Dt*: the contact stated the buckle system gets stuck when trying to release the infant from the child safety seat and would not unlatch.the car seat was a convertible seat with a lap over belt to secure the infant passenger. The contact called and said they received a copy of the complaint by postal mail, but the name used in the mailing had the wrongt first name gerald instead of jared, so the contact returne dit to the postmaster. Realizing what it was the contact returned to the postmaster nd requested the letter due to incorrect name, but the postmaster refused to release it back to the c ontact. The contct requested an additional copy of the complaint be sent to them with correctfirst name.
Dt*: the contact stated the parent entered the vehicle placing the baby in the car seat.the seven year old sibling entered the back seat and expressed that the baby's lips appeared blue.the parent patted the baby on the back and blew into the baby's mouth.the baby began to cry.the parent took the sibling to school and then commenced to transport the baby to their primary care physician to have the baby's cyanotic episode addressed.on the way, the parent stopped at a family member's residence to request the parent's siblingto accompany them to the doctor's office.the distance between the school and residence was 1.5 miles.upon arrival, the baby was observed to be unresponsive and not breathing.respiratory efforts were commenced and 911 was called. The child died, and the primary cause of death was hypoxic brain injury due to positional asphyxia.the manner of death was accidental.updated 12/28/2006 -
Coscomodel 02-759, dom09/24/97. Carrying handle becomes loose while using the seat as a carrier.a recall was issued on css mode. ; however this seat was not included due to the manufacturer'sdate.
The car seat is not able to be carried by the handle due to the handle hanging out of the seat.
Cosco model # 02758, dom 26-may-1999; was pulling handle backwards into place when one side would move, and the other side failed to move at all.was acting as if the attachment end was hung up on something inside ofthe attachment.

Dt: cosco eddie bauermodel # 22-720, dom 10/24/2004. The contact states that car seat broken in the joint where the shield connects to the seat. The child was not in the seat when the shieldloosened from the seat. Thecontactwas getting the seat out of the car when this happened. Cosco has not been contacted as of now. The seat has been discarded. The piece that goes into the hole from the shield cracked.
The contact owns a cosco turnabout booster seat, model number 22-293-bvl.the seat was manufactured on april 24, 2008.while the contact was driving, her child was able to place her leg between the cup holder and the arm rest of the booster seat.as a result, the child's leg became stuck in between the two objects.the child was not injured during the incident.the child seat was installed by the contact using the seat belt in the vehicle.the purchase date and failure type were unknown.
