I was backing up from a parking slot in a neighborhood shopping center, when the air bag on the steering wheel just exploded, and i was pinned to my seat. I pushed on the bag and it started to deflate. My seatbelt which had locked me into the seat, moved enough to allow me to dislodge it. When i got out of the vehicle and inspected the car. The only thing i found was a scrape mark underneath the center of the front bumper. Located in the parking slot was a concrete barrier situated in the front of the parking slot. The barrier is described as being five(5) foot long by four(4)inches high and five (5) inches wide. As i was backing out i did not feel any friction, vibration or hear any noise.

The air bag light on control panel is flashing when i start the engine after a working day. No rhyme or reason at all. There is no any impaction from outside and/or going over bumpers. It comes in and out in the first day and two, and then it's stayed on and flashed the code 19, which according to the manual means restraint control module - memory full. So i took it to mazda dealership and diagnosed the issue at my expense. According to the dealer report, they cannot reset the computer module such as clean memory; instead, i am asked to replace the clock spring for fix the driver side air bag. However, in the meantime, all the controls on the steering are working perfectly, horn/cruse control/ volume control for the stereo.

Fire in engine compartment, driver's side near front located at the fan controller unit.electrical high-resistance connection due to improper length of wire in harness causing loose connection in controller unit.

While driving the car (approximately 35 miles per hour)there is a sudden loss of power, including loss of power steering.car would not restart.we replaced alternator and had battery checked, which was working fine when the same problem occurred while driving on highway at 45 mph.problem is recurring.car is unsafe to drive.

In this case i left my lights on causing the battery to run down. The problem with this car is when there is no power it is impossible to open the rear sliding doors or tailgate. Only the driver's door opens with a key and you cannot open the rear doors from the inside without power. In the event of an accident that disables the power system these doors cannot be opened and passengers (especially children who are in safety seats) can be trapped. There are 3 rows of 2 bucket seats per row.i feel this car is a death trap and would seriously recommend it not be purchased.

The alarm keeps going off for no reason.i had this problem for long time. I turn it off and it will go off again, this is almost daily.

While driving 10 to 20 mph power steering fails followed by all electronics flashing and then by car engine dying.control of car is very difficult and dangerous.power steering has failed about 20 times engine failure about 10 times.dealer told me they found hits on the onboard computer, were able to repeat the problem and eventually tracked it down to the power steering. But have failed to find a problem.they now deny all of this. I question if the car is safe.

Broken fuel cap in a 2009 mazda5... Tried to unscrew fuel cap to fuel vehicle.it would not come off.tried several times until it broke off.a quick internet search found dozens of similar incidents with mazda 3's and mazda 5's. The cap has 2 parts, the outer part is what broke off, while the inner part remains stuck.after the outer part breaks off, it becomes evident the inner part is mistreaded, something not evident while the cap is intact.will have dealer remove stuck part and check this out, as there are obvious safety concern.

Occasionally (4 times now) when stopping or starting out (foot on brake, car in drive)the accelerator races full throttle, like i stomped on accelerator, for a few seconds. I have to press on the brake full force and/or put it in neutral. Scarey!!! called dealer, but was told they were not aware of any problems. No offer to check it out. Emailed mazdausaon jan2 but no response other than automated response showing they received my email. Took it to repair shop to check for codes - no codes.

The contact owns a 2009 mazda 5. While driving 35 mph, the rear driver side sliding door opened independently. The contact attempted to close the door, but the sliding door failed to latch. The failure recurred on numerous occasions. The vehicle was taken to a dealer where it was diagnosed that both rear door latches needed to be replaced. The vehicle was not repaired. The manufacturer was notified of the failure. The contact was aware of nhtsa campaign number: 06v463000 (latches/locks/linkages), but the vin was not included. The failure mileage was 100,000.

The third row seatbelts in my '09 mazda 5 being totally unusable for children and some adults, including myself. The seatbelts are positioned so high and so far forward that they are completely unsafe, therefore unusable. The problem is masked with a high back booster seat but even with a high back booster seat, when i jerked against the seatbelt to make it lock, there was at least 6 inches of motion forward and side-to-side before the seatbelt locked. When i tried this with a high back booster in the 2nd row, there was motion only 2 inches forward and none side-to-side. The seatbelt problem is totally unacceptable and am shocked there hasn't been a recall. There also isn't latch in the third row so how am i supposed to put anyone back there?

