The contact owns a tms motorcycle helmet, model jx-a111. While driving a 1996 yamaha 900rsr motorcycle and attempting to avoid a vehicle, the contact swerved and fell into a ditch. The helmet face covering exploded when it impacted the pavement. As a result, the contact sustained injuries to the head, back, ribs, spine, and scrapes all over the body and face. Medical attention was received. The contact recently received notification of nhtsa campaign number: 15e023000 (equipment). The serial number was unknown.
