This referral involves a dump truck injury accident resulting in the vehicle overturning. I am a ky osha compliance officer assigned a complaint in regards to this vehicle. The complainant alleges that the truck needed new tires. They allege that a blown worn tire caused this accident. The accident happened near marion, ky. The truck is now in a collision repair center at 945 nandino blvd in lexington, ky. I inspected the vehicle and it had a worn inner right rear tire. The truck was loaded with 25 tons of gravel and traveling at approximately 45 mph when the driver lost control of the vehicle which eventually overturned. No other vehicles were involved. Since this is more of a dot issue i am referring the matter to dot for possible follow up with ky cve. My name is [xxx]i am attaching the police report on the incident.