Gas pedal is too close to the brake pedal. I often have accelerated depressing the wrong pedal due to the extreme close proximity. While pulling up to the garage hit the gas pedal instead of the breaks and caused both vehicle and garage damage. I have thought to complain about this many times. The front end of the vehicle was damaged and the garage masonry has bricks and block work damage. Two of my wheels have been warped. I have only a medium foot size 10. This is dangerous for anyone.

Driving 20-309 mph on the 495 beltway during rush hour on wednesday, may 5th around 4 pm when the steering wheel of my new 2009 mazda 5 begins to lock.the power-steering wheel lock signal turns on and had to slowly get off the beltway.we had to turn off the car and turn it back on.thankfully we were not injured.we do not understand why the steering wheel locked and wondering if there were other complaints out there.

The steering column will not lock in place..mazda has made 6 repair attempts.repair dates:11/04/09, 4/14/10, 4/28/10, 6/10/10, 8/4/10, 10/25/10.the problem persists to date.

Today september 6, 2010, lit the light of a malfunction in the steering system of a mazda 5 2009, in heavy slow traffic, fortunately the car could drive to my house. Parked, went out and re-ignite the engine and the light disappeared.

The contact owns a 2009 mazda 5. While driving 55 mph, the power steering malfunctioned and the steering veered to the left. The failure almost caused a crash. In addition, the entire instrument panel illuminated. The contact had to exert excessive force to turn the steering wheel and pull the vehicle off the road. When the vehicle was turned off and restarted, the power steering returned to normal and the contact was able to drive home. The manufacturer and the dealer were not notified. The vehicle was not diagnosed or repaired. The failure mileage was 86,000.

At about 0630hrs, 02-03-11, while operating a 2009 mazda, mazda5 touring. The weather was clear and the roadway, okeechobee blvd., royal palm beach, palm beach county, florida, was dry, straight and level and of macadam construction. Traffic conditions were light to moderate. The portion of the roadway being driven upon was of four (4) lanes with a center median of concrete curbing and soil/landscaping with grass and shrubs and trees and there are left/right turn lanes in addition to the through roadway. Okeechobee blvd. Runs east and west and the area in which the incident occurred is a residential area. While driving eastbound on okeechobee blvd. In the inside lane of traffic at a speed of about 35 (thirtyfive) mph and approaching the intersection of crestwood blvd., the car suddenly began to drift to the left, without any input from the driver, and began entering into the left turn lane of okeechobee blvd, and all attempts to regain directional control of the car were thwarted by the sudden and apparent total loss of steering control, as the effort to turn or even move the steering wheel caused no change in direction and was beyond the physical strength of the driver, a 54 yoa, white female in good health and strength. As the car continued traveling further left into the intersection and with the application of braking, the right front wheel and right front lower bumper and chassis struck the curbing, traveling about 5 (five) feet before the car came to a halt in the westbound inside left turn lane of okeechobee blvd. At the crestwood blvd. Intersection. The vehicle suffered a shattered oilpan/lower crankcase and other major engine damage as well as damage to the right front suspension components, swingarm and wheel/tire along with the destruction of the front plastic bumper cover and lower plastic pans. The driver suffered minor bumps/bruising and musculo/skeletal pain to the arms, shoulders and neck.*ln

On may 12, 2010, our 2009 mazda5 with 18,800 miles on the odometer was being driven on the freeway during rush hour in los angeles at a speed of 40-50 miles an hour when the power steering failed suddenly.as a result the car was difficult to steer and control.fortunately, we were able to maneuver the vehicle off the freeway and call for help. The vehicle was towed to the mazda dealer.the dealer diagnosed the problem and replaced the power steering pump and pipe.the repair took two days as parts were not immediately available.

1.was driving my 2009 mazda5 in new york city in heavy traffic.as i as getting onto the lincoln tunnel i noticed my power steering had failed and the warning light was on.as i was in heavy traffic in the tunnel i had to drive until i could get to a less congested area.after stopping i turned the car off to call the dealer.when i restarted the car the power steering had returned.2: occurred just once.3. I took the car to a dealer the next day, am still waiting for resolution.

Power steering light comes on during operation, unable to use power steering until the car has been turned off and back on.this has occurred twice while driving the vehicle.received a recall notice; however the dealership says the parts are in short supply and cannot be fixed until at least december 2010.

