The contact owns a dionosafety seat, model type: radian rxt model, model number us16900 and manufactured in 4/01/2013. The safety seat was being utilized in a 2013 honda odyssey. The contact stated that while attempting to release the crotch buckle, the buckle failed to release and required excessive pressure to release. The latch that held the seat in place also failed to secure the seat. The manufacturer was made aware of the failure and advised the contact to purchase replacement parts, but then stated that the manufacturer would replace the crotch buckle. The seat was not repaired. The vin was not available.

Diono is telling parents they do not need to fix the recall if they are using a tether.i find this irresponsible.

Make: dionomodel: radian rxtserial #: 101208057dom: 10/12i can not get the harness to tighten.i can tug and pull with all of my weight and it still will not pass the pinch test.i am a member of several carseat pages and realized that this is a common problem.just last night i chatted with at least four different people who have radians with the same problem.all of whom myself included have contacted diono within the last week and are awaiting response from them.this is not safe, as an unsnug harness will not protect a child in the event of a crash.there was no event that lead up to the harness tightener not working, the seat is well maintained and routinely cleaned.it has also primarily been a back up seat and not frequently used.

Cpsc:i have a dione rxt car seat that has been having head wing issues. My sons car seat head wings keep falling down to the improper position and will not stay up even after doing as the company instructed. They ask for the bolts to be tightened in the back, which i did. So i contacted the company again letting them know that did not remedy the situation. Updated 04/21/2017

Cpsc:this car seat (diono radian rxt all-in-one) does not work properly in booster mode if it was made in 2014 or prior. (more years could be affected, but mine was made in 2014 and thecompany told me it has since changed the covers in newer models. Updated 08/18/2017

The pull strap to tighten my carseat rubs on the locking mechanism causing the strap to fray over time (every ~6-8 months, occurred 3 times so far, one car seat was disposed of after my wife was rear ended, diono replaced strap one time, and i am now out of warranty by a couple months but attempting to get them to replace it again). The manufacturer advised me that this can happen if you pull up rather than straight out from the child seat, but, i have the seat installed rear facing so it is impossible to pull straight out (the back of the actual seat of the car prohibits this). It appears to be a systematic problem with this car seat as it has occurred to both that i purchased (don't ask me how my wide convinced me to buy it a second time) and a google search provides amply accounts of others having the same issue.each time the strap frays i fear that it is putting my child's life in danger, probably even before it is visually damaged.

Cpsc: i14a0032a. Diono rainier car seat. Consumer stated that her child was able to reach the foam on the head wings and break pieces off, and was putting it in his mouth. Also stated this poses a significant risk of choking. *lnthe manufacturer sent the consumer a replacement car seat. However, her son was able to do the same thing, to the replacement seat.serial # 30310

Purchased a diono cambria booster seat. It showed up damaged. Called the retailer for a replacement and the replacement seat showed up damaged as well in the same exact place. Both seats manufactured on the same day. Diono did not show any concern nor barely bat an eye at the fact that they had the same manufacture date and were damaged in the same place. It's right where the high back connects to the low back. It could have failed in an accident and caused my child to be severely hurt.

As i was placing my daughter in her diono pacifica car seat, i noticed the eps foam on the head wing was not laying in place. I removed the cover to discover that one entire piece of the eps foam had popped off from the seat, and the rest of the foam on the head rest was loose. We are incredibly gentle and care for the seat as instructed by the manufacturer, so there is no reason for this to have happened.after contacting diono customer service, i was sent two replacement eps foam pieces with no instructions regarding how to adhere the foam to the seat. I had to ask how to adhere the pieces, only to be told that i should visit my local craft store and purchase a craft glue.my concerns regarding the handling of this problem are: 1. If i had not asked, i would not have been informed about what glue to use. What if i had attempted to use super glue (which the representative told me not to use in her response) or a simple elmer's glue? this is a huge safety concern.2. The eps foam absorbs energy in an impact (per diono, it must be in place). What if i had used the wrong glue and the foam did not stay in place? should i be checking the seat each time i place my daughter in the car seat now that it has come apart? 3. The remnants of the original glue adhering the foam are clearly visible and are, in my opinion, inadequate. There is simply one line of glue adhering the plastic, rather than glue placed along the entire surface of the plastic that the foam covers. Clearly this is a problem with manufacturing, as other foam pieces are also loose on my seat. 4. Diono's solution is to send me a "replaceable part", the foam was compared to the harness straps, cover, and shoulder straps. The eps foam is intended to be a permanent component of the seat. It seems negligent to ask a consumer to glue an essential piece back on independently.

The contact stated that while driving with her 18-month-old daughter rear facing in the child seat, her daughter started picking apart and eating the foam in the velcro shoulder straps. The contact stated that the flaps to the shoulder straps were not sewn shut to prevent a child from removing the foam padding.
