The contact owns a 2015 mazda 6. The contact stated that the air bag warning indicator flashed on and off and the front passenger air bag off indicator remained illuminated. The vehicle was not diagnosed or repaired. The manufacturer was contacted and advised the contact to take the vehicle to the dealer for diagnostic testing. The dealer was not contacted. The failure mileage was 60,000.

When involved in a collision in an intersection, the airbag deployed incorrectly. It popped out of the steering wheel but failed to inflate. Not only did this not protect me from whiplash but it also burned a hole in my dress and caused 1st and 2nd degree burns on my upper inner thighs due to the malfunction. The airbag however was fully inflated 48 hours later when sitting in a towing yard.

Takata recall

Waited months for dealer to repair recalled air bag inflator. Have not been contacted. Car may have been driven in high humid areas as it was originally a rental.

Vehicle has had 3 windshield replacements in just three years of use and ownership.the windshield design seems to be extremely fragile and induces safety related sensor function anomalies. My grand touring model is also equipped with a bose speaker system which causes popping from a faulty speaker.this seems to be a common problem with these vehicles.

This car is not safe and mazda has known for years. The door sash molding has caused me permanent brain damage. At age 43, i had to retire/disability due to defects mazda knew of and just inform consumer of in 2018? i was making right hand turn, legally. Someone in lane next to me decided to turn and hit side of car. I as not injure on impact. It was the three week had to drive damage car while wait for lady's insurance to pay. The metal piece that in sspa5 status stuck out and hit head on it multiple times. My neurologist was dumfounded and could not figure out where my various horrible symptoms came from. The accident occur march 2017,mazda has known for years, check mazda forum. Had this fix 3 times and is falling apart on passenger side again. As far as electrical, i will be at red light or stopped and car turn off for no reason, the bluetooth which need for directions goes in and out. It is safety concern for me due to confusion symptom of brain damage. I just received in febuary 2018 safety recall 1217f on brake issue. It state mazda concerned about safety, but again knew of issue and hat sent recall.the car has many serious manufacture defect that have caused my life to be ruin, and as i research, bodily harm to other people. Mazda, even when shown medical record and note from neurologist will not release me from my lease unless i get another mazda, which i will never do. I am on restricted driving and need to feel safe in the car i drive. I absolutely do not. Can only imagine what safety recall will come next month. If mazda cared about humans, these recalls would have been sent minute issue was acknowledged. The very least mazda can do is release me from lease, but they wont. Please get me out of this deadly machine so i am not afraid of car. Thankyou. I don't know how to add documents, another symptom of brain damage, memory loss,

I bought this car in january of 2017. Lately, i noticed the right side of dash went black, as steering info switch fails while driving.before our summer vacation road trip, i read through the manual and tested out electronic "bells and whistles", i discovered the master control switch in console and the steering wheel's radio volume control werenon-functional(never worked).there was no fuse to check. Mazda dealer claims shorted out due to beverage spill as cup holders are located next to master control switch.if so, why aren't relatively "high" voltage wires protected and waterproof at this location?dealer has had vehicle more that two weeks; as dealer claims electrical problems need to be traced back into steering wheel and dash. Back in january, as i was signing a purchase agreement, the dealership's finance employee strongly suggested i also buy an additional repair contract because "this model has like 15 computers in it."i did, but now i wish i didn't buy the vehicle. I don't believe wiring in this vehicle is safe.

The contact owns a 2015 mazda 6. While driving 26 mph, the vehicle was involved in a rear end collision. The contact stated the warning system did not activate and the automatic braking feature did not engage. The vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. A police report was filed and no injuries were reported. The approximate failure mileage was 4,700.

After stopping, the car does not power off all the way, dashboard and radio are still on. It happens very frequently and in some cases i waste a lot of time to try this 10-20 time before it actually turns off. On some occasions, i did not have much time and i left it that way and i could not close the car and after few hours the battery drained and i could not start the car.