The contact owns a 2009 mazda5. The contact stated that while driving 20 mph, the power steering failed and the steering wheel became extremely stiff. The contact was able to maneuver the vehicle onto the emergency lane and shut off the engine. The vehicle was towed to the dealer for diagnostic testing. The manufacturer was not contacted. The failure and current mileages were approximately 6,700.updated 09/01/10. *ljupdated 09/02/10

The steering wheel started to uncontrollable and wobbling suddenly while driving a freeway at 60mph and ended up causing the center divider crash. The vehicle was purchased one month ago as brand new car.the power steering has been difficult to handle and finally caused this major accident.the car is waiting for inspection from insurance company and the dealer refused to take any responsibility.

After ten minute drive time from home, the power steering failed while driving in a parking deck. Steering failure indicator light on dash was activated. With great difficulty, i exited the deck and parked on the street. After an hour of being parked and several engine restarts, the power steering came back on. We took the 2009 mazda5 to the dealer for repair and were told that they could find nothing wrong.

The contact owns a 2009 mazda 5.while driving at speeds of 60 mph, the power steering warning light illuminated and the steering wheel became extremely difficult to maneuver. The dealer replaced the power steering pump, piping hose and gasket. The mazda manufacturer was not contacted. The failure mileage was 17,000.updated 11/15/10

Sliding passenger door will not latch. Will open while driving.

I was heading to work, i cannot open sliding door to get my daughter into her seat.have to get her in on the driver's door and have to reach so far and bend to get her buckled in.when i drove off, the door signal on the panel is warning that a door is open.to avoid any accidents, i stop and recheck doors that can open, everything is shut but the signal still states it's open.i cannot check daughter's sliding door because it is still stuck....cannot open nor shut it.i drove slower just to make sure it will not suddenly open up, that will be horrifying event - for debris coming in, hitting my daughter or even my daughter's restrain system failing and for my daughter to be thrown out.when the weather warms up, the sliding door works again.it's intermittent and usually when temperature drops.the 2 side sliding doors, gets stuck either at same time or at times just one side.right now it's warmer, it's ok but yet cannot bring it to have it check.i called the dealership where i got the car from and tom at service, is in a hurry and i just have to let him go.i did check the internet, and there's actual safety recall for mazda 5 sliding doors freezing up, but for 2006 (recall number 4306k).it is still happening even for 2009 model, like ours.the safety concerns are still there.

Intermittent car shaking.after driving for a while, at speeds ofaround 60 or above, the car violently shakes, especially on bumpy roads.this is an intermittent problem.the dealer (faulkner - philadelphia, pa) - recently did an alignment and fixed a tire rim.the issue went away for a while, but occurs once in a while now.i am not sure exactly what part it is.note:about a year ago, i hit a curb which had debris on it which ruined two tires.all 4 tires did end up on the curb, and i did replace one of the tires and had the other one plugged and got an alignment.

Left rear shock failed at 21222 miles on march 23, 2012, covered by warranty.right rear shocked failed at 28073 miles on october 20, 2012. Mazda refused to replace right rear shock on march 23, 2012 due to no visible failure at this time. Mazda would not replace right rear shock unless i paid for it on september 20, 2012 due to lapse of warranty. These shocks have shown a history of failure at low mileage and mazda should be forced to recall these shocks due to their potential to cause loss of vehicle control.

Auto hashad to have complete set of tires because of premature wearing on outer edge of tires after only 32,000 miles. New tires are already showing signs of premature wearing even though they are being aligned and rotated regularly at only 52,000 miles.

I purchased a used 2009 mazda5 knowing that it needed tires.after buying new tires i had the car aligned.i was told the alignment was "within spec" despite the fact that the rear camber was set in such a manner that the tires shop said i'd be lucky to get 15,000 miles out of my new tires.i was also informed that there was no adjustment for rear camber without buying a $600 camber kit.i took it to the dealer, assuming they had more experience with the car, but was told the same thing.in order to properly align the car i would have to buy an aftermarket kit.i believe this is a safety issue because with the extreme and unadjustable negative camber tires can wear down to the cord on the inside edge while looking nearly new on the outer edge.all this happens rather quickly while the car is technically within the rather wide specification set by mazda.mazda customer service denies any knowledge about this problem despite hundreds, if not thousands of people claiming to have brought it to their attention.mazda knows this is a dangerous problem, but ignores the issue.

Kept hearing a rattling noise coming from rear driver's side, took the car in two times b/c of this.turned out to be a problem with the rear seat rubber bushing which is requiringthe dealer to grind the arch weld on the spring coil.there is a service bulletin about this very problem, which indicated that they have redesigned the parts in newer models so this does not happen.i feel that this is a design problem for which i have to pay for the repair and that it is not due to normal wear and tear on the car.it seems to me that they should fix the parts due totheir faulty manufacturing/design?

