The contact owned an evenflo evolve 3-in-1 seat, model number: 34411700. The seat was manufactured on april 20, 2015. The seat was utilized in a 2007 honda pilot. The contact stated that food became stuck in the seat belt release mechanism and the seat belt became loose. The child was in the safety seat at the time of the failure. The manufacturer was not made aware of the failure.

In january of 2017 i was placing my 4 year old son in his evenflo platinum evolve dlx 3-in-1 combination booster seat in the forward facing car seat as usual. After buckling both straps i tightened the straps and checked them for tightness like i always do and as i pulled back on both straps they immediately loosened. I again pulled the harness adjustment strap tight through the harness adjustment button assembly and checked the shoulder straps again and they loosened again. This is very scary. No matter what the cause, this should never happen. Serious injury or death could occur in the event of a collision. I had to drive back from vacation in wv to fl with my child in the defective car seat. I believe the problem is with the harness adjustment button not properly holding the straps tight.