On nov 27 16 the following safety issue occurred to my auto.while driving about 20-25 mph in slow traffic my vehicle threw itself out of gear, the engine raced, after a couple of seconds it violently went back in gear.prior to this happening the transmission had locked up twice and the auto could not be moved. By turning the ignition on and off that lock-up was freed. During this same driving period the following malfunction lights illuminated intermittently; engine light, low tire pressure, air bag, traction control (repeatedly). The autos engine was missing intermittently running rough.operating conditions:ambient temp: 78f, no wind, dry roadway, no precipitation.i contacted manufacturers mazda assist, they requested towing, auto is now at the dealer in (wallace) in stuart, florida,i informed mazda assist that i considered the car unsafe to drive, as i am handicapped and the holiday traffic was totally jammed i was force to drive slowly home on the 20 mph fla turnpike..

Even though it says it can use the lowest octane rating, i noticed some hesitation when starting from a red light. The cure for this was to go up one grade of octane now no more hesitation.

The brand new car idles very rough at stop lights.it shakes the entire car.

Was following a car and came up to a stop light. I depressed brake pedal and felt some resistance but car would not stop. Felt like the brakes were being bled off by perhaps computer control. I switched into a right hand turn lane that was clear, car still not stopping. Shifted into neutral, and engine started to rev up perhaps to 4500 rpm or so, then engine went back to idle on its own. The car had stopped and i put it in park. We then turned right and cautiously proceeded home, without a further problem.took to dealer and requested the memory be analyzed, they said they could not read the black box at the dealership. It showed no faults. Test drive was uneventful. I again requested the black box be analyzed as the brakes appeared to be bleeding off. They said they would let me know if mazda needed anything more.my background includes auto mechanic, tow truck driver, car dealership, technician, software engineer, and analyst.

I've owned this car for about 6 months. I usually buy gas by the dollar amount, but did a complete complete fill up for the first time with this vehicle. After driving away then parking at home, i noticed a heavy gasoline smell and checked underneath the car. There was a constant drip (every 15 sec or so) coming from underneath, near the rear driver's side wheel. I now have a puddle. I'm afraid to drive it. What if someone flicks a cigarette and it catches fire while parked?!my mechanic says that this is not normal after a complete full up, and should never happen.

The contact owns a 2015 mazda 6. While driving at a low speed, the accelerator pedal was depressed but the vehicle failed to accelerate. The engine rpms increased. The contact mentioned that the automatic transmission malfunctioned and the scbs warning and engine check required was displayed across the message board. The failure recurred on numerous occasions. Automatic transmission failure occurred as follows: transmission disengaged while driving 35 mph in city traffic (taylor street, columbia sc).car lost power as if the transmission was in neutral.warning displayed on dash "ats failure."i powered off the engine and re-started engine and drove approximately 500 feet before transmission failed again.was able to drive to the mazda dealership after 4th attempt.technicians labeled the issue as a "fluke" and they could find no reason for failure, although they did verify the code p0780 which stated to re-program the transmission.if i had been driving on the interstate (which i had been earlier in the day), the consequences could have been much more serious.

Audible whine from transmission when in motion. I can't recreate the noise when stopped or in neutral and can be heard when in driving and picking up speed.

This car has had many random computer problems starting with transmission locking in low gear until you stop the car and restart. Car shuts off displaying automatic stop break got very hard and steering very hard. My daughter had an accident last week 2/2/17 due to vehicle loss of power hard breaking and steering. Called mazda with no response.

The contact owns a 2015 mazda 6. The contact stated that the vehicle was repaired per an unknown recall regarding the brakes in march of 2018. After the repair, the contact heard a grinding sound coming from the brakes. The initial repairs were performed by bradshaw mazda service (2512 laurens rd, greenville, sc 29607, (864) 458-3500). The vehicle was taken back to the dealer who wanted to charge the contact a diagnostic fee. The vehicle was not repaired. The manufacturer was not contacted. The failure mileage was 42,172.

After making a left turn i depressed the brakes to stop for a red traffic signal. The brake pedal was excessively hard to operate and failed to stop the car. To avoid an accident, i had to use the parking break. At the same time the engine rpm increased dramatically. I shifted the transmission to neutral which brought the rpm to normal velocity. After this incident the car performed normally. I have experienced this problem three times, the last time was on april 4,2016. I noticed the failure occurs at slow speed, around 15 to 25 miles/hour.i went twice to berge mazda in mesa,az. Unfortunately they were unable to duplicate the break failure during test. This predicament is still unresolved and could be the cause of a deadly accident. Some help in this matter would be appreciated.

