The contact owns a 2006 toyota sienna with a 2010 graco booster seat, model number pd144968a. The contact stated that the seat belt would not hold the child in place while in the booster. The child had the ability to move freely in the seat from one side to another and was also able to unclip the seat belt. The booster seat manufacturer was contacted and asked if there was a clip that could be installed for the vehicle seat belt to have a tighter fit. I own a turbo booster child car seat ,manufactured 5-28-2012 by graco children's products inc.. The child safety seat will snap out from the base and it can recline all the way back like laying position! there was not enough safety installation instructions when we bought it. On november 19 8 am i get ready drive to work and my 3 year old was buckled in her car seat behind the drivers side of my 2001chrysler pt cruiser she was belted over her shoulder correctly, she was not restrained safe enough because we are lucky to be alive. We were tboned by a kenworth semi logging truck coming out of our driveway, she was thrown out of the carseat on thefloor behind my drivers seat. She caught her finger on the belt it was dislodged before thrown to the floor and she received stitches on her index finger laceration she also was hurt on the head back and belly she was taken to the hospital by ambulance(related vehicle information: model year=2001, make=chrysler, model=pt cruiser)

The size of the hole that the strap travels through was extremely narrow; and both booster seat arms were poorly designed. The manufacturer was made aware of the failure. The vin and failure mileage were unavailable.updated 9/20/11 \the consumer stated significant assembly was required, the written assembly instructions were poor, and even with pictures provided they were not adequate to ensure safe installation. If the screws were not installed in the arms, they could easily detach from the seat and release the seat belt. The arms of the booster seat were not secure enough or strong enough to retain the shoulder strap in an accident. The arms of the booster seat does not prevent a child from removing the vehicle shoulder strap from its required position below the arm rest. The five slots on the back of the booster seat presented too many possible attachments points for the strap that was provided with the seat. The three slots on the back of the booster seat were too weak to secure the strap, but was the only location where the strap could easily be installed. The two corner slots located on the outer edges of the seat was strong enough to secure the straps, but they were too narrow to make installation of the strap easy. The seat belt attachment clip on the booster seat strap was slotted and could allow the vehicle seat belt to become detached in a violent accident or be removed by a determined child. The shoulder positioning clip is not shipped assembled in the manner shown on pages 32-35 of product manual. Therefore, if picture accurately portrays how the product should look and be installed, user must reassemble product. That is, the strap is threaded backwards through the clip according to the manual's pictures.

The booster seat should include a tether strap to keep the seat in place. Just using a seatbelt around the booster is not enough to hold it in place in an accident. A one car wreck occurred with a 4 yr old in a proper child restraint in bartlett, tn. The car flipped and landed on the passenger side.my complaint is the fact that there was a graco booster seat for a lager child of 6 in the car that was not in use. Because there is no way to strap this down securely to the car the booster seat then became a projectile object and hit the 4 yr old. There was blood on the back of the booster seat and the pad and in the cup holders. A better design is needed to keep these necessary objects that are not in use from being an additional cause of trauma during an accident. I think it is in the best interest of the car seat companies and the families who use these products on a daily basis to address this issue with the up-most importance. We purchased the seat and used it for a couple weeks before i noticed the problem. Whenever i moved the seat the headrest would be lowered, even if i was careful not to bump the top. After this happened a few times, i noticed it would end up lower even if i hadn't moved the seat (child used the seat, and left the vehicle. When child returned to seat, headrest was lower). I also discovered that if i pushed on the top of the headrest, it would click down, even without pushing the red button that allows it to click down. I called graco and they sent a replacement head piece. After a couple weeks, the same problems happened again. I called graco again and they sent a replacement back and head piece. Another couple week of using the seat and again the headrest is not working properly. I contacted graco and asked for a refund of the money. They will send a refund and i will return the seat to them.

Today, 8/6/16. I took the cover off my daughter's carseat and the foam fell off the headrest. I attempted to call graco but they are closed and i have to wait until monday to see what to do to fix the issue.

The back of the booster does not stay in position. If the back is raised to a position other than the lowest position it slips back into the lowest position with very little pressure applied to the top. It should remain locked unless the adjuster is squeezed.
