumped 10,000 , miles because a month before we tried to trade carfor a bigger vehicle( i have 6 people in family- growing out of this one)in and it only had 39,000 miles on it another dealer can verify itbecause we going to use the mazda for a trade in, after the battery went dead , all the sudden i had 51,000 miles .... Go figure that one outin 30 days ....

The engine coils have failed 4 times since we have owned the vehicle.last night while traveling on the interstate in the rain, key words, in the rain it started again.i am sure at least one if not two coils went this time.just had a coil and all the boots that are suppose to protect the coils and keep the moisture out replaced in february, 2012.it always happens when it rains.mazda is well aware of the situation and has done nothing to fix this issue except to stop using this system and start making their cars differently.but what about the people who own the older models?this is a dangerous situation as i was on the highway, at night, and it was pouring down rain and the van starts slowing down and sputtering along, with traffic traveling behind me.this should be a recall issue as it is a known problem and it is because of a manufacturing defect not anything that owners can avoid.

2004 mazda mpv. Engine went out without warning while cruising at 65 mph on the highway. Check engine light on. The car had only 3000 miles on it at that time. Got towed by mazda roadside assistance to the closest dealer, who had no time to work on it for more than a week (!). Therefore the car was towed at mazda's expense to the dealership i had bought it from. There they did not find anything wrong with it and therefore concluded that the ecu must be faulty. A new ecu was installed and the problem has so far not recurred. The car now has about 14000 miles on it.

2004 mazda mpv engine completely and suddenly dies when driving - usually when pressing the accelerator pedal as the engine is decelerating. Drove car to shoulder, placed in neutral to restart.appears to be the idle control valve (madza forum sites report numerous similar events).

The contact owns a 2004 mazda mpv.whenever the air conditioner is activated, the fan that blows the air is very loud and continues to run 3-5 minutes after the vehicle is turned off.the vehicle was taken to the dealer and they stated that the fan module needed to be replaced.the contact did not notice any differences prior to the failure.the current and failure mileages were 79,600.

The contact owns a 2004 mazda mpv.every time it rains, the engine misses and the check engine light illuminates.the contact took the vehicle to the mechanic and was informed that moisture enters the engine and fouls the cylinder when it rains.the failure causes the vehicle to run roughly and travel only at low speeds.the vehicle has not been taken to the mazda dealer, but the manufacturer stated that there was no recall.the current and failure mileages were 80,000.

2004 mazda mpv with 63k miles. Has broken down 5 times with same problem affecting one of two different coils. Engine light comes on. Van hesitates/shakes. No loss of life, but plenty of lost wages etcandmazda rep not cooperative. $400 and average3 days in shop each time to repair.mechanic guessed problem before i even told him. States that they see this all the time with the mpv's.

The contact owns a 2004 mazda mpv.while driving 65 mph on the interstate the vehicle automatically shut off on four occasions.the contact restarted the vehicle after each failure and brought the vehicle to two different mazda dealers.neither dealer could diagnose the cause of the failure.the contact stated that the failurewas random and occurredevery 1,000 miles.the current mileage is 60,700 and failure mileage was 57,500.

2004 mazda mpvengine failure due to engine coils, one went out in may driving in the rain on the highway and another went out two months later also in the rain on the highway.dealer said it is common.well then there needs to be a recall.the vehicle only has about 50k miles on it.

Engine coil issue.rough idle, check engine light.

2004 mazda mpv. Consumer states that while driving speeds of 70 mph the vehicle just lost all acceleration dropping to below 5 mph. She also states that the vehicle just shut off.the mechanics were unable to find a problem.

2004 mazda mpv purchased on feb. 28, 2004. On mar. 27, 2004, engine failed without warning lights. Fortunately, car was idle when engine failed. Only had 1,100 miles on vehicle. Manufacturer has not been cooperative in diagnosing the problem. Initial assessment by the dealer tech at star mazda in glendale, ca was that connecting rod failed and engine seized. Dealer stated that mazda's own tech people would have to take a look at the engine to diagnose problem. It has already been a week. Requested vehicle to be towed to 1000 oaks mazda, where i purchased the vehicle. Mazda still not given approval for that towing. Still waiting

At approx. 50,000 miles my 2004 mazda mpv's 'check engine light' went on.when i brought the car in for service i was told an ignition coil had to be replaced.at approx. 60,000 miles, the same thing happened ... The car's check engine light ' came on and, again, an ignition coil had to be replaced.i have since learned from web postings that this ignition coil problem may be a fairly common repair on 2004 mazda mini-vans and i'm wondering if this is a design defect that should be addressed through a recall and repair by mazda.were i on a highway when the light came on ... The car lost acceleration capacity and became very difficult to drive ... I could have been in a serious accident.i am now concerned that this ignition coil problem may continue to re-occur in the future.

