The most recent occurrence- of many- where the vehicle not operating as it was shifted to/ not staying on/or turning off when the ignition button is pressed is as follows. In a (flat) parking lot the van was parked and turned off> the brake was pressed> the ignition button was pressed to turn the van to 'on' and the word 'run' was illuminated- it was running in full electric mode with a battery of ~85% > the shift knob was turned from p to r> the van was reversed out of a parking spot > the brake was pressed and the shift knob was spun to d > the brake was released as the steering wheel was turned to pivot out of the parking lot however the car continued in reverse. At this point i started filming to 'catch' this problem on film since similar reported issues are always 'non duplicate-able' at the dealership. The ignition button showed 'run', the "d" above the shifting knob was blinking blue, there was an orange 'check engine' light type icon showing on the main screen where the message "shift to p to start" was also showing. I had to press on the brake, shift to p, and turn off the car, then turn on the car in order to have the car be able to drive in a forward motion. While this was in a quiet parking lot me sitting in the middle of a lane was not a problem, and thankfully i was not close to the next car while i was reversing since upon shifting to 'd' i continued backwards towards a row of cars. However, i live on a busy street and often have to reverse out onto a 4 lane road where it is imperative that the van move forward when shifted as such to avoid being hit by fast moving traffic.i have also had similar occurrences happen, and frequently have to press the ignition button multiple times to get the van to turn off once parked.*dt
Three notable issues.first, the electrical system randomly crashes and i receive a huge glaring 9-1-1 error screen that the vehicle phone system needs service. Second, the onboard charging system does not charge correctly leading me to believe failure is occuring.lastly, the drivers automatic side sliding door often malfunctions which could cause injury
Driving on interstate when the car basically lost forward drive in the middle of the highway. Was in traffic, going approximately 5mph with the adaptive cruise control on, braked for slowing traffic and on trying to re-accelerate got no power.and drifted slowly to a stop while trying to re-engage drive. Vehicle is phev, was running on battery with plenty of charge at the time and plenty of gas for hybrid gas engine backup, but neither provided power.tried unsuccessfully to turn the car off, restart, reset, etc., but nothing worked despite power to dash electronics.was pushed to shoulder by state police and subsequently towed to dealer....updated 11/03/17 updated 11/9/2017
On three seperate occasions over the past five months, i have encountered the same issue.while driving between 75-80mph on the highway with 32-44% life on the hybrid battery, the vehicle has shut off the engine.a message comes up on the cluster that i must put the vehicle in park to drive.the accelerator does nothing and power steering is lost.i must coast off the highway.once parked, the vehicle is in a state that cannot be turned on/off.the vehicle start button shows that the vehicle is on, but the engine is still off.all other electronics in car like stereo and bluetooth work as the center touchscreen remains active.touching the start button makes this flicker to off, but it immediately returns to this state.my vehicle has been at the chrysler dealer twice already and currently is there for a third time to resolve this issue, but they have no solution.this is deadly!
On friday, 7/12, while my wife, [xxx]along with my 5 children, were sitting at a stoplight in olathe, ks near the intersection of 119th and blackbob , she noticed smoke coming from under the hood. As the light turned green and she accelerated forward, turning west onto 119th st. Within seconds of thelight turning green, flames were noticed coming from the underside of the front passenger section of the car.my wife quickly pulled to the side of 119th st, about 50 yards to the west of the intersection with blackbob.she proceeded to help all 5 kids from the vehicle and as they were running away, the vehicle exploded into a fireball.the total time from her seeing smoke to the fireball was less about 4 minutes.information redacted pursuant to the freedom of information act (foia), 5 u.s.c. 552(b)(6).
The car had been operating normally while driving for about 15 minutes when we realized we missed a turn and decided to turn around using a driveway. The car was never switched out of drive. After turning around we started accelerating to cross the two lane road and then all motive power cut out, we lost power steering and could not operate the gear shifter. The vehicle stalled, dangerously blocking both lanes of traffic with three children and two adults inside. We got everyone out of the vehicle and off of the road. A few minutes later after reentering the vehicle, it started normally.
While driving to work in the morning on 19-feb-2019, the mil check engine lamp illuminated as well as the "red turtle" limp mode indicator and a dashboard message stating "vehicle speed may be limited." this happened while driving on local surface streets about one mile from my place of work. I had just turned off a 45 mph main street onto a 35 mph side street. The gas engine in my phev hybrid vehicle stopped working but could still operate at normal speed in all-electric mode. I got to work and plugged my phev van into a chargepoint evse to recharge. At lunch, i ran some errands and noticed that the gas engine still would not operate under any conditions, which is very rare during the harsh, cold weather we were having at the time in the chicago area. After returning to work, i made a service appointment at my local dealership for that evening. On my drive to the dealership after work, the all-electric motor also failed and stopped working about two miles from the dealership. This happened on a 35 mph main street, and i was driving straight down the road at the time. I shut the vehicle off and restarted it and was able to move about 100 feet before it shut down again. I was able to repeat this process 2-3 times in order to turn my vehicle off onto a side street so that i was not impeding traffic on the main thoroughfare. I had my phev van towed to the dealership that evening. It has now been at the dealership for repair for 49 days, and they still cannot determine what caused the loss of power and forward motion.
Car was powered on, put in reverse and we began backing down a steep driveway. Part of the way down the motive power cut out, we lost power steering and power brakes and could not operate the gear shifter. We could not get the vehicle to restart on the hill but were able to roll down to the bottom. Once at the bottom we were able to restart the vehicle normally.
The van's engine stopped while traveling at 30 mph on a city street. The ac had been running. The hybrid and engine light came on. The day board stated i needed to put vehicle in park to start van. After coasting to a stop the vehicle did not respond to trying to use gear selector. This changed after sitting for 15 minutes. While driving 45 mph on 4-lane divided surface street, a warning chime sounded, all the dash lights illuminated, and the engine died. Once stopped, was unable to restart the engine. All the lights remained on, as did the radio, etc. Was able to 'start' the system in order to put it in neutral and get pushed to side of road. Engine would still not turn over. Electronics remained on - would not turn off even when door was opened. Tow truck driver pulled a switch to enable him to put it in neutral and the engine started. Apparently when that happened, it cleared the codes and the chrysler garage was unable to identify the problem. After four days of trying to recreate the problem, they gave up and gave me the car back. I am extremely uncomfortable driving it now. Had one previous incident where the chime sounded and all the dash lights came on, but did not lose power, or steering. Engine light came on, but once car was stopped and then restarted, light did not come back on. Both times that happened, the electric battery was depleted and car was in hybrid mode, using the gas engine.
The contact owns a 2018 (na) chrysler pacifica hybrid. While driving 60 mph, the vehicle stalled. Once it was restarted, the temperature and traction control indicators illuminated. The vehicle was taken to a dealer (ry dell chrysler dodge jeep ram, 700 san fernando road, san fernando, ca 91340) where it was diagnosed that there was a short in the temperature sensor and a new transmission was needed. The manufacturer was notified and informed the contact that they would not honor the warranty on the vehicle. The failure mileage was 278.
I bought a new plug in hybrid car with an advertised "automated emergency breaking" and 32 mpg gas onlymilage, and mpge of 84.it does not have the automatic breaking, and gets 24 mpg only.i have had 25 documented communications with the dealer, chrysler customer service (case 33518003) which was "escalated" two levels - but never addressed. The dealer proposed i sell it back to them as a "used car" at a huge discount - and i want a full refund.north carolina lemon law officials (mr. David fox - 1-877-5-no-scam) said i should take case to your agency.
While driving, i experienced a sudden loss of power. The vehicle put itself into a low-speed mode (aka "turtle mode") and then would no longer accelerate at all. It has been determined that an entirely new transmission is required.