On the mechanical (cable) type parking brake, water may enter the brake caliper due to inappropriate sealing performance of the rear brake caliper protective boot, particularly under low temperature conditions, and the parking brake actuator shaft, which is the component of rear brake caliper, may be rusted. But no remedy available yet and this was back in june 2017

Hand operated parking brake safety recall.i received a recall notice from mazda stating that there is a defect in the brake caliper of mazda6 vehicles produced from october 25, 2012 through december 1, 2014.my vehicle falls into this recall category.i received the recall notice from mazda on august 19, 2017 making me aware of the issue and stating that replacement parts are not yet available to correct this safety issue. I called mazda usa on november 2 to get an update and they still don't have an eta for the parts.the recall notice states that the defect could result in unexpected movement of the vehicle if parked on a slope, increasing the risk of a collision.i have asked mazda to take the vehicle back and reimburse me.the representative made a note of this, gave me a case number and left it up to me to figure out how to resolve the issue.i'm concerned that the vehicle in its present state is at risk of causing physical harm and property damage to the surrounding area it may be parked in, especially places like san francisco, where high grade slopes are pretty common. Since this is a defect to the brake caliper i can only assume that the problem has existed ever since i purchased the vehicle.

Mechanical type parking brake, water may enter the brake caliper protective boot due to inappropriate sealing performance of the rear brake caliper. The actuator shaft may rust.which may stick resultin the rear brakes dragging while driving and also a decreased parking brake holding force.ref: recall1217f(mazda) 6 months and still haven't got parts for repair

While driving my 2015 mazda 6 gt, with 40,0000 miles on it, on a main local road, with a max speed limit requirement of 45 mph, i attempted to turn into the right turn lane at a speed of approx 35 mph, while lightly depressing the brake. As i began to turn, the steering just locked up, and the power steering stopped working. A few dashboard lights also came on at the same time.. One said power steering malfunction, the other was a red hazard symbol, and i don't specifically recall the other.i finally shut the car off after a few minutes.when i restarted the car, after several seconds the wheel regained mobility and the dashboard lights went off.i carefully and slowly drove my car home, aprrox 2 miles from where it happened.i haven't driven the car since, due to fear of it happening again.i was almost rear ended twice when it happened.

The car does not steer in a trustworthy manner and is unresponsive at times which negatively impacts the driver's assertion of where the car is on the road and how it will handle. The steering wheel itself behaves loosely and the turning capability of the car is unpredictable. Thus far the cause and a solution to it has not been found. The dealership's service department has had it 5 times in 4 months, including extended periods, and despite valiant attempts to remedy the problem, it continues to elude the technicians.

I have my 2015 mazda 6 for only one and half month. My windshield has a crack of almost 20 inch long. Almost everyday as i drive, i would hear little things hitting my car that leaves marks on my windshield and body. The reason i hear the sounds is because of its less than average soundproof structure. I do not drive any differently then i have been for years, and mostly i am just driving to work. I have never had this consistent problem before with my cars, in fact, for all the cars i ever drove, new or old, own or borrowed, no car gets mark and dents as easily as this one. I believe there is a quality problem with this model and it is unethical for mazda to sell customers vehicles that are completely resistant-less in wear and tear on a normal use.

My wife moved our 2015 mazda6. She moved the car down the driveway, pressed the brake to bring the car to a complete stop and shifted the car into park. She then proceeded to press the button to turn off the vehicle. In doing so, the engine turned off and when she released the brake the car rolled 3 feet. Frantically, she pushed down on the brake and pulled up the emergency brake. However, the car rolled into a post at the end of the driveway causing damage to the hood, molding and bottom bumper of our brand new vehicle.

We were on a highway access road, slowing for a turn onto an overpass. A piece of gravel from a gravel truck hit the windshield. We were not following closely. We did not see the gravel. Within seconds of the gravel impact, and as we slowed and turned onto the overpass, the windshield developed an over 20 inch crack. We had to replace the entire windshield, which cost us a $500 deductible. Gravel trucks are a fact of life. On all of our other cars, we have had to repair windshield rock chips. This is the first car we've owned where a piece of gravel resulted in a replaced windshield. This is concerning.

