The contact owns a harmony juvenile products child booster seat, model number: 0304003lrw, manufactured july 16, 2010. The child restraint was being utilized in a 2007 saturn outlook. The child was placed in the booster seat in a forward facing position. The booster seat buckle was released upon exiting the vehicle and as the contact opened the door for the child to exit, the booster seat began to tilt toward the door. The child immediately fell onto the pavement and sustained minor lacerations to the forehead. The rounded edges on the base of the child restraint contributed to the failure. The booster seat was installed with the seat belt. The contact was unable to reach the manufacturer in order to report the defect.

I currently have two harmony juvenile backless boosters.they constantly tip over when my kids grasp them to get into the car. Last week my friend's daughter fell out of our parked car due to the tipping of the booster.she basically landed head first on the cement!these boosters are extremely unstable!please recall them!!!

We purchased a harmony defender 360 and installed it about a year ago. It has not been removed from the car since until today. Today when i went to buckle my four-year-old son into the seat, i noticed that a bolt holding the seat back to the base of the seat had completely fallen off and was laying on the seat of my car. The seat still appeared to be attached and i'm lucky i noticed the missing bolt. It would have fallen apart in a crash. Very dangerous. I've quit using it.model 0302002mrs, manufactured 2015/05/09. Date of purchase 8-4-15.

The contact stated that the booster seat base was abnormally narrow and would cause the child restraint to tilt. As a result, the child was ejected from the seat while the vehicle was parked and sustained injuries to the face and jaw. The manufacturer was made aware of the failure.

The contact stated that the child safety booster seat was equipped with a removable foam padding attached with velcro to the structure of the booster seat. The contact discovered that there were sixteen holes on the booster seat. The contact mentioned that the child inserted his finger in a hole and became stuck to the seat. The fire department had to take the child to the hospital to remove his finger from the hole. The purchase date was unknown.