2004 mazda mpv - transmission issues and engine coil failures.transmission has shift shock, where the vehicle slams into the next gear at 20 mph.the shock sends the vehicle dangerously forward.mazda had two service bulletins for this defect which included replacing sensors and switches.mazda also supplied a diagram to instruct drivers to "wiggle" the gear selector between gears.none of this solved the problems.engine coils were replaced three times at 24,000 miles, 32,000 miles, and 35,000 miles. This caused the vehicle to stop running correctly while driving.

Mazda mpv 2004vehicle's engine hesitated when driving in rain at any speed.this began happening before mileage and date listed below. This is a safety hazard when particularly when it occurred while driving on the washington metro beltway.an assistant service manager at, rosenthal mazda arlington, 750 n. Glebe road, arlington, va. ( 703/527-3000) advised that this problem was known to mazda for the past five years and required air induction service every 12,000-15,000 miles.we never were warned of this problem or the need for the specific preventable maintenance by either the company or rosenthal mazda.the repair cost $2000 and could have been avoided if mazda had advised owners of the need for preventable maintenance (air intake manifold).i repeat that this is a safety issue, causing the driver to be distracted and the car to accelerate poorly, particularly whenthe engine is hesitating while driving at high speeds in congested traffic.please contact mazda and investigate and require them to send warnings and reimburse those who suffered monetary damage as a result of their lack of warning.

The engine light turned on three times since i bought this car. Also when i searched internet i found a lot of users reported similar issue.

2004 mazda mpv with only 400 miles on it is starting to develop a sulfur smell coming from tail pipe area. This has been a issue in the past with the mpv. Is there a fix. I have not contacted dealer as of yet.

I own a 2004 mazd mpv van. When the transmission lever is moved to d for drive, thethe air conditionin (a/c ) on/off swicth is hidden behind the lever no matter how what the driver seat settings may be.i have contacted mazda, but they have failed to provide any remedy for thisproblem.i strongly believe that this is a driving hazard as i have had a few close calls trying to turn the a/c on or off while driving.

Car is running fine, and then engine stops. After setting for a minute or so, engine will start again and run fine for days ...then the above recurs. Replaced idle control valve...no more trouble.

Dt*:the contact stated while depressing the accelerator pedal, the vehicle would not accelerate.the vehicle was taken to the dealership, where it was determined the bladder separated in the fuel tank and was getting into the fuel pump.the gas tank needed to be replaced.

Accelerator (gas pedal) stock open when pedal pressed for acceleration onto interstate highway. Vehicle accelerated to very high speed. Pressing brake slowed vehicle to 55 mph. Shifted to neutral, heard a low pop and accelerator mechanism disengaged.

I was on i-75 when i accelerated hard while trying to merge into a gap in heavy, merging traffic.when i took my foot off the accelerator my van continued to accelerate as the engine continued to rev at a high rpm.i stabbed at the accelerator with my foot to see if i could unstick it, but it only caused the engine to rev even higher - to full-throttle.i jabbed at it multiple times to no avail.i tried to lift the gas pedal with my foot by jamming it underneath the pedal and pulling it up, but it floated freely and continued to fall back to its full-throttle position. Although i was confident that the floor mat was not the cause of the problem i pulled it away from the pedals and, as expected, there was no effect. I was able to keep control of my van by applying extremely heavy pressure to the brake while shifting between neutral and drive.during that time i was still jabbing and lifting the accelerator with my right foot trying to 'unstick' the gas pedal.i was entering and crossing and overpass that was undergoing construction.concrete barriers lined both sides of the highway and bridge where the emergency lanes would have otherwise been located.i had to maintain some speed, for fear of being rear-ended, while searching for the first opportunity to pull over. My vehicle was towed to the nearest mazda dealership where the mechanic was able to quickly ascertain that the screwing mechanism from an unsecured air hose clamp had snagged the throttle arm - the clamp attaches the hose to the engine.as i pushed harder on the gas pedal it further exacerbated the problem by locking the arm in a more wide-open throttle position.the clamp had been left unsecured from previous work done at a mazda dealership. According to the mechanic, this could have also occurred if the clamp had been secured, but not positioned out of the way of the arm.

