This safety seat has an adjustable head restraint that does not stay in the locked position.when the restraint falls to the lowest position, the straps are loosened and do not provide the best protection for my child.i would like to either return the product or receive replacement parts for the head restraint.

Child's adjustable headrest does not lock into place after being adjusted to height requirements for safety harness/restraint. The child's car seat headrest slips out of position which allows the straps to loosen. This could lead to a severe injury. This occurs every time the car seat is in use, there are currently no replacement parts to fix this issue.

The adjustable headrest on the booster car seat does not lock in the position desired for the child. It slides back down to the lowest position resulting in reduced or no side impact protection for child.

The contact owns a safety 1st prospect 22-595-wal child safety seat.while securing the booster seat with a safety belt she noticed that the child seat was moving back and forth. The manufacturer was notified and informed her that the failure was not a safety issue.whenever the booster seat was secured with a safety belt the failure recurred.

I recently purchased a saunter travel system stroller set. The stroller seems to be fine, however the car seat carrier does not work well in grocery store shopping carts where they are supposed to slide down and fit into the top of the horizontal edge of the baskets cart to secure the seat for child safety. The carrier is unsafe because it does not have the two narrow slats on each side that slips down over and into the shopping carts edge of the basket allowing it to securely lock into place. However this particular carseat model has a half moon rounded smooth plastic molding that will cause it to slip and slide causing the carrier to fall off of the carts edge and causing injury or death to the infant. I tried to take this item back to the store that i purchased it from but the store manager refused to exchange it. The manager said that because it was purchased with a state voucher, it was the state organization's rule that no exchanges could be made for any reason except in the case that i had purchased a clothing item whose size was not the right size. I do not know what to do at this time, i need a carseat carrier that is safe for shopping as well as vehicle safety. Can you help? any assistance you can offer would be very much appreciated.

Complaint about safety 1st child restraint forward facing car seat. Model: bc026-atv linenumber:08 b 264460 mfg: 04/26/2011 model: rumithe issue is that the seat belt is very difficult to tighten and loosen. This was observed from day one and still is very difficult. First i though it's not so bad since the child will not be ejected or be in any danger until now. We were in a situation where we had to get the child out in seconds. This wasn't possible since it took longer than usual to release the seat belt, compared to a non at fault seat. This also applies to get the child in the seat during a emergency. There is friction so the seat belt is difficult to loosen and tighten. It is imperative the seat belt to work without flaws. Meaning to hold the child as it is to use the seat belt. Please make sure this will not result in loss of life. I am an engineer and took a look at the seat. There is a tray underneath the seat, i guess to store the equipment when not in use. That tray when inserted will touch the roller where the seat belt is routed around to make a 90 degree turn and routed up the seat. When the seat is installed in the car the upwardforce created against that tray and the roller makes the seat belt difficult to tighten and loosen.

The contact owns a safety first child restraint, model gb4359-660, manufactured date 6/19/2010. The restraint was installed in a 2000 lexus gx300. The contact stated that while turning left, the restraint flipped over while the child was seated in the restraint. The child did not suffer any injuries. The manufacturer was not contacted.

I purchased a cosco saftey 1st car seat a few months ago. This seat frequantly has problems locking into it's base. We recently worked for 10 minutes attempting to get it to lock properly prior to my infant son's doctor appointment.

It is a safety first blue/ black car seat the straps get loosen and my baby was choking he is 16 pounds nine months and moves the restraints were loosen and he was choking. My oldest son was fortunately, in the back and informed us that he was choking. The model number is 01929-pac and it was manufacture 11/7/06.

Infant carrier child passenger restraint was defective out of box. Carry handle does not lock into place, potentially allowing seat to fall forward when picked up and allow child to fall out. The locking indictor on each side of the handle shows that it is not locking into place.

After buying a brand new safety 1st forward facing/booster for up to 100lbs. For my son, i went home to install it. I read the instruction manual, then i placed it in my 92 chevy astro van, tightened the straps, and put the locking clip on, i noticed a bulge in the back of the seat. The manual told me to put the path of the seat belt behind the seat for forward facing. However, this(recommended) placement put the seat belt in my childs back because there is no plastic support from the shoulder supports to the bottom of the seat! there is only cloth that separates the child from the seat belt path. Luckily, i noticed the bulkiness and felt it. Otherwise, in an accident, my child's back would probably been permanently injured due to his back not being protected from the hard metal seat belt!! i promptly removed the seat from my vehicle and returned it to wal-mart for a refund. I think this is an issue with several seats sold at wal-mart as i am having a difficult time finding a seat for my child that does have full back support. I am unsure if this is a result of my car, or the seats themselves. But, ifear my childs back would have been seriously injured in an accident by the seat belt itself if not the accident!

