While driving vehicle about 50 mph he was involved in a frontal collision and the air bag did not deploy, causing face and chest injuries.
Electrical connection between the main buss in the main fuse box and the fuse holderto the fuel pump relay fuse failed causing the vehicle to not start or to stall while driving.nlm
On long trips, often in cold, wet or snowy weather, the engine will occasionally rev out of control.this is not an issue of the throttle cable icing up because turning the engine off and restarting it (which is the last thing you want to do while still moving on icy roads) temporarily resolves the issue.therefore, it seems to be most likely an electrical problem.no diagnostic trouble codes (dtcs) are registered in the computer's memory, therefore no information exists by which to diagnose the problem.i have been to dealers and they have said they cannot help me unless they can replicate the problem in the shop.as i said, it has only happened after many hours of driving on long trips, which makes it impossible to replicate in the shop.
While driving the whole electrical system went out manufacturer offer no assistance.tt

I don't know what the problem is but the truck stalls out at idle and during driving at highway speeds. I have not lost power steering but am concerned that with stallling at highway speeds this couild pose a safety problem.i was wondering if there were any other reported problems or recalls that would fit this description.i have had thevehicles serviced andthe mechanic says that they can find no problems.

During a trip from home to store, the dash lights for the abs and brake system came on without any prior warning. Another driver made a u-turn in front of me prior to my arrival at the store causing me to have to apply the brakes in an evasive manuever. The brakes l,ocked up and caused me to lose control of the vehicle. Fortunately, i was able to avoid the collision. I took the vehicle to a local repair shop and they informed me that the abs hydraulic control valve needed to be replace. They indicated that there had been 3 revisions of the original abs system valve, indicating that mazda was aware that there had been a problem with this valve. We have never received any indication of a recall or problem.
Approximately 4-5 months ago i began to experience engine surging while driving in town.unable to find any vacuum leaks, i inspected the fuel filler neck to find it badly deteriorated in the area just above the gas tank.i replaced it with a new oem quality neck.at the same time i replaced the throttle position sensor, the coolant temperature sensor, rebuilt the fuel injectors and replaced the intake manifold gasket. Afterward, i did not experience any surging until a few weeks ago when, approaching a stop sign, the engine began racing so badly that i quickly shifted into neutral and steered to the shoulder of the road, narrowly avoiding a rear end collision.since then i have driven the truck about 100 miles without incident.the truck has been driven 90,000 miles and well maintained. It may not be relevant to this problem, but this engine was built without an egr valve and egr vacuum solenoid.
Knocking noise in cooling system, also leak near 't' in heater hose.
Engine hesitates/misses, also engine squeals.

Have been told on at least three seperate occations that problem w/ o2, throttle position and mass air flow sensors was " nothing that could be fixed and that problem would not intensify " and check engine light would remainonly a nusance. Recently, check engine light remained on and engine ran erratic and fuel mixture rich.now being told problemshould have been fixed before and now i must bear cost of repair due to non- warranty. Have also been told by others that this is an ongoing problem withford ranger,explorer/mazda b4000,navaho
Vehicle exhibits knocking sound coming from passenger's area or rear of vehicle, generally from heat shield. Also, brakes either hesitate/slam or grind.in addition, rough idling.
Request forassistance-check attached paid bills to determine if any repairs qualify for recall.the drivability problem was constant and was present during normal road conditions and the egr was found rusted and allowed incorrect egr flow. *sc

