Rv trailer tow ball came loose form ball mount caused by the add on sway control mount. Husky 39585 sway control ball mount adapter sold by:one source rv amazon.the sway control ball mounting pad was added to the standard ball mount. Hitch ball clamped the adapter in place, while a bolt-on shim was to keeps it from twisting.the shim plate did not keep the sway control ball mounting pad from being twisted by the sway controller.this movement loosen the nut on the trailer tow ball by moving the ball while the lock washer held the nut.once the ball was loose the nut was loose.the loose ball cause the trailer behave erratically at interstate speed.the nut could have unthread and fall off. The trailer would have been un attached form the tow vehicle. If this had happen at interstate speeds a serous accident would have occurred.*dt