The contact owns a 2017 chrysler pacifica hybrid. The contact received notification of nhtsa campaign number: 17v371000 (hybrid propulsion system) however, the parts to do the repair were unavailable. The contact stated that the manufacturer exceeded a reasonable amount of time for the recall repair. The dealer was not contacted. The manufacturer was contacted 800-853-1403, was made aware of the issue and were not able to confirm when the parts were to become available. The contact had not experienced a failure. Vin tool confirms parts not available.

The brakes on my vehicle randomly go out. Sometimes the brakes go out long enough the entire car starts screaming at you to break because you get too close to the vehicle in front of you. It's unpredictable. The car has always been in motion when this happens.

Chrysler says possible diode failure, possible sudden loss of power, possible crash (fatal), but chrysler has no replacement, says nothing can be done until third quarter 2017.that's unacceptable.i can't drive a deathmobile.my car has not shown symptoms, but that's not the point.my vin is on recall list. Recall notice attached.

The rear view backup camera fails to function (goes virtually dark) under certain lighting conditions, presenting a condition that is favorable to collisions with vehicles, object, or pedestrians.the web has plenty such complaints on this vehicle, and the problem may not be specific to the hybrid.two specific scenarios where i experienced the problem: 1) in a parking garage, in broad daylight, when backing out of a space, and 2) when backing out of the garage in my house at night.to speculate on the cause: in both of these scenarios, light (either from the sun or a lamp) was hitting the front of the vehicle, while the area behind the vehicle was relatively dark.it is possible that light sensors on the vehicle miscalculate the amount of available light and so fail to adjust the light level on the camera.we purchased this vehicle new about a month ago and there are no open recalls on it.