The brake switch has failed 3 times on this vehicle since the purchase date. This failure results in no brake lights. The switch has been replaced by mazda on 5/29/97 and 6/30/00. It was replaced for the third time by another repair shop on 7/16/01. I believe this high rate of failure needs to addressed because of failure to have break lights.
Because the brake light switch failed i could not disengage the cruise control by pressing on teh brake.almost rear ended vehicle in front of me at 70 mph.had switched replaced.happened again about 30,000 miles later., had switched replaced a second time
Brake light switch failure.
Because the brake light switch failed i could not disengage the cruise control by pressing on teh brake.almost rear ended vehicle in front of me at 70 mph.had switched replaced.happened again about 30,000 miles later., had switched replaced a second time
The brake light switch has failed four times (approximately every 15-20,000 miles).thismeans that there are no brake lights and no indication of failure inside the vehicle.the dealer says that the equivalent model ford (f150?)has the same problem and that the dealership can not fix theunderlyingproblem. The switch and / or mounting system for the switch was changed in 1996 (at least that is what the dealer says).the failure of this system creates a significant saftey problem.please advise if this problem has been identified by others.
The brake light switch failed over and over again due to the design flaw of the switch and the mounting bracket. Yh

The filler tube going to the gas tank ruptured. When i filled the tank, gas spilled on the ground. Fumes are always around the truck. I paid to have this tube replaced. The truck had 53 k miles. Now it has done the same thing at 74 k miles. I think that this is a very dangerous condition and is costly to keep repairing.
Parking brake non-functional.fuel leak, not able to locate. Repair shop claims probable no under warranty.brake vacuum booster isfailing. Rotors have been turned, pads replaced.drivers seat back broken. Repaired. Broken again.
The contact owns a 1994 mazda b4000. The contact stated that when refueling, fuel would spill back out from the filler neck. She also noticed a fuel leak when the vehicle was in motion. The vehicle was taken to an independent mechanic and they advised the contact that the fuel filler hose was fractured and needed to be replaced. The vehicle was repaired. The manufacturer was contacted and they offered no assistance. The failure mileage was approximately 202,000. The vin was unavailable.
Gas fumes inside vehicle afteritwarmed up, usually after driving 30-40 miles.
The contact owns a 1994 mazda b3000. The contact stated that he noticed fuel leaking from the vehicle. The contact inspected and noticed a fracture in the fuel line hose. The contact called both the dealer and the manufacturer and was told by both parties that the vehicle was not included in nhtsa campaign id number: 94v111002 (fuel system, gasoline:delivery:hoses, lines/piping, and fittings). The vehicle was not repaired. The current and failure mileage was approximately 189,000.
The contact owns a 1994 mazda b-3000 pick up. The fuel line hasdeveloped a crack.consequently during fueling gasoline leaks from the fuel system.fuel leakage in the presence of an ignition source can cause a fire. The current mileage was 323,000 miles.
Dt: the contact stated the fuel filler hose crackedwhich resulted in a leak.when attempting to fill the fuel tank there was a large amount of gasoline that leaked around the filler neck and formed a large puddle underneath the vehicle.the filler hose was previously replaced by an independent repair shop, but the hosecracked again.no further repairs have been made at this time.updated 12/27/2005 -