I own two safety 1st carseats for my twins. One of them the handle has been stuck in the upright position for some time and we have learned to deal with it. However the other handle had given way once two weeks ago while carrying my son out to the yard for fresh air. He fell forward onto the grass and i felt terrible believing that i had not locked it in place. I made sure to double check it from then on. But to my horror it occurred again last week while leaving a restaurant. This time my little one fell forward onto tile floor causing contusions to his head, face and nose. He was treated at the local er. I have emailed dorel corp. Called three times and still no one has responded. Customer serv. Has stated only that the handles cannot be repaired buti am not to return the car seats to the store but to wait for a call from them. Walmart has offered to refund or replace them but the company itself has not even returned my call. Being in the military with 6 children including baby twins we are on a fixed income and cannot afford to put out the expense to buy two more car seats so i simply wanted these replaced. Having to carry two babies everywhere makes the handles a necessity so this is causing me alot of difficulty. I do not know what to do at this point. I am frustrated and getting more angry by the minute at the irresponsibility and lack of attention dorel corp is giving this situation. I fear this incident may occur to other families using this product.

I attempted to adjust the height of the headrest of a safety first apex 65 child restraint, manufactured in october 2008 and purchased in november 2008. When i first purchased the restraint, the knob on the back of the headrest that "locks" the headrest into position was difficult to turn, but i was able to adjust the headrest into the appropriate position for the child needing to use the restraint. I needed to change the headrest setting, and was able to easily turn the knob on the back of the headrest, but could not extend the headrest to a higher setting, nor decrease it to a lower height. I could hear something rattling within the restraint, and when i lifted the restraint, a small (~1/4" in length) screw fell out of the restraint. This restraint is a spare, used only three times when i have needed to transport additional children. It has been carefully stored in my home when not in use, leading me to believe this issue may be due to either a manufacturing defect or damage during the shipping process.upon contacting the manufacturer, dorel, they have offered to replace the restraint upon proof of destruction.

I am a cpst who ordered 2 safety first chart air 65 convertible seats from the safety first / dorel "hot deals" section of their website recently.when seats arrived i noted that there was seeminglyample "double"body pillow padding for an infant or small child butwhen the " air protect" feature is moved up (to allow access to higher shoulder slots) and the body pillow(s) for the smaller child are removed ... The area behind the back of a toddler or preschool age child's back is completely devoid of padding..in addition to not having any padding...the fabric in this area is much thinner than elsewhere on the seat. Assuming the "body pillows" are removed as the child grows ( instr. Pp 28) this would render the seat very uncomfortable if not painful with day to day use. Questionable as to outcome in a mvc.additionally i noted that with moving the " air protect " feature upward ...the track it slides upon leaves approx.1.5 cm of "open space"that is easily accessible to a toddler or pre-school child. Thespace is & wide enough to allow a child's fingers (possibly a hand) to become entrapped in the track.i am not sure there are standards that address the amount of padding behind a child's back ..but i am bringing this lack of padding to your attention such that if there are no current standards perhaps one might be developed.i am pretty sure there are standards to addressthe potential for a child's finger or hand to be injured. Just not sure if an injury actually must occur before the matter can be addressed. I do believe there is potential for injury in that a small hand could be entrapped i did contact dorel to see if i could return the seat due to the concerns above. They will allow a return if i want to pay the postage.they see this as an issue of buyers remorse and not a problem with the product,i have decided not to return the product to dorel. Please let me know if nhtsais interested in having the seat.

After watching a video of another safety 1st vantage carseat with harness failure, i decided to check the seat at my in-laws house that is luckily not used very often. Low and behold, with a small amount of force, you can completely pull the harness loose without pressing the button to loosen it. I am a car seat technician and deal with many seats every day. I am shocked (and a bit embarrassed) that my child has been in this seat. Clearly, a 35lb child in a 20mph accident would have much greater force then me yanking on the harness... This seat is a real danger to any child riding in it. I will discontinue to use this seat and hopefully send it back to safety 1st for their own testing. Clearly the failure is in the mechanism that loosens and keeps the harness tight. My seat was made in 03/28/2004 and is model number 22-561-wal.

