The contact owns a 1996 ford taurus gl, equipped with a duralast gold dlg773667 alternator. The contact stated that the driver was traveling 35 mph when black smoke emitted inside the vehicle as the vehicle caught on fire.the driver noticed that the after market alternator was melted after the fire. The vehicle was not taken to the dealer and the manufacturer was not contacted. The vehicle was not repaired. The failure mileage was 145,327 and the current mileage was 145,337.
I recently (3 dec., 2016) purchased front disc brake rotors for my 2003 honda accord from autozone store in orange connecticut. Brake rotors (duralast gold rotor, p/n 582278dg914).the brake rotors were purchased along with their best ceramic brake pads.prior to installing the brake rotors i wiped them down with solvent to remove any grease or oil.as i wiped them down i saw no grease on the rotor which had a smooth black surface which i assumed was a protective coating like alodine or similar to prevent rust.nothing came off on the rag when i wiped them down.the installation went well (i have done this job several times), but the brakes made a grinding noise as soon as the car was rolling in the driveway.upon disassembly, to assess the issue, i found that the black coating was actually a fairly thick layer of black paint which had been applied to the brake rotors by the brake rotor manufacturer.the brake pads also now had a significant buildup of partly melted black paint gooped into the ceramic brake pads.i sanded the pain off the rotors with a disc sander (the paint was quite hard), replaced the brake pads with new ones from honda, and re-assembled the brakes on the car.this resolved the problems.my concern is that some owners may be using these brake rotors with gummed up brake pads and be unaware that the braking efficiency may be degraded.[xxx] mechanical engineer. *dlinformation redacted pursuant to the freedom of information act (foia), 5 u.s.c. 552(b)(6).'

The contact stated that while starting the vehicle a loud noise emitted from the rear of the vehicle. After inspecting the vehicle, the contact discovered that the battery had exploded and debris from the battery was present in the trunk of the vehicle. The dealer and manufacturer of the battery were notified of the incident. The vehicle mileage at the time of the incident was 145,000.