The contact owns a 1994 mazda b400.the contact stated that fuel leaked from the vehicle and onto the ground while he was pumping fuel into the tank.his vehicle is not included in recall # 94v111002 (fuel system, gasoline:delivery:hoses, lines/piping, and fittings).the current mileage is 98,000 and failure mileage was 92,000.
Had taken vehicle for inspection, which it could not pass due to lack of fuel pressure.consumer contacted mechanic and he said to check fuel pipe hose.in fact there was a leakage below hose clamp.dealer was contacted.
Plastic fuel tank cracked, leaking gas.
After smelling gas and seeing a puddle in the garage floor, owner had truck towed to the dealership.dealer found the fuel tank to be leaking from the forward part of the tank, right next to the strap tie down point.fuel tank was replaced at owner's expense. Please provide any further information.
The original fuel tank leaked gasoline.this was not caused by an accident or any other external force. The entire tank needed to be replaced. It seems to me that a tank this old should not rupture.i work with a fellow who owns a 1994 ford ranger (same truck) that also had to replace a bad tank.if i had not noticed the odor, this could have easily created a fire. Have there been any other reports of ruptures in fuel tanks for 1994 mazda or ford pickups?
I have a 1994 mazda b3000 with 75000 miles and after ten years the tank has developed a crack in an area that is protected from road debris and has no strees or flex . I beleive that this tank while it is ten years old is defective and should be replaced by the manufacturer. The result could be fire from the leaking fuel . The crack is not at a seam . The vehicle is not used in rough terrain causing stress.
My 1994 mazda b3000 pickup (i.e. Ford ranger) leaks gasoline when being filled at the filling station due to a faulty fuel tank filler neck.fuel spills on the ground of filling station (approx 1/2 gal) and is a fire hazard.cost to replace fuel filler neck is $280 from ford dealership, not including labor - this is prohibitively expensive for a rubber hose.many other ranger owners experience this same problem, thus indicating that there's a defect in the design/construction of the filler neck.
Dt: the gas tank filler hose on the mazda pick up truck is faulty and is leaking gasoline when the tank is full, and fumes when the tank is half full or less. In the consumer's research he found that this is a common problem. Madza is charging $400.00 for the replacement part.consumer has been in contact with mazda, with no help.consumer has placed on order for the replacement part. Consumer had remedy for this issue and it 4 years. However, problem recurred.
I have replaced the fuel assembly twice on my 1994 mazda b-3000 due to leaking fuel and paid $400.00 each occurence since i purchased the truck from the dealer. I took note of recall number 94v111002 and mazda states my vin number does not match. My truck specifications match as much as i can tell andi would appreciate a detailed explanation. Thank you.

Seat belt sticks when belt is inserted down side up into buckle.
June of 96 the seat belt on the passenger's side detached from anchorage and could not be used. On jan. Of 97 driver's side started going bad. Both belts could be used ifseatswere moved back as far as possible.when consumer drives she needs the seat up close.when seat is up too close belts are not long enough or they are not being released from anchorage.consumer calledmanufacturer who told her to take truck to vehicle to dealer andlet them check it out because there's only a 3 yr. Warrenty .
Seat belts have poor design, preventing occupants from moving once seat belts are fastened
While locking seat beltsthey become undone to due to latch not latching, consumer unable to wear seat belt. Please provide further information.
Driver's side seat belt assembly won't retract once it has been extended and fastened. Will not retract when buckle released. Driver must manually pull belt tight when attaching belt, usually with much effort because the take-up reel is very stiff. When leaving the truck, driver must again re-tighten belt because it will not retract and subsequently hangs outside the driver's door. Was replaced--i believe--about two years ago, but dealer is now out of business and i'm not able to access the service records. Selling dealer is also no longer in business. I'm not sure how secure i am in this sloppy seat belt, and would like it repaired to assure my--and other driver's--safety. As you know, this is a ford made product. It has had a few other "strange" defects, but this is the one i'm very concerned about now.
Back of driver's seat collapsed while driving within 4 blocks of home, throwing me suddenly backwardto a horizontal position. I lost controlmomentarily butvery carefully drove home, which was very dangerous without the back support, particularly difficult for braking and loss of seat belt shoulder strap.i have relatively low mileage on this vehicle and i feel that the set should have been made to last at least twice as long. I have never before experienced this kind of problem with even much older cars than this mazda. The dealer told me that there is only a three year warranty on the seat, which i think is unreasonable for a structural defect that compromises driver safety.

I don't know if there's anything to this complaint, but in view of the firestone tire recall (on wilderness p23575r15 tires - dot vd), i thought i'd provide you with some input on this tire problem episode.i suspect that you look for trends...if folks don't let you know about something, then the statistics are never developed for something that would otherwise go unnoticed.approx. 1/4 of the face of 1 of my truck's tires peeled away...the tire appears to have good tread.i am the second owner of the p/u.the truck came with the tires that are described in this questionnaire.i don't know how many miles were on the tires upon purchasing it, but they appeared to be in pretty good shape as i assumed ownership.the tire that appears to be defective is available for your experts to review if you so desire.thanks for the time that you might devote to this report.gery m.