Parents had difficulty attaching the safety 1st onboard infant child restraint onto the base. Model# 22-077 fsm. Cps technicians attempted to put the infant child restraint onto the base and noticed that when they were putting it on, that it was difficult to attach it securely. (*note: parent arrived without the child restraint securely attached due to problem) once the infant child restraint was on the base, it would then not release from the left side looking directly at the child restraint to be removed. When pulling on the base release lever, the right side would release and it was apparent that there were no obstructions from the seatbelt or the lower anchor belts. Upon removal of the padding to view the carrier shell, one screw was removed from the inside panel to examine the inner workings of the base attachment hooks.by manually moving the inside base attachment hook mechanism, the infant carrier immediately released from the base. When the carrier was off of the base and an examination was completed it was observed that the base attachment hook on the left side was not functioning appropriately and would not move back as far as the hook on the right side, preventing the child restraint from initially being secured properly on the base and then creating extreme difficulty in removing said carrier.i am a child passenger safety technician instructor and was called to assist in this matter.

Safety 1st rumi air - one hand adjustable headrest does not stay in place. After 1 year from purchasing my child grew and now i need to readjust the height of the headrest. Unfortunately the headrest does not stay in place. It drops down to the original position which makes the harness straps go down my child's shoulders and the headrest is not properly aligned with my childs head. This is not safe and no one wants to replace it. Ive read reviews of other people having the same issue and to my surprise there is no recall for this problem. Do not buy this carseat.

In february of 2012 we purchased a rumi air harnessed booster for my daughter. Several months later i noticed that i was frequently having to readjust the head rest. My husband and i started paying more attention to what was going on. We checked the instruction manual to ensure we were adjusting it properly. However, it continued to lower on it's own. I even took the seat completely out of our vehicle and adjusted it. The head rest appears to be locked in place, but if it is jiggled enough, it will lower on its own. We live on a gravel road, so this happens frequently. I have noticed that the head rest will lower faster when i take our vehicle without my daughter. I'm assuming that her weight helps keep the seat from jiggling. We did not notice this issue at first because our daughter did not need the head rest raised for the first few months after purchasing the product. I believe that i registered the seat with the company, but have not received any notices about safety concerns/issues; however, i may not have sent in the card as i was still in early postpartum for our son and can't remember for sure.

I am a certified cps technician.i was helping a father install a safety 1st intera forward facing using the latch system in the center rear position of his 2003 chevy impala.after the install, i tightened and loosened the harness to ensure it was moving properly.when i pushed in the silver harness adjuster lever to loosen the harness straps, the lever stuck in the inward position, so when you pulled the adjuster strap it would tighten the harness, but you could pull the harness straps out to loosen without touching the lever because it was stuck in the loosen position.this means an unsuspecting parent may think the harness is tight on their child, when in fact the child could move forward easily.this would obviously be a critical safety factor in a crash as the child would not be harnessed tightly to the seat.**** the model # and product year 9999 are incorrect, your drop down screens would not allow correct info to be placed.the child restraint was a safety 1st intera, model #-22-460 bdf, dom - 7/29/04 and the defective part was the harness adjuster lever, not the harness.

Bought 2 harnessed booster car seats. The headrest adjustment does not lock after you raise the headrest . I amnot able to adjustheadrest and harness height to accommodate thegrowth of my grandchildren. This makes the booster seats unsafe.

After watching a video of another safety 1st vantage carseat with harness failure, i decided to check the seat at my in-laws house that is luckily not used very often. Low and behold, with a small amount of force, you can completely pull the harness loose without pressing the button to loosen it. I am a car seat technician and deal with many seats every day. I am shocked (and a bit embarrassed) that my child has been in this seat. Clearly, a 35lb child in a 20mph accident would have much greater force then me yanking on the harness... This seat is a real danger to any child riding in it. I will discontinue to use this seat and hopefully send it back to safety 1st for their own testing. Clearly the failure is in the mechanism that loosens and keeps the harness tight. My seat was made in 03/28/2004 and is model number 22-561-wal.

Infant carrier carseat's harness straps have come outof the 2 slot from the lowest setting numerous times and over a few days time i have noticed that they loosen up. I have notice this ever since i moved the straps up to better fit my son while he is in his seat.model # 22-311-wal manufacture # 8b643290 manufactured date 1/30/2007

Loosening the straps on the car seat to put child in or remove and one strap comes completely out of its restraint in the back.the entire seat has to be removed and put back together.worried about child safety in a crash.the back of the seat has a metal bracket which the two shoulder restraints just slip over.when you loosen the shoulder straps to put a child in or take him out then there can be so much slack on that metal bracket that it slips right off the shoulder strap.if you don't notice the next time then the child can be put back in the seat with one side not fully restrained.the child could be in danger in a crash or even a sudden stop.to fix the situation, you have to take the entire seat out and put the harness back together.this seat is made to hold up to 65 lbs with the 5 point harness.my child is only 42 lbs and i am worried how it would really do with any larger child who has the straps pulled even more loose to get in and out.the type of seat is not properly listed so i couldn't correctly report it on the next screen.it is the safety 1st, apex 65, model 22-531.

