My headlights are intermittently failing on my 2006 prius.the dealership has replaced both the left and right (as a pair) headlights at ~20,000 miles and again at ~48,000 miles.and now at 55,000 miles, they are replacing another front headlight hid bulb.there were two additional times that i requested the toyota service department check the lights as i had been told by a police officer that front and/or back lights were out, but the dealership said all of the lights were functioning.my lights appear to work intermittently.i may have both headlights working for a week or a month or more, then one of both may go out, and then one or both may start functioning again.it's completely variable.i never know if one of both headlights will work at any given time.it's frustrating and scary- as i don't know what to do to fundamentally resolve the issue and ensure that i have functioning headlights!approximate dates:jan 2008 - police officer told me a light was out, dealer could not reproduceapril 2008 - both headlights replacedseptember 2009- both headlights replaced february 2009- police officer told me lights were out (1 front, 1 back)march 2010- headlight replaced *dt

My 2007 prius has the hid headlamps, which are too costly to repair and will randomly go out while driving at night...clearly dangerous.i try to have the bulbs replaced and toyota dealer says its a 200.00 to 300.00 dollar job to repair (money that i do not have) while the car was still under warranty the las vegas dealer fixed the issue ( or i thought they did) but less then two years later i am having the same problem and now toyota will not repair it cause it is not under warranty. This is wrong they tried and failed to fix now i guess i am no longer a good customer.why will they not fix a danger situation.[xxx] information redacted pursuant to the freedom of information act (foia), 5 u.s.c. 552(b)(6).

Driver-side hid headlamp in 2004 toyota prius was noticed to be very, very, dim (effectively off) around 2008/10/14.turning headlamps off and on restored brightness.but the problem recurred at unpredictable times--frequently enough to be a serious safety problem.frequently the driver can't tell while driving, whether one headlamp is off, and obviously it's unsafe to fix it by turning it off and on while driving. Toyota dealer replaced driver-side headlamp on 2008/10/15, for $600!on 12/15 the driver noticed the same headlamp again was misbehaving in exactly the same way, and the problem has recurred since then at apparently random times.priuschat.com's many posters about this problem make it unlikely that just the bulb is the problem.probably the ballast.but toyota won't comment.i do still have the original bulb, and the box from the new bulb. Lisa at toyota corporate customer experience center replied around 2008/10/18 that toyota would reimburse half of the $600.but as of 2009/1/3 no reimbursement has been gotten.i called toyota corporate again on 2008/12/16 and damian said he must forward my new complaint about the repeated failure to lisa, and that she was "required" to respond in one business day.she has not responded as of 2009/1/3. Headlight malfunction. Right headlight has malfunctioned (turned off), with no consistent cause apparent. The specific error message that appears in the driver information screen is "right headlight dipped". This condition is both unsafe, and illegal - as many states will issue a traffic violation for driving with a non-functioning headlight. It has happened about 7-8 times, the headlight flickers, then goes out, but will come back on when the car is restarted, then be fine for weeks or months at a time. The dealership has been notified of the problem on at least 3 visits, and has claimed that everything is fine and they can not reproduce. They have suggested/guessed that it is the xenon light bulb itself, however it has checked out fine each time, and as mentioned, has functioned flawlessly after each occurrence with merely a restart of the vehicle. No parts have been repaired or replaced, however they have claimed to perform a visual inspection of the electrical connections on at least one visit. Annecdotal evidence (conversational, and via internet owners' forums) suggests that the problem is widespread among a substantial portion of audi's model range and years.