Rear shocks fail at 40k miles.have the shocks replaced at the dealer, making the same noise at 50k miles.i think the quality of shocks and suspension for this car is poor.

This van has 67,000 miles on it and i've had to replace the tires two times.i have flats going down the road.the inside of the tire is eat away. We have had the tires balanced and aligned to no avail and complained to mazda.looking at forums on the web, there seems to be many issues with this happening.

Loud clunking noise in left/front over uneven surface.worse during cold weather.checked by independent dealer then by mazda dealership.told sway bar bushings were the problem.car was under warranty at time of initial complaint, but technician was unable to hear it.[bushing warm by the time i got to dealer].claim made to mazda to repair under warranty but was declined.internet search shows this to be a known problem on this model.

Tpms sensor-valve failure:i was driving to work going about 40 mph when tpms light went on.i could not pull off immediately because of road conditions.shortly after that i could hear the sound of a very flat tire. When i managed to pull off and check the tires, one was completely flat.tire was too damaged to re-inflate.the relatively new tire and the tpms sensor had to be replaced.

The tire pressure sensor illuminated and i checked to see if the tire was flat.it was not.i drove to a filling station to top off the pressure and as i was driving away from the air pump the valve stem to the rear driver side tire broke and the tire deflated instantly.the valve stem was corroded.apparently it was initially cracked slightly causing the tire to first lose air slowly.then, upon manipulation from the pump fitting, the stem was further damaged causing the stem to break off.i did have this happen once before.the explanation from a service station was the same.in this model the valve stem and pressure sensor is a single assembly so when the tire is changed the valve stem is not changed even if it is old and showing signs of aging.otherwise the cost of replacing a tire would be nearly doubled.the sensor itself will last much longer than its valve stem.consequently, the stems do eventually corrode and break off at inconvenient, possibly dangerous, times.the service stations say this is common for this vintage of mazda.the service rep said mine was the second they repaired on the very same day!as i said, this happened once before when a stem broke off as i was backing out of my home parking spot.a couple of days before the this reported event, i took a long road trip and a sudden deflation like this could have happened at highway speed.it would have been tragic!i have checked on-line and have found many identical complaints.if it has not been reported to nhtsa before now, i am quite amazed.

Tpms sensor-valve slow leak:four new tires (pirelli p6 four seasons) installed at mazda dealer on jan 2011. The original tpms sensor-valves where used on the new tires.after that the tpms warning light would go on every couple of weeks. I would check the pressure in all the tires using a digital gauge which showed slightly low pressure (high 20?s psi) in one tire, the same tire each time.i would inflate the tire to the correct pressure.one reason i am reporting this is that there is a well-documented history of tpms sensor valves breaking easily when putting air in the tire.the more one has to inflate the tires the more likely the tpms sensor-values will break.

On dec.8,2011, i was driving eastbound on interstate hwy 167., the rear end of the vehicle on the passenger side had gone flat. I was able to get on the auxiliary road without getting into an accident. I inspected the tire and could not find any holes. I used a portable tire inflater and drove approximately 1/4 mile to a tire repair station. The tire was removed and upon closer inspection, it was revealed that the entire inner side of the tire had been worn off. The mechanic checked the other tires and showed me that the same type of wear was event on the rear drivers side tire. It was then that the mechanic informed me that this was common in mazdas' and that a simple parthas been used on mazda5s'to correct the problem. I take my vehicle for regular checkups and fluid changes every four months. Yet not once was i ever told of this potential dangerous hazard, that could have been fatal had it occurred on an open highway, where there are numerous turns on stretches of road that located on curves and are well over 200 feet above ground. (i have the tires involved).

Valve stem broke off while low filling tire with air.each time the tire looked a little low and when air nozzle was used it broke off and flew forcefully and all the air came suddenly rushing out.the computer low tire light was on the first time this happened, that is why i checked the pressure.i had to call service both times and had them changed tire and replace valve stem with non-computerized kind.they did not find any reasons that the tires had gone low other than the valve stem may have been already leaking.i then went to dealer to get new parts, which were still under warranty, and they said it was from misuse and wouldn't do anything about it.they were very rude.said it was from hitting curb.it was not.the valve stem is well protected inside the rim.this has happened twice within the last six months.both failures were nearly identical.i did not report the first time, but after checking around i found out this has happened to others and tire repairmen said they have had to change out several.had i not seen that the tire was a little low and checked it in my garage this could have been much more devastating.i'm almost afraid to drive in case one of these would break on the road and kill me.the failures happened in december 2010 and april 2011.the vehicle i'm driving is a 2009.