Rear liftgate door gas strut mounting stud snapped off, there is a recall in canada for this issue for model years 2000-2004. Total transmission failure.

2004 mazda mpv - north park mazda is san antonio has been unable to fix a noise coming from the front of the vehicle.i took the van in 5 times in july.they were unable to duplicate the noise on the first few attempts.i finally had to take the service manager for a ride in it.they thought they had fixed it the last 2 times i took it in but apparently did not actually locate the problem.i

While driving consumer applied the brakes and pedal went to the floor. A loud knocking noise was heard from the front. Consumer was able to slow down, and pulled over.consumerhad vehicle towed to the dealer, and mechanic determined that transmission blewout due to a worn shaft solenoid.

On my mazda mpv in temperatures below 40 degrees the transmission slips when shifting into 3rd gear.this causes loss of power and a whiplash action when the transmission re-engages. In high traffic situations this can be very dangerous.

2004 mazda mpv with transmission problems. Consumer states that the vehicle has a transmission shift shock problem and the warranty is getting ready to expire. When accelerating vehicle and passenger get jolted forward.the consumer stated the last time the problem happened, the transmission light illuminated.

Water gets in the door panel every time it rains or we wash the car. Three different mazda dealers tried to fix the problem five times, unsuccessfully. I am afraid to drive my children in the car. The door is electric and it could cause a serious problem. Also mold could develop in there. I have contacted the mazda corporation. They inspected the car. The problem has been identified by mazda. They promised to resolve the problem, but are not doing anything about it. They ignore my phone calls and letters. What should i do? thanks.

Over the past 3 months, passenger-side power sliding door on a 2004 mpv, intermittently would:1) not close. Door will attempt to close, but bounces open at the end.door power has to be turned off at the drivers control, so that the doorcan be manually closed.2) door would not open. Several occasion, the door would appear to be stuckand cannot be open with any of the controls and not manually either. Door buttonwill "beep" but door would not open. Door usually "recovers" after driving the vehicle for a short trip.3) door will make motor whinning sound...like it's trying to open while vehicle is in motion, and once it did open on its own while vehicle was traveling at low speed.reported to mazda of roswell dealer, they could not reproduce problem and cannot offer much help.

Passenger sliding van door;when it rains, allows water to enter track space of vehicle; over time may cause rust;already took mpv in once, mazda did not fix correctly, made the leak worse and now the window has problems rolling up and down; taking vehicle in again to mazda for a 2nd time???

There is no way to adjustment the alignment of the rear wheels on this year mazda mpv, thus the tires ware out early because they are not in alignment. Tire failure occurred while driving on highwaymazda dealer says no adjustment can be done as mazda did not provide a way to correct this alignment problem so early tire failure can result. Mazda dealer work sheet for alignment states that the rear wheels are out of factory specification and if not corrected can lead to excessive tire wear, handling and safety issues. Mazda by there own admission did not provided any way to correct this safety issue. I'm driving a vehicle with tire safety issues due to lack ofability to align the rear wheels to mazdas own specifications.

Suspension subframe rusted and cracked.

Front cross member corrosion; frame damaged; car dangerously unstable. Wheel alignment cost me $72.64 and frame repair cost $693.36. The vehicle in the shop for 4 days. Mental anguish. Mazda dealer declined repairs. Mazda customer service declined to pay.now they are admitting that problem on mazda 6 vehicles. It's an old problem probably related to metallurgy or improper paint protection.

Lately while driving my car will suddenly jump forward.it seems to happen more so when going from 2nd to 3rd gear.also, the engine is becoming more hot, even after only running for a few minutes and a strange smell is emitting from the engine.i have taken it to the dealership here and they have said nothing about the situations.they claim, i am pushing too hard on the gas, which i don't do.they also say to make sure i have enough coolant in the engine which i always do.i am afraid the engine will catch fire or something with the way it is running.unfortunately, it is my main car to get around with for the children.the car is only 3 years old.i am tired of taking it to the dealership and not getting an answer.

Front windshield wiper arms broke off during use on highway and heavy rains.upon later inspection, the cast metal part of both arms were cracked near the hinge point. The drivers side arm broke off and was caught in the passenger side wiper blade for a few seconds causing it to smack against the windshield and cracking it.mazda refused to offer any assistance in determining if the parts were defective or in obtaining replacement parts. Replacements were purchased from the original dealer.