The child's car seat headrest slips out of position which allows the straps to loosen. This could lead to a severe injury. This occurs every time the car seat is in use, there are currently no replacement parts to fix this issue.

Complaint about safety 1st child restraint forward facing car seat. Model: bc026-atv linenumber:08 b 264460 mfg: 04/26/2011 model: rumithe issue is that the seat belt is very difficult to tighten and loosen. This was observed from day one and still is very difficult. First i though it's not so bad since the child will not be ejected or be in any danger until now. We were in a situation where we had to get the child out in seconds. This wasn't possible since it took longer than usual to release the seat belt, compared to a non at fault seat. This also applies to get the child in the seat during a emergency. There is friction so the seat belt is difficult to loosen and tighten. It is imperative the seat belt to work without flaws. Meaning to hold the child as it is to use the seat belt. Please make sure this will not result in loss of life. I am an engineer and took a look at the seat. There is a tray underneath the seat, i guess to store the equipment when not in use. That tray when inserted will touch the roller where the seat belt is routed around to make a 90 degree turn and routed up the seat. When the seat is installed in the car the upwardforce created against that tray and the roller makes the seat belt difficult to tighten and loosen.

The contact owns a 2014 safety first on board 22lx (na) child seat, model number: 1c228cyj. The seat was manufactured on november 5, 2014. When the contact attempted to buckle the child in the safety seat, the harness straps failed to extend. As a result, the contact was unable to buckle the child. The manufacturer was not made aware of the failure. The safety seat was not repaired. The purchase date was unknown.

Dt*: the contact stated the child pulled harness straps through the safety latches in the back of the safety 1st child safety seat, model number 01948ml0.this caused the harness to become unlatched from the seat.

This safety seat has an adjustable head restraint that does not stay in the locked position.when the restraint falls to the lowest position, the straps are loosened and do not provide the best protection for my child.i would like to either return the product or receive replacement parts for the head restraint.

Power train and transmission problem. When put in reverse it accelerates with foot still on brake, could cause an accident. I am a cpst who ordered 2 safety first chart air 65 convertible seats from the safety first / dorel "hot deals" section of their website recently.when seats arrived i noted that there was seeminglyample "double"body pillow padding for an infant or small child butwhen the " air protect" feature is moved up (to allow access to higher shoulder slots) and the body pillow(s) for the smaller child are removed ... The area behind the back of a toddler or preschool age child's back is completely devoid of padding..in addition to not having any padding...the fabric in this area is much thinner than elsewhere on the seat. Assuming the "body pillows" are removed as the child grows ( instr. Pp 28) this would render the seat very uncomfortable if not painful with day to day use. Questionable as to outcome in a mvc.additionally i noted that with moving the " air protect " feature upward ...the track it slides upon leaves approx.1.5 cm of "open space"that is easily accessible to a toddler or pre-school child. Thespace is & wide enough to allow a child's fingers (possibly a hand) to become entrapped in the track.i am not sure there are standards that address the amount of padding behind a child's back ..but i am bringing this lack of padding to your attention such that if there are no current standards perhaps one might be developed.i am pretty sure there are standards to addressthe potential for a child's finger or hand to be injured. Just not sure if an injury actually must occur before the matter can be addressed. I do believe there is potential for injury in that a small hand could be entrapped i did contact dorel to see if i could return the seat due to the concerns above. They will allow a return if i want to pay the postage.they see this as an issue of buyers remorse and not a problem with the product,i have decided not to return the product to dorel. Please let me know if nhtsais interested in having the seat.

The contact owns a dorel juvenile group safety 1st prospect,model number 22-595wal child booster seat.both of the plastic arms separate when they are connected to the child seat.the separation creates a wide gap between the pieces when leaning on them, which could pinch or injure the child.the purchase date was unknown. Cpsc#i1240318a. Safety 1st s1 rumi air harnessed booster seat. Consumer stated at first they were fine. But soon the head rest which also adjust the safety straps fell from the proper position to the lowest setting. It also became very loose, presenting a potentially life threatening issue. *ln

Cpsc#i1220155a. Safety one infant car seat. Consumer stated a few months after purchasing the infant car seat both of the upper straps that hold baby in, regularly slip out. *lnthe consumer stated the strap would come out of its holder in the back of the car seat.

Unable to secure avg size newborn in a cosco safety 1st carrier seat. Model 02-621 sal, manufacture 12/02.harness straps extremely long even in tightest position. Need to be able to tighten more(4-5"). Do you have any acceptable modifications?

Handle bar is very loose. Happens all the time.