Tpms / tire valve stem leaked air from tire.this is the second tpms occurrence within last 2 months (from 2 different sensors).service shop said that it was corroded.

While driving at 70mph on us23 in michigan, tpms low pressure telltale illuminated.within 30 secs, vehicle began to pull hard to the left.pulled off the side of this 4 lane 70mph highway in the rain to investigate.left front tire completely flat.nut holding the tpms sensor to the rim was missing & had corroded, allowing the tpms sensor to fall inside tire cavity, resulting in rapid air loss.threads of tpms sensor exhibited corrosion.drove vehicle to gmc dealership in milan mi which confirmed the tpms sensor nut failure due to corrosion.tire destroyed, rim edge chewed up.contacted mazda usa and they would not offer any consideration, stating that the 3 year warranty had expired 1 month prior, and that corrosion was not covered beyond 3 years.i stated that this isn't a cosmetic rust issue, that the corrosion resulted in a safety concern for me and my family, but mazda would not offer any resolution.the vehicle has been driven during 3 mild ohio winters; there is notany signs of corrosion anywhere else on the vehicle; i drive the car thru automatic car washes every 3 weeks or so in the winter just to wash off any road salt.no other component on the vehicle is exhibiting corrosion.had vehicle on lift to inspect the underbody.it does not seem technically sound that a vehicle component specifically mandate by nhtsa to prevent tires from being ran low in inflation pressure on a vehicle should instead cause my tire to rapidly lose all air pressure.this is a safety issue. I bought this vehicle new and know its service life.this tpms sensor was in use for only 37 months.a 3 cent nut put my family at significant safety risk.

The contact owns a 2009 mazda mazda 5. While attempting to add air to the rear drivers side tire the aluminum valve stem end broke due to corrosion. The vehicle was taken to the dealer where the technicians replaced the valve stem. The vehicle was repaired. The approximate failure mileage was 23,000.

My husband purchased a brand new 2009 mazda5 and in february 10th 2011, as i was driving my tire lost all air, the valve had snapped in half. The dealer said i must have hit something but i know i didn't, i would have felt something if it was strong enough to break the valve, plus there was nothing on the side of the road close enough to hit the car. They made me pay $150 to replace it. In may 29th i was pumping air and it happened again on a different tire. At this time i had 37,200 miles. I called mazda and the representative said that they never heard of this problem and it wasn't covered. I google it and there are plenty of people complaining about the same and i even saw some reports of mazda knowing about faulty valves. I had another one break on december 15th, this was #3. This is beyond ridiculous!!!this product is definitely defective. At this point i do not want to have another one of these valves put on my tires. Mazda should recall them before someone dies on the highway when the tires go flat because of the valves. I bought this vehicle new from the dealer and had extended warranty on it, which did not cover my valves. They said i was probably pushing too hard when i was pumping air. Again, the valves should be recalled before someone dies.

The car dealershop didn't inform me about this car had 3 recall ,and i need to cancel the contract.i haven't pick up the car yet.if dealer sold me defected car with recall problem,i want to protect myself from fraud.

At 42,300 miles both front windshield wiper blades ripped off the wiper arm at exactly the same time, while driving in very heavy rain. When i went to get new blades, the parts manager explained that the driver side 26" blades are not sold separately from the arm, and i would have to purchase a whole new arm w/blade, although distributors do offer the right sized blade replacement blade for the passenger side. This vehicle is not even two years old, why are the rubber blades ripping off the arm?luckily there were no injuries or accidents as the result of this problem.

I have had 2 tire pressure monitoring valves break into 2 parts each in the past 4 months.the latest was when i was driving today on city streets.the tire pressure alarm went off so i stopped the car to check the tires.when i touched one of the valves it just came apart in my hand and i was holding part of the valve, the rest of it was still in the tire.the tire then completely deflated in about 2 seconds.this could be extremely hazardous.if you lost all tire pressure in a few seconds at 65 mph you could easily lose control of the car and get in an accident.i no longer trust the 2 valves that have yet to break so i am going to be proactive and replace them before they do break at an inopportune time.i see a lot of other complaints about the same problem on the internet.i included a picture of the piece of the valve that broke off into my hand.

Went to inflate tires, valve stem broke and tire went completely flat.i was lucky that this happened when the car wasn't moving - i have read about the valves failing at highway speed, when it could have caused an accident.