My vehicle has had strange grunting, squeaking, popping noises coming from the frontend, particularly the drivers side. It has had this issue since i bought the car in june 2017. The noise seems to be worse when driving over small bumps. It's very noticeable when i back out of my driveway. I have addressed this issue with the dealership i purchased my car from. They can't duplicate. I have researched this exact issue and found there is a tsb for the suspension issue i'm describing.i'm just getting worried about my safety. I'm afraid something will fail and i feel it's the manufacturers responsibility to correct this unsafe issue with the suspension that affecting many other mazda 6 vehicles.

While driving home from the shore today going 70mph i heard a popping sound and within 15 second my 2015 mazda 613,000 miles was out of control i barley made it crossing four lanes of traffics to get onto the shoulder. Once i exited the car i noticed the left rear tirep245/65r19 sport was the issue upon further examination i noticed the tire broke off approx 1" above where the dot numbers are located. The inside of tire to the rim also broke off completely from the rim.the tire was still completely round but was separated from the rim. Still approx2" of the rubberwas still attached to the both sides of the rim.i took numerous pictures of the tire and rim. I'm now afraid the three remaining tires may still have this type of defect.i was a police officer for the past 35 years and would occasionally travel well over 100mphwhile performing my duties and not once in those 35 years did i ever experience a tire separating from the rim like today. I really feel that if this happened to an average driver that driver would've completely loss control of the vehicle and could've been seriously injured or death to him or to other drivers. Please respond back to me asap. I also would like to know how i can send you the pictures i took on iphone.[xxx]https://www.dunloptires.com/en-us/company/contact-usinformation redacted pursuant to the freedom of information act (foia), 5 u.s.c. 552(b)(6).

No rock hit my car.. Then all of a sudden there was a large crack on my window. That just kept getting bigger.when i first purchased the car i thought something was wrong (purchased brand new), but when the defroster was in, it was as if there was a line in the window where it wouldn't work.now a year later, i have a crack that cannot be fixed and the whole window has to be replaced.

Intermittently, the cruise control buttons do not perform their proper function.when experiencing the issue, all of the following occur:1) cancel button functions as the minus (decelerate) button, 2) the minus button functions as the plus (accelerate) button, 3) the plus button doesn't work, and4) the off button disengages the cruise control but does not shutoff the cruise control main power.this behavior is an issue because with the cancel button acting as the minus button, the cruise control will activate with the cancel button.in addition, having the minus button function as the plus button could cause an accident if the car behaves in the opposite manner as expected.the issue occurs intermittently while driving, and the dealer claims they are unable to recreate the problem on two separate visits.the problem has been occurring for over 18 months.i've found that hitting the edge of the steering wheel hard will clear the problem - generally for the rest of the day, but it returns several times a week.i'm reporting the problem to the nhtsa because an internet search has shown similar problems have been reported.no one has posted a cause of the problem or resolution.here is a separate incident similar to mine from a mazada 6 forum:http://forum.mazda6club.com/electrical/350433-cruise-main-off-not-working.htmland another, separate incident with video on youtube:https://www.youtube.com/watch?v=0ycu3vdfieu&t=90s&list=flb8sh0os-z9o2rc1i0i4zlg&index=1and my posts on youtube showing how my car responds.the first video is from july 2016.the second is from december 2017.https://www.youtube.com/watch?v=_tr6hg-6tq8&list=flb8sh0os-z9o2rc1i0i4zlg&index=2https://www.youtube.com/watch?v=ape_lzxacyathank you for your assistance with this systemic and potentially serious defect.

The contact gradually applied force to the accelerator pedal and the vehicle accelerated in excess independently. The failure recurred on numerous occasions. The vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. The approximate failure mileage was 55.

Crack in windshield.went into store, no crack. Came back out, crack.not aware of anything hitting the windshield.have owned many vehicles over the years, have never had a windshield crack, even when it had been chipped by rocks.called dealership, they said call your insurance company.the crack is in the center of windshield, starts below the seal at the bottom and goes about a third of the way up.

I had done tons of research and finally found the vehicle that i was wanting. I purchased a brand new mazda6. I had only had the car about a week and a half, when suddenly, as i was entering onto the highway, i got my speed up to approximately 35-45 mph when i heard three loud pops. I saw my windshield beginning to split from the seal at the base. It split central above my defroster. By the time i traveled two miles to pull over, i had watched the crack grow, forming an s shape onto my windshield. When i contacted the dealership, i was immediately advised if it was a rock chip, it would not be covered. There was no rock chip on my windshield, i looked at it and took pictures of it. By the end of the day when i had gotten off of work, the crack had grown, covering about 1/3 of the height of my windshield. The split began widen a bit and a couple of little glass fragments were breaking off. I brought it up to the dealership and within seconds they stated it wouldn't be covered due to a rock chip when i watched it split from the base seal of the windshield. There was no indication or sign of a chip in the windshield. They made such a quick judgment that i feel they had made this decision prior to my arrival.

I am the original owner of a 2015 mazda 6. While traveling on a straight 60 mph highway, my sunroof window suddenly exploded, with a loud "bang!". The window was closed at the time and had not been opened in approximately 3 days. Fortunately, the interior deflector (sliding cover) was closed at the time, or glass shards would have rained down on my face and hair. No objects or sudden motions provoked the explosion. Police would not write a report because there were "no suspects of vandalism" and "no collision with other vehicles or property". $725 later, i have a new replacement window from the manufacturer and hope for no similar future incidents or possible mold/mildew issues. The cause of the safety risk is unknown but is certainly unacceptable, given that i maintain my vehicle to the highest standards and am the original owner of only 3 years. I have repair invoices, should they be necessary to substantiate this complaint.

Brand new vehicle and received a twenty-six inch long glass break on the front windshield from a small tiny rock. The break begins at the structure of the windshield frame and extends beyond. This is the first vehicle i've ever experienced to receive any break in the glass from a tiny rock along with the need to replace the entire windshield. Furthermore, days later another tiny rock hit the front windshield in the same area and created once again another break but a chip instead.

After my first trip on the highway in the 2015 mazda6, it got three chips/cracks in the windshield.this is not from a sizable stone, but tiny pebbles that in other cars would not have been affected.after owning many cars over the years, i have never witnessed one to be so quick to get cracks.i am quite bothered that in only seven days my new car needs something replaced due to a weak component.

Last week the while driving the car it was felt that the vehicle was going out of control and a metallic knocking sound was coming from the front right side.after parking the vehicle it was noticed that the front right tire had completely deflated.but he tpms did not show any abnormality ever during the drive.the tire was damaged to an extent that it had to be replaced.thankfully this happened at a low speed and it appears that the tpms sensors (on which there was an earlier recall) is not working correctly.

I was driving to work in my 2015 madza6 when i noticed the car is swerving a bit to the right. I thought my car had a small alignment problem. So i continue driving as i did not see here any beeps and there's no warning on my dashboard. It was about 3 to 4 miles later when all of the sudden, the car was swerving a bit to the left and right and i can hear growling sound which seems to come from the tires. I was driving at about 45 mph at that time and there were some snow and ice on the road due to the 3 to 4 inches of snow overnight. Without hesitating, i drove to a nearby parking lot and checked my car. Yap! i had a flat tire on the front passenger side. If that's is so, why didn't the tire pressure monitoring system (tps) gave me a beep and lights up on the dashboard? also, i remembered taking my car to the suburban madza6 dealer in troy michigan sometime in december 2014 to have the tps fixed due to a madza recall. So why isn't the tps on my car working? my odometer shows only about 8065 miles when this happens. I changed the flat tire with my spare and took the flat tire to discount tire (248-3931900) in auburn hills, mi and unfortunately, they told me my tire is bad as it was driven for quite a distance with low tire pressure. The tire still has about 80 to 90% thread on it.the next morning, i complained to madza usa (1-800-2225500) about the tire pressure monitoring system. I explained to jessie whom i spoke to about what happened. The case number is 1-1-91037814. He checked my car record and admit i did have the tps recall done. But he said there is a recent tps recall but i was not notified through mail or phone. I told jessie that madza should be responsible for the defective tps and should also pay for my tire. Instead he only give me a $100 coupon to be used for madza product and services. I hope to record this complaint for other madza6 owners.

Coming home from vacation on interstate highway large explosion.the rim blew out and the tire with it.there was no road hazard that we hit.the hole in the rim is approx. 3-4" and 6-8" split in the rim.
